Post-inpatient Attrition from Care “As Usual” in Veterans with Multiple Psychiatric Admissions

Disengagement from outpatient care following psychiatric hospitalization is common in high-utilizing psychiatric patients and contributes to intensive care utilization. To investigate variables related to treatment attrition, a range of demographic, diagnostic, cognitive, social, and behavioral variables were collected from 233 veterans receiving inpatient psychiatric services who were then monitored over the following 2 years. During the follow-up period, 88.0 % (n = 202) of patients disengaged from post-inpatient care. Attrition was associated with male gender, younger age, increased expectations of stigma, less short-term participation in group therapy, and poorer medication adherence. Of those who left care, earlier attrition was predicted by fewer prior-year inpatient psychiatric days, fewer lifetime psychiatric hospitalizations, increased perceived treatment support from family, and less short-term attendance at psychiatrist appointments. Survival analyses were used to analyze the rate of attrition of the entire sample as well as the sample split by short-term group therapy attendance. Implications are discussed

GIDEON: a comprehensive Web-based resource for geographic medicine

GIDEON (Global Infectious Diseases and Epidemiology Network) is a web-based computer program designed for decision support and informatics in the field of Geographic Medicine. The first of four interactive modules generates a ranked differential diagnosis based on patient signs, symptoms, exposure history and country of disease acquisition. Additional options include syndromic disease surveillance capability and simulation of bioterrorism scenarios. The second module accesses detailed and current information regarding the status of 338 individual diseases in each of 220 countries. Over 50,000 disease images, maps and user-designed graphs may be downloaded for use in teaching and preparation of written materials. The third module is a comprehensive source on the use of 328 anti-infective drugs and vaccines, including a listing of over 9,500 international trade names. Analysis of intrapulse chirp in CO2 oscillators

Pulsed single-frequency CO2 laser oscillators are often used as transmitters for coherent lidar applications. These oscillators suffer from intrapulse chirp, or dynamic frequency shifting. If excessive, such chirp can limit the signal-to-noise ratio of the lidar (by generating excess bandwidth), or limit the velocity resolution if the lidar is of the Doppler type. This paper describes a detailed numerical model that considers all known sources of intrapulse chirp. Some typical predictions of the model are shown, and simple design rules to minimize chirp are proposed

A Quillen model structure for Gray-categories

A Quillen model structure on the category Gray-Cat of Gray-categories is described, for which the weak equivalences are the triequivalences. It is shown to restrict to the full subcategory Gray-Gpd of Gray-groupoids. This is used to provide a functorial and model-theoretic proof of the unpublished theorem of Joyal and Tierney that Gray-groupoids model homotopy 3-types. Oddballs and a Low Odderon Intercept

We report an odderon Regge trajectory emerging from a field theoretical Coulomb gauge QCD model for the odd signature JPC (P=C= -1) glueball states (oddballs). The trajectory intercept is clearly smaller than the pomeron and even the omega trajectory's intercept which provides an explanation for the nonobservation of the odderon in high energy scattering data. To further support this result we compare to glueball lattice data and also perform calculations with an alternative model based upon an exact Hamiltonian diagonalization for three constituent gluons.Comment: 4 pages, 2 figures, 1 tabl

Assessment of the Progress of Nations on Core Labor Standards: Measures of Freedom of Association and Collective Bargaining

The linkage between labor standards and trade agreements pursued by the US, and the burgeoning corporate codes of conduct that seek to strengthen core labor standards in global supply chains, has resulted in interest in the development of measures (or indicators) of core labor standards by a variety of organizations, such as the US dept of Labor, the ILO and several NGOs. We argue in this paper that measures of freedom of association and collective bargaining that are in use currently are incomplete and flawed, partly because they focus almost exclusively on whether the rights exist, without regard to practice, and partly because they tend to focus on easily available quantitative indicators that are necessary but insufficient indicators of the freedom of association and collective bargaining process. We develop new measures that draw on decades of comparative industrial relations research and which are based on the existing cross-national variation in industrial relations practice. Our suggested measures require national experts to use both quantitative data and qualitative research and judgment in their evaluation, and report it in consistent and transparent ways. Given that the connection between trade and labor standards makes the consequences of violation quite severe for developing countries, reliance on imperfect measures to make decisions about country performance on core labor standards is problematic. Creating Contagious: How Social Networks and Item Characteristics Combine to Drive Persistent Social Epidemics

Why do certain cultural items capture persistent collective interest while others languish? This research integrates psychological and sociological perspectives to provide deeper insight into social epidemics. First, we develop a psychologically plausible individuallevel model of social transmission behavior. We then situate this model in a social network and perform a series of simulations where we vary different item- and networkrelated characteristics in an experimental setting. The results (1) demonstrate how item and network characteristics combine to drive persistent collective enthusiasm and (2) shed light on the underlying mechanisms through which such social epidemics occur. Interest in most items or products naturally decays over time, so item characteristics (e.g., talkability) and the network positions of early consumers are critical for bolstering consumer enthusiasm. Importantly, however, they do so via different mechanisms, determining how frequently, and with what level of enthusiasm, items are discussed
