On the Flow-level Dynamics of a Packet-switched Network
The packet is the fundamental unit of transportation in modern communication
networks such as the Internet. Physical layer scheduling decisions are made at
the level of packets, and packet-level models with exogenous arrival processes
have long been employed to study network performance, as well as design
scheduling policies that more efficiently utilize network resources. On the
other hand, a user of the network is more concerned with end-to-end bandwidth,
which is allocated through congestion control policies such as TCP.
Utility-based flow-level models have played an important role in understanding
congestion control protocols. In summary, these two classes of models have
provided separate insights for flow-level and packet-level dynamics of a
network
User involvement in healthcare technology development and assessment: Structured literature review
Purpose – Medical device users are one of the principal stakeholders of medical device technologies. User involvement in medical device technology development and assessment is central to meet their needs.
Design/methodology/approach – A structured review of literature, published from 1980 to 2005 in peer-reviewed journals, was carried out from social science perspective to investigate the practice of user involvement in the development and assessment of medical device technologies. This was followed by qualitative thematic analysis.
Findings – It is found that users of medical devices include clinicians, patients, carers and others. Different kinds of medical devices are developed and assessed by user involvement. The user involvement occurs at different stages of the medical device technology lifecycle and the degree of user involvement is in the order of design stage > testing and trials stage > deployment stage > concept stage. Methods most commonly used for capturing users’ perspectives are usability tests, interviews and questionnaire surveys.
Research limitations/implications – We did not review the relevant literature published in engineering, medical and nursing fields, which might have been useful.
Practical implications – Consideration of the users’ characteristics and the context of medical device use is critical for developing and assessing medical device technologies from users’ perspectives.
Originality/value – This study shows that users of medical device technologies are not homogeneous but heterogeneous, in several aspects, and their needs, skills and working environments vary. This is important consideration for incorporating users’ perspectives in medical device technologies.

Medical device technologies: Who is the user?
A myriad of medical devices deployed by many users play an essential role in healthcare, and they, and their users, need to be defined, classified and coded effectively. This study provides definitions of terms frequently employed to describe the users of medical device technologies (MDT) as well as a classification of such users. Devices are widely used, developed and assessed by many others than clinicians. Thus, users of medical devices need to be classified in various relevant ways, such as primary and secondary users; user groups such as healthcare professionals, patients, carers, persons with disabilities, those with special needs, as well as professionals allied with healthcare. Proper definition and classification of MDT users is particularly important for integrating the users’ perspectives in the process of MDT development and assessment, as well as in relation to the regulatory, health and safety, and insurance perspectives concerning MDT

Benefits of and barriers to involving users in medical device technology development and evaluation
Objectives: This study investigated the benefits of, and barriers to user involvement in medical device technology development and evaluation. Methods: A structured review of published literature in peer-reviewed journals.
Results: This review revealed that the main benefits of user involvement were an increased access to user needs, experiences and ideas; improvements in medical device designs and user interfaces; and an increase in the functionality, usability and quality of the devices. On the other hand, resource issues, particularly those of time and money were found the key impediments to involving users in the development and evaluation of medical device technologies. This study has categorised both the benefits and barriers to user involvement also.
Conclusions: The involvement of users in MDTD&E requires resources, which are limited; however, it is essential from both users and manufacturers perspectives
Sequence determination of a satellite RNA isolated from Aspergillus foetidus
Aspergillus foetidus virus (AfV) has at least two distinct particle types, designated as AfV-fast (F) and AfV-slow (S). AfV-S includes AfV-S1, a victorivirus; AfV-S2, an unclassified satellite RNA; and AfV-S3, a previously uncharacterized dsRNA element. Here, we describe the complete sequence of AfV-S3, which is a short non-coding RNA with no known homologs. AfV-S3 is predicted to form an extended secondary structure, shares a 5’ terminus with AfV-S2, and is a satellite RNA possibly dependent on both AfV-S1 and AfV-S2. Freight Rates and Productivity Gains in British Tramp Shipping 1869-1950
The standard source for pre-WWII global freight rate trends is the Isserlis British tramp shipping index. We think it is flawed, and that its sources offer vastly more information than the Isserlis aggregate contains. The new data confirm the precipitous decline in nominal freight rates before the World War I, but it also extends the series to the 1940s. Furthermore, our new series is linked to the post-World War II era (documented by David Hummels), so that we can be more precise about what has happened over the very long run. We also create route-specific deflators by using the prices of commodities transported. Previous scholars have deflated their nominal freight rate indices by a price index that includes tradables not carried on all routes and non-tradables not carried on any route. Our deflated indices offer a more effective measure of the contribution of declining freight rates to commodity-price convergence across trading regions. Using the pricedual method and new indices for factor prices, we then calculate total factor productivity growth pre-war and interwar for five global routes. Finally, we identify the sources of the total factor productivity growth.
Prevalence and association of obesity with self-reported comorbidity: a cross-sectional study of 1321 adult participants in Lasbela, Balochistan
Association of fatness with chronic metabolic diseases is a well-established fact, and a high prevalence of risk factors for these disorders has increasingly been reported in the third world. In order to incorporate any preventive strategies for such risk factors into clinical practice, decision-makers require objective evidence about the associated burden of disease. A cross-sectional study of 1321 adults from one of the districts of Balochistan, among the most economically challenged areas of Pakistan, was carried out for the measures of fatness and self-reported comorbidities. Body mass index (BMI), waist circumference (WC), and waist-to-hip ratio (WHR) were measured and demographic information and self-reported comorbidities were documented.The prevalence of obesity was 4.8% (95% CI: [3.8, 6.1]) and 21.7% (95% CI: [19.5, 24.0]), as defined by the World Health Organization (WHO) international and Asia/Asia-Pacific BMI cut-offs, respectively. The proportion exhibiting comorbidity increased with increasing levels of fatness in a dose-response relationship

The Only Eye Study (OnES): a qualitative study of surgeon experiences of only eye surgery and recommendations for patient safety
OBJECTIVE: Performing surgery on patients with only one seeing-eye, where complications may result in catastrophic vision loss, presents unique challenges for the ophthalmic care team. There is currently no evidence regarding how surgeons augment their care when treating only eye patients and no guidelines for how these patients should be managed in hospital eye services. This study aimed to explore ophthalmic surgeons' experiences of only eye surgery and perceptions of current practice.
DESIGN AND PARTICIPANTS: Ten ophthalmic surgeons were asked to relate their experiences and views on performing only eye surgery in indepth, semistructured interviews. Interviews were audio-recorded and transcribed. Qualitative data were subjected to thematic analysis to identify key themes.
SETTING: Hospital eye service.
RESULTS: Five key themes emerged relating to surgeons' experiences and perceptions of only eye surgery: (1) differences in approach to consent, (2) strategies for risk reduction, (3) unmet training needs, (4) value of surgical mentor and (5) emotional impact of unsuccessful outcomes. Recommendations for improving the surgical journey for both the patient and the surgeon related primarily to better recognition and understanding of the complexities inherent with only eye surgery.
CONCLUSIONS: Outcomes of only eye surgery may be improved through a number of methods, including development of purpose-designed training fellowships, adoption of stress-reducing strategies and enhancement of available support services. The findings identify emerging themes unique to only eye surgery and the need for guidelines on the provision of care for these high-stakes surgical patients
THE PHYSIOLOGICAL RESPONSE OF CARDIORESPIRATORY FITNESS PARAMETERS TO EXERCISE IN PREDIABETIC POPULATION: AN EXPERIMENTAL PRE- POST DESIGN
Background: The cardiorespiratory fitness in prediabetic population at pre- and post-interventional stage after 8 weeks of moderate intensity exercise was determined. This study is part of PhD project and carried out in Physiology department, Institute of Basic Medical Sciences, Khyber Medical University, Peshawar, Pakistan. Methods: It was an experimental study design. Adult prediabetics (n=50), 22 females and 28 males of 18 to 35y age group were included in the study. Diagnosis of prediabetes was made with HbA1c falling in the range of 5.7– 6.4%, and fasting blood glucose (100-125mg/dL). Cardiorespiratory fitness parameters (Ventilation, oxygen consumed during exercise VO2, carbon dioxide exhaled VCO2, metabolic equivalents (ME), heart rate (HR), heart rate reserve (HRR), rate of carbohydrate oxidation (RCHO), fat oxidation (RFO) and energy expenditure (EE)) were determined at pre- and post-exercise intervention using breath-by-breath analyzer. The participants performed moderate exercise protocol of 30 min with HRmax% of 70 ± 5% for 5 days a week for 8 weeks during their leisure time, monitored with pedometers. Results: The results showed a significant improvement in cardiorespiratory fitness parameters at post exercise analysis. Similar changes were observed for fasting blood glucose (P value < 0.001) and HbA1c (P value < 0.001).Conclusion: Moderate physical activity for 8 weeks showed significant Improvement in cardiorespiratory fitness parameters and glycemic status of patients with prediabetes
Electro-gravimetric recovery of silver from aqueous solutions and its precursors
A simple electrolytic cell was used for electrochemical recovery of silver from aqueous solutions containing 100 mg/L Ag(I). Two different sets of electrodes were applied to find the enhanced recovery of silver. Rocks and ores samples were processed through fire assay method and acid digestion. A set of electrodes comprised of stainless steel anode and aluminum cathode gave maximum recovery (96.5%) of silver. This simple, robust, environment friendly and highly sensitive method was effectively applied to various ores and rock samples.
