    Following a systematic review of the literature, a novel theory was developed to explain the relationship between trauma and hearing voices. The theoretical model was tested using cross-sectional, prospective, and qualitative research methods with clinical and non-clinical samples. This thesis concludes with a comprehensive research agenda to guide future research that may lead to the development of more effective treatments for trauma-affected voice hearers

    Through the eyes of a woman: Using oral history to explore the enigmatic world of Saudi Arabia’s female population

    The Kingdom of Saudi Arabia (KSA) remains cloaked in mystery and stereotypes especially when it pertains to its female population. Oppression? Subjugation? Discontent? These typecasts reverberate around the world, but interestingly when one speaks to a Saudi woman her story offers diverging representations of Saudi life that often debunk those expressed elsewhere. But without scholars asking “real” people about their lives and analyzing their lived responses, these dominant narratives are reaffirmed over and over again. To move beyond these stereotypes, this paper attempts to address the lives of Saudi Arabian females by using firsthand accounts obtained for an undergraduate student project. “The Oral History Project”, initiated by the two authors using traditional oral history methods to teach history, geography and leadership at a private university in the kingdom, has female students interviewing family members about their past focusing on topics such as their life, family, tribe, and transformations due to modernity. The goal is to focus on the female perspective. Students asked common questions such as: What is life-like for a Saudi woman? How does it differ from when you were a child? How have women navigated the constraints of a traditional society and how do they perceive themselves in it? Familial responses were recorded, translated, and transcribed to create an overview of each family’s history

    A regional and international framework for evaluating seagrass management and conservation

    Seagrass meadows provide a range of key ecosystem services that are of high economic and societal value; seagrass meadows enhance biodiversity, provide food security through fisheries support, and are increasingly recognised for the role they play in mitigating climate change by the process of carbon sequestration. Whilst there is an increasing understanding of the global significance of seagrass habitats, the extent of these habitats is declining globally. The requirement to implement effective seagrass conservation and management strategies is thus becoming increasingly important. If the ambitions of the United Nations 2030 Agenda for Sustainable Development and the Sustainable Development Goals are to be achieved, then nations need ambitious and applicable marine conservation plans. This includes management and protection to vulnerable ecosystems such as seagrass meadows. This study aims to evaluate a range of seagrass management and conservation approaches identified in different geographic regions, using a critique framework developed from the United Nations Environment Programme 2020 report on seagrass “Out Of The Blue: The Value Of Seagrasses To The Environment And To People’. Using the framework, seagrass management in Scotland is used as a case study and compared nationally to the rest of the UK (England, Wales and Northern Ireland) and internationally, to Europe (Wadden Sea), Australia (Great Barrier Reef Marine Park) and West Africa (Senegal). The results identify potential areas of development in Scotland to enhance its seagrass conservation effort, including increased research in seagrass science, widespread mapping of seagrass, long-term monitoring programmes, improved marine protected areas, inclusion of seagrass protective measures into environmental laws and policies and the implementation of appropriate habitat restoration schemes. The results also identify the need for open data if effective knowledge sharing is to take place, and to ensure that ocean science can fully support countries to achieve the 2030 Agenda for Sustainable Development

    The principles of expected and realised genetic relatedness among individual honeybees

    Monitoring honeybee genetic variability is essential to manage global and local ge-netic diversity. Coefficients of relatedness are regularly used to measure genetic similar-ity within and between populations and their individuals. Although the haplo-diploid inheritance of honeybees is well understood, interpreting the various types of related-ness coefficients based on pedigree and genotype data is a challenge for researchers and practitioners in honeybee breeding. To demonstrate the principles of genetic relatedness in honeybees and its different individual-based estimators, we simulated three honeybee populations each containing 400 colonies over 10 years using the stochastic simulator SIMplyBee. We kept two populations closed and hybridised the third one by importing drones from one of the closed populations. We evaluated the relatedness between indi-viduals within a colony, between queens of the same population, and between queens of different populations. We calculated three types of relatedness: expected identity by descent using pedigree information, realised identity by descent using pedigree and genotype information, and identity by state using genotype information. Our results demonstrated an alignment of mean relatedness across different types when calculated using the same founder population, regardless of their data source. Identity by state relatedness varied significantly when calculated with different founder populations. Al-though this is an expected result, it shows that caution is needed when comparing values between studies using different populations with different allele frequencies. We expectedly showed increased relatedness over time in closed populations and decreased in the hybrid population. Our results underscore the significance of understanding the methodology for computing relatedness coefficients

    Ocular Point-of-Care Ultrasonography to Diagnose Posterior Chamber Abnormalities: A Systematic Review and Meta-analysis

    Importance: Diagnosing posterior chamber ocular abnormalities typically requires specialist assessment. Point-of-care ultrasonography (POCUS) performed by nonspecialists, if accurate, could negate the need for urgent ophthalmologist evaluation. Objective: This meta-analysis sought to define the diagnostic test characteristics of emergency practitioner-performed ocular POCUS to diagnose multiple posterior chamber abnormalities in adults. Data sources: PubMed (OVID), MEDLINE, EMBASE, Cochrane, CINAHL, and SCOPUS were searched from inception through June 2019 without restrictions. Conference abstracts and trial registries were also searched. Bibliographies of included studies and relevant reviews were manually searched, and experts in the field were queried. Study selection: Included studies compared ocular POCUS performed by emergency practitioners with a reference standard of ophthalmologist evaluation. Pediatric studies were excluded. All 116 studies identified during abstract screening as potentially relevant underwent full-text review by multiple authors, and 9 studies were included. Data extraction and synthesis: In accordance with PRISMA guidelines, multiple authors extracted data from included studies. Results were meta-analyzed for each diagnosis using a bivariate random-effects model. Data analysis was performed in July 2019. Main outcomes and measures: The outcomes of interest were diagnostic test characteristics of ocular POCUS for the following diagnoses: retinal detachment, vitreous hemorrhage, vitreous detachment, intraocular foreign body, globe rupture, and lens dislocation. Results: Nine studies (1189 eyes) were included. All studies evaluated retinal detachment, but up to 5 studies assessed each of the other diagnoses of interest. For retinal detachment, sensitivity was 0.94 (95% CI, 0.88-0.97) and specificity was 0.94 (95% CI, 0.85-0.98). Sensitivity and specificity were 0.90 (95% CI, 0.65-0.98) and 0.92 (95% CI, 0.75-0.98), respectively, for vitreous hemorrhage and were 0.67 (95% CI, 0.51-0.81) and 0.89 (95% CI, 0.53-0.98), respectively, for vitreous detachment. Sensitivity and specificity were high for lens dislocation (0.97 [95% CI, 0.83-0.99] and 0.99 [95% CI, 0.97-1.00]), intraocular foreign body (1.00 [95% CI, 0.81-1.00] and 0.99 [95% CI, 0.99-1.00]), and globe rupture (1.00 [95% CI, 0.63-1.00] and 0.99 [95% CI, 0.99-1.00]). Results were generally unchanged in sensitivity analyses of studies with low risk of bias. Conclusions and relevance: This study suggests that emergency practitioner-performed ocular POCUS is an accurate test to assess for retinal detachment in adults. Its utility in diagnosing other posterior chamber abnormalities is promising but needs further study

    Effects of maternal geohelminth infections on allergy in early childhood.

    BACKGROUND: Maternal geohelminth infections during pregnancy may protect against allergy development in childhood. OBJECTIVE: We sought to investigate the effect of maternal geohelminths on the development of eczema, wheeze, and atopy during the first 3 years of life. METHODS: A cohort of 2404 neonates was followed to 3 years of age in a rural district in coastal Ecuador. Data on wheeze and eczema were collected by means of questionnaire and physical examination at 13, 24, and 36 months of age. Atopy was measured based on skin prick test (SPT) reactivity to 9 allergens at 36 months. Maternal stool samples were examined for geohelminths by microscopy. Data on potential confounders was collected after birth by questionnaire. RESULTS: Geohelminths were observed in 45.9% of mothers. Eczema and wheeze were reported for 17.7% and 25.9%, respectively, of 2069 (86.1%) children with complete follow-up to 3 years, and allergen SPT reactivity to any allergen was present in 17.2% and to house dust mite in 8.7%. Maternal geohelminth infections were not significantly associated with eczema (adjusted odds ratio [OR], 1.26; 95% CI, 0.98-1.61), wheeze (adjusted OR, 1.02; 95% CI, 0.82-1.27), and SPT reactivity to any allergen (adjusted OR, 0.79; 95% CI, 0.61-1.01). In subgroup analyses maternal geohelminths were associated with a significantly reduced risk of SPT reactivity to mite and other perennial allergens, and maternal ascariasis was associated with an increased risk of eczema and reduced risk of SPT reactivity to all allergens. CONCLUSION: Our data do not support a protective effect of maternal infections with geohelminth parasites during pregnancy against the development of eczema and wheeze in early childhood, although there was evidence in subgroup analyses for a reduction in SPT reactivity to house dust mites and perennial allergens

    Biopharmaceutics of Topical Ophthalmic Suspensions: Importance of Viscosity and Particle Size in Ocular Absorption of Indomethacin

    Eye drops of poorly soluble drugs are frequently formulated as suspensions. Bioavailability of suspended drug depends on the retention and dissolution of drug particles in the tear fluid, but these factors are still poorly understood. We investigated seven ocular indomethacin suspensions (experimental suspensions with two particle sizes and three viscosities, one commercial suspension) in physical and biological tests. The median particle size (d50) categories of the experimental suspensions were 0.37–1.33 and 3.12–3.50 µm and their viscosity levels were 1.3, 7.0, and 15 mPa·s. Smaller particle size facilitated ocular absorption of indomethacin to the aqueous humor of albino rabbits. In aqueous humor the AUC values of indomethacin suspensions with different particle sizes, but equal viscosity, differed over a 1.5 to 2.3-fold range. Higher viscosity increased ocular absorption 3.4–4.3-fold for the suspensions with similar particle sizes. Overall, the bioavailability range for the suspensions was about 8-fold. Instillation of larger particles resulted in higher tear fluid AUC values of total indomethacin (suspended and dissolved) as compared to application of smaller particles. Despite these tear fluid AUC values of total indomethacin, instillation of the larger particles resulted in smaller AUC levels of indomethacin in the aqueous humor. This suggests that the small particles yielded higher concentrations of dissolved indomethacin in the tear fluid, thereby leading to improved ocular bioavailability. This new conclusion was supported by ocular pharmacokinetic modeling. Both particle size and viscosity have a significant impact on drug concentrations in the tear fluid and ocular drug bioavailability from topical suspensions. Viscosity and particle size are the key players in the complex interplay of drug retention and dissolution in the tear fluid, thereby defining ocular drug absorption and bioequivalence of ocular suspensions
