    A new antifungal phenolic glycoside derivative, iridoids and lignans from Alibertia sessilis (vell.) k. schum. (Rubiaceae)

    A new antifungal phenolic glycoside, 3,4,5-trimethoxyphenyl-1-O-beta-D-(5-O-syringoyl)apiofuranosyl-(1->6)-beta-D-glucopyranoside (1), together with four known iridoids, geniposidic acid (2), geniposide (3), 6alpha-hydroxygeniposide (4) and 6beta-hydroxygeniposide (5); two lignans, (+)-lyoniresinol-3alpha-O-beta-D-glucopyranoside (6), (-)-lyoniresinol-3alpha-O-beta-D-glucopyranoside (7); and two phenolic acids, chlorogenic (8) and salicylic acids (9) and D-manitol (10), were isolated from the ethanolic extract of the stems of Alibertia sessilis. Structures of 1 and of the known compounds were determined by spectroscopic analysis. All compounds isolated were evaluated for their antifungal activities against two phytopathogenic fungi strains Cladosporium cladosporioides and C. sphaerospermum by direct bioautography

    Transporte Inter-Hospitalar de recém-nascido em estado crítico: revisão integrativa da literatura

    Modelo do estudo: revisão integrativa. Objetivo: identificar condutas organizacionais descritas na literatura sobre transporte inter-hospitalar de recém-nascido em estado crítico de saúde realizado por meio de ambulância. Metodologia: revisão integrativa com consulta nas bases de dados: LILACS, CINAHL, MEDLINE, Web of Science e SCOPUS, no período de setembro a novembro de 2013. Para realizar a pesquisa, foram utilizados os descritores não controlados newborn transport, newborn infants, protocol e os controlados transporte de pacientes, recém-nascido e ambulâncias. A busca limitou-se a artigos publicados em português, inglês e espanhol, sem período estabelecido, que versassem sobre transporte inter-hospitalar de recém-nascido crítico por ambulância. Resultados: selecionou-se 12 artigos. As condutas identificadas foram: comunicação pré-transporte; composição da equipe que realizará o transporte; referência e contrar-referência; seleção do veículo para o transporte; equipamentos e medicações que poderão ser utilizados durante o transporte. Conclusões: Os estudos encontrados versam sobre importantes condutas a serem aplicadas em todas as fases do transporte inter-hospitalar do recémnascido por ambulância, importantes para uma assistência de qualidade e efetivaStudy model: integrative review. Objective: to identify organizational behaviors described in the literature about inter-hospital transport of newborns in critical health condition performed by means of ambulance. Methodology: integrative review with query in the databases: LILACS, CINAHL, MEDLINE, Web of Science and Scopus, during the period from September to November 2013. In order to perform the research, the non-controlled descriptors ‘newborn transport’, ‘newborn infants’ and ‘protocol’ were used, as well as the controlled descriptors ‘transport of patients’, ‘newborn’ and ‘ambulances’. The search was limited to papers published in Portuguese, English and Spanish, without an established period, which addressed the inter-hospital transport of critically ill newborn by ambulance. Results: 12 papers were selected. The identified behaviors were: pre-transport communication; composition of the team that will conduct the transport; reference and counter-reference; selection of vehicle for transportation; equipment and medications that may be used during transportation.     Fatigue Effects in Daily Life Activities of Kidney Transplant Recipients

    After undergoing kidney transplantation, some patients still face one symptom that continues after the dialysis sessions: fatigue (physical and mental tiredness that does not get better after resting). Fatigue effects in the everyday lives of kidney transplant patients can be beneficially modified early by changing this scenario. This is a quantitative study about the intensity and impacts of fatigue in kidney transplant patients admitted to the Hypertension and Kidney Hospital from October 2011 to March 2012. the fatigue pictogram was used to evaluate the level of fatigue interference in the daily life activities of kidney transplant patients. the sample consists of 39 patients, and was developed in 2 phases: data collection and attendance after and before the transplantation until hospital discharge. Descriptive statistical analyses were used. in the group at issue, we have noticed the following profile of the sample: 84.3% of transplantations with live donors, most were men, average age 36.5 years old, average hospitalization time 11.1 days, average time of renal failure 66.4 months, systemic arterial hypertension prevalence 66.7%, and the prevalence of at least 1.8 diseases in each individual. the self-referred causes of chronic renal failure were uncontrolled systemic arterial hypertension, glomerulonephritis, and overuse of anti-inflammatory drugs, among others. the study shows that fatigue is directly related to the level of activities of daily living, causing less ability to perform activities in the higher level of fatigue, which is in the immediate postoperative period and only settling fully on the 9th postoperative day.Universidade Federal de São Paulo, Nursing Sch São Paulo, São Paulo, BrazilUniversidade Federal de São Paulo, Kidney & Hypertens Hosp, São Paulo, BrazilUniversidade Federal de São Paulo, Nursing Sch São Paulo, São Paulo, BrazilUniversidade Federal de São Paulo, Kidney & Hypertens Hosp, São Paulo, BrazilWeb of Scienc

    Quality Of Life Assessment In Children With Cerebral Palsy

    Objectives: Evaluate the QoL of children with CP followed at Núcleo de Tratamento e Estimulação Precoce, in Fortaleza, Ceará, Brazil; identify the most affected domains of the Pediatric Quality of Life Inventory. Methods: This was a cross-sectional, of which target population consisted of parents of children with CP (N = 62). Data collection was carried out by applying the socioeconomic questionnaire and the Health-related Quality of Life tool – HRQoL-PedsQL version 4.0, module applied to parents. Data analysis was performed with SPSS statistical software, version 23.0. Results: The results after transformation of scores into a scale of 0 to 100 in PedsQL, the data showed statistical difference between QOL domains except for physical capacity and social aspect and in descending order of impairment, the domains were related to school activity, the emotional aspect, physical capacity and the social aspect. Conclusions: Given the above, to attain improvement in QoL indicators, it is reasonable to propose interdisciplinary intervention among healthcare professionals in order to develop improved strategies aiming to promote early integration of children with CP in schools, to obtain significant results

    The study elaborated approaches for adequated use of the surgical scrub and it evaluated, through systematized observation, the performance of the surgical team during the surgery. The adopted procedure was very valid, allowing to recognize positive and problematic aspects. Except the protecting glasses, all the other components of the surgical scrub were used. The subject located in the use form, where caps and masks were more problematic among anesthesists and circulate nurses; apron and gloves among surgeons and, mainly, scrub nurses.O estudo elaborou critérios para uso adequado da paramentação cirúrgica e avaliou, por observação sistematizada, o desempenho da equipe cirúrgica durante a cirurgia. O procedimento adotado foi válido, permitindo reconhecer aspectos positivos e problemáticos. A exceção do óculos protetor, todos os outros componentes da paramentação foram utilizados. A questão se situou na forma de utilização, onde gorros e máscaras foram mais problemáticos entre anestesistas e circulantes; avental e luvas entre cirurgiões e, principalmente, instrumentadores

    O estudo elaborou critérios de avaliação dos componentes da paramentação cirúrgica de hospitais do município de São Paulo. A maior dificuldade nessa elaboração deveu-se à inexistência de normas e consenso sobre a porosidade dos tecidos como barreira microbiológica e de contato com sangue e outros fluidos. Contudo, foi possível reconhecer aspectos positivos e problemáticos. Observou-se que existe semelhanças dos componentes entre os hospitais, no que se refere a modelo e tipo de tecido. As luvas foram os componentes com maior índice de adequação e os propés, os de maior índice de inadequação. O aspecto mais problemático referiu-se aos controles de aquisição e de reprocessamento dos componentes re-utilizáveis.The study elaborated evaluation patterns of the components of the surgical scrub in hospitals of the district of São Paulo. The most important difficulty was the lack of rules and consent on the types of the fabrics, as a microbiological and contact with blood and other fluids barrier. However, it was possible to identify positive and problematic aspects. It was observed that the components are similar among the hospitals, considering the design and the way of fabrics production. Gloves were the ones with the best quality results, while shootweares were the worst. The most problematic aspect was referred to the acquisition controls and the re-use process of the components

    Evaluation of Glycemic and Lipid Profile of Offspring of Diabetic Wistar Rats Treated with Malpighia emarginata Juice

    Knowing that maternal diabetes is related to hyperglycemia and fetal hyperinsulinemia, which affect the lipid metabolism, the aim of this study was to evaluate the effects of Malpighia emarginata (acerola) juice on the glycemic and lipid profile of offspring of diabetic and nondiabetic Wistar rats. The adult offspring of non-diabetic dams and of dams with severe streptozotocin-induced diabetes were divided into groups: G1, offspring (of control dams) treated with water, G2, offspring (of diabetic dams) treated with water, G3, male offspring (of control dams) treated with acerola juice, and G4, male offspring (of diabetic dams) treated with acerola juice. The offspring of diabetic dams treated with acerola juice showed significantly decreased levels of glucose, cholesterol, triglycerides, and increased HDL-c. The use of acerola juice is a potential strategy to aid in the prevention of DM and dyslipidemia and its complications or to act as an auxiliary in the treatment of these diseases

    Normal weight obesity is associated with metabolic syndrome and insulin resistance in young adults from a middle-income country

    Objective: This population-based birth cohort study examined whether normal weight obesity is associated with metabolic disorders in young adults in a middle-income country undergoing rapid nutrition transition. Design and Methods: The sample involved 1,222 males and females from the 1978/79 Ribeira˜o Preto birth cohort, Brazil, aged 23–25 years. NWO was defined as body mass index (BMI) within the normal range (18.5–24.9 kg/m2) and the sum of subscapular and triceps skinfolds above the sex-specific 90th percentiles of the study sample. It was also defined as normal BMI and % BF (body fat) .23% in men and .30% in women. Insulin resistance (IR), insulin sensitivity and secretion were based on the Homeostasis Model Assessment (HOMA) model. Results: In logistic models, after adjusting for age, sex and skin colour, NWO was significantly associated with Metabolic Syndrome (MS) according to the Joint Interim Statement (JIS) definition (Odds Ratio OR = 6.83; 95% Confidence Interval CI 2.84–16.47). NWO was also associated with HOMA2-IR (OR = 3.81; 95%CI 1.57–9.28), low insulin sensitivity (OR = 3.89; 95%CI 2.39–6.33), and high insulin secretion (OR = 2.17; 95%CI 1.24–3.80). Significant associations between NWO and some components of the MS were also detected: high waist circumference (OR = 8.46; 95%CI 5.09–14.04), low High Density Lipoprotein cholesterol (OR = 1.65; 95%CI 1.11–2.47) and high triglyceride levels (OR = 1.93; 95%CI 1.02–3.64). Most estimates changed little after further adjustment for early and adult life variables. Conclusions: NWO was associated with MS and IR, suggesting that clinical assessment of excess body fat in normal-BMI individuals should begin early in life even in middle-income countries
