    Psychotherapy Augmentation through Preconscious Priming.

    OBJECTIVE: To test the hypothesis that repeated preconscious (masked) priming of personalized positive cognitions could augment cognitive change and facilitate achievement of patients' goals following a therapy. METHODS: Twenty social phobic patients (13 women) completed a 36-weeks study beginning by 12 weeks of group behavioral therapy. After the therapy, they received 6 weeks of preconscious priming and 6 weeks of a control procedure in a randomized cross-over design. The Priming condition involved listening twice daily with a passive attitude to a recording of individualized formulations of appropriate cognitions and attitudes masked by music. The Control condition involved listening to an indistinguishable recording where the formulations had been replaced by random numbers. Changes in social cognitions were measured by the Social Interaction Self Statements Test (SISST). RESULTS: Patients improved following therapy. The Priming procedure was associated with increased positive cognitions and decreased negative cognitions on the SISST while the Control procedure was not. The Priming procedure induced more cognitive change when applied immediately after the group therapy. CONCLUSION: An effect of priming was observed on social phobia related cognitions in the expected direction. This self administered addition to a therapy could be seen as an augmentation strategy

    Impacts of Atmospheric Nitrogen Deposition on Surface Waters of the Western North Atlantic Mitigated by Multiple Feedbacks

    The impacts of atmospheric nitrogen deposition (AND) on the chlorophyll and nitrogen dynamics of surface waters in the western North Atlantic (25 degrees N-45 degrees N, 65 degrees W-80 degrees W) are examined with a biogeochemical ocean model forced with a regional atmospheric chemistry model (Community Multiscale Air Quality, CMAQ). CMAQ simulations with year-specific emissions reveal the existence of a hot spot of AND over the Gulf Stream. The impact of the hot spot on the oceanic biogeochemistry is mitigated in three ways by physical and biogeochemical processes. First, AND significantly contributes to surface oceanic nitrogen concentrations only during the summer period, when the stratification is maximal and the background nitrogen inventories are minimal. Second, the increase in summer surface nitrate concentrations is accompanied by a reduction in upward nitrate diffusion at the base of the surface layer. This negative feedback partly cancels the nitrogen enrichment from AND. Third, gains in biomass near the surface force a shoaling of the euphotic layer and a reduction of about 5% in deep primary production and biomass on the continental shelf. Despite these mitigating processes, the impacts of AND remain substantial. AND increases surface nitrate concentrations in the Gulf Stream region by 14% during the summer (2% on average over the year). New primary production increases by 22% in this region during summer (8% on average). Although these changes may be difficult to distinguish from natural variability in observations, the results support the view that AND significantly enhances local carbon export

    Biocurators and Biocuration: surveying the 21st century challenges

    Curated databases are an integral part of the tool set that researchers use on a daily basis for their work. For most users, however, how databases are maintained, and by whom, is rather obscure. The International Society for Biocuration (ISB) represents biocurators, software engineers, developers and researchers with an interest in biocuration. Its goals include fostering communication between biocurators, promoting and describing their work, and highlighting the added value of biocuration to the world. The ISB recently conducted a survey of biocurators to better understand their educational and scientific backgrounds, their motivations for choosing a curatorial job and their career goals. The results are reported here. From the responses received, it is evident that biocuration is performed by highly trained scientists and perceived to be a stimulating career, offering both intellectual challenges and the satisfaction of performing work essential to the modern scientific community. It is also apparent that the ISB has at least a dual role to play to facilitate biocurators’ work: (i) to promote biocuration as a career within the greater scientific community; (ii) to aid the development of resources for biomedical research through promotion of nomenclature and data-sharing standards that will allow interconnection of biological databases and better exploit the pivotal contributions that biocurators are making

    Glenohumeral joint capsular tissue tension loading correlates moderately with shear wave elastography: a cadaveric investigation

    Purpose The purpose of this study was to investigate changes in the mechanical properties of capsular tissue using shear wave elastography (SWE) and a durometer under various tensile loads, and to explore the reliability and correlation of SWE and durometer measurements to evaluate whether SWE technology could be used to assess tissue changes during capsule tensile loading. Methods The inferior glenohumeral joint capsule was harvested from 10 fresh human cadaveric specimens. Tensile loading was applied to the capsular tissue using 1-, 3-, 5-, and 8-kg weights. Blinded investigators measured tissue stiffness and hardness during loading using SWE and a durometer, respectively. Intraobserver reliability was established for SWE and durometer measurements using intraclass correlation coefficients (ICCs). The Pearson product-moment correlation was used to assess the associations between SWE and durometer measurements. Results The ICC3,5 for durometer measurements was 0.90 (95% confidence interval [CI], 0.79 to 0.96; P<0.001) and 0.95 (95% CI, 0.88 to 0.98; P<0.001) for SWE measurements. The Pearson correlation coefficient values for 1-, 3-, and 5-kg weights were 0.56 (P=0.095), 0.36 (P=0.313), and -0.56 (P=0.089), respectively. When the 1- and 3-kg weights were combined, the ICC3,5 was 0.72 (P<0.001), and it was 0.62 (P<0.001) when the 1-, 3-, and 5-kg weights were combined. The 8-kg measurements were severely limited due to SWE measurement saturation of the tissue samples. Conclusion This study suggests that SWE is reliable for measuring capsular tissue stiffness changes in vitro at lower loads (1 and 3 kg) and provides a baseline for the non-invasive evaluation of effects of joint loading and mobilization on capsular tissues in vivo

    Influence of standardized procedures on the reliability of hip clinical assessment

    Objective: This study evaluated a standardized and personalized approach to verify the effects of conditions on intrarater and interrater reliability, standard error of measurement, and minimal detectable difference for provocative tests and range-of-motion (ROM) tests used in hip pain assessment: flexion-adduction-internal rotation (FADIR), flexion-abduction-external rotation-extension (FABER), and hip internal rotation with 90° of hip flexion (hip IR). Methods: Nineteen participants (mean [± SD] age = 24 ± 2 years; 10 women and 9 men) without lower limb or back pain were recruited. Three raters evaluated each participant during 2 testing sessions, 1 day apart. Raters performed the 3 tests in 4 conditions: classic (C), controlled pressure duration (CPD), subject-specific position (SSP), and mixed (M = CPD + SSP). Results: For intrarater reliability, the CPD condition showed the highest intraclass correlation coefficients (ICCs; mean and 95% confidence interval [CI]) for hip IRROM (0.83; 95% CI, 0.53-0.94) and FADIRROM (0.75; 95% CI, 0.60-0.89). The SSP condition showed the highest ICCs for FABERheight (0.71; 95% CI, 0.42-0.87) and FABERROM (0.62; 95% CI, 0.27-0.83). Concerning interrater reliability, the classic condition presented the highest ICCs for FABER variables (height: 0.54; 95% CI, 0.28-0.76; ROM: 0.58; 95% CI, 0.32-0.79) and hip IR ROM (0.72; 95% CI, 0.51-0.87). The CPD condition showed the highest ICC for FADIRROM (0.57; 95% CI, 0.32-0.78). Conclusion: In the conditions of this study, CPD showed the highest ICCs for hip IRROM and FADIRROM, and SSP showed the highest ICCs for FABERheight and FABERROM. © 202

    Quantification and reliability of hip internal rotation and the FADIR test in supine position using a smartphone application in an asymptomatic population

    Objective: The purpose of this study was to quantify and report the intrarater and interrater reliability of hip internal rotation (IR) range of motion supine with the hip and knee at 90° of flexion and for the flexion-adduction-internal rotation (FADIR) test. Hip internal rotation measured in a lying supine position with the hip and knee at 90° of flexion revealed information on hip impairments. To date no simple quantification approach has been presented in this position; therefore, the FADIR test has not been quantified yet. Methods: Twenty participants (mean ± standard deviation [SD] age, 24.0 ± 2.1 years; 10 women and 10 men) without lower-limb or back pain were recruited. Three raters evaluated each participant during 2 testing sessions, 1 day apart. A built-in smartphone compass application was used to obtain the hip IR range of motion in both procedures. Results: Mean (± SD) supine IR was 51.7° (± 9.7°) and 62.6° (± 11.4°) for men and women, respectively. Concerning the FADIR test, mean values were 41.8° (± 9.64°) and 50.1° (± 8.0°) for men and women, respectively. The mean intrarater and interrater reliability coefficients were 0.80 and 0.72 for hip IR and 0.75 and 0.40 for the FADIR test. The standard error of the mean ranged from 4.8° to 8.3° (minimal detectable difference [MDD], 13.3° to 22.9°) for hip IR and from 4.6° to 10.3° (MDD, 12.8° to 28.6°) for the FADIR test. Conclusion: Overall, the smartphone compass application is adequate to quantify hip IR in a lying supine position. However, the poor to moderate interrater reliability in the FADIR test and the size of the MDD values suggest that the FADIR test should be standardized. © 202

    ‘I had to pop a wheelie and pay extra attention in order not to fall:’ embodied experiences of two wheelchair tennis athletes transgressing ableist and gendered norms in disability sport and university spaces

    When bodies move in certain contexts, it can mean very different things for different people. In our society, some bodies are more valued than others, and detrimentally, this can mean that certain types of bodies are ostracised and segregated to the outskirts of production economies and society. Dis/ability sport spaces, able-bodied sports spaces and able-bodied university spaces have been an under-researched area when considering how the body moves throughout these spaces for elite wheelchair athletes taking part in university courses. To learn more, this paper drew on feminist poststructuralism and new materialist perspectives and shared an insight into how two athletes with dis/abilities transgressed abled and gendered norms in different spaces and how they positioned themselves as athletic bodies and disabled bodies in these spaces. Employing a post-critical ethnographic design, we found that dependent on the space a dis/abled body is in constant flux as to when it feels marginalised and different (typically able-bodied spaces) and when it feels included, valued, and strong (typically dis/abled spaces). Significantly, the materiality of the institutional structures of universities, founded upon historic aesthetics of beauty dictated the physical spaces the athletes entered and created spaces of exclusion based on capitalist and ableist ideologies

    In Situ mechanical effects of a specific neurodynamic mobilization of the superficial fibular nerve: A cadaveric study

    Context: A specific neurodynamic mobilization for the superficial fibular nerve (SFN) has been suggested in the reference literature for manual therapists to evaluate nerve mechanosensitivity in patients. However, no biomechanical studies examined the ability of this technique to produce nerve strain. Therefore, mechanical specificity of this technique is not yet established. Objective: The aim of our study was to test whether this examination and treatment technique was producing nerve strain in the fresh frozen cadaver and the contribution of each motion to total longitudinal strain. Design: Quantitative original research, controlled laboratory study Methods: A differential variable reluctance transducer was inserted in ten SFN from six fresh cadavers to measure strain during the mobilization. A specific sequence of plantar flexion (PF), ankle inversion (INV), straight leg raise (SLR) position and 30{degree sign} of hip adduction (ADD) was applied to the lower limb. The mobilization was repeated at 0°, 30°, 60° and 90° of Straight Leg Raise (SLR) position to measure the impact of hip flexion position. Findings: Compared to a resting position, this neurodynamic mobilization produced a significant amount of strain in the SFN (7.93% ± 0.51 P &lt; 0.001). PF (59.34% ± 25.82) and INV (32.80% ± 21.41) accounted for the biggest proportion of total strain during the mobilization. No significant difference was reported between different hip flexion positions. Hip ADD did not significantly contribute to final strain (0.39% ± 10.42 P&gt; 0,05) although high subject variability exists. Conclusion: Ankle motions should be considered the most important during neurodynamic assessment of the SFN for distal entrapment. These results suggest that this technique produces sufficient strain in the SFN and could therefore be evaluated In Vivo for correlation with mechanosensitivit
