    MR imaging of adult acute infectious encephalitis

    International audienceBackground. – Imaging is a key tool for the diagnosis of acute encephalitis. Brain CT scan must be urgently performed to rule out a brain lesion with mass effect that would contraindicate lumbar puncture. Brain MRI is less accessible than CT scan, but can provide crucial information with patients presenting with acute encephalitis.Method. – We performed a literature review on PubMed on April 1, 2015 with the search terms " MRI " and " encephalitis ".Results. – We first described the various brain MRI abnormalities associated with each pathogen of acute encephalitis (HSV, VZV, other viral agents targeting immunocompromised patients or travelers; tuberculosis, listeriosis, other less frequent bacterial agents). Then, we identified specific patterns of brain MRI abnomalies that may suggest a particular pathogen. Limbic encephalitis is highly suggestive of HSV; it also occurs less frequently in encephalitis due to HHV6, syphillis, Whipple's disease and HIV primary infection. Rhombencephalitis is suggestive of tuberculosis and listeriosis. Acute ischemic lesions can occur in patients presenting with severe bacterial encephalitis, tuberculosis, VZV encephalitis, syphilis, and fungal infections. Conclusion. – Brain MRI plays a crucial role in the diagnosis of acute encephalitis. It detects brain signal changes that reinforce the clinical suspicion of encephalitis, especially when the causative agent is not identified by lumbar puncture; it can suggest a particular pathogen based on the pattern of brain abnormalities and it rules out important differential diagnosis (vascular, tumoral or inflammatory causes).Introduction. – L'imagerie cérébrale est essentielle au diagnostic d'encéphalite aiguë. Le scanner cérébral doit être réalisé en urgence afin d'exclure une lésion cérébrale avec effet de masse qui contre-indiquerait la ponction lombaire. L'IRM cérébrale est moins accessible que le scanner, mais peut apporter des informations importantes en cas d'encéphalite aiguë.Méthode. – Revue de la littérature sur PubMed le 1 er avril 2015 avec les mots clés « IRM » et « encéphalite ».Résultats. – Nous décrivons les différentes anomalies IRM associées à chaque agent pathogène de l'encéphalite aiguë (HSV, VZV, autres virus ciblant les patients immunodéprimés ou voyageurs ; tuberculose, listériose, autres agents bactériens moins fréquents). Nous identifions des patterns spécifiques d'anomalies IRM qui peuvent orienter vers un pathogène particulier. L'encéphalite limbique est très évocatrice de l'HSV ; elle survient aussi moins fréquemment dans les encéphalites dues au HHV6, à la syphilis, à la maladie de Whipple et à la primo-infection par le VIH. Une rhombencéphalite doit fait rechercher la tuberculose et la listériose.     By consensus, the acyl-CoA synthetase (ACS) community, with the advice of the human and mouse genome nomenclature committees, has revised the nomenclature for the mammalian long-chain acyl-CoA synthetases. ACS is the family root name, and the human and mouse genes for the long-chain ACSs are termed ACSL1,3-6 and Acsl1,3-6, respectively. Splice variants of ACSL3, -4, -5, and -6 are cataloged. Suggestions for naming other family members and for the nonmammalian acyl-CoA synthetases are made

    Physical parameters of erupting Luminous Blue Variables: NGC 2363-V1 caught in the act

    A quantitative study of the Luminous Blue Variable NGC 2363-V1 in the Magellanic galaxy NGC 2366 (D = 3.44 Mpc) is presented, based on ultraviolet and optical HST/STIS spectroscopy. Contemporary WFPC2 and WHT imaging reveals a modest V-band brightness increase of ~ 0.2 mag per year between 1996 January and 1997 November, reaching V=17.4 mag, corresponding to Mv=-10.4 mag. Subsequently, V1 underwent a similar decrease in V-band brightness, together with a UV brightening of 0.35 mag from 1997 November to 1999 November. The optical spectrum of V1 is dominated by H emission lines, with Fe II, He I and Na I also detected. In the ultraviolet, a forest of Fe absorption features and numerous absorption lines typical of mid-B supergiants are observed. From a spectral analysis with the non-LTE, line-blanketed code of Hillier & Miller (1998), we derive stellar parameters of T*=11kK, R*=420Ro, log(L/Lo)=6.35 during 1997 November, and T*=13kK, R*=315Ro, log(L/Lo)=6.4 for 1999 July. The wind properties of V1 are also exceptional, with Mdot ~ 4.4 x 10e-4 Mo/yr and v v_{\infty} ~ 300 km/s, allowing for a clumped wind (filling factor = 0.3), and assuming H/He ~ 4 by number. The presence of Fe lines in the UV and optical spectrum of V1 permits an estimate of the heavy elemental abundance of NGC 2363 from our spectral synthesis. Although some deficiencies remain, allowance for charge exchange reactions in our calculations supports a SMC-like metallicity, that has previously been determined for NGC 2363 from nebular oxygen diagnostics. Considering a variety of possible progenitor stars, V1 has definitely undergone a giant eruption, with a substantial increase in stellar luminosity, radius, and almost certainly mass-loss rate.Comment: 29 pages, 10 figures, Submitted to Ap

    Development and Validation of Predictive Model for a Diagnosis of First Episode Psychosis Using the Multinational EU-GEI Case-control Study and Modern Statistical Learning Methods

    Background and Hypothesis: It is argued that availability of diagnostic models will facilitate a more rapid identification of individuals who are at a higher risk of first episode psychosis (FEP). Therefore, we developed, evaluated, and validated a diagnostic risk estimation model to classify individual with FEP and controls across six countries. Study Design: We used data from a large multi-center study encompassing 2627 phenotypically well-defined participants (aged 18-64 years) recruited from six countries spanning 17 research sites, as part of the European Network of National Schizophrenia Networks Studying Gene-Environment Interactions study. To build the diagnostic model and identify which of important factors for estimating an individual risk of FEP, we applied a binary logistic model with regularization by the least absolute shrinkage and selection operator. The model was validated employing the internal-external cross-validation approach. The model performance was assessed with the area under the receiver operating characteristic curve (AUROC), calibration, sensitivity, and specificity. Study Results: Having included preselected 22 predictor variables, the model was able to discriminate adults with FEP and controls with high accuracy across all six countries (rangesAUROC=0.84-0.86). Specificity (range=73.9-78.0%) and sensitivity (range=75.6-79.3%) were equally good, cumulatively indicating an excellent model accuracy; though, calibration slope for the diagnostic model showed a presence of some overfitting when applied specifically to participants from France, the UK, and The Netherlands. Conclusions: The new FEP model achieved a good discrimination and good calibration across six countries with different ethnic contributions supporting its robustness and good generalizability.</p
