Spontaneous breaking of time reversal symmetry in strongly interacting two dimensional electron layers in silicon and germanium
We report experimental evidence of a remarkable spontaneous time reversal
symmetry breaking in two dimensional electron systems formed by atomically
confined doping of phosphorus (P) atoms inside bulk crystalline silicon (Si)
and germanium (Ge). Weak localization corrections to the conductivity and the
universal conductance fluctuations were both found to decrease rapidly with
decreasing doping in the Si:P and Ge:P layers, suggesting an effect
driven by Coulomb interactions. In-plane magnetotransport measurements indicate
the presence of intrinsic local spin fluctuations at low doping, providing a
microscopic mechanism for spontaneous lifting of the time reversal symmetry.

Serum proteins and paraproteins in women with silicone implants and connective tissue disease: a case–control study
Prior studies have suggested abnormalities of serum proteins, including paraproteins, in women with silicone implants but did not control for the presence of connective-tissue disease (CTD). This retrospective case–control study, performed in tertiary-care academic centers, assessed possible alterations of serum proteins, including paraproteins, in such a population. Seventy-four women with silicone implants who subsequently developed CTD, and 74 age-matched and CTD-matched women without silicone implants, were assessed in the primary study; other groups were used for additional comparisons. Routine serum protein determinations and high-sensitivity protein electrophoresis and immunofixation electrophoresis were performed for detection of paraproteins. Women with silicone implants, either with or without CTD, had significantly lower serum total protein and α1-globulin, α2-globulin, β-globulin, γ-globulin, and IgG levels compared with those without silicone implants. There was no significant difference, however, in the frequency of paraproteinemia between women with silicone implants and CTD (9.5%) and age-matched and CTD-matched women without silicone implants (5.4%) (odds ratio, 1.82; 95% confidence interval, 0.51–6.45). Paraprotein isotypes were similar in the two groups, and the clinical characteristics of the 13 women with paraproteinemia were comparable with an independent population of 10 women with silicone breast implants, CTD, and previously diagnosed monoclonal gammopathies. In summary, this first comprehensive study of serum proteins in women with silicone implants and CTD found no substantially increased risk of monoclonal gammopathy. Women with silicone implants, however, had unexpectedly low serum globulin and immunoglobulin levels, with or without the subsequent development of CTD. The causes and clinical implications of these findings require further investigation
Counterpropagating topological and quantum Hall edge channels
The survival of the quantum spin Hall edge channels in presence of an
external magnetic field has been a subject of experimental and theoretical
research. The inversion of Landau levels that accommodates the quantum spin
Hall effect is destroyed at a critical magnetic field, and a trivial insulating
gap appears in the spectrum for stronger fields. In this work, we report the
absence of this transport gap in disordered two dimensional topological
insulators in perpendicular magnetic fields of up to 16 T. Instead, we observe
that a topological edge channel (from band inversion) coexists with a
counterpropagating quantum Hall edge channel for magnetic fields at which the
transition to the insulating regime is expected. For larger fields, we observe
only the quantum Hall edge channel with transverse resistance close to .
By tuning the disorder using different fabrication processes, we find evidence
that this unexpected plateau originates from extended quantum Hall edge
channels along a continuous network of charge puddles at the edges of the

The utility of rapid antigen detection testing for the diagnosis of streptococcal pharyngitis in low-resource settings

Thermo Biostar donated the STREP A OIA MAX rapid test kits for use in this study free of charge.Objectives: To evaluate the utility of rapid antigen detection testing (RADT) for the diagnosis of group A streptococcal (GAS) pharyngitis in pediatric outpatient clinics in four countries with varied socio-economic and geographic profiles. Methods: We prospectively evaluated the utility of a commercial RADT in children aged 2-12 years presenting with symptoms of pharyngitis to urban outpatient clinics in Brazil, Croatia, Egypt, and Latvia between August 2001 and December 2005. We compared the performance of the RADT to culture using diagnostic and agreement statistics, including sensitivity, specificity, and positive and negative predictive values. The Centor scores for GAS diagnosis were used to assess the potential effect of spectrum bias on RADT results. Results: Two thousand four hundred and seventy-two children were enrolled at four sites. The prevalence of GAS by throat culture varied by country (range 24.5-39.4%) and by RADT (range 23.9-41.8%). Compared to culture, RADT sensitivity ranged from 72.4% to 91.8% and specificity ranged from 85.7% to 96.4%. The positive predictive value ranged from 67.9% to 88.6% and negative predictive value ranged from 88.1% to 95.7%. Conclusions: In limited-resource regions where microbiological diagnosis is not feasible or practical, RADTs should be considered an option that can be performed in a clinic and provide timely results.publishersversionPeer reviewe
Contemporary themes in the design of AT for the ageing population: materiality, co-design and cultural influences
Products we purchase are much more than artefacts that fulfil functional needs in our life. We have grown to enact our consumer choices, even those regarding fast moving consumable goods, with careful considerations informed by numerous trials, recommendations and, growingly, environmental concerns in mind. Advanced manufacturing and progress in research and development are providing more choices for consumers even in quite specific and complex product markets. An exemption to this market trend is represented by assistive technologies (ATs). This is a relatively underdeveloped context despite the growing demands for assistive devices by those in later life who need either support in accomplishing everyday life to stay independent or have complex co-occurring conditions. In this chapter, we explore why ATs, especially for older adults, are underdeveloped by exploring issues related to design approaches and cultural and social perceptions that have contributed to making consumers more or less sensitive and demanding towards the role of ATs in their lives. The chapter will conclude with recommendations that may be able to shift the perception of assistive devices so as to facilitate the user’s emotional investment in the devices, attachment to them, which, in return, may lead to better adherence and faster adoption
Treatment of streptococcal pharyngitis with once-daily amoxicillin versus intramuscular benzathine penicillin g in low-resource settings : A randomized controlled trial
Background: Primary prevention of acute rheumatic fever is achieved by proper antibiotic treatment of group A β -hemolytic streptococcal (GAS) pharyngitis. Methods: To assess noninferiority of oral amoxicillin to intramuscular benzathine penicillin G (IM BPG). Children (2 to 12 years) meeting enrollment criteria were randomized 1:1 to receive antibiotic treatment in 2 urban outpatient clinics in Egypt and Croatia. Results: A total of 558 children (Croatia = 166, Egypt = 392) were randomized, with 368 evaluable in an intention-to-treat (ITT) analysis, and 272 evaluable in the per protocol (PP) analysis. In Croatia, ITT and PP treatment success rates were comparable for IM BPG and amoxicillin (2.5% difference vs 1.1% difference, respectively). In Egypt, amoxicillin was not comparable with IM BPG in ITT analysis (15.1% difference), but was comparable in PP analysis (-9.3% difference). Conclusion: If compliance is a major issue, a single dose of IM BPG may be preferable for treatment of GAS pharyngitis.publishersversionPeer reviewe

Molecular beam epitaxy of highly crystalline GeSnC using CBr4 at low temperatures
Tensile-strained pseudomorphic Ge1–x–ySnxCy was grown on GaAs substrates by molecular beam epitaxy using carbon tetrabromide (CBr4) at low temperatures (171–258 °C). High resolution x-ray diffraction reveals good crystallinity in all samples. Atomic force microscopy showed atomically smooth surfaces with a maximum roughness of 1.9 nm. The presence of the 530.5 cm−1 local vibrational mode of carbon in the Raman spectrum verifies substitutional C incorporation in Ge1–x–ySnxCy samples. X-ray photoelectron spectroscopy confirms carbon bonding with Sn and Ge without evidence of sp2 or sp3 carbon formation. The commonly observed Raman features corresponding to alternative carbon phases were not detected. Furthermore, no Sn droplets were visible in scanning electron microscopy, illustrating the synergy in C and Sn incorporation and the potential of Ge1–x–ySnxCy active regions for silicon-based lasers.The authors acknowledge support from the National Science

A search for W bb and W Higgs production in ppbar collisions at sqrt(s)=1.96 TeV
We present a search for W b \bar{b} production in p \bar{p} collisions at
sqrt{s}=1.96 TeV in events containing one electron, an imbalance in transverse
momentum, and two b-tagged jets. Using 174 pb-1 of integrated luminosity
accumulated by the D0 experiment at the Fermilab Tevatron collider, and the
standard-model description of such events, we set a 95% C.L. upper limit on W b
\bar{b}WH--$135
