    The Tipping Point in Psychopathy: Role of Psychopathic Traits in Trauma Exposure

    Though psychopathy has been associated with a socially deviant lifestyle, the idea of “successful” psychopathy has gained increased attention. Previous research has explored whether psychopathic traits play a role in trauma exposure/ PTSD, using clinical and forensic populations. Results from these studies suggest that Factor 1 traits may protect from trauma exposure (Factor 1 Theory) while Factor 2 traits may worsen the impact of trauma through exacerbated exposure to traumatic events. The objective of this study was to explore the relationship between each of Factor 1 traits and Factor 2 traits with trauma exposure and PTSD. In a sample of 86 emergency responders and 104 community members, Factor 1 did not show a significant association with trauma exposure (community or emergency responder samples) but showed a negative and significant association with PTSD (community and emergency responders samples); Factor 2 showed a significant positive association with trauma exposure (community sample) and PTSD (community and emergency responders samples). Factor 1 had a significant moderation effect on the relationship between trauma exposure and PTSD (community sample) suggesting a protective effect. Factor 2 did not have a significant moderating effect in any of the samples. This study used a population that has not been studied before (emergency responders) in the context of psychopathy and trauma. By furthering research on this topic, appropriate intervention methods can be developed to assist such sub-groups that are faced with high trauma exposure on a daily basis.     This paper is a narrative of the challenges experienced as a faculty member in an attempt to transform the existing educational model for how school and clinical mental health counselors are trained to enter field-based clinical experiences with students and clients. The author shares the pre-practicum laboratory training as an important step towards the growth of a counselor both personally and professionally. The laboratory training addresses aspects of student anxiety and self-efficacy with relation to live supervision with use of technology, nature of feedback, reflective journaling, reflective teams, and process group experiences as pedagogical adjuncts to enhance the training and developmental needs of beginning counseling students

    Privacy and Protection for Location Based Services

    An LBS (location based service) provides services such as finding the nearest location, favourite entertainment areas etc, to the users based on either their residing area or based on the input the give. The services provided by a location based service are typically based on a point of interest database. Therefore retrieval of the data from the database server takes place. The work proposed is a novel protocol for location based queries that has major performance improvements, which is performed based on two stages- firstly, the user determining his/her location privately and secondly, the server protecting its data from unauthorized users for which they have not paid. This protocol enables security for the userrsquo;s details as well as protects the serverrsquo;s data. The user is protected because the server is unable to determine his/her location. In the same way, the serverrsquo;s data is protected as a Malicious user can only decrypt the part of data obtained to the user with the encryption key acquired in the previous phase, that is he/she cannot decrypt the remaining server data that they are not supposed to authorise. In other words, users can never get the data more than what they have paid for. A phase called oblivious transfer phase is present so as to ensure the privacy of the user and a phase called private information retrieval phase is carried out to protect serverrsquo;s data

    Assessment of comorbidities in patients with deranged thyroid hormone levels

    Background: There is association of deranged thyroid hormone levels with various co-morbidities. Drugs for comorbidity may interact with each other and affect the outcome of treatment. So, this study was planned to find out comorbidities with deranged thyroid hormone levels and various possible drug interactions.Methods: It was a prospective, observational study carried out at tertiary care hospital from November 2017 to June 2018. Thyroid hormonal levels reports were followed in OPD and online information system facility. Drugs interactions were checked by referring standard pharmacology textbooks, review articles and Medscape drug interaction checker. Approval from the Institutional Ethics Committee was taken before initiation of the study. Patients with deranged thyroid hormone levels were included in the study.Results: Study was conducted in 111 patients of thyroid disorders. Spondylitis, asthma, acne cases were seen in hypothyroid patients whereas hypokalemic periodic paralysis, thyroid ophthalmopathy statistically significantly seen in hyperthyroid patients. Various concurrent medications such as calcium, carbamazepine decreases the effects of levothyroxine. For management of comorbidities various drugs are given which also interact among themselves significantly.Conclusions: Diabetes mellites, obesity, spondylitis, lichen planus were more common in hypothyroidism while comorbidities like hypertension, hypokalemic periodic paralysis, thyroid ophthalmopathy were found to be more in hyperthyroidism.  Incidence of drug interactions is found to be more with increased use of medications for comorbidities, so physicians should be careful while prescribing them. Due to drug interactions desired effects of drugs given for thyroid disorders may not be observed

    Feto-maternal outcome in patients with peripartum cardiomyopathy: a 5-year study in a tertiary care hospital in Kolar district

    Background: Peripartum cardiomyopathy (PPCM) is a disorder of unknown cause in which initial left ventricular systolic dysfunction and symptoms of heart failure occur between the last month of pregnancy and the ïŹrst 5 months postpartum. PPCM remains a diagnosis of exclusion. There have been numerous proposed causes including hormonal abnormalities, inflammation, viral pathogens, autoimmune response, and genetic predisposition. Aim of study was to study the clinical profile, risk factors, and the management along with obstetric and perinatal outcome, in women with peripartum cardiomyopathy.Methods: This retrospective observational study was conducted on the antenatal women of age group of 18 years to 40 years, admitted in the labour ward of R. L. Jalapa hospital who presented with heart failure in last month of pregnancy till 5 months postpartum, without previously having a heart disease over the 5 year period, January 20 15 to December 2019.Results: Majority of the patients (15/18) presented with complaints of exertional dyspnoea. Mean LVEF at the time of diagnosis was 38.39%. There were 5 (27%) maternal mortality and all of them had global hypokinesia on echocardiography and presented in NYHA class III and IV. Two (11%) out of eighteen patients had intrauterine death, and all the patients who had IUFD belonged to class IV. Four babies (22.22%) had intrauterine growth restriction.Conclusions: The present study came to conclusion that in rural tertiary center, maternal outcome and prognosis was poor as patients presenting to us were majority of them in cardiogenic shock and lower LVEF in terminal stages. The associated risk factors were preeclampsia, anemia and multiple gestation which could also contribute to the poor prognosis which was noticed in the study. Early recognition of the disease is of paramount importance as the clinical manifestations can conceal and can lead to high morbidity and mortality especially within 3 months postpartum

    An analysis of ear discharge and antimicrobial sensitivity used in its treatment

    Background: Ear discharge is one of the cardinal symptoms of ear infection along with progressive deafness, pain, tinnitus and vertigo. Main objectives of the study were to study the various causes of ear discharge, isolate and identify the microorganisms associated with different causes of ear discharge and study the antibiotic sensitivity patterns of the isolated organisms.Methods: All the patients matching the inclusion criteria were enrolled and sample of ear discharge was collected. This sample was sent to the microbiology laboratory for isolation of microorganism and antimicrobial sensitivity testing.Results: In present study 115 samples of ear discharge were examined for the presence of microorganisms. Out of 115, 93 (80.86%) samples were positive for growth of microorganisms and 22 (19.13%) samples were sterile. Out of 93% positive samples 61 (65.59%) samples were pure-bacterial growth, 8 (*8.60%) samples showed pure fungal growth and 24 (25.80%) showed mixed growth of both bacteria anti fungi.Conclusions: Overall bacterial isolates were higher than fungal and pseudomonas appeared to be most common. It was found sensitive to ceftazidime, amikacin, imipenem, colistin and aztreonam

    Performance Analysis of Transmit Antenna Selection with MRC in MIMO for Image Transmission in Multipath Fading Channels Using Simulink

    Multiple antenna configurations can be used to increase the data throughput reducing the effects of multipath fading and interference when channel bandwidth is limited. Orthogonal Space Time Block Codes along with Transmit antenna selection can improve the performance of multiple input multiple output systems. In this paper, we present the Transmit Antenna Selection (TAS) technique based on the Maximal Ratio Combining (MRC) scheme with single antenna selection for image transmission. The performance analysis of the system was carried out under different fading channels i.e. Rayleigh and Rician channel for image input. We design end to end TAS/MRC system in Simulink with advancements in the channel designs and receive diversity techniques along with the feedback models. The Bit Error Rate (BER) analysis was performed for the combinations of number of transmit and receive antennas for TAS/MRC system for various fading environments
