    Outcome analysis in patients with multi vessel coronary artery disease and left ventricular ejection fraction after percutaneous coronary intervention: single center experience

    Background: The present study evaluated the changes in baseline left ventricular function and clinical symptoms in multi-vessel coronary artery disease patients after multi-vessel percutaneous coronary intervention.Methods: This was a prospective, observational study conducted at Medical Super-speciality Hospital, Kolkata, India, between August 2017 and August 2019. The study included 48 patients who were diagnosed with ≥2 coronary artery stenosis of ≥50% in native coronary arteries with left ventricular ejection fraction (LVEF) <40%. Echocardiography was performed before and after 3 months of the procedure to observe LVEF. Canadian Cardiovascular Society (CCS) score was calculated before and after 3 months after PCI.Results: Mean age of the patients was 61.89±9.96 years and 89.6% patients were male. Mean LVEF before and after angioplasty was 34.9±4.95% and 42.06±8.78%, respectively (p=0.001). CCS score before and after angioplasty was 2.89 and 1.83, respectively (p=0.001).Conclusions: The results displayed significant improvement in clinical symptoms as well as LVEF after PCI in patients with multi-vessel disease with LVEF <40%. These results will be helpful to conduct larger randomized trials with long term follow-up in order to prove the safety and effectiveness of PCI in such patients over coronary artery bypass grafting

    Structural and functional characterisation of animal adenovirus capsid proteins

Fecha de lectura: 24-10-2014Adenoviruses are disease-causing agents, but are also explored as a model system to decipher mechanisms of molecular biology and used as vectors for gene and cancer therapy trials. Vectors wholly or partially based on animal adenoviruses may be interesting in this respect, both for avoiding pre-existing immunity in humans and the possibility of having novel cell-targeting properties. The capsid of the icosahedral, double-stranded DNA-containing adenovirus maintains the integrity of virus particle, but also has proteins involved in virus entry. The fibre is one such protein and is responsible for primary virus-host interaction. Hence, the homo-trimeric fibre protein is an important factor in adenoviral biology and to successfully exploit these viruses for medical applications. In this thesis work, X-ray crystallographic structures of four animal adenovirus fibre heads have been determined, three of which belong to recently established genera of Adenoviridae, namely Siadenovirus and Atadenovirus. Despite being members of distinct genera and targeting various organisms, the structures revealed overall conserved topology of the central beta-sandwich in their head domains. However, other features of these fibre heads, such as their size, shape and the length of connecting loops show more diversity, which may, in turn, have an impact on their tropism. Structures determined from two atadenoviruses have established their fibre heads as the smallest among all known adenovirus fibre head structures. Both the snake adenovirus 1 fibre head and the lizard adenovirus 2 short fibre heads are compact, with relatively short loops connecting their beta-strands. The presence of a prominent alpha-helix in their structures is also interesting. Turkey adenovirus-3, which exists in avirulent and virulent forms, has a fibre head more similar to reovirus fibre heads than to those of other adenoviruses. The presence of a betahairpin insertion is unique to the structure. Moreover, the striking similarity of turkey adenovirus-3 fibre head with reovirus attachment proteins has strengthened the theory of an evolutionary link between adenoviruses and reoviruses. Sialic acid-containing carbohydrates were identified as in vitro ligands by NMR spectroscopy and calorimetry and co-crystal structures were successfully determined. The structure of ligand-bound turkey adenovirus 3 fibre head revealed the sialic acid binding site located on the side of the trimer. Murine adenovirus 2, like most mammalian adenoviruses and all known human adenoviruses, belongs to the Mastadenovirus genus and could potentially be developed into a model system to study adenovirus pathogenicity. I have determined the high-resolution crystal XVI structure of the fibre head domain of murine adenovirus 2, along with the two directly preceding shaft domain repeats, which shows an overall resemblance to other adenovirus fibre heads of Mastadenovirus genus. Structural comparison with other fibre head structures, however, clearly shows variations that may have functional implications. Nacetylglucosamine, which is common component of glycosaminoglycans such as heparan sulfate and has a wide tissue distribution, has been identified as a ligand for murine adenovirus 2 fibre head in this thesis. In summary, I present new structural and functional findings on distinct adenovirus fibre heads that will be useful in elucidating their tropism and biology. The new structures may also be useful in the design and development of animal adenovirus-based applications

    A case of three year child with uncorrected tetralogy of Fallot for drainage of fronto-parietal abscess

    The Tetralogy Fallot was one of the commonest Right to Left shunting disease in the children. TOF is most common in association with brain abscess (13-70%). This case is of a 3 year old child who had tetralogy of Fallot (uncorrected) and was to be taken for drainage of Fronto-Parietal sub dural abscess via Burr hole. The abscess was drained and patient was shifted with inotropes and ventilator support. In next 24 hours patient improved and supports were gradually withdrawn

    Research Productivity and Visualization of the All India Institute of Medical Sciences (AIIMS) Bhubaneswar during 2012-2019: A Scientometric Approach

    The purpose of the present study is to evaluate the productivity of research at the Indian Institute of Medical Sciences (AIIMS), Bhubaneswar, in the first eight years (2012-2019) of its establishment. In this study, the authors used a number of Scientometric indices to assess research productivity. The results of the study showed that in the selected period of 2019, most research publications appear with an average growth rate of 46.43%. Most of the comments were written by five or more authors. In addition, R.R. Das is recognized as the most prolific author, and the Journal of Clinical and Diagnostic Research is one of the most popular sources of publications, with the largest number of publications with the help of scientists from AIIMS. The UK and AIIMS New Delhi are the countries and institutions that work best together. In addition, the study also found that the Indian Medical Research Council is the leading research institution with AIIMS Bhubaneswar. The title of Maiti R Metronomic Chemotherapy , published in the Journal of Pharmacology and Pharmacotherapeutics, received the most citations. this sentence is long and somewhat complicated. This study is useful for policymakers and stakeholders in medical institutions to improve their research prospects

    Domain of Existence and Uniqueness for Nonlinear Hammerstein Integral Equations

    [EN] In this work, we performed an study about the domain of existence and uniqueness for an efficient fifth order iterative method for solving nonlinear problems treated in their infinite dimensional form. The hypotheses for the operator and starting guess are weaker than in the previous studies. We assume omega continuity condition on second order Frechet derivative. This fact it is motivated by showing different problems where the nonlinear operators that define the equation do not verify Lipschitz and Holder condition; however, these operators verify the omega condition established. Then, the semilocal convergence balls are obtained and the R-order of convergence and error bounds can be obtained by following thee main theorem.     Nanomechanical inhomogeneities in CVA-deposited titanium nitride thin films: Nanoindentation and Finite Element Method Investigations

    Refractory metals that can withstand at high temperatures and harsh conditions are of utmost importance for solar-thermal and energy storage applications. Thin films of TiN have been deposited using cathodic vacuum arc deposition (CVA) at relatively low temperatures ~ 300 oC using the substrate bias ~ -60V. The nanomechanical properties of these films were investigated using nanoindentation and the spatial fluctuations were observed. The nanoindentation results were simulated using finite element method (FEM) through Johnson-Cook model. We have found the local nitridation plays an important role on nanomechanical properties of TiN thin films and confirms that the nitrogen deficient regions are ductile with low yield stress and hardening modulus. This study further opens the opportunities of modelling the nanoscale system using FEM analysis
