    Inadequate chemoprophylaxis and the risk of malaria

    BACKGROUND: Malaria is an important disease for Australian travellers, particularly to Papua New Guinea. Travellers often seek health advice from their general practitioner before travel or if they develop illness after travel. METHOD: A retrospective cohort investigation into malaria risk in a group of adult Australians that trekked the Kokoda trail in Papua New Guinea. RESULTS: Six of 38 group members were diagnosed with malaria on return from Papua New Guinea. None of the 12 individuals who took chemoprophylaxis for the recommended period post-travel developed malaria compared to 4/24 travellers who terminated prophylaxis prematurely or 2/2 who took no chemoprophylaxis. DISCUSSION: Chemoprophylaxis is effective if taken for the full recommended period following travel to a malaria endemic area; 4 weeks for doxycycline and mefloquine, and 7 days for atovaquone+proguanil. Malaria is a likely cause of illness in recently returned travellers from Papua New Guinea who develop a febrile illness

    What parents in Australia know and do about head lice

    INTRODUCTION: Although parents in developed market economies regard head lice infections (pediculosis) as a significant problem, health departments generally rate pediculosis as a low priority health issue, encouraging parents to manage and control it. But how well equipped and willing are parents to manage the infections? There do not appear to be any studies in the literature addressing these issues. This article presents the results of a survey conducted in Australia that aimed to answer these questions. METHODS: A cross-sectional survey of parents of primary school aged children in Victoria (Vic) and north Queensland (NQ) was conducted using a self-administered questionnaire. The study investigated the knowledge, attitudes and practices of parents regarding head lice infections. RESULTS: Only 7.1% of 1338 who completed the questionnaire answered all 10 knowledge questions correctly and more than one-third failed to answer half correctly. There was a weak negative correlation between parents\u27 knowledge and the prevalence of active pediculosis in the school. Almost all parents wanted the responsibility for treating pediculosis and more than three-quarters saw it as a health concern. A higher proportion of parents in NQ used preventative strategies (67% vs 41%). Most parents spent less than AU$50 per year on treatments. Alarmingly, however, the proportion of children missing school as a result of pediculosis was 24.4% and 30.3% in Vic and NQ, respectively. In Vic there was a positive correlation (r = 0.39) between missing school in the previous 12 months and prevalence of pediculosis in the school. CONCLUSIONS: This appears to be the most comprehensive study of parental knowledge, beliefs, and practices regarding head lice infections. Although parents wanted responsibility for the management of pediculosis, deficiencies in their knowledge indicate they may be inadequately equipped to do so. Given the high proportion of children in both states who have missed school as a result of head lice, it is recommended that health departments in Australia should work to ensure that consistent and accurate messages about pediculosis are disseminated, and that relevant legislation is amended to prevent children being excluded from school

    The use of hospital-based nurses for the surveillance of potential disease outbreaks

    Objective: To study a novel surveillance system introduced in Mpumalanga Province, a rural area in the north-east of South Africa, in an attempt to address deficiences in the system of notification for infectious conditions that have the potential for causing outbreaks. Methods: Hospital-based infection control nurses in all of Mpumalanga's 32 public and private hospitals were trained to recognize, report, and respond to nine clinical syndromes that require immediate action. Sustainability of the system was assured through a schedule of regular training and networking, and by providing feedback to the nurses. The system was evaluated by formal review of hospital records, evidence of the effective containment of a cholera outbreak, and assessment of the speed and appropriateness of responses to other syndromes. Findings: Rapid detection, reporting and response to six imported cholera cases resulted in effective containment, with only 19 proven secondary cholera cases, during the two-year review period. No secondary cases followed detection and prompt response to 14 patients with meningococcal disease. By the end of the first year of implementation, all facilities were providing weekly zero-reports on the nine syndromes before the designated time. Formal hospital record review for cases of acute flaccid paralysis endorsed the value of the system. Conclusion: The primary goal of an outbreak surveillance system is to ensure timely recognition of syndromes requiring an immediate response. Infection control nurses in Mpumalanga hospitals have excelled in timely weekly zero-reporting, participation at monthly training and feedback sessions, detection of priority clinical syndromes, and prompt appropriate response. This review provides support for the role of hospital-based nurses as valuable sentinel surveillance agents providing timely data for action

    Effectiveness of Lifestyle Measures in the Treatment of Gastroesophageal Reflux Disease — A Case Series

    Aim: To assess the effectiveness of lifestyle measures in the treatment of gastroesophageal reflux disease (GERD) among adults attending a dietetic practice.\ud \ud Methods: A retrospective case series of adult patients presenting with GERD to a dietetic practice over a three year period. The routine lifestyle counselling for treatment of symptoms of GERD included: not reclining within two to three hours of eating; a diet low in fat; small frequent meals; avoiding dietary components considered to relax the lower esophageal sphincter; and avoiding local irritants.\ud \ud Results: Twenty three cases were included (12 male). Eighteen, (9 female) were referred by their doctor, 7 (6 female) presented for GERD alone, 7 (4 female) presented for GERD together with comorbidities, and 9 (1 female) incidentally mentioned GERD during a dietary consultation for another disorder. Thirteen participants (9 female) had previously undergone endoscopies, 18 (11 female) were taking medication for GERD, and 19 (7 female) had comorbidities. Twenty two (10 female) reported an improvement in symptoms with 11/18 taking GERD medication at presentation reducing their medication following treatment.\ud \ud Conclusions: These results suggest that a more thorough investigation of lifestyle modification in the treatment of GERD is warranted

    [Extract] The aim of this tender was to design and trial a pilot survey to map the distribution of chytridiomycosis in Australian frogs. The project has 5 components: Scope Items 1 and 3 being desk top synthesis and evaluation activities, Scope Item 2 being a data gathering and proof of concept activity, Scope Item 4 being a field and laboratory based project testing the protocol developed in Scope Item 3, and Scope Item 5 assessing and evaluating the previous outputs and providing recommendations that need to be addressed to allow a national survey for chytridiomycosis to proceed in an effective manner. Diagnostic Tests: An analysis of published and unpublished literature identified that chytridiomycosis could be diagnosed by at least 11 tests, that can be collapsed into 5 categories; culture, microscopical examination of unstained epidermis, histology, capture ELISA and PCR. Although each diagnostic test has detection of the amphibian chytrid fungus, Batrachochytrium dendrobatidis (Bd), as its final outcome and could on superficial assessment appear to be competing with each other, we discovered that this was true only in part. The tests have different functions and complement, rather than exclude, one another. For example, examination of fresh tissue is an essential test when culture of B.dendrobatidis from infected skin is being attempted in the laboratory. Histology is the diagnostic test of choice for retrospective surveys of archived specimens, and the sensitivity of diagnosis is increased by immunohistochemistry using polyclonal antibodies. For the proposed mapping survey, real-time PCR is the recommended diagnostic technique owing to its increased sensitivity and non-invasiveness

    Endemic Infection of the Amphibian Chytrid Fungus in a Frog Community Post-Decline

    The chytrid fungus Batrachochytrium dendrobatidis has been implicated in the decline and extinction of numerous frog species worldwide. In Queensland, Australia, it has been proposed as the cause of the decline or apparent extinction of at least 14 high-elevation rainforest frog species. One of these, Taudactylus eungellensis, disappeared from rainforest streams in Eungella National Park in 1985–1986, but a few remnant populations were subsequently discovered. Here, we report the analysis of B. dendrobatidis infections in toe tips of T. eungellensis and sympatric species collected in a mark-recapture study between 1994 and 1998. This longitudinal study of the fungus in individually marked frogs sheds new light on the effect of this threatening infectious process in field, as distinct from laboratory, conditions. We found a seasonal peak of infection in the cooler months, with no evidence of interannual variation. The overall prevalence of infection was 18% in T. eungellensis and 28% in Litoria wilcoxii/jungguy, a sympatric frog that appeared not to decline in 1985–1986. No infection was found in any of the other sympatric species. Most importantly, we found no consistent evidence of lower survival in T. eungellensis that were infected at the time of first capture, compared with uninfected individuals. These results refute the hypothesis that remnant populations of T. eungellensis recovered after a B. dendrobatidis epidemic because the pathogen had disappeared. They show that populations of T. eungellensis now persist with stable, endemic infections of B. dendrobatidis

    Prevalence of soil transmitted nematodes on Nukufetau, a remote Pacific island in Tuvalu

    BACKGROUND: The population of Nukufetau, a remote coral atoll island in Tuvalu in the Western Pacific, received annual mass drug administration (MDA) of diethylcarbamazine and albendazole under the Pacific Elimination of Lymphatic Filariasis program in 2001, 2002 and 2003, with the last MDA occurring six months before a cross-sectional survey of the whole population for soil transmitted helminths (STH). METHODS: A cross-sectional survey in May 2004 recruited 206 residents (35.2% of the population) who provided a single faecal sample that was preserved, concentrated and examined microscopically. RESULTS: Overall prevalence of STH was 69.9%; only hookworm and Trichuris trichiura were diagnosed. Trichuris was present in 68.4% with intensity of infection being light in 56.3%, medium in 11.7% and heavy in 0.5%. Hookworm occurred in 11.7% with intensity of infection 11.2% being light and medium in 0.5%. Twenty individuals (9.7%) had dual infections. The prevalence of Trichuris was constant across all ages while the prevalence of hookworm was significantly lower in residents below 30 years of age. In the age group 5–12 years comparison of results with a 2001 survey [1] suggested that the prevalence of STH has declined minimally, due to sustained high prevalence of Trichuris, while hookworm has declined dramatically from 34.4% to 1.6%. CONCLUSION: The results of this survey suggest that although the MDA appears to have reduced hookworm prevalence in residents below 30 years of age, there has been minimal effect on Trichuris prevalence. An integrated program to control STH is required
