Streszczenie Osoba w wieku starszym, geriatrycznym a tym samym bardzo często z niepełnosprawnością jest zbyt skąpo i niedostatecznie holistycznie zaopatrywana w realizację świadczeń zdrowotnych w środowisku domowym. Jedną z form jaka funkcjonuje na rynku świadczeń zdrowotnych i dedykowanych osobom starszym i niepełnosprawnym jest pielęgniarska opieka domowa długoterminowa. Ten rynek świadczeń zdrowotnych od ponad 17 lat zorganizowały i prowadzą pielęgniarki jako samodzielna grupa zawodowa w oparciu o zawierane kontrakty z Narodowym Funduszem Zdrowia. Pomimo stale zwiększającego się zapotrzebowania na tego typu opiekę płatnik czyli Narodowy Fundusz Zdrowia traktuje te świadczenia jako limitowane. Świadczenia te są najtańszą formą opieki na rynku świadczeń zdrowotnych skierowaną do osób starszych i niepełnosprawnych. Wobec faktu, że społeczeństwo polskie starzeje się w bardzo szybkim tempie a tym samym zmaga się z wielochorobowością należałoby te świadczenia zdrowotne umieścić w świadczeniach gwarantowanych nielimitowanych. Zmiana ta wprowadziłaby ogromne oszczędności w ochronie zdrowia. Osoba, która ze względu na stan zdrowia kwalifikuje się do realizacji świadczeń zdrowotnych typu pielęgniarska opieka domowa długoterminowa, a z powodu braku miejsc jest wpisywana na listę osób oczekujących. Summary The elderly person in the geriatric age, and thus very often with a disability, is too sparingly provided with holistic health care services in the home environment. One of the forms, which operates in the market of health care services and which is dedicated to the elderly and disabled is a long-term home-based nursing care. For over 17 years the market for health care services has been organized and led by nurses who are an independent professional group and whose actions have been based on the contracts with the National Health Fund. Despite the ever increasing demand for this type of care, the payer or the National Health Fund treats these benefits as limited. These benefits are for the payer the cheapest form of care addressed to the elderly and disabled. In view of the fact that Polish society is aging at a rapid rate and is thus faced with multiple morbidities, these health benefits should be guaranteed and unlimited. This change would bring huge savings in health care. Due to the insufficient number of places, the person who is eligible for long-term home-based nursing care is put on a waiting list

    Evaluation of the life quality of patients with lumbosacral spine pain performing sedentary work after using Kaltenborn-Evejth manual therapy techniques

    Introduction: Pain in the lumbosacral spine is a substantial problem of modern civilization. It leads to lowering the quality of life and to the reduced functioning in social and professional context. This problem mainly affects people with sedentary work such as drivers and office workers. Long-term work in one position and a frequent lack of physical activity are two of the factors conducive to the occurrence of pain in people doing work in a sitting position. Aim of the study: To assess the influence of Kaltenborn-Evejth manual therapy techniques on the quality of life of people doing sedentary work. Material and Method: The research was carried out in the Medi-Sport Rehabilitation and Sports Center from March to October 2018. The study included 60 people doing sedentary work of which 37 were women and 23 men. The Oswestry Questionnaire was the research tool. In the study group there were 21 people aged 30-40, 23 aged 41-50 and 16 aged 51-60. Results: The use of Kaltenborn-Evejth manual therapy techniques in people doing sedentary work reduced pain in the lumbosacral spine, improved its functions, which improved the quality of life of patients. After the therapy, the severity of pain in the lumbosacral spine decreased. Conclusions: Techniques of Kaltenborn-Evejth manual therapy mobilization significantly reduce pain in the lumbosacral spine, which improves the quality of life of persons performing sedentary work.     A case-control study of the lipid profile of women with breast cancer

    Purpose: Neoplastic diseases remain the second most common cause of death, behind cardiovascular diseases, and breast cancer is the leading cause of death among women worldwide. Increased incidence of neoplastic diseases is also known to be accompanied by lipid metabolism disorders.Method: Blood samples for lipid profile testing were taken from the two studied groups, which comprised 110 women with breast cancer and 70 healthy women. Lipid profile testing was conducted on venous blood samples collected among both groups of women on an empty stomach. Statistical analyses were performed using the IBM SPSS Statistics 20 software package.Results: A statistically significant (p<0.05) majority of women with breast cancer had abnormal TC results (67.3%). A statistically significant (p<0.05) majority of control group women (90.0%) had normal HDL-C results. More women with breast cancer (87.3%) had abnormal LDL-C results than healthy women (62.9%), which represented a statistically significant difference (p<0.05). Conclussions: Abnormal lipid profile significantly impacts breast cancer incidence. At the same time it is under influence of abnormal eating habits and low physical activity. Community and oncology nurses, having considerable knowledge about health and lifestyle of their patients, should be more involved in basic education to decrease cancer risk, especially by women with alarming LDL-C level

    Support received by women with breast cancer in the event of side effects of chemotherapy

    Admission. Correct chemotherapy introduces many side effects. These effects, spread over time and in connection with a chronic disease such as breast cancer, put the patient in a crisis situation, which verifies the needs in terms of social support. Goal: Showing the type of social support expected and received by women treated with cytostatics for breast cancer. Material and methods: The study was conducted on a group of 110 women - patients of oncology departments of two Lublin hospitals diagnosed with breast cancer and during chemotherapy. The research tool was the author's questionnaire. The results from the obtained data were subjected to statistical analysis. Results: Patients expect information support about their illness and treatment from a doctor. In some respects, most of them receive such support. The respondents receive emotional support from their families, more than half of them received an offer of material support. There is a statistically significant relationship between education and the need for material assistance, and a relationship between the next cycle of chemotherapy and the demand for spiritual support. Conclusions: Patients treated with cytostatics, regardless of their level of education, expect information support from professionals. Such support cannot be provided by family or friends. They receive emotional support from loved ones, but during chemotherapy, the demand for spiritual and material support increases, regardless of education

    Molecular characteristics of Polish field strains of Marek's disease herpesvirus isolated from vaccinated chickens

    <p>Abstract</p> <p>Background</p> <p>Twenty-nine Marek's disease virus (MDV) strains were isolated during a 3 year period (2007-2010) from vaccinated and infected chicken flocks in Poland. These strains had caused severe clinical symptoms and lesions. In spite of proper vaccination with mono- or bivalent vaccines against Marek's disease (MD), the chickens developed symptoms of MD with paralysis.</p> <p>Because of this we decided to investigate possible changes and mutations in the field strains that could potentially increase their virulence. We supposed that such mutations may have been caused by recombination with retroviruses of poultry - especially reticuloendotheliosis virus (REV).</p> <p>Methods</p> <p>In order to detect the possible reasons of recent changes in virulence of MDV strains, polymerase chain reaction (PCR) analyses for <it>meq </it>oncogene and for long-terminal repeat (LTR) region of REV were conducted. The obtained PCR products were sequenced and compared with other MDV and REV strains isolated worldwide and accessible in the GeneBank database.</p> <p>Results</p> <p>Sequencing of the <it>meq </it>oncogene showed a 68 basepair insertion and frame shift within 12 of 24 field strains. Interestingly, the analyses also showed 0.78, 0.8, 0.82, 1.6 kb and other random LTR-REV insertions into the MDV genome in 28 of 29 of strains. These genetic inserts were present after passage in chicken embryo kidney cells suggesting LTR integration into a non-functional region of the MDV genome.</p> <p>Conclusion</p> <p>The results indicate the presence of a recombination between MDV and REV under field conditions in Polish chicken farms. The genetic changes within the MDV genome may influence the virus replication and its features <it>in vivo</it>. However, there is no evidence that <it>meq </it>alteration and REV insertions are related to the strains' virulence.</p

Badania prowadzono w Białymstoku, Bydgoszczy, Krakowie, Lublinie, Łodzi, Olsztynie, Sosnowcu, Szczecinie, Warszawie i we Wrocławiu, z zastosowaniem metody wolumetrycznej, przy użyciu aparatów typu Burkard i Lanzoni. Najwyższe stężenia pyłku komosy zanotowano w Łodzi 27 sierpnia (68 z/m3) oraz w Olsztynie w dniu 9 sierpnia (16 ziaren/m3).This paper presents the course of goosefoot pollen season in selected cites of Poland in 2009. The measurements were performed in Białystok, Bydgoszcz, Kraków, Lublin, Łódź, Olsztyn, Sosnowiec, Szczecin, Warszawa and Wrocław, use of volumetric method with Burkard and Lanzoni Spore Trap.     The quality of life of people with slow-healing wounds is a multi-faceted concept which focuses on the functioning of patients in many spheres of life.Two characteristics of patients qualified for long-term homecare nursing are multi-morbidities and senior age. The coexistence of several chronic diseases, including atherosclerosis, is associated with the presence of a number of healing and caring problems, for example, slow-healing wounds. The social security in Poland in the domain of long-term homecare nursing is at a very low level, as reported by the Central Statistical Office in 2015, with only 4 out of 1000 people using  long-term nursing care in a home environment.</p

    Background . Childhood injuries and accidents constitute an important health, social and economic problem. Serious injuries require surgical intervention and hospitalization. In other cases, patients leave hospital within several hours after diagnosis and non-operative treatment, and then they receive outpatient care. Despite a downward trend, injuries remain the main cause of death among children and adolescents. Therefore, understanding their epidemiology, which would allow us to create prevention programs based on scientific evidence (evidence based medicine), is so important. Objectives. The study’s objective was an attempt at analyzing childhood injuries treated in outpatient clinics. Material and methods. The study comprised 2182 children aged 0–18, including 692 females and 1490 males. Among them 1024 children treated surgically and 1158 outpatients. In the retrospective study the authors used a method of analysis of medical records of patients treated in 2012 at the Department of Child Surgery and Traumatology and Orthopaedics with the Operating Theatre in the Nicolaus Copernicus Province Hospital in Koszalin. The analysis was conducted on the basis of operative reports in the case of afflictions and injuries requiring surgery, and outpatient admissions reports in the case of outpatients. Results. Among the outpatients, the biggest group of injuries (34.83% or 403) included bruises, cuts, lacerations etc. Regarding frequency, injuries of hand area – 14.35% (166) and arm fractures – 13.4% (159) followed. Conclusions . 1. The age and gender of patients were factors determining injuries. 2. Seasons of the year have an influence on the structure of injuries among children. 3. It is necessary to increase social awareness on the prevention and negative effects of burns
