    Plasmonic optical fiber meta-tip for cancer biomarkers detection

    Differential diagnosis between Marfan syndrome and Loeys–Dietz syndrome type 4: A novel chromosomal deletion covering tgfb2

    Marfan syndrome (MFS) and Loeys–Dietz syndrome type 4 (LDS4) are two hereditary connective tissue disorders. MFS displays ectopia lentis as a distinguishing, characterising feature, and thoracic aortic ectasia, aneurysm, dissection, and systemic features as manifestations overlapping with LDS4. LDS4 is characterised by the presence of hypertelorism, cleft palate and/or bifid uvula, with possible ectasia or aneurysms in other arteries. The variable age of onset of clinical manifestations makes clinical diagnosis more difficult. In this study, we report the case of a patient with Marfan syndrome diagnosed at our centre at the age of 33 on the basis of typical clinical manifestations of this syndrome. At the age of 38, the appearance of ectasia of the left common iliac artery and tortuosity of the iliac arteries suggested the presence of LDS4. Next Generation Sequencing (NGS) analysis, followed by Array-CGH, allowed the detection of a novel chromosomal deletion including the entire TGFB2 gene, confirming not only the clinical suspicion of LDS4, but also the clinical phenotype associated with the haploinsufficiency mechanism, which is, in turn, associated with the deletion of the entire gene. The same mutation was detected in the two young sons. This emblematic case confirms that we must be very careful in the differential diagnosis of these two pathologies, especially before the age of 40, and that, in young subjects suspected to be affected by MFS in particular, we must verify the diagnosis, extending genetic analysis, when necessary, to the search for chromosomal alterations. Recently, ectopia lentis has been reported in a patient with LDS4, confirming the tight overlap between the two syndromes. An accurate revision of the clinical parameters both characterising and overlapping the two pathologies is highly desirable

    A study of the prevalence and genotypes of Giardia duodenalis infecting kennelled dogs

    Giardia duodenalis is a protozoan parasite of animals that is zoonotic. Given the capacity of this organism to spread via the faecal–oral route, animals held in overcrowded and unhygienic conditions are at high risk of infection. Faecal samples from dogs in three kennels in Rome were examined by microscopy and PCR for G. duodenalis, and the prevalence data generated were correlated with variables such as kennel identity, age of dog, length of time the dog had been kennelled and clinical signs. The overall prevalence of the parasite in the faecal samples was 20.5% and was higher in samples from the largest kennel, which had the greatest turnover of dogs, and in faecal samples from younger animals. Giardia cysts were found more frequently in diarrhoeic animals but were also found in dogs with no clinical signs. Although the finding that the majority of isolates were dog-specific rather than zoonotic genotypes suggests that the zoonotic risk from this pathogen is less than previously thought, the higher prevalence of infection in younger dogs may pose a specific public health issue as such animals are more frequently re-homed with families. 2008 Elsevier Ltd.     Investigation of a VLSI neural network chip as part of a secondary vertex trigger

    Abstract An analog VLSI neural network chip (ETANN) has been trained to detect secondary vertices in simulated data for a fixed target heavy flavour production experiment. The detector response and associative memory track finding were modelled by a simulation, but the vertex detection was performed in hardware by the neural network chip and requires only a few microseconds per event. The chip correctly tags 30% of the heavy flavour events while rejecting 99% of the background, and is thus well adapted for secondary vertex triggering applications. A general purpose VME module for interfacing the ETANN to experiments, equipped with ADC/DAC circuits and a 68070 CPU, is also presented

    Seasonally Forced SIR Systems Applied to Respiratory Infectious Diseases, Bifurcations, and Chaos

    We investigate models to describe respiratory diseases with fast mutating virus pathogens such that after some years the aquired resistance is lost and hosts can be infected with new variants of the pathogen. Such models were initially suggested for respiartory diseases like influenza, showing complex dynamics in reasonable parameter regions when comparing to historic empirical influenza like illness data, e.g., from Ille de France. The seasonal forcing typical for respiratory diseases gives rise to the different rich dynamical scenarios with even small parameter changes. Especially the seasonality of the infection leads for small values already to period doubling bifurcations into chaos, besides additional coexisting attractors. Such models could in the future also play a role in understanding the presently experienced COVID-19 pandemic, under emerging new variants and with only limited vaccine efficacies against newly upcoming variants. From first period doubling bifurcations, we can eventually infer at which close by parameter regions complex dynamics including deterministic chaos can arise.Marie Skłodowska-Curie grant agreement No. 79249

    A possible cause of misdiagnosis in tumors of the axilla: schwannoma of the brachial plexus

    The Authors report a rare case of a 57 years old man affected by a left radial nerve schwannoma that occurred as an asymptomatic lesion of the axilla. At clinical examination the lump was undistinguishable from the most common axillary lymphadenopathy. A lymphoadenopathy was erroneously diagnosed with ultrasonography (US). This mistake was due to the low specificity of the instrumental methodology and to the rarity of an asymptomatic schwannoma of the infraclavicular brachial plexus. The neoplasia was excised without using the microscope. In the early post-operative follow up, a “falling” attitude of the wirst, the hand and the fingers appeared, peculiar for a lesion of the radial nerve. Furthermore a hypoaesthesia of the skin of first finger and of the first interosseus space was associated. The sensitive and motor electromyography showed a radial nerve suffering. The “stupor” of the nerve trunk was treated with steroid therapy for 7 days and the patient underwent to some series of neuro-rehabilitative physical therapy for 12 weeks. The postoperative total body CT, showed that the lesion was unique: therefore it was possible to exclude the diagnosis of neurofibromatosis. After 28 months electromyography and axillary US were performed showing the complete resolution of the motor and sensitive deficit and the absence of local recurrence

    [Early laparoscopic cholecystectomy in the treatment of acute cholecystitis].

    BACKGROUND: The aim of the study was to demonstrate the importance of early laparoscopic cholecystectomy for acute cholecystitis. METHODS: From 1998 to 2000, 66 patients were submitted to laparoscopic cholecystectomy. All patients were submitted to US scans preoperatively and operated on by surgeon skilled in emergency laparoscopic operative technique. RESULTS: Only one patient (1.5%) had conversion to open cholecystectomy. There was no mortality and no bile duct or major vascular injuries. The overall operative morbidity rate was 3%. The mean postoperative hospital stay was 3.1 days. CONCLUSIONS: Author's experience and results support the validity of early laparoscopic cholecystectomy in the treatment of acute cholecystitis, since it reduces the postoperative length of hospital stay and hospital costs. Early treatment is always helpful for inflamed and oedematous tissue which favours dissection

    Bidirectional lipid droplet velocities are controlled by differential binding strengths of HCV Core DII protein

    Host cell lipid droplets (LD) are essential in the hepatitis C virus (HCV) life cycle and are targeted by the viral capsid core protein. Core-coated LDs accumulate in the perinuclear region and facilitate viral particle assembly, but it is unclear how mobility of these LDs is directed by core. Herein we used two-photon fluorescence, differential interference contrast imaging, and coherent anti-Stokes Raman scattering microscopies, to reveal novel core-mediated changes to LD dynamics. Expression of core protein’s lipid binding domain II (DII-core) induced slower LD speeds, but did not affect directionality of movement on microtubules. Modulating the LD binding strength of DII-core further impacted LD mobility, revealing the temporal effects of LD-bound DII-core. These results for DII-core coated LDs support a model for core-mediated LD localization that involves core slowing down the rate of movement of LDs until localization at the perinuclear region is accomplished where LD movement ceases. The guided localization of LDs by HCV core protein not only is essential to the viral life cycle but also poses an interesting target for the development of antiviral strategies against HCV

    Surgical site infection after caesarean section. Space for post-discharge surveillance improvements and reliable comparisons

    Surgical site infections (SSI) after caesarean section (CS) represent a substantial health system concern. Surveying SSI has been associated with a reduction in SSI incidence. We report the findings of three (2008, 2011 and 2013) regional active SSI surveillances after CS in community hospital of the Latium region determining the incidence of SSI. Each CS was surveyed for SSI occurrence by trained staff up to 30 post-operative days, and association of SSI with relevant characteristics was assessed using binomial logistic regression. A total of 3,685 CS were included in the study. A complete 30 day post-operation follow-up was achieved in over 94% of procedures. Overall 145 SSI were observed (3.9% cumulative incidence) of which 131 (90.3%) were superficial and 14 (9.7%) complex (deep or organ/space) SSI; overall 129 SSI (of which 89.9% superficial) were diagnosed post-discharge. Only higher NNIS score was significantly associated with SSI occurrence in the regression analysis. Our work provides the first regional data on CS-associated SSI incidence, highlighting the need for a post-discharge surveillance which should assure 30 days post-operation to not miss data on complex SSI, as well as being less labour intensive
