    Partial rupture of the quadriceps muscle in a child

    <p>Abstract</p> <p>Background</p> <p>The quadriceps femoris muscle ruptures usually occur in the middle-aged population. We present a 4-year-old patient with partial rupture of the quadriceps femoris muscle. To our knowledge, this is the youngest patient reported with a quadriceps femoris muscle rupture.</p> <p>Case Presentation</p> <p>A 4-year-old girl admitted to our clinic with left knee pain and limitation in knee movements. Her father reported that she felt pain while jumping on sofa. There was no direct trauma to thigh or knee. We located a palpable soft tissue swelling at distal anterolateral side of thigh. The history revealed that 10 days ago the patient was treated for upper tract respiratory infection with intramuscular Clindamycin for 7 days. When we consulted the patient with her previous doctor and nurse, we learnt that multiple daily injections might be injected to same side of left thigh. MRI showed a partial tear of vastus lateralis muscle matching with the injection sites. The patient treated with long leg half-casting for three weeks. Clinical examination and knee flexion had good results with conservative treatment.</p> <p>Conclusions</p> <p>Multiple intramuscular injections may contribute to damage muscles and make prone to tears with muscle contractions. Doctors and nurses must be cautious to inject from different parts of both thighs.</p

    On the feasibility of a channel-dependent scheduling for the SC-FDMA in 3GPP-LTE (mobile environment) based on a prioritized-bifacet Hungarian method

    We propose a methodology based on the prioritization and opportunistic reuse of the optimization algorithm known as Hungarian method for the feasible implementation of a channel-dependent scheduler in the long-term evolution uplink (single carrier frequency division multiple access system). This proposal aims to offer a solution to the third generation system’s constraint of allocating only adjacent subcarriers, by providing an optimal resource allotment under a fairness scheme. A multiuser mobile environment following the third generation partnership project TS 45.005v9.3.0/25.943v9.0.0 was also implemented for evaluating the scheduler’s performance. From the results, it was possible to examine the channel frequency response for all users (four user equipments) along the whole bandwidth, to visualize the dynamic resource allocation for each of the 10,000 channel realizations considered, to generate the statistical distribution and cumulative distribution functions of the obtained global costs, as well as to evaluate the system’s performance once the proposed algorithm was embedded.     The ac stark shift and space-borne rubidium atomic clocks

    open7sìDue to its small size, low weight, and low power consumption, the Rb atomic frequency standard (RAFS) is routinely the first choice for atomic timekeeping in space. Consequently, though the device has very good frequency stability (rivaling passive hydrogen masers), there is interest in uncovering the fundamental processes limiting its long-term performance, with the goal of improving the device for future space systems and missions. The ac Stark shift (i. e., light shift) is one of the more likely processes limiting the RAFS' long-term timekeeping ability, yet its manifestation in the RAFS remains poorly understood. In part, this comes from the fact that light-shift induced frequency fluctuations must be quantified in terms of the RAFS' light-shift coefficient and the output variations in the RAFS' rf-discharge lamp, which is a nonlinear inductively-couple plasma (ICP). Here, we analyze the light-shift effect for a family of 10 on-orbit Block-IIR GPS RAFS, examining decade-long records of their on-orbit frequency and rf-discharge lamp fluctuations. We find that the ICP's light intensity variations can take several forms: deterministic aging, jumps, ramps, and non-stationary noise, each of which affects the RAFS' frequency via the light shift. Correlating these light intensity changes with RAFS frequency changes, we estimate the light-shift coefficient, K-LS, for the family of RAFS: K-LS = -(1.9 +/- 0.3) x 10(-12) /%. The 16% family-wide variation in K-LS indicates that while each RAFS may have its own individual K-LS, the variance of K-LS among similarly designed RAFS can be relatively small. Combining K-LS with our estimate of the ICP light intensity's non-stationary noise, we find evidence that random-walk frequency noise in high-quality space-borne RAFS is strongly influenced by the RAFS' rf-discharge lamp via the light shift effect.     Gibbsian Method for the Self-Optimization of Cellular Networks

    In this work, we propose and analyze a class of distributed algorithms performing the joint optimization of radio resources in heterogeneous cellular networks made of a juxtaposition of macro and small cells. Within this context, it is essential to use algorithms able to simultaneously solve the problems of channel selection, user association and power control. In such networks, the unpredictability of the cell and user patterns also requires distributed optimization schemes. The proposed method is inspired from statistical physics and based on the Gibbs sampler. It does not require the concavity/convexity, monotonicity or duality properties common to classical optimization problems. Besides, it supports discrete optimization which is especially useful to practical systems. We show that it can be implemented in a fully distributed way and nevertheless achieves system-wide optimality. We use simulation to compare this solution to today's default operational methods in terms of both throughput and energy consumption. Finally, we address concrete issues for the implementation of this solution and analyze the overhead traffic required within the framework of 3GPP and femtocell standards.Comment: 25 pages, 9 figures, to appear in EURASIP Journal on Wireless Communications and Networking 201

    Roadmap towards the redefinition of the second

    This paper outlines the roadmap towards the redefinition of the second, which was recently updated by the CCTF Task Force created by the CCTF in 2020. The main achievements of optical frequency standards (OFS) call for reflection on the redefinition of the second, but open new challenges related to the performance of the OFS, their contribution to time scales and UTC, the possibility of their comparison, and the knowledge of the Earth's gravitational potential to ensure a robust and accurate capacity to realize a new definition at the level of 10-18 uncertainty. The mandatory criteria to be achieved before redefinition have been defined and their current fulfilment level is estimated showing the fields that still needed improvement. The possibility to base the redefinition on a single or on a set of transitions has also been evaluated. The roadmap indicates the steps to be followed in the next years to be ready for a sound and successful redefinition

    Demonstrator of Time Services based on European GNSS signals: the H2020 DEMETRA Project

    During 2015-2016, a European Consortium of 15 partners from 8 different countries, developed the DEMETRA (DEMonstrator of EGNSS services based on Time Reference Architecture), a project funded by the European Union in the frame of the Horizon 2020 program. This project aims at developing and experimenting time dissemination services dedicated to specific users like traffic control, energy distribution, finance, telecommunication, and scientific institutions. Nine services have been developed. These services provide time dissemination with accuracy levels from millisecond to the sub-ns, and also additional services like certification, calibration, or integrity. Five of these services are based on the European GNSS. After a development phase (see PTTI 2016 presentation) the full DEMETRA system has been working during six months for demonstration. The paper will report about the experimentation results, showing performances and limits of the proposed time dissemination services, aiming to foster the exploitation of the European GNSS for timing applications

    Mapping of health system functions to strengthen priority programs. The case of maternal health in Mexico

    <p>Abstract</p> <p>Background</p> <p>Health system strengthening is critical to ensure the integration and scaling-up of priority health promotion, disease prevention and control programs. Normative guidelines are available to address health system function imbalances while strategic and analytical frameworks address critical functions in complex systems. Tacit knowledge-based health system constructs can help identify actors' perspectives, contributing to improve strengthening strategies. Using maternal health as an example, this paper maps and analyses the health system functions that critical actors charged with formulating and delivering priority health programs consider important for their success.</p> <p>Methods</p> <p>Using concept mapping qualitative and statistical methods, health system functions were mapped for different categories of actors in high maternal mortality states of Mexico and at the federal level. Functions within and across maps were analyzed for degree of classification, importance, feasibility and coding.</p> <p>Results</p> <p>Hospital infrastructure and human resource training are the most prominent functions in the maternal health system, associated to federal efforts to support emergency obstetric care. Health policy is a highly diffuse function while program development, intercultural and community participation and social networks are clearly stated although less focused and with lower perceived importance. The importance of functions is less correlated between federal and state decision makers, between federal decision makers and reproductive health/local health area program officers and between state decision makers and system-wide support officers. Two sets of oppositions can be observed in coding across functions: health sector vs. social context; and given structures vs. manageable processes.</p> <p>Conclusions</p> <p>Concept mapping enabled the identification of critical functions constituting adaptive maternal health systems, including aspects of actor perspectives that are seldom included in normative and analytical frameworks. Important areas of divergence across actors' perceptions were identified to target capacity strengthening efforts towards better integrated, performing health systems.</p
