    Impact of Routine Use of Drug-eluting Stents in Contemporary Interventional Cardiology at a Tertiary Center: One-decade Experience of the DESIRE Registry

    ABSTRACTBackgroundDrug-eluting stents (DES) have changed contemporary interventional cardiology practice, enabling the approach of increasingly more complex clinical and angiographic scenarios. The objective of this study was to demonstrate the changes observed in the last 10 years in the indication and practice of percutaneous coronary intervention (PCI) at a tertiary private hospital in the State of São Paulo.MethodsDESIRE is a single-center prospective registry aiming at following the acute and late outcomes of consecutive patients treated by DES.ResultsFrom 2002 to 2011, 4,299 patients were included, with mean age of 64.3±11.2years, 23% were female and 30.5 were diabetic. The total number of lesions treated was 6,518 of which 61.5% were type B2/C. During the course of the study, DES were progressively more used, reaching a penetration of 88.4% in 2011. The complexity of PCIs has increased and in the past year 1.76 lesions per patient were treated with an average of 1.89 DES. The SYNTAX score increased from 12.3±4.4 (2002-2006) to 15.7±4.7 (2007-2011). Clinical follow-up was obtained in 98.2% of the patients, with a median of 5.2years, and during this period target-lesion revascularization rate was 5%, myocardial infarction was 6.7% and cardiovascular death was 4.1%. Stent thrombosis was observed in 2.4% of the cases.ConclusionsOur results showed a marked increment in the complexity profile of patients treated in the last 10 years and at the same time confirm the long-term effectiveness of DES, despite the clinical and angiographic profile of patients

    An unusual cause of myocardial infarction

    Copyright © 2019 European Society of CardiologyBackground: In order to direct the treatment it, is well established that is fundamental to clarify the aetiology of heart failure and the cause of myocardial infarction (MI) with non obstructive coronary artery disease (MINOCA), with CMR being one of the methods of choice in both clinical situations. Case report: A 70 years-old male patient was admitted in our emergency department with complaints of irregular palpitations, progressive dyspnoea and fatigue on exertion, with two weeks of evolution; these symptoms were associated to retrosternal chest pain in the last twelve hours. He had a previous medical history of dyslipidemia, no other cardiovascular risk factors were known. On admission, his heart rate was 130bpm, with an irregularly irregular pulse and the pulmonary auscultation revealed bibasal crackles. The remaining physical examination was unremarkable. The ECG showed an atrial fibrillation rhythm, with a mild ST elevation and T wave inversion in inferior leads. The echocardiogram revealed a diffuse hypokinesia of left ventricle with an ejection fraction of 35-40%. The lab tests documented an elevation of troponin (hs-TnT 210ng/L) and NTproBNP (1945pg/ml).     Isolation of novel microsatellite markers for tambaqui (Colossoma macropomum, Cuvier 1818), an important freshwater fish of the Amazon.

    Colossoma macropomum is an endemic species from Amazon basin. It is widely commercialized as food, becoming one of the main items in Amazonian fisheries. Despite its economic importance, genetic information of small captivity populations is not currently available. The present contribution describes 14 new microsatellite loci used to analyze 30 individuals of C. macropomum. The number of alleles for each locus ranged from 4 to 24. The observed (Ho) and expected (HE) heterozygosity values ranged from 0.318 to 1.000 and 0.729 to 0.949, respectively. Out of 14 polymorphic loci, nine did not deviate from Hardy-Weinberg Equilibrium after Bonferroni correction. These new microsatellite loci will contribute towards the genetic of small artificial populations, as well as pedigree control of fish farms of C. macropomum

    High performance free-standing films by layer-by-layer assembly of graphene flakes and ribbons with natural polymers

    In this work, novel free-standing (FS) films based on chitosan, alginate and graphene oxide (GO) were developed through layer-by-layer assembly. First, GO was synthesized from graphite and multi-walled carbon nanotubes using a modified Hummer's method, yielding oxidized graphene flakes (o-GFs) and oxidized graphene nanoribbons (o-GNRs), respectively, which were then characterized. Then FS films were produced and their morphological, thermal and mechanical properties, as well as the o-GF and o-GNR dispersion along the films were assessed. Their degradation and swelling profiles as well as their biological behavior were evaluated. Graphite and nanotubes were successfully oxidized and exfoliated forming stable suspensions that could be combined with chitosan (CHI) and alginate (ALG) solutions by layer-by-layer processing. The addition of o-GFs and o-GNRs resulted in rougher, hydrophilic FS films with significantly improved mechanical properties relative to CHI/ALG films. The presence of o-GFs or o-GNRs did not affect the thermal stability and the addition of o-GFs resulted in films with enhanced cytocompatibility.     Seismic vulnerability assessment and characterisation of the buildings on Faial Island, Azores

    The earthquake of the 9th of July 1998 that hit in the central group of the Azores archipelago greatly affected the islands of Faial, Pico and SĂŁo Jorge, reaching a magnitude of Mw 6.2 with the epicentre located about 15km northeast of the Faial Island. This earthquake allowed the collection of an unprecedented quantity of data concerning the characterisation of the building stock and the damage suffered by construction. This is the main purpose of this research, consisting essentially of three main aspects: (i) A detailed characterisation of the building stock, assigning a five category classification, from old traditional rubble stone masonry to reinforced concrete moment framed buildings; (ii) A detailed damage grade classification based on the different damage mechanisms observed; and, (iii) A seismic vulnerability assessment of the building stock. The results of the vulnerability assessment together with the building stock database and damage classification were integrated into a GIS tool, allowing the spatial visualation of damage scenarios, which is potentially useful for the planning of emergency response strategies and retrofitting priorities to mitigate and manage seismic risk

    Polyomavirus Nephropathy: Ten-Year Experience

    BACKGROUND: Polyomavirus nephropathy (BKVN) is an important cause of chronic allograft dysfunction (CAD). Recipient determinants (male sex, white race, and older age), deceased donation, high-dose immunosuppression, diabetes, delayed graft function (DGF), cytomegalovirus infection, and acute rejection (AR) are risk factors. Reducing immunosuppression is the best strategy in BKVN. The objective of our study was to evaluate CAD progression after therapeutic strategies in BKVN and risk factors for graft loss (GL). METHODS: Retrospective analysis of 23 biopsies, from patients with CAD and histological evidence of BKVN, conducted over a period of 10 years. Glomerular filtration rate was <30 mL/min in 16 patients at the time of the BKVN diagnosis. RESULTS: BKVN was histologically diagnosed in 23 recipients (19 men, 4 women). All patients were white, with age of 51.2 ± 12.1 years (6 patients, age >60 years), and 22 had a deceased donor. Diabetes affected 4 patients, DGF occurred in 3, cytomegalovirus infection in 2, and AR in 15. All patients were medicated with calcineurin inhibitors (CNI) (95.7% tacrolimus) and corticoids, and 16 also received an antimetabolite. One year after antimetabolite reduction/discontinuation and/or CNI reduction/switching and/or antiviral agents, graft function was decreased in 11 patients, increased/stabilized in 10, and unknown in 2. GL occurred in 9 patients. Older age (hazard ratio, 1.76; 95% confidence interval, 0.94-3.28) and DGF (hazard ratio, 2.60; 95% confidence interval, 0.54-12.64) were the main risk factors for GL. The lower GFR at the time of the BKVN diagnosis was associated with an increased risk of initiation of dialysis. CONCLUSIONS: GL occurred in 39.1% of patients with BKVN and DGF; older age and lower GFR at the time of diagnosis were important risk factors.     We report the case of a 66-year-old white man with a 6-month history of a blue-gray plaque of the glans. The patient was heterosexual, with 2 sexual partners in the last 6 months, and denied condom use. There was no personal or family history of melanoma. Physical examination of the uncircumcised penis found a round, well-demarcated, smooth, blue-gray plaque measuring 8 7 10 mm, arising within an erythematous background plaque of the glans (Fig 1).     Flipping the Classroom: Reflections on the flipped classroom and teaching practice in Philosophy in the Higher Education

    The new Coronavirus Pandemic has revealed the need for a methodological redefinition of teaching and learning attitude of teachers and students, through the pedagogical use of digital technologies. Therefore, this article aims to present reflections on the use of the Flipped Classroom methodology as a re-signifying possibility for the teaching of Philosophy in Higher Education, considering the context of remote emergency teaching. Assuming that there is a concern with the didactic-pedagogical and sociopolitical process of teaching and learning that constitutes teaching practice in the teaching of Philosophy in Brazilian Academies, the question is: what are the advantages and challenges of using the flipped classroom methodology in the process of teaching and learning Philosophy in Higher Education? For this, two fundamental aspects of the formal educational process are discussed: the first refers to the didactic-pedagogical and sociopolitical aspects of the teaching and learning process that constitute the teaching practice, as the content and didactics are emphasized. that is, the “what” and the “how to teach” and the second, it is about the need for re-signification/updating of traditional teaching in face of active methodologies. Based on the current context of remote teaching and/or hybrid teaching in Higher Education in Brazil, it is concluded that is necessary to reframe pedagogical practice in terms of the production of academic knowledge, without losing sight of the conditions in which it is produced
