    Apexification in Non-Vital Teeth with Immature Roots: Report of Two Cases

    Apexification is a method of inducing apical closure for non-vital immature permanent teeth. During this treatment a mineralized barrier is induced [with long term calcium hydroxide (CH) treatment]; or artificially created [with mineral trioxide aggregate (MTA) plug]. This article describes two cases of apexification in immature necrotic teeth treated with these two different techniques. After 6 years of follow-up, clinical and radiographic control showed that both treatments were successful.

    Heterogeneous seismic anisotropy beneath Madeira and Canary archipelagos revealed by local and teleseismic shear wave splitting

    SUMMARY Mid-plate upward mantle flow is a key component of global mantle convection, but its patterns are poorly constrained. Seismic anisotropy is the most direct way to infer mantle flow as well as melt distribution, yet the convection patterns associated with plume-like mantle upwelling are understudied due to limited seismic data coverage. Here, we investigate seismic anisotropy beneath the Madeira and Canary hotspots using a dense set of shear wave splitting observations and combining teleseismic and local events recorded by three-component broad-band and short-period seismic stations. Using a total of 26 stations in the Madeira archipelago and 43 stations around the Canary Islands, we obtain 655 high-quality measurements that reveal heterogeneous flow patterns. Although local event results are sparse around most islands, we can observe a small average of S-wave splitting times of 0.16 ± 0.01 s, which significantly increase with source depth beneath El Hierro (>20 km) and Tenerife (>38 km) up to 0.58 ± 0.01 and 0.47 ± 0.05 s. This suggests an influence of melt pocket orientation in magma reservoirs developed at uppermost-mantle depths. Likewise, anisotropy increases significantly beneath the islands with shield stage volcanism (up to 9.81 ± 1.78 per cent at El Hierro, western Canaries, against values up to 1.76 ± 0.73 per cent at Lanzarote, eastern Canaries). On average, teleseismic SKS-wave splitting delay times are large (2.19 ± 0.05 s), indicating sublithospheric mantle flow as the primary source for anisotropy in the region. In the Canaries, the western islands show significantly smaller average SKS delay times (1.93 ± 0.07 s) than the eastern ones (2.25 ± 0.11 s), which could be explained by destructive interference above the mantle upwelling. Despite complex patterns of fast polarization directions throughout both regions, some azimuthal pattern across close stations can be observed and related to present-day mantle flow and anisotropy frozen in the lithosphere since before 60 Ma. Additionally, we infer that the current presence of a mantle plume beneath the archipelagos leads to the associated complex, small-scale heterogeneous anisotropy observations

    Evaluation of three different rotary systems during endodontic retreatment: analysis by scanning electron microscopy

    Background: Endodontic therapy is considered a series of important and interdependent steps, and failure of any of these steps may compromise the treatment outcome. This study aimed to evaluate the effectiveness of three different rotary systems in removing obturation materials during endodontic retreatment using scanning electron microscopy (SEM) analysis. Material and Methods: Thirty-six endodontically treated teeth were selected and divided into 3 groups of 10 and 1 control group with 6 dental elements. The groups were divided according to the rotary system used for removing gutta-percha, as follows: G1: ProTaper system; G2: K3 system; G3: Mtwo system; and G4: Control group. Thereafter, the roots were split and the sections were observed under SEM, for analysis and counting of clear dentinal tubules, creating the variable “degree of dentinal tubule patency” (0: intensely clear; 1: moderately clear; 2: slightly clear; 3: completely blocked). The data were subjected to the Friedman and Kruskal-Wallis statistical tests. Results: No differences were observed in the “degree of dentinal tubule patency” neither between the root thirds (to each evaluated group) nor between the groups (to each evaluated third). Nevertheless, when the three root thirds were grouped (providing evaluation of all root extension), the “degree of dentinal tubule patency” was lower in G1 than in G3 ( p <0.05), but showed no differences neither between G1 and G2 nor G2 and G3.     Importance of rotary systems in dental care by undergraduate students in patients of a public health service of Belo Horizonte

    Background: The aim of this study was to evaluate the fracture rates of ProTaper rotary files used in the endodontics extension clinic of the Undergraduate Dentistry Course of the Pontifical Catholic University of Minas Gerais during the prior eight years. Material and Methods: Control record data regarding file usage by students were collected and analyzed by semester. For each period, the total number of patient consultations, the frequency of file use, the occurrence of fractures and the file numbers for which fractures occurred were noted. Descriptive statistics, including frequency of file fractures were calculated for all file types. The chi-square test was used to evaluate differences in the frequency of file fractures between all file types. Results: The study results revealed that during the examined period, there were 1006 consultations and 7993 uses of files. A total of 30 file fractures were recorded throughout this period; thus, fractures occurred in 0.37% of total file uses and 2.98% of all consultations. The most frequently used files were S1, S2 and F1, and these files also accounted for the most fractures. However, no differences in the frequency of file fractures were observed between the file types ( p >0.05). Conclusions: The low fracture rates observed in this study indicate that the examined instruments can be used to safely provide dental care to patients

    Scanning Electron Microscopic Evaluation of Smear Layer Removal Using Isolated or Interweaving EDTA with Sodium Hypochlorite

    Introduction: The aim of this study was to verify the effect of alternating 2.5% sodium hypochlorite (NaOCl) and 17% ethylenediaminetetraacetic acid (EDTA) on the smear layer removal from root canal surfaces. Methods and Materials: A total of 15 single-rooted human teeth, instrumented with ProTaper files, were randomly distributed in 3 groups. In group 1 (n=7) the canals were irrigated with 1 mL of 2.5% NaOCl between files and final irrigation was done with 1 mL of 2,5% NaOCl, followe by 1 mL of 17% EDTA, for a perio of 15 sec with new irrigtion of 1 mL of 2,5% NaOCl at each change of files. In group 3 (control group) (n=1), saline solution was used. All samples were cleaved into two sections, metalized and analyzed under scanning electron microscopy (SEM). The presence or absence of smear layer in the cervical, middle and apical thirds, with scores varying from 1 to 3, respectively were evaluated. The data were submitted to nonparametric Mann-Whitney U test. The level of significance was set at 0.05. Results: It was observed that there was a greater discrepancy between groups with respect to the apical third. In the other areas there was a greater similarity between the scores attributed to the groups. There was a statistically significant difference between the groups only in the apical third, when group 1 presented the higher median (P&lt;0.05). Conclusion: The alternating use of EDTA during instrumentation with NaOCl was the most effective irrigation method to remove the apical smear layer. Both forms of irrigation were effective on removal of the smear layer in the coronal and middle thirds of the canals.Keywords: Root Canal Irrigants; Scanning Electron Microscopy; Smear Laye

    Clinical Approach to Pulp Canal Obliteration: A Case Series

    This article describes four cases with safe and feasible clinical treatment strategies for anterior teeth with pulp canal obliteration (PCO) using cone-beam computed tomography (CBCT), digital radiography (DR), dental operating microscopy (DOM) and ultrasonic tips (US). Four anterior teeth with PCO were chosen. DR was taken with different angulations and analyzed with different filters. Subsequently, the access cavity was performed with the aid of DOM. If the canal was not identified, CBCT was requested. Sagittal and axial slices guided the direction of the ultrasonic tips. After identification of the canal, it was then negotiated and instrumented with the rotary instruments. All four canals were successfully identified, with no complications. In case 1, the canal was identified using DR, DOM and US tips. In cases 2, 3 and 4, the canals were identified with DR,DOM,UStips and CBCT. Complete root canal obliteration identified in radiography did not necessarily mean that pulp tissue was not visible clinically, either. The clinical evaluation of the access cavity with the aid of MO was crucial. If the canal was not identified, CBCT was mandatory in order to show more detailed view of the precise position of the canals, their directions, degrees of obstruction and dimensions.     Guided Endodontics in Nonsurgical Retreatment of a Mandibular First Molar: A New Approach and Case Report

    This case report describes the use of the guided endodontics for a non-surgical endodontic retreatment of the mandibular molar. A 38-year-old female reported apical swelling and localized pain on the tooth #30, exacerbated when chewing hard food. Periapical radiographic examination showed pulp canal obliteration in the apical third associated with extensive radiolucent area. Cone-beam computed tomography (CBCT) images were requested to support the diagnosis and enable preparation of a surgical guide, used to direct access to the canals that were obliterated and incompletely filled. The follow-up at 24 months radiographically showed completely healed apical area in the involved tooth.     The importance of bioceramics and computed tomography in the late clinical management of a horizontal root fracture : a case report

    Root fractures resulting from dental trauma involve dentin, cementum and pulp. The present study aimed to demonstrate the importance of cone-beam computed tomography (CBTC) and bioceramics in the correct planning and intervention of a horizontal root fracture case in tooth 11 with late treatment in an 18-year-old patient. Clinical and radiographic examinations revealed tooth displacement, pain on vertical percussion and images suggestive of a horizontal root fracture. Pulp necrosis was diagnosed and CBTC was requested for treatment planning. Subsequently, endodontic treatment was performed using a bioceramic apical plug. A 2-year follow-up indicated the absence of root resorption and normal periodontal and periapical tissues. It was concluded that endodontic treatment associated with the use of bioceramics and the aid of CBTC is an effective therapeutic option in cases of permanent horizontal root fractures
