    Time-scale analysis non-local diffusion systems, applied to disease models

    The objective of the present paper is to use the well known Ross-Macdonald models as a prototype, incorporating spatial movements, identifying different times scales and proving a singular perturbation result using a system of local and non-local diffusion. This results can be applied to the prototype model, where the vector has a fast dynamics, local in space, and the host has a slow dynamics, non-local in space

O objetivo é analisar os métodos aplicados na projeção de vendas comerciais e verificar a metodologia na realização em uma empresa do setor de Prestação de Serviços em Seguros. Para tal, os capítulos a seguir mostrarão os sistemas e métodos considerados para projeção das vendas em sua fundamentação teórica e os efeitos método aplicado em uma empresa do ramo acima citado, modo a proporcionar resultados positivos para empresa no que diz respeito à lucratividade da mesma. O ambiente de negócios atual (pós-década de 1990) apresenta oportunidades e desafios bastante distintos daqueles encontrados até bem pouco tempo atrás. Para serem efetivadas em seus mercados, as empresas devem buscar novas formas de gestão. Várias propostas têm sido implementadas nesse sentido, dentro das quais quatros são destacados: A Gestão Estratégica, que trata da necessidade da empresa estar permanentemente em sintonia com seu ambiente competitivo; a Gestão Participativa, relacionada ao aumento da autonomia, do comprometimento e da participação dos funcionários, visando fornecer respostas rápidas às demandas ambientais; a Gestão Holística, que representa um passo além da teoria dos sistemas, visualizando o executivo e a empresa como parte de um todo que se influencia mutuamente, e a Gestão Empreendedora, que diz respeito à busca da transformação de planos e sonhos em realidade. A melhoria dos processos de gestão empresarial está diretamente relacionada à forma como seu desempenho é avaliado. Na gestão moderna é fundamental avaliar o desempenho dos processos organizacionais por meio das medidas não financeiras, apesar de estarem atreladas ao desempenho financeiro, tendo em vista que as práticas que garantiram bons resultados no passado podem não ser mais suficientes.     Overdiagnosis and overimaging: an ethical issue for radiological protection

    Aims and objectives: This study aimed to analyse the key factors that influence the overimaging using X-ray such as self-referral, defensive medicine and duplicate imaging studies and to emphasize the ethical problem that derives from it. Materials and methods: In this study, we focused on the more frequent sources of overdiagnosis such as the total-body CT, proposed in the form of screening in both public and private sector, the choice of the most sensitive test for each pathology such as pulmonary embolism, ultrasound investigations mostly of the thyroid and of the prostate and MR examinations, especially of the musculoskeletal system. Results: The direct follow of overdiagnosis and overimaging is the increase in the risk of contrast media infusion, radiant damage, and costs in the worldwide healthcare system. The theme of the costs of overdiagnosis is strongly related to inappropriate or poorly appropriate imaging examination. Conclusions: We underline the ethical imperatives of trust and right conduct, because the major ethical problems in radiology emerge in the justification of medical exposures of patients in the practice. A close cooperation and collaboration across all the physicians responsible for patient care in requiring imaging examination is also important, balancing possible ionizing radiation disadvantages and patient benefits in terms of care

    Thermal Ablation of Liver Tumours: How the Scenario Has Changed in the Last Decade

    Surgical resection has long been considered the gold standard for the local treatment of primary and secondary liver tumours. Until recent years, percutaneous thermal ablation (PTA), in particular radiofrequency ablation (RFA), was not accepted as a first-line option for the treatment of liver tumours and was reserved for patients who were unsuitable for surgery. However, in the last decade the scenario has changed: interesting technical developments and innovations have improved the performance of RFA and broadened the availability of other ablative technologies, such as microwave ablation (MWA) and laser ablation (LA). The latest generation of MWA systems can achieve larger ablation areas than RFA and LA, with a multifibre technique that uses very thin needles, allowing physicians to treat nodules in at-risk locations with high flexibility and a very low risk ofcomplications. Nowadays, there is an increasingly accepted consensus on the role of PTA as a first-line option for the treatment of liver tumours <2 cm in size, as well as in patients potentially eligible for surgery, and it is likely that in the near future the 2 cm barrier could also be surpassed and extended to at least 3 cm. PTA is becoming more effective and important in the treatment of primary and secondary liver tumours, and, in the well-established scenario of a multimodal tailoredtreatment, it plays and will continue to play a central role.     Percutaneous Laser Thermal Ablation in a Patient with 22 Liver Metastases from Pancreatic Neuroendocrine Tumours: A Case Report

    The relatively indolent nature of well-differentiated neuroendocrine tumours (NET) and their proclivity to be hormonally active warrants aggressive multimodal treatment, even for advanced stage disease. Good results have been reported in well-selected patients with a median of 23 liver metastases (LM) from NET treated with surgical resection combined with intraoperative radiofrequency ablation. We report the case of a patient who underwent percutaneous laser thermal ablation (LTA) of 22 small LM from NET, treated over three consecutive sessions. After 2 years, five new LM were detected and treated with LTA. At present, 82 months after the first LTA session, the patient is still alive and disease-free. Due to enabling the use of one to four optical fibres at once to tailor the thermal lesion size to the nodule size, LTA could represent the ablation technique of choice in the presence of multiple, small, and variably sized LM

The IUC Independent Policy Report argues for a radical change of perspective, capable of restoring the supremacy of the law over the economic system. It is not only about finance, nor is it only about economics or policy. In this sense a transnational set of normative principles is needed in order to establish a global legal system capable of controlling economic processes, rather than being controlled by them. Within this framework a series of policy proposals are presented in order to effectively implement a new system of global standards. The current Western standard of living is unsustainable. Should the rest share the model of development of the West, our planet will simply not be capable of resisting the growth in consumption and pollution. Within this fundamental setting of scarcity in resources, using the rhetoric of the end of history as the polar star for growth, development and ultimately happiness of the whole world is simply a cynical lie. We argue here for the beginning of a necessary process aimed at the development of a legal system that is much less about creating an effcient backbone for an exploitive economy and much more about a vision of civilization, justice and respect where the laws of nature and those of humans converge in a sustainable long-term philosophy. Principles of justice, responsibility and long term environmental protection, rather than short term economic contingency and strong interests must set the legal agenda. A new governance and bottom-up inclusive integration of knowledge-based economies (wherever located), which is crucial to the very survival of humankind, cannot happen without defning new terms of a widely accepted standard of long term justice in the transnational context, hence the urgency to conceive legitimate transnational legal structures and possibly some apparatus of “superlegality.” The report is composed of fve sections. After having presented the pitfalls of the prevailing theoretical apparatus, an alternative cultural grid upon which policy actions should be shaped is presented. In this sense several normative proposals - revisiting the key characteristics of the current system - are offered aiming at acquiring a wider perspective over the actual global crisis

    Ultrastructural pathology of nephropathies with organized deposits: a case series

    Renal organized or structured deposits are much less frequent than those with usual type immunocomplex deposits and are encountered in a wide variety of primary and systemic disorders. It has been suggested that immunoglobulins (Igs) are responsible for organized deposits. We report 5 cases who have been diagnosed and treated in our hospital. Patients were aged 52 to 72 years, three of them were males and had variable degree of renal function, from normal serum creatinine to uraemia. Proteinuria was detected in all patients while monoclonal component was present only in the serum of one subject. Ultrastructural analysis of renal specimens revealed organized deposits. Diagnoses that were made are the following: membranoproliferative glomerulonephritis with finger print, immunotactoid glomerulopathy, membranoproliferative glomerulonephritis with arched deposits, primary amyloidosis and light chain deposition disease. In systemic disorders ultrastructural pathology could be particularly valuable for correct deposits classification, precise localization and pattern of deposition of Igs

    Brain Switches Utilitarian Behavior: Does Gender Make the Difference?

    Decision often implies a utilitarian choice based on personal gain, even at the expense of damaging others. Despite the social implications of utilitarian behavior, its neurophysiological bases remain largely unknown. To assess how the human brain controls utilitarian behavior, we delivered transcranial direct current stimulation (tDCS) over the ventral prefrontal cortex (VPC) and over the occipital cortex (OC) in 78 healthy subjects. Utilitarian judgment was assessed with the moral judgment task before and after tDCS. At baseline, females provided fewer utilitarian answers than males for personal moral dilemmas (p = .007). In males, VPC-tDCS failed to induce changes and in both genders OC-tDCS left utilitarian judgments unchanged. In females, cathodal VPC-tDCS tended to decrease whereas anodal VPC-tDCS significantly increased utilitarian responses (p = .005). In males and females, reaction times for utilitarian responses significantly decreased after cathodal (p<.001) but not after anodal (p = .735) VPC-tDCS. We conclude that ventral prefrontal tDCS interferes with utilitarian decisions, influencing the evaluation of the advantages and disadvantages of each option in both sexes, but does so more strongly in females. Whereas cathodal tDCS alters the time for utilitarian reasoning in both sexes, anodal stimulation interferes more incisively in women, modifying utilitarian reasoning and the possible consequent actions. The gender-related tDCS-induced changes suggest that the VPC differentially controls utilitarian reasoning in females and in males. The gender-specific functional organization of the brain areas involved in utilitarian behavior could be a correlate of the moral and social behavioral differences between the two sexes
