Impact of defocus in the measurement of light distortion: simulation and experimental measurements
Dissertação de mestrado em Optometria AvançadaCurrently it has been given more clinical relevance to visual disturbances that affect the
performance of people in everyday activities, especially in low lighting conditions. In fact, vision quality
is related with the resulting intraocular straylight from light scattering on optical means, the intrinsic
aberrations of each subject and un-corrected refractive errors. All this can degrade the vision quality
by perceiving dysphotopsias around light sources. There are several photic phenomena (like halos and
starburst) associated to light distortion limiting the visual performance, especially in low luminance
conditions, such as night driving. The association between intraocular light scattering and high-order
aberration with visual quality and performance is well known and it is already taken into account in
clinical practice for pathological situations, surgical and other visual treatments. It is known how
defocus can influence the visual performance related to visual acuity, contrast sensitivity, reading and
shapes and patterns recognition. However the increase on light distortion with defocus and its
differences between myopes and hyperopes has not been yet studied. Therefore, it is relevant to study
the impact of defocus in light distortion measures and how it is perceived, and any additional factors
that may influence the visual quality along with the blur caused by non-corrected refractive errors as
well, namely the action of accommodation, corneal surface regularity and symmetry and pupil size.
In this cross-sectional study involving 20 healthy subjects the size and irregularity of light
distortion surrounding a bright source of light against a dark background were evaluated with the Light
Distortion Analyzer (LDA) under low luminance levels with positive and negative induced defocus of
1.00D, before and after the instillation of a cycloplegic drug. From this study we observed that there
was a relevant increase on light distortion with both types of defocus (positive and negative) and both
types of induced defocus produced similar values of light distortion in size and irregularity. The
spherical-like HOA was the ocular aberration that most influenced light distortion measures, mainly its
size, but the pupil by itself did not show to have a significant influence. Besides, the topographic
indexes SRI and SAI showed to be predictors of visual quality by being positively related with light
distortion size. The same it was not verified for the simulated PSF with the psychophysical measure
from LDA in both positive and negative induced defocus.
Given the results, we verified that despite the psychophysical capacity of the binocular system
to attenuate the monocular effects, the importance and need to correct low refractive errors should
not be dismissed, not only for a matter of improvement of visual acuity but also visual quality under
dim light conditions.Atualmente a importância clínica dos distúrbios visuais que afetam a performance das
pessoas em atividades do quotidiano, nomeadamente em condições de baixa iluminação, tem
merecido relevo pela comunidade científica. De facto, a qualidade visual está relacionada com a
dispersão da luz nos meios óticos, as aberrações oculares intrínsecas de cada sujeito e de erros
refrativos não corrigidos. Tudo isto pode degradar a qualidade visual através da perceção distorcida
de fenómenos luminosos (como halos e estrelas à volta das luzes) que limitam a performance visual
dos sujeitos, nomeadamente em condições de baixa iluminação, como é o caso da condução
noturna. A relação das aberrações oculares e da dispersão da luz intraocular com a qualidade e
performance visual é já bem conhecida e tida em conta na prática clínica em situações patológicas,
cirúrgicas e outros tratamentos. Sabe-se de que forma o desfocado pode influenciar a performance
visual em termos de acuidade visual, sensibilidade ao contraste, velocidade de leitura,
reconhecimento de formas e padrões. No entanto, não se conhece de que forma o desfocado
aumenta a distorção luminosa e se difere ou não entre míopes e hipermétropes. Assim, torna-se
relevante conhecer de que forma o desfocado tem impacto em medidas de distorção luminosa e
como é percebida, bem como quais os fatores adicionais que poderão influenciar a qualidade visual
juntamente com o desfocado provocado pela não correção de erros refrativos.
Neste estudo transversal utilizou-se o Light Distortion Analyzer (LDA) para medidas do
tamanho e irregularidade da distorção luminosa em situações de baixa iluminação aquando da
simulação de desfocado positivo e negativo de 1,00D, após a atuação de um fármaco cicloplégico.
Este estudo mostrou haver um aumento relevante da distorção luminosa perante a presença dos dois
tipos de desfocado, sendo que ambos desfocados hipermetrópico e miópico produziram valores
semelhantes de distorção, tanto em tamanho como irregularidade. A aberração esférica de alta
ordem foi a aberração que mais influenciou as medidas de distorção, mas a pupila por si só não
mostrou ter influência. Além disso, os índices topográficos SRI e SAI mostraram ser preditores de
qualidade ótica ao relacionarem-se positivamente com o tamanho da distorção. O mesmo não se
verificou entre a simulação da PSF e a medida psicofísica do LDA para ambos os desfocados
positivos e negativo.
Face aos resultados, verificamos que apesar da capacidade psicofísica do sistema binocular
de atenuar os efeitos monoculares, não se deve descartar a importância e a necessidade de corrigir
os baixos erros refrativos, não só por uma questão de melhoria da acuidade visual mas também de
qualidade visual
DIFUSÃO E APROPRIAÇÃO DO PARADIGMA BIM NO BRASIL
Este trabalho apresenta resultados parciais de uma pesquisa que vem sendo desenvolvida no (texto removido para avaliação cega) e que objetiva acompanhar a difusão e apropriação do paradigma BIM – Building Information Modeling no Brasil. Ele apresenta e discute os principais resultados da tabulação de um questionário aplicado durante o (texto removido para avaliação cega – nome do evento, data e local). Neste questionário, procurou-se conhecer os graus de familiaridade e de utilização do paradigma BIM entre os participantes do evento, além de suas opiniões sobre diferentes questões, como vantagens e desvantagens observadas no uso das ferramentas BIM, facilidades e dificuldades que este novo paradigma traz para os atores envolvidos no setor da construção civil, dentre outros. As análises foram realizadas em duas etapas: a primeira, composta de uma amostra de 186 respostas, identificou o perfil do participante do encontro e o que ele entende por BIM. A segunda parte das análises trabalhou questões que objetivavam conhecer as experiências e opiniões de usuários efetivos do paradigma BIM, sendo constituída por uma amostra de 48 entrevistados. Ao final, sinaliza questões relevantes apontadas pelos dados coletados, inferindo a necessidade de ações que devem ser cumpridas pelos diversos seguimentos do setor da AEC, para que o paradigma BIM se consolide no Brasil

The Concept of Experience Applied to Tourist Products Commercialized in São Luis (Maranhão, Brazil)
Analysis of the importance of applying the concept of experience in tourism products and services as a way to add value to tourism. Its objective is to verify if the dimensions of the tourist experience are applied to tourism products and services marketed, in the city of São Luís, Maranhão, with the object of analysis being the Historical Soiree Project of the Municipal Tourism Secretariat of São Luís, Maranhão, Brazil. Strategy of promotion of tourism in the region based on playful work and interaction with the viewer. The research was characterized as exploratory-descriptive, carried out by means of bibliographic and documentary research and field research, having as a data collection instrument interviews with tourists and visitors from January to February 2019. It discusses the concept of experience in its multiple meanings and how tourist experiences can provide memorable moments for tourists. It highlights the concept of experience tourism or experiential tourism when listed in tourism products and services. It characterizes the Historical Soiree, showing how this action is performed to provide interaction with visitors, aligning the discussions of the dimensions of the experience with the reports of the participants

Mathematical estimation of axial length increment in the control of myopia progression
This study aims to evaluate the existing mathematical approach for the theoretical estimation of axial length (AL) in a cross-sectional study, developing a new mathematical model and testing it in a longitudinal sample. Many professionals do not have a device to measure the AL due to clinic space and cost of equipment. However, this parameter plays an important role in the assessment of myopia progression to monitor treatment effects with myopia control strategies. First, a cross-sectional study based on the mathematical equation proposed by Morgan was performed. The AL was estimated based on the mean values of keratometry and spherical equivalent in 1783 subjects (52% female), aged 14.6 ± 4.6 years (6 to 25 years), of whom 738 were myopic, 770 emmetropic and 275 hyperopic. On average, the AL estimated with the Morgan formula was 0.25 ± 0.48 mm larger than the real AL value (95% limits of agreement: +0.70 to −1.20 mm). The study by gender, ametropia, type of astigmatism and age showed statistically significant differences between the real AL and predicted AL_Morgan (r > 0.750, spearman). Based on the previous sample, a multiple linear regression was applied, and a new mathematical model was proposed. The model was tested on a longitudinal sample of 152 subjects whose mean age was 13.3 ± 3.1 years (9 to 24 years) and of whom 96 were female (64%). The sample consisted of 46 myopes, 82 emmetropes and 24 hyperopes. The longitudinal study of the differences in axial length at one year between the models showed no statistically significant differences and that the mathematical equations are valid for estimating differences in axial increment for ages between 9 and 24 years, despite errors in the predicted value for axial length.This work was supported by the Portuguese Foundation for Science and Technology (FCT)

Microbioma oral de pacientes em unidade de terapia intensiva: revisão de literatura
Knowledge of the most prevalent microorganisms and their control protocols are fundamental for the comprehensive care of patients in intensive care. The objective of this work was to determine the main pathogens that make up the oral microbiome of intensive care patients. A literature review was carried out in the Pubmed and Scielo databases. The descriptors used were “nosocomial pneumonia” AND “oral cavity” and “oral microbiome” AND “ICU”. Articles published in the last 10 years were included. Gray literature was excluded. After applying the criteria, 7 articles were selected for this review. More than 500 types of bacteria have been described in the oral cavity, with approximately 22 classified as dominant. Among the main bacteria, the following stand out: Pseudomonas aeruginosa, S. aureus, Acinetobacter spp, Klebsiella spp, Enterobacter spp, S. pneumoniae, E. coli, Klebsiella pneumoniae and Enterococcus faecalis. Species variations are found between dentate and edentulous patients. Microbiological coincidence is high between dental biofilm and lower airways. Other microorganisms may be present in the oral microbiome, such as Redondoviridae, associated with both periodontitis and respiratory failure, and Candida albicans. In view of the diversity of pathogens, the cleaning agent must have a broad spectrum, with few side effects for the patient. The literature shows that the most common oral pathogens in critically ill patients are gram-negative bacteria and Staphylococcus aureus. Chlorhexidine digluconate 0.12% has proven to be the gold standard for cleaning the oral cavity in patients in these environments, due to its broad spectrum and substantivity.O conhecimento dos microrganismos mais prevalentes e os protocolos de controle dos mesmos são fundamentais para o cuidado integral do paciente em terapia intensiva. O objetivo deste trabalho foi determinar os principais patógenos que compõem o microbioma oral de pacientes em terapia intensiva. Foi realizada uma revisão da literatura nas bases de dados Pubmed e Scielo. Os descritores utilizados foram “nosocomial pneumonia” AND “oral cavity” e “oral microbioma” AND “ICU”. Foram incluídos artigos publicados nos últimos 10 anos. A literatura cinzenta foi excluída. Após aplicação dos critérios, 7 artigos foram selecionados para esta revisão. Mais de 500 tipos de bactérias já foram descritos na cavidade oral, sendo aproximadamente 22 classificadas como dominantes. Dentre as principais bactérias destaca-se: Pseudomonas aeruginosa, S. aureus, Acinetobacter spp, Klebsiella spp, Enterobacter spp, S. pneumoniae, E. coli, Klebsiella pneumoniae e Enterococcus faecalis. São encontradas variações de espécies entre pacientes dentados e edêntulos. A coincidência microbiológica é alta entre biofilme dental e vias aéreas inferiores. Outros microrganismos podem estar presentes no microbioma oral como é o caso do Redondoviridae, associado tanto à periodontite quanto à insuficiência respiratória e Candida albicans. Em vista da diversidade de patógenos, o agente de limpeza deve ter amplo espectro, como poucos efeitos colaterais para o paciente. A literatura demonstra que os patógenos orais mais frequentes em pacientes críticos são bactérias gram-negativas e Staphylococcus aureus. O digluconato de clorexidina 0,12% tem se mostrado o padrão ouro para a limpeza da cavidade oral em pacientes nesses ambientes, devido ao amplo espectro e substantividade
Educational technologies and inclusion experiences: You Tube’s potential for teaching History to deaf people
A teaching proposal for deaf people will be outlined here, mediated through You Tube, that contemplates the topics covered in History classes, through the joint and collaborative construction of a digital library of Libras videos on this platform. Therefore, a brief contextualization is outlined on how the discipline has been organized in the Basic Education curriculum in face of the technological changes that take place at the turn of the 20th century with the advent of information and communication technologies (ICTs) The methodology used is a qualitative approach in a descriptive and exploratory character, which will analyze some bibliographical productions on the subject and the discussion of two You Tube channels.The dialogues with Pinsky (2016), Bispo and Barros (2016), Castells (1999), Queiroga Junior (2018), Coelho and Bottentuit Junior (2019), Correa (2002) and Pereira (2017), open up possibilities for thinking about the teaching of History mediated by ICTs. Among the results obtained, there is the relevance of thinking about mediated educational projects by educational technologies that contribute to the achievement of autonomy of the deaf person, respecting their different ways of learning
ABORDAGEM DA VIOLÊNCIA INFANTIL NA ESTRATÉGIA SAÚDE DA FAMÍLIA: FATORES INTERVENIENTES E ESTRATÉGIAS DE ENFRENTAMENTO
Objetivo: identificar, na percepção de enfermeiros, os fatores intervenientes e as estratégias empregadas para a abordagem da violência infantil na Estratégia Saúde da Família. Método: estudo qualitativo com 22 enfermeiros de Teresina, Piauí. Os dados foram coletados em outubro de 2019, mediante entrevistas individuais orientadas por um roteiro semiestruturado, submetidas à análise de conteúdo temática. Resultados: os enfermeiros revelaram inabilidade para lidar com situações de violência infantil. Eles buscavam abordar esses casos por meio de estratégias pautadas no diálogo, no trabalho interprofissional e na intersetorialidade. Considerações finais: foram fatores intervenientes para a abordagem da violência infantil na Estratégia Saúde da Família, o (não)reconhecimento de sinais de abuso infantil, as barreiras para o estabelecimento de vínculo com as famílias e dificuldades de interlocução na rede assistencial. As estratégias para o enfrentamento da violência infantil utilizadas foram trabalho interprofissional, cuidado holístico, articulação com a rede intersetorial e diálogo com a família.Descritores: Enfermagem. Validation of Rare Structural Variants in Portuguese Azoospermic Patients
Azoospermia affects approximately 15% of infertile males. Despite considerable research efforts in the last decades, in the majority of cases the cause remains unidentified. Chromosomal abnormalities and Yq microdeletions have been thoroughly studied, yet only account for 17% of azoospermic men. In fact, little is known about the contribution of the hemizygous X-linked and autosomal genes to male infertility. This study focuses on the validation of rare deletions encompassing candidate genes on the X chromosome and on the autosomes, previously identified by Affymetrix 6.0 SNP Array, in a cohort of 166 Portuguese individuals with severe spermatogenic impairment (non-obstructive azoospermia and severe oligozoospermia). As expected, the protein-coding genes CXORF48, and MAGEA8, as well as a miRNA (hsa-mir-4330) could not be amplified by PCR from the single X chromosome of the patients suspected of carrying deletions. These rearrangements will be further validated by aCGH (array Comparative Genomic Hybridization). Additionally, by MLPA analysis on 11p13 we confirmed a large deletion (~1Mb) spanning the WT1 gene - a conserved transcription factor known to play a crucial role in gonadal differentiation. A retrospective clinical evaluation of this patient revealed partial gonadal dysgenesis, consistent with a causal role for the newly discovered deletion. These results reveal new candidate genes for a role in spermatogenic pathways and suggest that haploinsufficiency of proteins important for the development of the male reproductive system can lead to spermatogenic dysfunction
