    Affordances guiding Forest School practice: The application of the Ecological Dynamics approach

    Forest School focuses on child development underlining nature-connection and play pedagogy. Practitioners facilitate child-led learning through a deep observation approach. However, challenges and assumptions exist in such approaches. Additionally, a critical examination of the practice reveals that it may be lacking a solid theoretical underpinning that can respond to diverse contexts and participants while escaping a one-size-fits-all approach encouraged by commercialisation. Ecological Dynamics offers a theoretical framework that has the potential to guide Forest School practice and clarify its effectiveness. Specifically, notions of affordances combined with analysis at the level of person-environment relationships could guide future design and implementation of activities. Benefits could include realising and attuning to affordances which have sociocultural and individual connotations, thereby respecting local cultures and their community resources. The role of the Forest School practitioner becomes one of facilitating diverse populations in their perception of affordances in nature for individualised benefits, including well-being

    Prognostic model to predict postoperative acute kidney injury in patients undergoing major gastrointestinal surgery based on a national prospective observational cohort study.

    Background: Acute illness, existing co-morbidities and surgical stress response can all contribute to postoperative acute kidney injury (AKI) in patients undergoing major gastrointestinal surgery. The aim of this study was prospectively to develop a pragmatic prognostic model to stratify patients according to risk of developing AKI after major gastrointestinal surgery. Methods: This prospective multicentre cohort study included consecutive adults undergoing elective or emergency gastrointestinal resection, liver resection or stoma reversal in 2-week blocks over a continuous 3-month period. The primary outcome was the rate of AKI within 7 days of surgery. Bootstrap stability was used to select clinically plausible risk factors into the model. Internal model validation was carried out by bootstrap validation. Results: A total of 4544 patients were included across 173 centres in the UK and Ireland. The overall rate of AKI was 14·2 per cent (646 of 4544) and the 30-day mortality rate was 1·8 per cent (84 of 4544). Stage 1 AKI was significantly associated with 30-day mortality (unadjusted odds ratio 7·61, 95 per cent c.i. 4·49 to 12·90; P < 0·001), with increasing odds of death with each AKI stage. Six variables were selected for inclusion in the prognostic model: age, sex, ASA grade, preoperative estimated glomerular filtration rate, planned open surgery and preoperative use of either an angiotensin-converting enzyme inhibitor or an angiotensin receptor blocker. Internal validation demonstrated good model discrimination (c-statistic 0·65). Discussion: Following major gastrointestinal surgery, AKI occurred in one in seven patients. This preoperative prognostic model identified patients at high risk of postoperative AKI. Validation in an independent data set is required to ensure generalizability

    Nucleic acid chaperone properties of ORF1p from the non-LTR retrotransposon, LINE-1

    Long interspersed element-1 (LINE-1 or L1) is a non-long terminal repeat (LTR) retrotransposon that has amplified to hundreds of thousands of copies in mammalian evolution. A small number of the individual copies of L1 are active retrotransposons which are presently replicating in most species, including humans and mice. L1 retrotransposition begins with transcription of an active element and ends with a newly inserted cDNA copy, a process which requires the two element-encoded proteins to act in cis on the L1 RNA. The ORF1 protein (ORF1p) is a high-affinity, non-sequence-specific RNA binding protein with nucleic acid chaperone activity, whereas the ORF2 protein (ORF2p) supplies the enzymatic activities for cDNA synthesis.     Homeless youth face numerous challenging life circumstances, both prior to leaving home and while homeless. Examples of such difficult experiences include exceptionally high rates of pregnancy and early parenthood, particularly in comparison to their housed peers. This chapter reviews the complexity of causes, risk factors, and adverse outcomes associated with homeless youth pregnancy and early parenthood. Homeless youth pregnancy and early parenthood are notably under-researched topics, and as homeless youth have unique and complex life experiences, this chapter also highlights opportunities for further advancing culturally responsive prevention and intervention services in efforts to more effectively decrease pregnancies and improve sexual, reproductive, maternal-child health, and parenting outcomes among this highly vulnerable population

    Metabonomics in neonatal and paediatric research: studying and modulating gut functional ecology for optimal growth and development

    Gut microbiota play a fundamental role in human health by promoting intestinal homeostasis, stimulating development of the immune system, and providing protection against pathogens. Relatively little is known about the acquisition and development of this complex microbial community during infancy. However, emerging 'omics' technologies are now being applied to the study of the gut microbial ecology, generating new opportunities to deepen the functions of the gut microbiota in human health. All the published literature on paediatric and neonatal nutrimetabonomics is presented in a synthetic way, including studies on maternal milk and formula. The role of the disruption of the gut microbiota in various gastrointestinal diseases is considered, focusing the metabonomics approach in gut ischemia, chronic inflammatory diseases, cystic fibrosis, diabetes, and obesity. As a general rule, the best biofluid to study nutrition or to identify food-specific biomarkers is urine. In food consumption monitoring, the same concepts behind drug testing and drug monitoring can be used. In the opinion of the authors, in the near future improved tools for the analysis of the metabolic profile (simplified like 'dipsticks' for urine) and its integration with the other 'omics' data will move metabonomics beside the child, from top research to bedside
