    Role of serum interleukin-6 in deciding therapy for multidrug resistant oral lichen planus

    Background: Oral lichen planus (OLP) is a T cell mediated immune response. T cells locally present in the involved tissues release cytokines like interleukin-6 (IL-6), which contributes to pathogenesis of OLP. Also IL-6 has been associated with multidrug resistance protein (MRP) expression by keratinocytes. Correspondingly, upregulation of MRP was found in OLP. We conducted this study to evaluate the effects of various drugs on serum IL-6 in OLP; and correlation of these effects with the nature of clinical response and resistance pattern seen in OLP lesions with various therapeutic modalities. Thus we evaluated the role of serum IL-6 in deciding therapy for multidrug resistant OLP. Material and Methods: Serum IL-6 was evaluated in 42 erosive OLP (EOLP) patients and 10 normal mucosa and 10 oral squamous cell carcinoma cases using ELISA technique. OLP patients were randomly divided into 3 groups of 14 patients each and were subjected to Pimecrolimus local application, oral Mycophenolate Mofetil (MMF) and Methotrexate (MTX) alongwith Pimecrolimus local application. IL-6 levels were evaluated before and after treatment. Results: Serum IL-6 levels were raised above 3pg/ml in 26.19% erosive OLP (EOLP) cases (mean- 3.72±8.14). EOLP (5%) cases with IL-6 levels above 5pg/ml were resistant in MTX group. However significant decrease in serum IL-6 corresponding with the clinical resolution was seen in MMF group. Conclusions: Significantly raised IL-6 levels in EOLP reflect the chronic inflammatory nature of the disease. As serum IL-6 levels significantly decreased in MMF group, correspondingly no resistance to treatment was noted. However with MTX there was no significant decrease in IL-6 and resistance to treatment was noted in some, especially plaque type lesions. Thus IL-6 can be a possible biomarker in deciding the best possible therapy for treatment resistant OLP

    Seed germination behaviour of Terminalia paniculata Roth (Combretaceae), an economically important endemic tree to peninsular India

    The present study was conducted to investigate the seed germination behaviour of Terminalia paniculata, a commercially important tree component of deciduous forests of Peninsular India. The study included the identification of maturity index to determine the optimum collection period of the most viable seeds with good seed longevity. Species are characterized by very low germinability and poor regeneration status. Observations were made throughout the flowering and fruiting period on selected matured trees. Seed viability was tested at various stages of maturity and seed longevity was evaluated. The results indicated that the 16th week after anthesis with red-colored fruits recorded the maximum viability (2.60% germination) that shows it is the best period for the collection of seeds. Seed emptiness is very high in T. paniculata (4 to 2%) compared to other Terminalia species (low emptiness). Pre-sowing treatments were not effective in enhancing seed germination and viability. Seeds can be stored up to 6 months in an air-tight metallic container at 4oC and 45 ± 5 % relative humidity. The results of the study can be taken as a reference for various other tree improvement programs and further studies. 

    Factors associated with cognitive decline and delirium after transcatheter aortic valve implantation: Preliminary evidence

    Background: Transcatheter aortic valve implantation (TAVI) has become the standard for treating severe symptomatic aortic stenosis in those with prohibitive surgical risk. Cognitive complications, including delirium and cognitive decline are common following TAVI, yet an understanding of pre-procedural factors associated with these outcomes is lacking. This prospective observational study set out to identify geriatric pre-procedural factors associated with post-procedural delirium and cognitive decline in patients undergoing TAVI. / Methods: Cognitive outcomes of TAVI patients aged ≥60 years (N=32) were measured over one-year post-TAVI. Pre-procedural measures included frailty, gait, visual symptoms, voice pitch, dysphagia, blink rate, mood, and sleep. Primary outcomes were post-procedural delirium and cognitive decline. / Results: Delirium was present in 25% of patients over two days following TAVI and 26% experienced cognitive decline in the year post-TAVI. Daily physical activity was a protective factor against cognitive decline, and worse baseline visual memory was associated with delirium. While non-significant and with very large confidence intervals, moderate to large effect sizes were found for associations between slowed gait speed, pre-existing atrial fibrillation, and dysphagia for delirium, and slower gait speed, higher blink rate, pre-existing atrial fibrillation for cognitive decline. / Conclusion: Though underpowered, measures of considerable effect size were identified (although non-significant and with large variability). In larger studies, these novel geriatric factors could further be explored for predicting cognitive complications following TAVI. Improvement of risk prediction for cognitive decline and delirium following TAVI could assist with early identification of those at risk, informing clinical decision-making and allowing for targeted intervention to reduce post-procedural incidence of these complications

Items, creators, fans, presidents, composers present themselves from different perspectives to speak, in an autoetnographic methodology, of their points of view about the views of the points where they are located.     Effect of early protein restriction on growth and subsequent performance of egg-type chickens

    Typescript (photocopy).Fifteen crosses of day-old Shaver Single Comb White Leghorn egg-type chicks were used to study the effect of early protein restriction on growth and subsequent performance during lay. Four dietary protein regimes (starter-grower/developer-layer protein levels) 17-16-16, 17-16-18, 20-16-16 and 20-16-18 percent were fed during the study. Each cross consisted of 225 chicks which were separated into four groups, two of which were randomly assigned to receive either a 20% protein or 17% protein starter diet. The two starter diets were formulated to be isocaloric, containing 2924 kcal ME/kg of feed, and were fed to the chicks for the first eight weeks of growth. Feed and water were fed ad libitum during the test period. At both six and eight weeks of age, protein restriction resulted in lighter birds, shorter shank (tarsometatarsus) bones and a reduction in feed consumption per bird. Unlike the slow growing birds, the fast growing birds that were protein restricted showed evidence of an early rebound in body weight by compensatory growth after six weeks of protein restriction. From the beginning of the ninth to the end of the eighteenth week all birds were fed the same grower/developer basal diet containing 16% protein and 3056 kcal ME/kg of feed. Generally, the protein restricted birds continued to maintain smaller body frames and body weights, shorter shanks and reduced feed consumption per bird compared with the protein un-restricted birds at both 12 and 18 weeks of age. The slow growing crosses that were protein restricted made a phenomenal rebound in body weight between 12-18 weeks of age, demonstrating a "delayed compensatory growth" compared with the fast growing crosses. Feeding a high protein (18%) layer diet as compared to a low protein (16%) diet during lay to the crosses that were protein restricted for the first eight weeks of growth significantly increased adult body weight, egg size and shell weight per egg, but did not significantly improve egg production, livability, yolk size or albumen per egg, shell thickness or any egg quality factor investigated. A 17-16-16 protein feeding regime is recommended for the Single Comb White Leghorn egg-type birds. Cross genotypes significantly influenced all traits evaluated

    Treacher Collins syndrome (TCS) is an autosomal dominant disorder with craniofacial abnormality. Typical features are downslanting palpebral fissures, lower eyelid colobomas, microtia, and malar and mandibular hypoplasia. Management requires a multidisciplinary team from birth through adulthood with proper planning, counselling, and surgical techniques.
