    A szívfrekvencia-variabilitás jelentősen csökken nem diabeteses hypertoniás betegek körében

    Bevezetés: A szívfrekvencia-variabilitás csökken hypertoniában vagy diabetes mellitusban szenvedők körében. A hypertonia és a diabetes mellitus egymással gyakori komorbiditást mutatnak. Nincs elegendő adat arról, hogy a kontrollhoz képest diabetes mellitusban nem szenvedő hypertoniás betegek körében hogyan változik a szívfrekvencia-variabilitás. Célkitűzés: A szerzők hypertoniában szenvedő diabeteses és nem diabeteses betegekben, valamint kontrollegyénekben a szívfrekvencia-variabilitás vizsgálatát tűzték ki célul. Módszer: 130 hypertoniában, 48 hypertoniában és 2-es típusú diabetes mellitusban szenvedő beteget, valamint 87 kontrollszemélyt vontak be a vizsgálatba. A minimális, átlagos és maximális szívfrekvenciát, valamint az egymást követő RR-intervallumok időtartamának és 5 perces szegmensekben mért átlagának szórását határozták meg. Eredmények: Az átlagos minimális szívfrekvencia szignifikánsan nem különbözött a csoportok között. A kontrollhoz képest a többi paraméter szignifikánsan csökkent mind a hypertoniában, mind a hypertoniában és diabetesben szenvedő betegek csoportjában. A hypertoniás csoporthoz képest a hypertoniában és diabetesben szenvedő csoportban a paraméterek nem különböztek szignifikánsan. Következtetések: Diabetes mellitusban nem szenvedő hypertoniás betegek körében a szívfrekvencia-variabilitás jelentősen beszűkül. Úgy tűnik, hogy hypertoniás betegek körében a 2-es típusú diabetes mellitus szignifikánsan már nem csökkenti tovább a szívfrekvencia-variabilitást. Orv. Hypertension and diabetes show frequent co-morbidity, but it is still not entirely clear whether heart arte variability is reduced in non-diabetic patients with hypertension. Aim: The aim of the authors was to evaluate the heart rate variability in hypertensive patients with and without diabetes and in control subjects. Method: 130 patients with hypertension, 48 patients with hypertension and type 2 diabetes mellitus, and 87 control subjects were involved in the study. Minimum, mean and maximum heart rate, and parameters of heart rate variability were measured. Results: The mean of minimum heart rate did not differ significantly between the three groups. However, all other parameters were significantly reduced in patients with hypertension with and without diabetes as compared to the control group. No significant differences were observed between hypertensive patients with and without diabetes mellitus. Conclusions: Heart rate variability is significantly reduced in non-diabetic patients with hypertension.     Evaluation and Selection of Gel Base for the Formulation of Dexpanthenol Products

    Purpose: To formulate dexpanthenol gels with enhanced in vivo absorption properties via skin.Methods: Carboxyvinyl derivatives (Carbopol 980 and Ultrez 10) and poloxamer (Lutrol F 127) were used as the hydrogel base in the formulations. Changes in rheological properties (apparent viscosity and penetration values) during the storage period were examined by Rheotest RN rotational viscometer and PNR12 penetrometer. In vitro release study using Franz diffusion cell was employed to compare the release  characteristics of the formulated hydrogels with those of a reference cream.Results: The flow curves of the gels with Carbopol 980 and Ultrez 10 showed pseudoplastic flow. Lutrol F 127 gels presented thixotropic  behaviour. The consistency of the studied gels was in the following rank order: Lutrol F 127 > Ultrez 10 > Carbopol 980. In vitro results showed that dexpanthenol was released in lower amounts from the reference cream than from the three test gels. No significant differences were observed in the amount of active substance released from the gels due probably to the fact that Carbopol 980 and Ultrez 10 are both carboxyvinyl polymers. The highest amount of dexpanthenol was released from Lutrol F 127 gel.Conclusion: The hydrogel made with Lutrol F 127 gel base possesses the best properties of all the gels and is recommended for the formulation of a suitable dexpanthenol gel.Keywords: Hydrogel, Dexpanthenol, Carboxyvinyl polymers, Gels,  Carbopol, Poloxamer, Rheology, Drug release, Penetromete

    The user support programme and the training infrastructure of the EGI Federated Cloud

    The EGI Federated Cloud is a standards-based, open cloud system as well as its enabling technologies that federates institutional clouds to offer a scalable computing platform for data and/or compute driven applications and services. The EGI Federated Cloud is based on open standards and open source Cloud Management Frameworks and offers to its users IaaS, PaaS and SaaS capabilities and interfaces tuned towards the needs of users in research and education. The federation enables scientific data, workloads, simulations and services to span across multiple administrative locations, allowing researchers and educators to access and exploit the distributed resources as an integrated system. The EGI Federated Cloud collaboration established a user support model and a training infrastructure to raise visibility of this service within European scientific communities with the overarching goal to increase adoption and, ultimately increase the usage of e-infrastructures for the benefit of the whole European Research Area. The paper describes this scalable user support and training infrastructure models. The training infrastructure is built on top of the production sites to reduce costs and increase its sustainability. Appropriate design solutions were implemented to reduce the security risks due to the cohabitation of production and training resources on the same sites. The EGI Federated Cloud educational program foresees different kind of training events from basic tutorials to spread the knowledge of this new infrastructure to events devoted to specific scientific disciplines teaching how to use tools already integrated in the infrastructure with the assistance of experts identified in the EGI community. The main success metric of this educational program is the number of researchers willing to try the Federated Cloud, which are steered into the EGI world by the EGI Federated Cloud Support Team through a formal process that brings them from the initial tests to fully exploit the production resources. © 2015 IEEE

    Feasibility of Group Schema Therapy for Outpatients with Severe Borderline Personality Disorder in Germany:A Pilot Study with Three Year Follow-Up

    Borderline Personality Disorder (BPD) is a severe, challenging to treat mental disorder. Schema therapy (ST) as an individual therapy has been proven to be an effective psychological treatment for BPD. A group format of ST (GST) has been developed and evaluated in a randomized controlled trial in the United States and piloted in The Netherlands. These results suggest that GST speeds up and amplifies treatment effects of ST and might reduce delivery costs. However, feasibility in the German health care system and with BPD patients with high BPD severity and comorbidity, and frequent hospitalization, has not been tested to date. We investigated GST in 10 severely impaired, highly comorbid female patients with BPD, that needed frequent hospital admission. Patients received an outpatient ST-treatment program with weekly group and individual sessions for 1 year. Outcome measures including BPD severity, general psychopathology, psychosocial functioning, quality of life, happiness, schemas, and modes, and days of hospitalization were assessed at the start of treatment and 6, 12, and 36 months later with semi-structured interviews and self-report measures. We observed significant decreases in severity of BPD symptoms, general symptom severity, dysfunctional BPD-specific modes and schemas, and days of hospitalization. Functional modes, quality of live and happiness improved. The results of this feasibility study are promising and encourage further implementation of ST outpatient treatment programs even for patients with severe BPD and high hospitalization risk. However, small sample size and the missing of a control group do not allow the generalizability of these findings

    From Mott state to superconductivity in-1T-TaS

    The search for the coexistence between superconductivity and other collective electronic states in many instances promoted the discovery of novel states of matter. The manner in which the different types of electronic order combine remains an ongoing puzzle. 1T-TaS is a layered material, and the only transition-metal dichalcogenide (TMD) known to develop the Mott phase. Here, we show the appearance of a series of low-temperature electronic states in 1T-TaS with pressure: the Mott phase melts into a textured charge-density wave (CDW); superconductivity develops within the CDW state, and survives to very high pressures, insensitive to subsequent disappearance of the CDW state and, surprisingly, also the strong changes in the normal state. This is also the first reported case of superconductivity in a pristine 1T-TMD compound.
