    La desinstitucionalització i la crisi de les institucions

    Parlar d'institucions s'ha convertit en parlar de la seva crisi. Amb l'assentament de la societat postindustrial sembla que s'han accentuat les dificultats de legitimació d'unes institucions creades per mantenir un tipus de formació social que esta en procés de desaparició. De fet, es pot dir que la crisi de les institucions no és més que un aspecte d'una altra de global que podríem anomenar crisi del projecte modern.Hablar de instituciones se ha convertido en hablar de crisis de las instituciones. En este sentido, la noción de desinstitucionalización ocupa un lugar relevante en la reflexión institucional. Por este motivo, en este artículo se lleva a cabo una revisión del concepto de institución v de las principales críticas que ha recibido como paso previo al análisis más detallado del fenómeno desinstitucionalizador en la práctica psiquiátrica. Tal análisis lleva a la elaboración de una propuesta alternativa que cuestiona la pertinencia de seguir hablando de instituciones, así como de procesos desinstitucionalizadores. Concretamente, se plantea que las especiales características de algunas prácticas psiquiátricas contemporáneas se ajustan mejor a la noción de ex-titución tomada de la obra de SerresSpeaking of the institutions implies nowadays speaking of the crisis of institutions. In that sense, the concept of disinstitutionalization fills a relevant role in the institutional reflection. For this reason, this article carries out a review of the concept of institution and the main criticisms that it has received as a previous step to a more detailed examination of the disinstitutionalizing phenomenon, in the framework of psychiatric experience. Such an examination leads to the making of an alternative proposal which questions the pertinence of keeping speaking of both institutions and disinstutionalizing processes.     An elastic software architecture for extreme-scale big data analytics

    This chapter describes a software architecture for processing big-data analytics considering the complete compute continuum, from the edge to the cloud. The new generation of smart systems requires processing a vast amount of diverse information from distributed data sources. The software architecture presented in this chapter addresses two main challenges. On the one hand, a new elasticity concept enables smart systems to satisfy the performance requirements of extreme-scale analytics workloads. By extending the elasticity concept (known at cloud side) across the compute continuum in a fog computing environment, combined with the usage of advanced heterogeneous hardware architectures at the edge side, the capabilities of the extreme-scale analytics can significantly increase, integrating both responsive data-in-motion and latent data-at-rest analytics into a single solution.     Profibrotic role of inducible heat shock protein 90α isoform in systemic sclerosis

    Systemic sclerosis (SSc) is an autoimmune disease that affects skin and multiple internal organs. TGF-β, a central trigger of cutaneous fibrosis, activates fibroblasts with the involvement of the stress-inducible chaperone heat shock protein 90 isoform α (Hsp90α). Available evidence supports overexpression and secretion of Hsp90α as a feature in profibrotic pathological conditions. The aim of this work is to investigate the expression and function of Hsp90α in experimental models of skin fibrosis such as human fibroblasts, C57BL/6 mice, and in human SSc. For this purpose, we generated a new experimental model based on doxorubicin administration with improved characteristics with respect to the bleomycin model. We visualized disease progression in vivo by fluorescence imaging. In this work, we obtained Hsp90α mRNA overexpression in human skin fibroblasts, in bleomycin- and doxorubicin-induced mouse fibrotic skin, and in lungs of bleomycin- and doxorubicin-treated mice. Hsp90α-deficient mice showed significantly decreased skin thickness compared with wild-type mice in both animal models. In SSc patients, serum Hsp90α levels were increased in patients with lung involvement and in patients with the diffuse form of SSc (dSSc) compared with patients with the limited form of SSc. The serum Hsp90α levels of patients dSSc were correlated with the Rodnan score and the forced vital capacity variable. These results provide new supportive evidence of the contribution of the Hsp90α isoform in the development of skin fibrosis.     Everolimus plus minimized tacrolimus on kidney function in liver transplantation: REDUCE, a prospective, randomized controlled study

    Background and aim: reduction in calcineurin inhibitor levels is considered crucial to decrease the incidence of kidney dysfunction in liver transplant (LT) recipients. The aim of this study was to evaluate the safety and impact of everolimus plus reduced tacrolimus (EVR + rTAC) vs. mycophenolate mofetil plus tacrolimus (MMF + TAC) on kidney function in LT recipients from Spain. Methods: the REDUCE study was a 52-week, multicenter, randomized, controlled, open-label, phase 3b study in de novo LT recipients. Eligible patients were randomized (1:1) 28 days post-transplantation to receive EVR + rTAC (TAC levels <_ 5 ng/mL) or to continue with MMF + TAC (TAC levels = 6-10 ng/mL). Mean estimated glomerular filtration rate (eGFR), clinical benefit in renal function, and safety were evaluated. Results: in the EVR + rTAC group (n = 105), eGFR increased from randomization to week 52 (82.2 [28.5] mL/min/1.73 m2 to 86.1 [27.9] mL/min/1.73 m2) whereas it decreased in the MMF + TAC (n = 106) group (88.4 [34.3] mL/min/1.73 m2 to 83.2 [25.2] mL/min/1.73 m2), with significant (p < 0.05) differences in eGFR throughout the study. However, both groups had a similar clinical benefit regarding renal function (improvement in 18.6 % vs. 19.1 %, and stabilization in 81.4 % vs. 80.9 % of patients in the EVR + rTAC vs. MMF + TAC groups, respectively). There were no significant differences in the incidence of acute rejection (5.7 % vs. 3.8 %), deaths (5.7 % vs. 2.8 %), and serious adverse events (51.9 % vs. 44.0 %) between the 2 groups. Conclusion: EVR + rTAC allows a safe reduction in tacrolimus exposure in de novo liver transplant recipients, with a significant improvement in eGFR but without significant differences in renal clinical benefit 1 year after liver transplantation

    Stroke and traumatic injury in brain or spinal cord are often life-threating conditions and major causes of death or permanent disability with high impact in the health care system. There are several stages of intervention to improve the neurological outcome. Acutely, fast interventions aiming to reestablish cerebral blood flow in ischemic stroke, to stop bleeding after brain hemorrhage, and to reduce edema after contusions are amongst mandatory actions. Current studies aim to develop accompanying strategies for brain cell protection based on enhancing endogenous protective mechanism, blocking cell death pathways, or through immunomodulation. After the acute phase, interventions are intended to promote recovery of function using rehabilitation with state-of-the-art technologies enabled by robotics. Other advanced strategies include cell, gene, and immune therapies, and brain function modulation with the aid of smart nanotechnologies.
