    Symptomatic giant left atrial aneurysm in a child : a rare entity

    Isolated left atrial aneurysms are rare entities in clinical practice. Usually the condition is diagnosed in the second to fourth decades of life. The presence of such lesions in the pediatric age group is scantily described. We present a 2 year boy who presented with complaints of excessive irritability, respiratory distress and swelling of the feet. On examination, child was tachypnoeic with irregularly irregular rhythm. Echo showed a huge aneurysmal LA appendage with severe left ventricle dysfunction. The child underwent surgical resection for same. Findings were confirmed intraoperatively but he continued to have low cardiac output state after the surgery, with frequent arrhythmias and expired on day 7 of surgery. The case is reviewed and compared with the available English literature.peer-reviewe

    Visual detection of Brucella in bovine biological samples using DNA-activated gold nanoparticles

    Brucellosis is a bacterial disease, which, although affecting cattle primarily, has been associated with human infections, making its detection an important challenge. The existing gold standard diagnosis relies on the culture of bacteria which is a lengthy and costly process, taking up to 45 days. New technologies based on molecular diagnosis have been proposed, either through dip-stick, immunological assays, which have limited specificity, or using nucleic acid tests, which enable to identify the pathogen, but are impractical for use in the field, where most of the reservoir cases are located. Here we demonstrate a new test based on hybridization assays with metal nanoparticles, which, upon detection of a specific pathogen-derived DNA sequence, yield a visual colour change. We characterise the components used in the assay with a range of analytical techniques and show sensitivities down to 1000 cfu/ml for the detection of Brucella. Finally, we demonstrate that the assay works in a range of bovine samples including semen, milk and urine, opening up the potential for its use in the field, in low-resource settings

    Total body irradiation in a case of thalassemia major with source to axis distance based planning: A case report

    Background: The use of total body radiation (TBI) before hematopoietic stem cell transplantation (HSCT) would increase the engraftment without transplant-related morbidity or mortality among Thalassemia major (TM) cases. Case presentation: A 2-year-old female child, diagnosed with TM was scheduled for haploidentical allogenic transplant-based protocol, and after that, based on protocol she was scheduled to undergo a single session of TBI as a conditioning regimen before haploidentical allogenic hematopoietic stem cell tranplant. A total dose of 4 Gy was administered.. The incidence of graft failure was reduced as TBI was used before allogeneic stem cell transplantation. TBI provided a uniform dose of radiation to the entire body, penetrating areas such as the central nervous system (CNS) and testes. Conclusion: Total Body Irradiation with the SAD technique is the most effective way of treatment. As it is comfortable for the patient to undergo, easily reproducible, and it helps to achieve a uniform dose distribution

    Clinical application of acellular matrix derived from the bubaline diaphragm and caprine rumen for the repair of abdominal wall defects in animals

    The abdominal wall hernias resulting due to trauma or other clinical conditions are common in animals. Large hernias required the use of synthetic mesh, which is costly and may result in infection, fistula formation, and pain. Application of biomaterials in hernia repair causes a reduction in pain, reduced recovery time, and rate of recurrence. The study was undertaken to test the acellular bubaline diaphragm matrix (BDiaM) and acellular caprine rumen matrix (CRuM) for the repaired hernia in clinical cases. Fresh bubaline diaphragm and caprine rumen were decellularized using sodium deoxycholate (1% for CRuM and 2% for BDiaM) for 48h. Acellularity was ascertained histologically and by DNA quantification. Histologically, both the matrices showed complete acellularity and orderly arranged collagen fibers after 48 h. The DNA contents were significantly (P0.05) reduced in both the matrices in comparison to the native matrices. The BDiaM and CRuM matrices were applied in eight and nine clinical cases of abdominal wall defects, respectively. Animals with BDiaM and CRuM matrices recovered uneventfully and remained sound at least up to 3 months. Hematological and immunological findings were unremarkable.     The objective of the study was to investigate the effect of supplementation of a supranutritional dose of zinc sulfate (ZnSO4) and Bacillus firmus derived bio-response modifier (BRM) on growth, blood neutrophil functions, pro-inflammatory and anti-inflammatory cytokine responses in early weanling piglets. In total, 45 piglets (age of 19.25 ± 0.84 days) were randomly divided into five groups: I (basal diet only), II (basal diet supplemented with ZnSO4), III (basal diet supplemented with BRM), IV (basal diet supplemented with ZnSO4 plus BRM) and V (basal diet without weaning from dam). The production of myeloperoxidase (MPO) and superoxide anion (O2-), and the concentration of transforming growth factor-β1 (TGF-β1) were markedly reduced, whereas the concentrations of intercellular adhesion molecule 1 (ICAM1), and monocyte chemoattractant protein 1 (MCP1) were significantly (P0.05) but a marked increase (P0,05), ali kod prasadi iz skupine II. i III. je 14. i 7. dana opaženo znakovito povišenje (P<0,05) ICAM1, MCP1, te sniženje koncentracije TGF-β1. U prasadi iz skupine IV dodavanje BRM i ZnSO4 u osnovnom obroku poboljšalo je MPO (2. dan) i O2- (7. dan), bez znakovitih promjena u rastu i koncentraciji citokina. Na kraju, zaključeno je da dodavanje kombinacije BRM i ZnSO4 u obroku potiče urođenu imunost prasadi što nije slučaj kada se ZnSO4 ili BRM dodaju zasebno. Rezultati ovog istraživanja pomoći će u formuliranju učinkovitog upravljanja hranidbom kod rano odbijene prasadi u krdu svinja

    Females with paired occurrence of cancers in the UADT and genital region have a higher frequency of either Glutathione S-transferase M1/T1 null genotype

    Upper Aero digestive Tract (UADT) is the commonest site for the development of second cancer in females after primary cervical cancer. Glutathione S-transferase (GSTM1 and / or T1) null genotype modulates the risk of developing UADT cancer (primary as well as second cancer). The aim of this study was to evaluate the difference in GST null genotype frequencies in females with paired cancers in the UADT and genital region as compared to females with paired cancers in the UADT and non-genital region. Forty-nine females with a cancer in the UADT and another cancer (at all sites-genital and non-genital) were identified from a database of patients with multiple primary neoplasms and were analyzed for the GSTM1 and T1 genotype in addition to known factors such as age, tobacco habits, alcohol habits and family history of cancer. Frequencies of GSTM1 null, GSTT1 null, and either GSTM1/T1 null were higher in females with paired occurrence of cancer in the UADT and genital site (54%, 33% and 75% respectively) in comparison to females with paired occurrence of cancer in the UADT and non-genital sites (22%, 6% and 24% respectively). The significantly higher inherited frequency of either GSTM1/T1 null genotype in females with a paired occurrence of cancers in UADT and genital region (p = 0.01), suggests that these females are more susceptible to damage by carcinogens as compared to females who have UADT cancers in association with cancers at non-genital sites

    Abstract- An intrusion detection system framework for mobile wireless network is designed to support heterogeneous network environments to identify intruders at its best. The landscape of network security has drastically changed due to the rapid increase of wireless networks and mobile computing applications. Firewalls and encryption software methods have now become outdated for securing networks and are no longer sufficient and effective. There is a requirement for new architecture and mechanism to protect the wireless networks and mobile computing application. In this paper, we will be examining the openness of wireless networks and put up an argument for inclusion of intrusion detection in the security architecture for mobile computing environment.
