    Ultradian rhythms in glucose regulation: A mathematical assessment

    Glucose regulation is an essential function of the human body which enables energy to be effectively utilized by the brain, organs and muscles. This regulation operates in a cyclic manner, in different periodic regimes. Indeed, ultradian rhythms with a period of 70 to 150 minutes have been clinically observed in healthy patients under various glucose stimulation patterns. Various models of these oscillations in plasma glucose and insulin have shown that the presence of two delays in hepatic glycogenesis and pancreatic insulin secretion provide a pathway for explaining these oscillations. The efficacy of this control is typically reduced in the presence of diabetes. In this contribution, we adopt the presence and the accurate tuning of ultradian rhythms as a criterion for healthy glucose regulation. We then investigate a model with two delays of these ultradian rhythms which incorporates parameters accounting for insulin sensitivity and insulin secretion. Additionally, the effect of diabetic deficiencies on this feedback loop is explored by quantifying the joint contribution of delays and diabetic parameters on the limit cycle of this model, which is generated through a Hopf bifurcation. Strategies for restoring an oscillatory regime in a physiologically appropriate range are discussed. Finally, a simple polynomial model of the oscillations is introduced to give further insight into the influence of each physiological subsystem.     Prediction of distal tibial articular extension in tibial shaft fractures: both posterior malleolar fracture and non posterior malleolar fracture intra-articular extension

    Abstract Background Multiple authors have highlighted the increased incidence of occult posterior malleolar fractures (PMFs) with spiral tibial shaft fractures, although other reported associated risks of intra-articular extension have been limited. The aim of our study is to investigate both PMFs and non-PMFs intra-articular extensions associated with tibial diaphyseal fractures to try to determine any predictive factors. Methods We undertook a retrospective review of a prospectively collected database. The inclusion criteria for this study were any patient who had sustained a diaphyseal tibial fracture, who had undergone surgery during the study period and who had also undergone a CT scan in addition to plain radiographs. The study time period for this study was between 01/01/2013 and 9/11/2021. Results Out of 764 diaphyseal fractures identified, 442 met the inclusion criteria. A total of 107 patients had PMF extensions (24.21%), and a further 128 patients (28.96%) had intra-articular extensions that were not PMF’s. On multivariate analysis, spiral tibial fracture subtypes of the AO/OTA classification (OR 4.18, p &lt; 0.001) and medial direction of tibial spiral from proximal to distal (OR 4.38, p &lt; 0.001) were both significantly associated with PMF. Regarding intra-articular fractures, multivariate analysis showed significant associations with non-spiral (OR 4.83, p &lt; 0.001) and distal (OR 15.32, p &lt; 0.001) tibial fractures and fibular fractures that were oblique (OR 2.01, p = 0.019) and at the same level as tibia fracture (OR 1.83, p = 0.045) or no fracture of the fibular (OR 7.02, p &lt; 0.001). Conclusion In our study, distal tibial articular extension occurs in almost half of tibial shaft fractures. There are very few fracture patterns that are not associated with some type of intra-articular extension, and therefore, a low threshold for preoperative CT should be maintained.     Investigation of effect of nutritional drink on chemotherapy-induced mucosal injury and tumor growth in an established animal model

    Chemotherapy-induced mucositis represents a significant burden to quality of life and healthcare costs, and may be improved through enhanced nutritional status. We first determined the safety of two nutritional drinks (plus placebo), and then potential gut protection in tumor-bearing rats in a model of methotrexate-induced mucositis. In study 1, animals were fed one of two test diets (or placebo or control chow pellets) for a total of 60 days and were monitored daily. All diets were found to be safe to administer. In study 2, after seven days of receiving diets, a Dark Agouti Mammary Adenocarcinoma (DAMA) was transplanted subcutaneously. Ten days after starting diets, animals had 2 mg/kg intramuscular methotrexate administered on two consecutive days; after this time, all animals were given soaked chow. Animals were monitored daily for changes in bodyweight, tumor burden and general health. Animals were killed 10, 12 and 16 days after initially starting diets, and tissues were collected at necropsy. In study 1, animals receiving diets had gained 0.8% and 10.8% of their starting bodyweight after 60 days, placebo animals 4.4%, and animals fed on standard chow had gained 15.1%. In study 2, there was no significant influence of test diet on bodyweight, organ weight, tumor burden or biochemical parameters. Only animals treated with MTX exhibited diarrhea, although animals receiving Diet A and Diet C showed a non-significant increase in incidence of diarrhea.     IGL-1 solution reduces endoplasmic reticulum stress and apoptosis in rat liver transplantation

    Injury due to cold ischemia reperfusion (I/R) is a major cause of primary graft non-function following liver transplantation. We postulated that I/R-induced cellular damage during liver transplantation might affect the secretory pathway, particularly at the endoplasmic reticulum (ER). We examined the involvement of ER stress in organ preservation, and compared cold storage in University of Wisconsin (UW) solution and in Institute Georges Lopez-1 (IGL-1) solution. In one group of rats, livers were preserved in UW solution for 8 h at 4 °C, and then orthotopic liver transplantation was performed according to Kamada's cuff technique. In another group, livers were preserved in IGL-1 solution. The effect of each preservation solution on the induction of ER stress, hepatic injury, mitochondrial damage and cell death was evaluated. As expected, we found increased ER stress after liver transplantation. IGL-1 solution significantly attenuated ER damage by reducing the activation of three pathways of unfolded protein response and their effector molecules caspase-12, C/EBP homologous protein-10, X-box-binding protein 1, tumor necrosis factor-associated factor 2 and eukaryotic translation initiation factor 2. This attenuation of ER stress was associated with a reduction in hepatic injury and cell death. Our results show that IGL-1 solution may be a useful means to circumvent excessive ER stress reactions associated with liver transplantation, and may optimize graft quality

    Predictors and Outcomes Associated with Household Hunger in Lusaka, Zambia: Secondary Analysis of a Citywide Survey

    Background: Food insecurity has important social and health consequences for affected individuals and households. We sought to measure one aspect of food insecurity—household hunger—and evaluated its possible association with household morbidity and mortality. Methods: We analyzed data from the final two rounds of a repeat cross-sectional, population-based survey conducted in Lusaka Zambia (May and August 2011). Using the Household Hunger Scale, we categorized participating households into three groups using established convention in the public health literature: little to no hunger, moderate hunger, and severe hunger. We used multilevel logistic regression to investigate associations between household hunger and the following morbidities, adjusting for individual, household, and cluster characteristics: malaria, persistent cough, tuberculosis, diarrhea, hospitalization, and death. Results: Overall, 90.0%, (95%CI: 88.1–91.7%) participating households were considered to have little to no household hunger; 9.8% (95%CI: 8.2–11.6%) reported moderate household hunger; and 0.2% (95%CI: 0.1–0.4%) reported severe household hunger. Marital status, functional status, education, employment, household member requiring nursing care, and household wealth index were associated with all levels of hunger. Adjusted for individual and household characteristics and sampling cluster, hunger was associated with malaria (OR:1.29, 95%CI:1.03–1.63 [mild] and OR:3.68, 95% CI:1.76 –13.74 [severe]), persistent cough (OR:1.64, 95%CI:1.13–2.38 [mild]), tuberculosis (OR:2.24, 95%CI:1.45–3.46 [mild], OR:6.06; 95%CI:1.56–23.57 [severe]), and hospitalization (OR:1.95; 95%CI:1.38–2.76 [mild]; OR:5.52; 95%CI:1.78-17.16 [severe]).Household hunger was not associated with death (p>0.05). Conclusions: Household hunger was associated with a number of adverse health outcomes. Although further studies are needed, our findings suggest that programs to alleviate household hunger—an important aspect of food insecurity—could lead to measurable public health impacts
