Beneficio de la realización de un taller en gestión emocional para enfermería nefrológica
Los profesionales de enfermería del servicio de nefrología
recibieron entrenamiento en gestión emocional
mediante un taller de 12 horas centrado en la tecnología
relacional de Counselling de amplia aplicabilidad
en entornos sanitarios. El objetivo fue evaluar cambios
en actitudes en relación a los principios bioéticos
y en conocimientos sobre comunicación y gestión
emocional antes y después de la implantación del
taller. La muestra estaba formada por 63 profesionales
del ámbito de la enfermería nefrológica (43
enfermeras y 20 auxiliares de enfermería) para la
variable conocimientos y por 23 profesionales para la
variable de actitudes (18 enfermeras y 5 auxiliares).
Los resultados muestran cambios en implicación con
los principios bioéticos (p<0,05) y conocimientos

Propiedades psicométricas preliminares de un cuestionario multidimensional de adaptación a la enfermedad para pacientes onco-hematológicos (Cmae-Oh) adaptado a pacientes renales en diálisis (Cmae-Rd)
Este trabajo tiene como objetivo la adaptación de un cuestionario empleado para la evaluación de necesidades y recursos en pacientes con enfermedad onco-hematológica, el CMAE-OH de Arranz y Ulla (2008) a pacientes renales en diálisis. Para ello realizamos tres estudios secuenciales. El primero de ellos es un estudio interjueces, para determinar la validez del cuestionario. El segundo es un estudio piloto para poner a prueba su funcionamiento en pacientes renales. El tercer estudio, consistirá en el empleo de la herramienta en una muestra mayor para determinar las propiedades psicométricas del instrumento. Tras la adaptación del cuestionario, concluimos que la herramienta es útil y adecuada para la evaluación de necesidades y recursos en los pacientes renales en diálisis, así como lo era para el caso de los pacientes oncológicos. Partimos de la adaptación de esta herramienta para iniciar una reflexión acerca de la cercanía, más que el distanciamiento, entre la psicooncología y otras subdisciplinas del ámbito de la Psicología
de la Salud como la psiconefrologíaThe aim of this research is to adapt a questionnaire designed for the assessment of needs and resources in patients with onco-haematological diseases – the CMAE-OH- to renal patients under dialysis treatment. We have conducted three different studies. The first one is an inter-judges study to determine the face validity of the tool. The second is a pilot study, to know how the designed tool works when used in renal patients under dialysis. The third study consists of the usage of the questionnaire in a biggest sample to determine the psychometric properties of the tool. After the adaptation of the tool we conclude that the questionnaire we have adapt it is useful to assess needs and resources in renal patients under dialysis, as well as CMAE-OH was when used with oncological patients. On the basis of our study, we reflexionate about the closeness between psychooncology and other subdisciplines in the area of health psychology like psychonephrolog
Functional relevance of the switch of VEGF receptors/co-receptors during peritoneal dialysis-induced mesothelial to mesenchymal transition
Vascular endothelial growth factor (VEGF) is up-regulated during mesothelial to mesenchymal transition (MMT) and has been associated with peritoneal membrane dysfunction in peritoneal dialysis (PD) patients. It has been shown that normal and malignant mesothelial cells (MCs) express VEGF receptors (VEGFRs) and co-receptors and that VEGF is an autocrine growth factor for mesothelioma. Hence, we evaluated the expression patterns and the functional relevance of the VEGF/VEGFRs/co-receptors axis during the mesenchymal conversion of MCs induced by peritoneal dialysis. Omentum-derived MCs treated with TGF-β1 plus IL-1β (in vitro MMT) and PD effluent-derived MCs with non-epithelioid phenotype (ex vivo MMT) showed down-regulated expression of the two main receptors Flt-1/VEGFR-1 and KDR/VEGFR-2, whereas the co-receptor neuropilin-1 (Nrp-1) was up-regulated. The expression of the Nrp-1 ligand semaphorin-3A (Sema-3A), a functional VEGF competitor, was repressed throughout the MMT process. These expression pattern changes were accompanied by a reduction of the proliferation capacity and by a parallel induction of the invasive capacity of MCs that had undergone an in vitro or ex vivo MMT. Treatment with neutralizing anti-VEGF or anti-Nrp-1 antibodies showed that these molecules played a relevant role in cellular proliferation only in naïve omentum-derived MCs. Conversely, treatment with these blocking antibodies, as well as with recombinant Sema-3A, indicated that the switched VEGF/VEGFRs/co-receptors axis drove the enhanced invasion capacity of MCs undergoing MMT. Tamoxifen ameliorates peritoneal membrane damage by blocking mesothelial to mesenchymal transition in peritoneal dialysis
Mesothelial-to-mesenchymal transition (MMT) is an auto-regulated physiological process of tissue repair that in uncontrolled conditions such as peritoneal dialysis (PD) can lead to peritoneal fibrosis. The maximum expression of peritoneal fibrosis induced by PD fluids and other peritoneal processes is the encapsulating peritoneal sclerosis (EPS) for which no specific treatment exists. Tamoxifen, a synthetic estrogen, has successfully been used to treat retroperitoneal fibrosis and EPS associated with PD. Hence, we used in vitro and animal model approaches to evaluate the efficacy of Tamoxifen to inhibit the MMT as a trigger of peritoneal fibrosis. In vitro studies were carried out using omentum-derived mesothelial cells (MCs) and effluent-derived MCs. Tamoxifen blocked the MMT induced by transforming growth factor (TGF)-β1, as it preserved the expression of E-cadherin and reduced the expression of mesenchymal-associated molecules such as snail, fibronectin, collagen-I, α-smooth muscle actin, and matrix metalloproteinse-2. Tamoxifen-treatment preserved the fibrinolytic capacity of MCs treated with TGF-β1 and decreased their migration capacity. Tamoxifen did not reverse the MMT of non-epitheliod MCs from effluents, but it reduced the expression of some mesenchymal molecules. In mice PD model, we demonstrated that MMT progressed in parallel with peritoneal membrane thickness. In addition, we observed that Tamoxifen significantly reduced peritoneal thickness, angiogenesis, invasion of the compact zone by mesenchymal MCs and improved peritoneal function. Tamoxifen also reduced the effluent levels of vascular endothelial growth factor and leptin. Efficacy of the creation of a patient school program in the detection of needs in patients with autosomal dominant polycystic kidney disease
Introducción: La Poliquistosis Renal Autosómica Dominante es una enfermedad renal crónica responsable del 10% de los casos de insuficiencia renal terminal. La participación y los grupos de apoyo entre iguales son herramientas que mejoran el bienestar, evitando complicaciones y retrasando el avance de la enfermedad. Objetivos: Detectar necesidades informativas, así como recursos de apoyo, en este grupo de pacientes mediante la puesta en marcha de una Escuela de Pacientes con poliquistosis renal autosómica dominante.
Material y Método: Se utilizó un diseño mixto (cuantitativo y cualitativo). El estudio se desarrolló mediante cuatro fases: 1) Grupo focal: pacientes con poliquistosis renal y sus cuidadores; 2) Selección de los pacientes expertos; 3) Elaboración de los contenidos del programa de la Escuela de pacientes con poliquitstosis renal autosómica dominante; 4) Pilotaje del programa.
Resultados: Se detectaron necesidades de información referentes al tratamiento oral y al afrontamiento de la poliquistosis renal que no están cubiertas por los equipos de nefrología.
Conclusiones: La Escuela de Pacientes ha demostrado ser una herramienta útil para detectar necesidades y recursos en pacientes con poliquistosis renal autosómica dominante que han de enfrentarse a una enfermedad crónica donde se requiere la participación del paciente para garantizar la adhesión al tratamiento.Introduction: Autosomal Dominant Polycystic Kidney Disease is a chronic kidney disease responsible for 10% of cases of end-stage renal failure. Participation and peer support groups are tools that improve well-being, avoiding complications and delaying disease progression.
Objectives: To detect information needs, as well as support resources, in patients with autosomal dominant polycystic kidney disease trough a Patient School.
Material and Method: A mixed design (quantitative and qualitative) was used. The study was developed through four phases: 1) Focus group: patients with autosomal dominant polycystic kidney disease and their caregivers; 2) Selection of expert patients; 3) Preparation of the contents of the program of the Patient School with autosomal dominant polycystic kidney disease; 4) Piloting the program.
Results: Information needs regarding oral treatment and coping with autosomal dominant polycystic kidney disease were detected, which are not covered by nephrology teams.
Conclusions: Patients School has proven to be a useful tool to detect needs and resources in patients with autosomal dominant polycystic kidney disease who have to face a chronic disease where patient participation is required to ensure adherence to treatmen

Are the mesothelial-to-mesenchymal transition, sclerotic peritonitis syndromes, and encapsulating peritoneal sclerosis part of the same process?
Mesothelial-to-mesenchymal transition (MMT) is an autoregulated physiological process of tissue repair that in uncontrolled conditions,
such as peritoneal dialysis (PD), can lead to peritoneal fibrosis. The maximumexpression of sclerotic peritoneal syndromes
(SPS) is the encapsulating peritoneal sclerosis (EPS) for which no specific treatment exists. The SPS includes a wide range of
peritoneal fibrosis that appears progressively and is considered as a reversible process, while EPS does not. EPS is a serious
complication of PD characterized by a progressive intra-abdominal inflammatory process that results in bridles and severe fibrous
tissue formation which cover and constrict the viscera. Recent studies show that transdifferentiated mesothelial cells isolated from
the PD effluent correlate very well with the clinical events such as the number of hemoperitoneum and peritonitis, as well as with
PD function (lower ultrafiltration and high Cr-MTC). In addition, in peritoneal biopsies from PD patients, the MMT correlates
very well with anatomical changes (fibrosis and angiogenesis). However, the pathway to reach EPS from SPS has not been fully and
completely established. Herein, we present important evidence pointing to the MMT that is present in the initial peritoneal fibrosis
stages and it is perpetual over time, with at least theoretical possibility that MMT initiated the fibrosing process to reach EPSThis work was supported by Grant SAF2010-21249 from the

Evaluación de un programa de Programa de Intervención Nutricional en pacientes con enfermedad renal crónica avanzada (ERCA)
Introducción y Objetivos: La Enfermedad Renal Crónica
Avanzada (ERCA) se asocia a una elevada prevalencia de
malnutrición. La práctica habitual en estos pacientes va
dirigida a reducir la ingesta proteica, recomendación que
podría favorecer esta situación. Por ello, el objetivo de este
estudio fue evaluar el efecto de un programa de intervención
nutricional (PIN) sobre la función renal y el estado
nutricional en pacientes con ERCA.
Pacientes y Métodos: Se diseñó un estudio longitudinal
y prospectivo con 93 pacientes (53,7% hombres, 66 ±
17años) que participaron en un PIN durante 6 meses con
visitas mensuales. Al inicio y al final de la intervención se
evaluaron: estado nutricional en función de los criterios
de Chang, datos antropométricos, dietéticos y bioquímicos
(albúmina, prealbúmina, aclaramiento de creatinina,
fósforo y potasio séricos, Colesterol-total, LDL, HDL, triglicéridos
y PCR).
Resultados: Tras el PIN la ingesta calórica disminuyó
en los normonutridos (1833 ± 318 vs 1571 ± 219 kcal p <
0,001) y se mantuvo en los malnutridos. Se redujo significativamente
la ingesta proteica (69,9 ± 16,6 vs 54,9 ± 11 g
p < 0,001), potasio (2938 ± 949 vs 2377 ± 743 mg, p <
0,0001) y fósforo (1180 ± 304 vs 946,6 ± 211 mg, p <
0,0001). Un 16,5% requirió suplementación. El porcentaje
de pacientes que presentaba malnutrición según criterios
de Chan disminuyó tras el PIN (41,7 % (27,8% leve,
10,10% moderada y 3,8% grave) vs 16,8% (8% leve, 5%
moderada y 3,8% grave)). Tras el PIN incrementó significativamente
el aclaramiento de creatinina (17,8 ± 5,2 vs
19,4 ± 6,9 ml/min, p < 0,01), albúmina (3,3 ± 0,5 g/dL vs
3,5 ± 0,4 g/dL, p < 0,05) y disminuyó el potasio sérico (4,8
± 0,6 vs 4,5 ± 0,5 mmol/L, p < 0,05), Colesterol Total
(179,8 ± 44,3 vs 170,0 ± 15,1 mg/dL, p < 0,05), LDL colesterol
(113,2 ± 37,0 vs 108,3 ± 27,3 mg/dL, p < 0.01) y triglicéridos

Disease (ACKD) is associated with high prevalence of
malnutrition. The habitual continuous dietary restrictions
used in clinical practice. increased the malnutrition
risk. The aim of this study was to evaluate the effects of a
Nutritional intervention Program (NIP) on renal function
and nutritional status in patients with ACKD.
Patients and Methods: 93 patients, (53.7% men, 66 ± 17
years) were included in a prospective longitudinal study.
The patients recived a NIP during 6 months with mensual
visits. At baseline and six months the outcome assessed
were: nutritional status by Chang criteria, anthropometric,
dietetic and biochemical parameters (albumin,
prealbumin, creatinine clearance, serum phosphorus,
potassium, total-Cholesterol, LDL, HDL, triglycerides,
and PCR).
Results: After intervention, caloric intake decreased in
nourished patients (1833 ± 318 vs 1571 ± 219 kcal p =
.001). and it was constant in malnourished patients. The
intake of protein (69,9 ± 16,6 vs 54,9 ± 11 g p < 0.001),
potassium (2938 ± 949 vs 2377 ± 743 mg p < 0.001) and
phosphorus (1180 ± 304 vs 946,6 ± 211 mg p < 0.001) significantly
decreased. 16.5% patients required supplementation.
A total of 41.7% of patients were malnourished at
baseline (27.8% mild, 10.10% moderate and 3.8%
severe), and 16.8% at the end (8% mild, 5% moderate
and 3.8% severe) by Chang criteria. At the end of NIP,
patients significantly increased creatinine clearance (17,8
± 5,2 vs 19,4 ± 6,9 ml/min, p < 0,01), albumin (3,3 ± 0,5 vs.
3,5 ± 0,4 g/dL, p < 0,05), and decreased serum potassium
(4,8 ± 0,6 vs 4,5 ± 0,5 mmol/L, p < 0,05), total cholesterol
(179,8 ± 44,3 vs 170,0 ± 15,1 mg/dL, p < 0,05), LDL (113,2
± 37,0 vs 108,3 ± 27,3 mg/dL, p < 0.01) and tryglicerides
(141.9 ± 60.8 vs 129.9 ± 52.7 mg/dL, p < 0.05).
Conclusions: The study reflected a NIP usefulness in
the nutritional status and renal function improvements
within an interdisciplinary framework during ACKD
consultation

Consulta de enfermería interdisciplinar de enfermedad renal crónica avanzada: apuntes para un modelo integral de cuidados: carta al Director
La consulta de enfermería de Enfermedad Renal Cróni-ca Avanzada (ERCA) constituye un escenario especialmente complejo(1,2) tanto para el paciente como para los profesionales. Por este motivo, y con el objetivo principal de ofrecer apoyo al personal de enfermería durante estas consultas, en la Unidad de ERCA del Servicio de Nefrología del Hospital Universitario La Paz, se en-cuentra dentro del protocolo de actuación la posibilidad de que el psicólogo este físicamente presente en esta consulta. Esta modalidad de actuación está en línea con la evidencia de que un equipo interdisciplinar es necesario para mejorar la comprensión de la enfermedad renal, y con ello la calidad de vida relacionada con la salud (CVRS) de los pacientes con ERCA(3). Este protocolo ha sido publicado como carta al director en la revista Nefrología(4), aunque en la presente versión hacemos énfasis en aspectos diferenciales, principalmente en las funciones específi cas de la enfermería en la consulta de ERCA y la defi nición del perfi l de paciente ante el cual activar este protocolo. El objetivo de promover la difusión del mismo en las distintas sociedades científicas de la especialidad es recordarnos que en el abordaje integral del enfermo renal es mucho más lo que nos une que lo que nos separa
