Novel emerging treatments for NMOSD
Neuromyelitis optica spectrum disorders (NMOSD) are inflammatory demyelinating diseases of the central nervous system (CNS) that cause optic neuritis, transverse myelitis, and some other CNS syndromes. Recently, diagnosis and understanding of these diseases has been markedly enhanced by the discovery that serum autoantibodies that target aquaporin-4 (AQP4) are strongly associated with the disease. This spectrum includes also a potential subset of patients with a phenotype of NMOSD who have anti-myelin oligodendrocyte glycoprotein (MOG) antibody. Although steroids and immunosuppressive drugs have been widely used for NMOSD treatment, until recently there was no approved therapy for these diseases. With improved understanding of the pathophysiology of NMOSD, numerous new therapeutic strategies have recently been evaluated. The results of these studies, involving monoclonal antibodies (mAbs) inhibiting terminal complement protein cleavage interfering with interleukin-6 receptor (IL-6 R) signaling and depleting CD19-positive B cells, have been published in recent months. All of these new therapeutics have shown a high degree of efficacy in diminishing NMOSD activity and inhibiting disability progression. At the same time, all these mAbs have demonstrated favorable safety and tolerability profiles, with a limited rate of adverse events. The first of these new drugs, eculizumab, have been approved in USA and Europe for NMOSD treatment within the last couple of months and it is expected that the other novel, effective and safe treatments for NMOSD will be approved in the near future

The issue of the environmental degradation and global character of the ecological crisis is one of the distinguishing features of the modern civilization. Thus, nowadays there are appearing many concepts concerned with overcoming the ecological crisis. This article presents the idea of peace with nature proposed by K.M. Meyer-Abich within the framework of the practical philosophy of nature. By the practical philosophy of nature I understand such a reflection on nature which includes an active presence of man in the environment. This understanding of the practical philosophy of nature is therefore equivalent with the demands of the times, in which human activity in nature is a major problem. Peace with nature is a political and philosophical concept attempting to reconcile the political and economic organization of an industrial society with the order of nature. K.M. Meyer-Abich’s departure point for the idea of peace with nature is a physiocentric conception of a human being, which is expressed above all by the belief in the phylogenetic origin of man and nature. He conceives of nature as a “connature” of human life. The phylogenetic origin of man and the surrounding nature makes us recognize the value of nature itself. Hence, K.M. Meyer-Abich’s conclusion is that environmental protection should be recommended, not only because of the man, but also because of the other elements of the connatural world.Zeszyty Naukowe TDU
Leczenie przyczynowe stwardnienia rozsianego
W związku z domniemaną autoimmunologiczną patogenezą stwardnienia
rozsianego (SM, sclerosis multiplex) leczenie przyczynowe tej
choroby polega na stosowaniu leków immunomodulujących. W wieloośrodkowych, randomizowanych badaniach klinicznych klasy I udowodnono
skuteczność intereferonu β 1A, interferonu β 1B oraz glatirameru
w postaci remitującej SM (RR SM). Skuteczność ta polega na
zmniejszeniu częstości rzutów choroby o 30-35% oraz na zwolnieniu
postępu rozwoju deficytu neurologicznego o średnio 9 miesięcy
w okresie 2 lat leczenia. Interferon β 1B wykazał także skuteczność
we wczesnym okresie postaci wtórnej przewlekłej choroby (SP SM).
Większą skuteczność inteferonu β obserwowano u chorych po jego
zastosowaniu w bardzo wczesnym okresie choroby, nawet po pierwszym
epizodzie objawów (CIS, clinical isolated syndrome). Interferon
β kilkukrotnie zmniejszał liczbę zmian aktywnych w badaniu rezonansu
magnetycznego w czasie leczenia. Badania rejestracyjne interferonu
β i glatirameru trwały 2 lub 3 lata, ale przedłużone badania
otwarte wskazują, że długoterminowe odległe wyniki leczenia są jeszcze
korzystniejsze. Ogólnoustrojowa tolerancja stosowanych leków
immunomodulujacych jest dobra, ale u pacjentów przyjmujących interferon
β po wstrzyknięciu leku występują objawy paragrypowe.
W terapii postaci przewlekłych i gwałtownie postępujących SM stosuje
się cytostatyczny lek antracyklinowy - mitoksantron, z zachowaniem
środków ostrożności charakterystycznych dla leków cytostatycznych,
w tym ze stałym nadzorem kardiologicznym
Stwardnienie rozsiane - kryteria diagnostyczne i naturalny przebieg choroby
Stwardnienie rozsiane (SM, sclerosis multiplex) jest najczęstszą choroba
demielinizacyjną w patologii ludzkiej. Chorobę klinicznie definiuje
się jako występowanie nawrotowych epizodów objawów neurologicznych,
spowodowanych rozsianymi demielinizacyjnymi uszkodzeniami
ośrodkowego układu nerwowego, z czasem prowadzących do
znacznych deficytów neurologicznych. W aktualnych kryteriach diagnostycznych
McDonalda zdefiniowano warunki, jakie muszą być
spełnione do rozpoznania SM. Według tych kryteriów wystąpienie
nowych zmian patologicznych w badaniu rezonansu magnetycznego
(MR, magnetic resonance) jest równoważne z kolejnym epizodem
klinicznym. Oprócz najczęstszej postaci SM rozpoczynającej się od
nawracających rzutów choroby (RR SM) występuje też postać przewlekle
postępująca (PP SM), w której objawy neurologiczne narastają w sposób ciągły. Po kilku latach trwania choroby u pacjentów
z postacią RR SM przechodzi ona w postać wtórnie przewlekłą
(SP SM). Objawy kliniczne SM są zróżnicowane, a ich postać kliniczna
zależy od lokalizacji zmian patologicznych. Do najczęstszych objawów należą: niedowłady piramidowe, zaburzenia czucia, widzenia,
koordynacji i zaburzenia kontroli zwieraczy. Naturalny przebieg SM
obejmuje szerokie spektrum tempa rozwoju deficytu neurologicznego.
Z jednej strony obserwuje się przypadki o bardzo powolnym postępie (łagodne SM), a z drugiej - przypadki postępujące bardzo
gwałtownie. Te ostatnie często określa się mianem choroby Schildera.
Wariantem SM jest też choroba Devica, w której objawy dotyczą
uszkodzenia rdzenia i nerwów wzrokowych. W diagnostyce różnicowej
SM należy uwzględniać wszystkie choroby z więcej niż jednym
objawem neurologicznym, występujące przewlekle lub nawrotowo, w przebiegu których w badaniu MR występują rozsiane zmiany hiperintensywne
w sekwencjach T2/PD
Polyunsaturated fatty acids in the pathogenesis and treatment of multiple sclerosis
Epidemiological, biochemical, animal model and clinical trial data described in this overview strongly suggest that polyunsaturated fatty acids, particularly n-6 fatty acids, have a role in the pathogenesis and treatment of multiple sclerosis (MS). Data presented provides further evidence for a disturbance in n-6 fatty acid metabolism in MS. Disturbance of n-6 fatty acid metabolism and dysregulation of cytokines are shown to be linked and a "proof of concept clinical trial" further supports such a hypothesis. In a randomised double-blind, placebo controlled trial of a high dose and low dose selected GLA (18:3n-6)-rich oil and placebo control, the high dose had a marked clinical effect in relapsing-remitting MS, significantly decreasing the relapse rate and the progression of disease. Laboratory findings paralleled clinical changes in the placebo group in that production of mononuclear cell pro-inflammatory cytokines (TNF-alpha, IL-1 beta) was increased and anti-inflammatory TGF-beta markedly decreased with loss of membrane n-6 fatty acids linoleic (18:2n-6) and arachidonic acids (20:4n-6). In contrast there were no such changes in the high dose group. The improvement in disability (Expanded Disability Status Scale) in the high dose suggests there maybe a beneficial effect on neuronal lipids and neural function in MS. Thus disturbed n-6 fatty acid metabolism in MS gives rise to loss of membrane long chain n-6 fatty acids and loss of the anti-inflammatory regulatory cytokine TGF-beta, particularly during the relapse phase, as well as loss of these important neural fatty acids for CNS structure and function and consequent long term neurological deficit in MS

This article presents the K. M. Meyer-Abich theory of responsibility. It discusses the essence of human responsibility as it arises especially at times of ecological crises. The circle of responsibility should be so wide as to prevent anything from existing merely for the sake of human needs. K. M. Meyer-Abich’s theory focuses on eight periods of human development, which correspond to eight circles of responsibility referring to eight ethical conceptions. Throughout human history, we can see how the scope of responsibility has been increasing to fall upon more and more entities. The extension of the circle of responsibility moved from the individual to include family and friends, the nation, all peoples of the world, and humanity as a whole, including future generations. The final step was to look beyond the boundaries of humanity to include all life forms
Multiple sclerosis: time for early treatment with high-efficacy drugs
This review addresses current changes in the approach to treating patients with multiple sclerosis (MS). The widely practiced approach of utilizing agents with lower treatment efficacy (LETA) at onset with subsequent escalation has been challenged by new data suggesting that MS patients derive greater benefit when therapy is initiated with high-efficacy treatment agents (HETA). Several recent studies compared treatment efficacy and safety of early administration of HETA versus LETA. The results of randomized, double blind, phase III studies with LETA as a control arm and population-based larger and longer studies using propensity scoring, marginal structural modeling and weighted cumulative exposure analysis support the benefit of early treatment with HETA. Patients initiating their treatment with HETA, regardless of prognostic factors and MRI burden at baseline, showed significantly lower annualized relapse rate (ARR) and reduced disability progression in follow-up periods of up to 10–15 years. Moreover, the safety profile of recently approved HETA ameliorates concerns about off-target effects associated with a number of earlier high-efficacy drugs. Patient perception has also changed with an increasing preference for medication profiles that both improve symptoms and prevent disease progression. Accumulating data from randomized studies and the results of large population-based studies demonstrating short-term and longer-term patient benefits support the view that HETA should be more widely used. The adoption of early treatment with HETA capitalizes on a window of opportunity for anti-inflammatory drugs to maximally impact disease pathology and heralds a sea change in clinical practice toward pro-active management and away from a philosophy routed in generating clinical benefit as a consequence of treatment failure
Leczenie objawowe stwardnienia rozsianego
Stwardnienie rozsiane (SM, sclerosis multiplex) jest przewlekłą, zapalno-
demielinizacyjną chorobą ośrodkowego układu nerwowego,
prowadzącą u większości chorych do kalectwa. Objawy SM, takie jak
spastyczność, zaburzenia zwieraczy czy drżenie, przysparzają pacjentom
znacznych cierpień, powodują izolację społeczną i obniżają jakość ich życia. Mimo stosowania coraz większej liczby leków modyfikujacych naturalny przebieg choroby, takich jak interferon beta, octan
glatirameru czy mitoksantron, większość z tych pacjentów wymaga
także zindywidualizowanego leczenia objawowego.
