    Urban share of the "burden" : impact of a support organisation on caregiver burden of people affected by dementia

    Purpose. This study investigated the experiences and expectations of unpaid caregivers who were members of a nonprofit social support organisation. Design and Methods. Colaizzi's phenomenological exploration was followed. Data were saturated after 15 in-depth individual interviews at a centre for people affected with dementia. Findings. The themes reported with the COREQ checklist were contributory to caregiver burden, the escalation of dementia symptoms, changes in family roles, psychological distress, social challenges, membership in a social network, and developing effective coping skills. Practice Implications. Access, availability, and continuity of psychosocial support programs are vital for the wellbeing of people affected with dementia

    Fournier gangrene due to Rhizobium Radiobacter

    Fournier's gangrene (FG) is a life-threatening, rapidly progressing acute necrotizing fasciitis of perianal, genitourinary and perineal areas. The most common isolated pathogens are E. coli, Streptococcus and Enterococcus in tissue and abscess cultures. However we present the first Rhizobium radiobacter FG in this case. A 47-year-old man presented with black necrotizing area with malodorous pus drainage in the bottom of the right hemiscrotum. Intravenous imipenem and Clindamycin were started prophylactically; all necrotizing tissues were debrided and right hemiscrotectomy was performed. Rhizobium radiobacter was isolated from tissue and abscess cultures. After successful treatment patient was discharged posteroperative 18th day. The combination therapy of early and totally surgical debridement of necrotic tissues and appropriate antibiotic use are adequate to control Rhizobium radiobacter infection in FG

This report presents a case of mental nerve paresthesia caused by endodontic infection of a mandibular left second premolar. Resolution of the paresthesia began two weeks after conventional root canal treatment associated with antibiotic therapy and was completed in eight weeks.     Paper-based sensors have great potential for use in a variety of areas, from environmental monitoring to clinical and pointof-care testings. Here, a microfluidic paper-based analytical device (µPAD) was integrated with a smartphone app capable of offline (without internet access) image processing and analysis for rapid colorimetric detection of glucose. A self-inking stamp was used to form hydrophobic channels on a piece of paper-towel due to its superior water absorption efficiency. As demonstrated, the developed sensor was employed for colorimetric detection of glucose in artificial saliva in the linear scope of 0-1 mM with a calculated detection limit of 29.65 µM. In addition, experimental results show that quantitative analysis of glucose with the proposed smartphone platform could be completed in less than one minute. The app developed for the smartphone platform is capable of extracting the color changing area with an embedded image processing tool which could address the problem of color uniformity in detection zones of µPAD. The total cost of a µPAD is less than $0.2. By integrating µPAD with a smartphone and user-friendly app together, the proposed smartphone-based platform could be used for the quantitative analysis of glucose with advantages such as portability, simple operation, rapid response, ultra-low cost, field-deployable, selectivity and sensitivity. The results show that the integrated platform has great potential to be used for non-invasive measurement of glucose in body fluids like a tear, sweat and saliva

    Evaluation of Surface Topography of Zirconia Ceramic After Er:YAG Laser Etching

    Objective: The aim of this study is to evaluate the effect of Erbium: yttrium-aluminum-garnet (Er:YAG) laser with different pulse lengths on the surface roughness of zirconia ceramic and airborne particle abrasion. Background data: Er:YAG laser treatment is expected to be an alternative surface treatment method for zirconia ceramics; however, the parameters and success of the application are not clear. Methods: One hundred and forty zirconia discs (diameter, 10 mm; thickness, 1.2 mm) were prepared by a computer-aided design and computer-aided manufacturing (CAD/CAM) system according to the manufacturer's instructions. Specimens were divided into 14 groups (n=10). One group was left as polished control, one group was air-particle abraded with Al2O3 particles. For the laser treatment groups, laser irradiation was applied at three different pulse energy levels (100, 200, and 300 mJ) and for each energy level at four different pulse lengths; 50, 100, 300, and 600 mu s. Surface roughness was evaluated with an optical profilometer and specimens were evaluated with scanning electron microscopy (SEM). Results: Data was analyzed with one way ANOVA and Tukey multiple comparison tests (alpha=0.05). For the 100 and 200 mJ laser etching groups, 50 and 100 mu s laser duration resulted in significantly higher surface roughness compared with air-particle abrasion (p0.05). For the 300 mJ laser etching groups; there was no statistically significant difference among the R-a values of 50 mu s, 100 mu s, 300 mu s, 600 mu s, and air-particle abrasion groups (p>0.05). Conclusions: In order to increase surface roughness and promote better bonding to resin luting agents, Er:YAG laser etching may be an alternative to air-particle abrasion for zirconia ceramics. However, high levels of pulse energy and longer pulse length may have an adverse effect on micromechanical locking properties, because of a decrease in surface roughnes
