CFD modeling of the catalyst oil slurry hydrodynamics in a high pressure and temperature as potential for biomass liquefaction
The paper presents the simulation of a catalyst-para n oil slurry hydrodynamics under
high pressure and temperature in a convex bottom reactor with a Rushton turbine which was
conducted with an application of computational fluid dynamics (CFD) modeling. An analysis to
obtain a uniform distribution of solid catalyst particles suspended in para n oil was carried out as a
potential for biomass liquefaction. The e ects of the particle diameter, bed density, liquid viscosity,
and the initial solid loading on slurry hydrodynamics in high pressure and temperature behavior
were investigated using the Eulerian–Eulerian two-fluid model and the standard k-" turbulence
model. The main objective was to assess the performance in agitating highly concentrated slurries to
obtain slurry velocity, concentration, the degree of homogeneity, and to examine their e ect on the
mixing quality. The results of the analysis are applied to predicting the impact of the most e cient
conditions on slurry suspension qualities as potential for biomass liquefaction
A comparison of the clinical course of preexcitation syndrome in children and adolescents and in adults
Background: Atrioventricular reentrant tachycardia (AVRT) in patients with preexcitation
syndrome (PS) is the main cause of paroxysmal regular arrhythmias in children and adolescents.
While the previously published data most commonly concern clinical consequences of PS
in adults, few researchers have evaluated the problem in children and adolescents. The aim of
the study was to compare the clinical course of PS between the population of children and
adolescents below 19 years of age and the population of adult patients.
Method: The study population consisted of 302 consecutive PS patients managed between January
2001 and June 2005 with radiofrequency catheter ablation (RFCA). The study population was
divided into two groups: Group 1 consisting of 52 patients aged 15.38 years on average (7-18 ± 2.53) and Group 2 consisting of 250 adult patients aged 38.67 years on average (19-72 ± 13.1).
Results: Patients from Groups 1 and 2 experienced their first episode of AVRT at the mean
age of 13.3 years and 29.1 years, respectively (p < 0.05). The mean annual numbers of AVRT
episodes in Groups 1 and 2 were 12.97 (range, 2-96; median, 8) and 8.86 (range, 2-25;
median, 6), respectively (p = non-significant). Two patients from Group 1 (3.85%) and
42 patients from Group 2 (16.8%) experienced episodes of atrial fibrillation (AF) (p < 0.05).
Location of the accessory pathways (AP): In Group 1, the right free wall and anteroseptal AP
locations were significantly more common [11 (21.15%) and 9 (17.31%) patients, respectively,
vs.q 19 (7.6%) and 13 (5.2%) patients in Group 2; p < 0.01]. In Group 2, the left anterolateral
AP location was more common [81 (32.4%) vs. 4 (7.69%) in Group 1; p < 0.01].
Conclusions: In children and adolescents with PS, a significantly lower incidence of AF was
found. In Group 1, RFCA was performed significantly more frequently due to the development
of AVRT caused by right free wall and right anteroseptal AP, while in the group of adults, the
left anterolateral AP location was found more commonly. (Cardiol J 2007; 14: 384-390
On-Pump vs Off-Pump coronary artery bypass surgery in atrial fibrillation : analysis from the polish national registry of cardiac surgery procedures (KROK)
BackgroundNo single randomized study has ever before addressed the safety of On-Pump coronary artery bypass grafting (CABG) vs Off-Pump CABG in the setting of atrial fibrillation (AF) and data from small observational samples remain inconclusive.Methods and findingsProcedural data from KROK (Polish National Registry of Cardiac Surgery Procedures) were retrospectively collected. Of initial 188,972 patients undergoing CABG, 7,913 presented with baseline AF (76.0% men, mean age 69.1±8.2) and underwent CABG without concomitant valve surgery between 2006-2019 in 37 reference centers across Poland. Mean follow-up was 4.7±3.5 years (median 4.3 IQR 1.7-7.4). Cox proportional hazards models were used for computations. Of included patients, 3,681 underwent On-Pump- (46.52%) as compared to 4,232 (53.48%) who underwent Off-Pump CABG. Patients in the latter group less frequently were candidates for complete revascularization (PConclusionsOff-Pump CABG offered 30-day survival benefit to patients undergoing CABG surgery and presenting with underlying AF. Children and adolescents with pulmonary arterial hypertension : baseline and follow-up data from the polish registry of pulmonary hypertension (BNP-PL)
We present the results from the pediatric arm of the Polish Registry of Pulmonary
Hypertension. We prospectively enrolled all pulmonary arterial hypertension (PAH) patients,
between the ages of 3 months and 18 years, who had been under the care of each PAH center
in Poland between 1 March 2018 and 30 September 2018. The mean prevalence of PAH was
11.6 per million, and the estimated incidence rate was 2.4 per million/year, but it was geographically
heterogeneous. Among 80 enrolled children (females, n = 40; 50%), 54 (67.5%) had PAH associated
with congenital heart disease (CHD-PAH), 25 (31.25%) had idiopathic PAH (IPAH), and 1 (1.25%)
had portopulmonary PAH. At the time of enrolment, 31% of the patients had significant impairment
of physical capacity (WHO-FC III). The most frequent comorbidities included shortage of growth
(n = 20; 25%), mental retardation (n = 32; 40%), hypothyroidism (n = 19; 23.8%) and Down syndrome
(n = 24; 30%). The majority of children were treated with PAH-specific medications, but only half of
them with double combination therapy, which improved after changing the reimbursement policy.
The underrepresentation of PAH classes other than IPAH and CHD-PAH, and the geographically
heterogeneous distribution of PAH prevalence, indicate the need for building awareness of PAH
among pediatricians, while a frequent coexistence of PAH with other comorbidities calls for a
multidisciplinary approach to the management of PAH children
Wczesna zakrzepica zastawki biologicznej w pozycji mitralnej
We present a case of a 70 year-old woman operated due to severe mitral regurgitation. Early after surgery transthoracic
echocardiography revealed the decreased effective orifice area of the implanted bioprosthetic valve and the stenotic features
of transvalvular flow. Transesophageal echocardiography (TEE) disclosed a thrombotic cause of heterograft dysfunction. Due
to the clinical deterioration and the unclear cause of prosthesis stenosis, the patient was reoperated. Intra-operatively bioprosthetic
mitral valve thrombosis was confirmed. Precipitating factors of this rare complication including cardiac devicerelated
infective endocarditis (CDRIE) and the diagnostic applicability of TEE in this clinical scenario are discussed.We present a case of a 70 year-old woman operated due to severe mitral regurgitation. Wstęp: Celem pracy była ocena przebiegu zabiegów przezżylnej implantacji stymulatora serca
u dzieci poddanych wcześniej złożonym zabiegom kardiochirurgicznym.
Materiał i metody: Analizie poddano przebieg implantacji kardiostymulatora u 7 dzieci
(w tym po zabiegu Senninga, ortotopowym przeszczepie serca, operacyjnej korekcji DORV, VSD, PDA, plastyce zastawki trójdzielnej, zamknięciu ubytku w przegrodzie międzyprzedsionkowej
typu sinus venosus z nieprawidłowym spływem żył płucnych przy współistnieniu dekstrokardii,
z wadą pod postacią 2-napływowej wspólnej komory i przełożeniem wielkich naczyń
po zabiegu bandingu tętnicy płucnej). Grupę kontrolną stanowiły 24 dzieci, u których
implantowano 2-jamowy układ stymulujący DDD z powodu wrodzonego bloku przewodzenia
przedsionkowo-komorowego III stopnia.
Wyniki: U wszystkich dzieci uzyskano fizjologiczny tryb stymulacji. Zabiegi u chorych po
leczeniu kardiochirurgicznym były istotnie dłuższe (90,7 min vs. 71 min) oraz wymagały
istotnie dłuższego użycia fluoroskopii (30,7 min vs. 9 min).
Wnioski: U wielu dzieci po złożonych zabiegach kardiochirurgicznych istnieje możliwość
bezpiecznej implantacji przezżylnej stymulatora serca. Jednak decyzję o zastosowaniu tego typu
zabiegu zawsze musi poprzedzać wnikliwa analiza kliniczna, w wybranych przypadkach połączona
z wykonaniem specjalistycznych badań dodatkowych, w tym z cewnikowaniem serca

Porównanie przebiegu klinicznego zespołu preekscytacji u dzieci i młodzieży z pacjentami dorosłymi
Wstęp: Nawrotny częstoskurcz przedsionkowo-komorowy (AVRT) u chorych z zespołem preekscytacji
(PS) stanowi główną przyczynę napadowych miarowych arytmii u dzieci i młodzieży.
Publikowane dane najczęściej dotyczą następstw klinicznych PS u dorosłych, w niewielu natomiast
ocenia się ten problem u dzieci i młodzieży. Celem pracy była analiza porównawcza przebiegu
klinicznego PS między populacją dzieci i młodzieży w wieku poniżej 19 lat a chorymi dorosłymi.
Metoda: Badaniem objęto 302 kolejnych chorych z PS leczonych w okresie od stycznia 2001
do czerwca 2005 roku za pomocą ablacji prądem o wysokiej częstotliwości (RFCA). Z badanej
populacji wyodrębniono dwie grupy: grupę I (52 chorych w wieku średnio 15,38 roku; 7-18 ± 2,53) i grupę II (250 chorych dorosłych w wieku średnio 38,67 roku; 19-72 ± 13,1).
Wyniki: W grupie I wiek, w którym pierwszy raz wystąpił AVRT, wynosił średnio 13,3 roku,
w grupie II - 29,1 roku (p < 0,05). Częstość występowania epizodów AVRT w skali rocznej
wynosiła w grupie I średnio 12,97 (zakres 2-96, mediana 8), w grupie II - średnio 8,86
(zakres 2-25, mediana 6), p = NS. Epizody migotań przedsionków przebyło 2 chorych (3,85%)
z grupy I i 42 pacjentów (16,8%) z grupy II (p < 0,05). W grupie I istotnie częściej stwierdzono
lokalizację dróg dodatkowych (AP) prawostronną boczną (11 chorych; 21,15%) oraz przednioprzegrodową
(9 chorych; 17,31%), natomiast w grupie II odpowiednio - u 19 (7,6%)
i 13 pacjentów (5,2%; p < 0,01). W grupie II częściej występowała lokalizacja lewostronna
przednio-boczna (81 chorych; 32,4%), w grupie I - u 4 pacjentów (7,69%; p < 0,01).
Wnioski: U dzieci i młodzieży z PS istotnie rzadziej stwierdzano migotanie przedsionków.
Częściej w grupie I wykonywano RFCA z powodu ujawnienia się AVRT zależnych od prawostronnych bocznych i prawostronnych przednio-przegrodowych AP, w grupie dorosłych natomiast
częściej obserwowano lewostronną przednio-boczną lokalizację AP. (Folia Cardiologica
