    Exercise recommendations for people with bone metastases: Expert consensus for healthcare providers and clinical exercise professionals

    Purpose: Exercise has been underutilized in people with advanced or incurable cancer despite the potential to improve physical function and reduce psychosocial morbidity, especially for people with bone metastases because of concerns over skeletal complications. The International Bone Metastases Exercise Working Group (IBMEWG) was formed to develop best practice recommendations for exercise programming for people with bone metastases on the basis of published research, clinical experience, and expert opinion. Methods: The IBMEWG undertook sequential steps to inform the recommendations: (1) modified Delphi survey, (2) systematic review, (3) cross-sectional survey to physicians and nurse practitioners, (4) in-person meeting of IBMEWG to review evidence from steps 1-3 to develop draft recommendations, and (5) stakeholder engagement. Results: Recommendations emerged from the contributing evidence and IBMEWG discussion for pre-exercise screening, exercise testing, exercise prescription, and monitoring of exercise response. Identification of individuals who are potentially at higher risk of exercise-related skeletal complication is a complex interplay of these factors: (1) lesion-related, (2) cancer and cancer treatment–related, and (3) the person-related. Exercise assessment and prescription requires consideration of the location and presentation of bone lesion(s) and should be delivered by qualified exercise professionals with oncology education and exercise prescription experience. Emphasis on postural alignment, controlled movement, and proper technique is essential. Conclusion: Ultimately, the perceived risk of skeletal complications should be weighed against potential health benefits on the basis of consultation between the person, health care team, and exercise professionals. These recommendations provide an initial framework to improve the integration of exercise programming into clinical care for people with bone metastases

    Economic Evaluation of a Geriatric Oncology Clinic

    Geriatric assessment (GA) is supported by recent trials and guidelines yet rarely implemented due to a lack of resources. We performed an economic evaluation of a geriatric oncology clinic. Pre-GA proposed treatments and post-GA actual treatments were obtained from a detailed chart review of patients seen at a single academic centre. GA-based costs for investigations and referrals were calculated. Unit costs were obtained for surgical, radiation, systemic therapy, laboratory, imaging, physician, nursing, and allied health care (all in 2019 Canadian dollars). A six-month time horizon and government payer perspective were used. Consecutive patients aged 65 years or older (n = 152, mean age 82 y) and referred in the pre-treatment setting between July 2016 and June 2018 were included. Treatment plans were modified for 51% of patients. Costs associated with planned treatment were CAD 3,655,015. Costs associated with GA and related interventions were CAD 95,798. Final treatment costs were CAD 2,436,379. Net savings associated with the clinic were CAD 1,122,837, or CAD 7387 per patient seen. Findings were robust in multiple sensitivity analyses. Combined with mounting trial data demonstrating the clinical benefits of GA, our data can inform a strong business case for geriatric oncology clinics in health care environments similar to ours, but additional studies in diverse health care settings are warranted

    Differential survival improvement for patients 20-29 years of age with acute lymphoblastic leukemia

    Objective: To compare improvement in survival from 1986 to 2009 for acute lymphoblastic leukemia (ALL) patients 1-14, 15-19 and 20-29 years in Ontario and United States.Methods: This population-based analysis used data from Ontario Cancer Registry (OCR) and Surveillance Epidemiology and End Results (SEER).Results: In OCR, there was steady improvement in survival by period of diagnosis in all three age groups. In SEER, there was no improvement in survival for patients aged 20-29 years.Conclusions: Survival in Ontario and the United States has improved for patients with ALL aged 1-19 years. However, survival has improved among patients aged 20-29 years only in Ontario

    Geriatric Assessment and Management in Cancer

    As the majority of patients with cancer are older than 65 years, most medical, surgical, and radiation oncologists treat a large number of older adults in their clinical practice. Older adults are a heterogeneous group of patients, in terms of physiologic reserves, comorbidity, and geriatric conditions such as cognitive impairment and disability. Comorbidities become increasingly common as people age, as does frailty (limited physiologic reserves that increase the risk of negative outcomes).1 Neither chronological age alone nor performance status does justice to characterizing this heterogeneity. Furthermore, since older patients are under-represented in clinical cancer trials, especially if they are frail with comorbidity or functional dependency, traditional treatment algorithms may not be applicable.2,3 As a result, treatment decisions in older adults with cancer are not straightforward
