    Incidence and predisposing factors of cognitive disorders following off-pump coronary artery bypasses graft surgery

    Patients and Methods: A total of 171 patients who had undergone off-pump CABG without any history of psychiatric disorders were enrolled. Samples were selected according to a purposive sampling method. The Mini-Mental State Examination (MMSE) questionnaire was given to each patients to assess the incidence of cognitive disorder during the first 24 hours of surgery in ICU. To compare creatinine, erythrocytes sedimentation rate, extubation time, and patients’ age between those with and those without postoperative cognitive disorder, independent-samplest test was employed. To compare two groups in terms of any history of diseases such as diabetes, hypertension, and hyperlipidemia, and qualitative C-reactive protein (CRP), Chi square test was used. Results: Results showed that 75% of patients had postoperative cognitive disorder. There was a significant association between the history of hypertension, CRP, and preoperative creatinine levels in both cognitive disorder and control groups. Background: Cognitive disorder, which is a common problem for the hospitalized patients, is a fluctuating cognitive destruction that leads to the loss of consciousness. It is usually accompanied by increased mortality, prolonged hospital stay, and decreased rehabilitation. Objectives: The purpose of this study was to determine the risk factors associated with cognitive disorder after coronary artery bypasses graft surgery (CABG). Conclusions: Given the significant prevalence of postoperative cognitive disorder and significant association between the history of hypertension, CRP, and preoperative creatinine and cognitive disorder, the detection of patient’s clinical symptoms may improve diagnosis, treatment, and even prevention of cognitive disorder. © 2014, Iranian Society of Regional Anesthesia and Pain Medicine (ISRAPM); Published by Kowsar

    Does a New Modification of the Two-Step Injection Technique for Inferior Alveolar Nerve Block Reduce Pain Compared to the Conventional Technique? A Randomized Clinical Trial

    Background. The ability to control pain is an essential part of dental procedures and the need for optimal pain control and reduction of discomfort is the primary concern of every dentist. This study aims to compare the pain and vital signs during inferior alveolar nerve block between conventional and a new modification of the two-step injection techniques. Methods. In this institutional single-blind randomized clinical trial, attendees of dental school at Mazandaran University of Medical Sciences from February to May 2022 were included. Inclusion criteria were 20-60 years old and healthy (ASA1) individuals who were willing to participate in this study. Individuals who were taking medications affecting their understanding of pain and patients with active infections at the injection site were excluded. These individuals were divided into two groups. First, superficial anesthesia was performed and afterward, conventional and two-step injection techniques were performed. For the two-step injection method, 6 mm of the needle was injected into the mucosa and one-third of the local anesthetic solution was released from the computer-controlled injection toolkit. Afterward, a 25 mm 30-gauge needle was reinserted into the previous hole delivering the remaining local anesthetic. The pain during injection was measured by a patient-reported numerical rating scale (NRS). Moreover, vital signs were monitored immediately before and after the injection. Kolmogorov-Smirnov test, Mann-Whitney U test, independent T-test, and Fisher's exact χ2 test were performed for statistical analysis (α = 0.05). Results. This study involved 32 adults aged between 20 and 50 years old with 1: 1 female/male sex distribution. The pain score was significantly higher in the conventional injection technique compared to the two-step injection technique in all sex and age groups. There were no significant differences in vital signs between the conventional and two-step injection techniques. There was no significant difference in the mean pain scores of females and males, regardless of their injection techniques. Conclusion. Utilizing the two-step injection technique in patients for inferior alveolar block reduces pain during injection without altering patients' vital signs significantly.     Comparison of antimicrobial and wound-healing effects of silver nanoparticle and chlorhexidine mouthwashes: an in vivo study in rabbits

    The objective is to formulate a silver nanoparticle mouthwash and then evaluate its antimicrobial and wound-healing effects in rabbit animal models. Microbial samples were collected from the oral cavity of 60 rabbits. Thereafter, standardized wounds were created in the lateral border of the tongue on the right side for all rabbits. After surgery, digital photographs were obtained from the wounds with standardized settings. To characterize the silver nanoparticles used in the synthetic mouthwash, transmission electron microscopy (TEM) and digital light scattering analysis were used. The animal models were then randomly divided into 4 groups: group 1 received 9.80 wt% silver nanoparticle mouthwash; group 2 received all the ingredients of the formulated mouthwash except for silver nanoparticles; group 3 received chlorhexidine 2.0% mouthwash; and the negative control group did not receive any postoperative mouthwash. Microbial samples were collected from oral cavity of the rabbits each day for four postoperative days. Colony-forming unit (CFU) counts were compared post-operatively with the pre-operative counts. In addition, standardized digital photographs were taken each day from the wounds and the area of the wounds was compared in postoperative and pre-operative images. Data were statistically analyzed using one-way ANOVA and repeated measures variance analysis (α = 0.05). TEM revealed spherical morphology of silver nanoparticles and digital light scattering showed an average size of 5 nm and optimal distribution of the nanoparticles. CFU count significantly decreased in groups 1 and 3 (P 0.05). Silver nanoparticle mouthwash possesses favorable antibacterial and wound-healing effects. The formulated 9.80 wt% silver nanoparticle mouthwash with a particle size of 5 nm can be a promising alternative for application after oral surgical procedures

    Entanglement study of the 1D Ising model with Added Dzyaloshinsky-Moriya interaction

    We have studied occurrence of quantum phase transition in the one-dimensional spin-1/2 Ising model with added Dzyaloshinsky-Moriya (DM) interaction from bi- partite and multi-partite entanglement point of view. Using exact numerical solutions, we are able to study such systems up to 24 qubits. The minimum of the entanglement ratio R ≡\equiv \tau 2/\tau 1 < 1, as a novel estimator of QPT, has been used to detect QPT and our calculations have shown that its minimum took place at the critical point. We have also shown both the global-entanglement (GE) and multipartite entanglement (ME) are maximal at the critical point for the Ising chain with added DM interaction. Using matrix product state approach, we have calculated the tangle and concurrence of the model and it is able to capture and confirm our numerical experiment result. Lack of inversion symmetry in the presence of DM interaction stimulated us to study entanglement of three qubits in symmetric and antisymmetric way which brings some surprising results.Comment: 18 pages, 9 figures, submitte

The Judd-Ofelt parameters measured from the absorption spectra have been used to calculate the radiative life-time (τr) and the stimulated emission cross section. The low phonon energy, a reduced quenching effect and a high quantum efficiency of 90% for the 1.53 μm expected laser emission into pumping at 980 nm are in favor of promising material laser application

    Accurate testing of aspheric surfaces using the transport of intensity equation by properly selecting the defocusing distance

    In the last decade, the transport of intensity has been increasingly used in microscopy, wavefront sensing, and metrology. In this study, we verify by simulation and experiment the use of the transport of intensity equation (TIE) in the accurate testing of optical aspheric surfaces. Guided by simulation results and assuming that the experimental setup parameters and the conic constants are known, one can estimate an appropriate defocusing distance Äz that leads to an accurate solution of the TIE. In this paper, this method is verified through the construction of a nonnulled experiment for testing the 2D profile of an aspheric surface. The theoretical method and experimental results are compared to validate the results. Finally, to validate the TIE methodology, the phase distribution obtained by TIE is compared with the phase distribution obtained by a Shack-Hartmann sensor. © 2016 Optical Society of America

The susceptibilities of the nonlinear medium are obtained and their relation to coupling functions are determined.     A guide to properly select the defocusing distance for accurate solution of transport of intensity equation while testing aspheric surfaces

    In this paper, the Transport of Intensity Equation (TIE) for testing of an aspheric surface is verified experimentally. Using simulation, a proper defocus distance Δo that leads to an accurate solution of TIE is estimated whenever the conic constant and configuration of the experiment are known. To verify this procedure a non-nulled experiment for testing an aspheric is used. For verification of the solution, the results are compared with the Shack-Hartmann sensor. The theoretical method and experimental results are compared to validate the results. © 2016 SPIE
