    Average chemical properties and potential formation pathways of highly oxidized organic aerosol

    Measurements of ambient organic aerosol indicate that a substantial fraction is highly oxidized and low in volatility, but this fraction is generally not reproduced well in either laboratory studies or models. Here we describe a new approach for constraining the viable precursors and formation pathways of highly oxidized organic aerosol, by starting with the oxidized product and considering the possible reverse reactions, using a set of simple chemical rules. The focus of this work is low-volatility oxidized organic aerosol (LV-OOA), determined from factor analysis of aerosol mass spectrometer data. The elemental composition and volatility of the aerosol enable the determination of its position in a three-dimensional chemical space (defined by H/C, O/C, and carbon number) and thus its average chemical formula. Consideration of possible back-reactions then defines the movement taken through this chemical space, constraining potential reaction pathways and precursors. This approach is taken for two highly oxidized aerosol types, an average of LV-OOA factors from ten field campaigns (average formula C[subscript 10.5]H[subscript 13.4]O[subscript 7.3]), and extremely oxidized LV-OOA (from Mexico City, average formula C[subscript 10]H[subscript 12.1]O[subscript 8.4]). Results suggest that potential formation pathways include functionalization reactions that add multiple functional groups per oxidation step, oligomerization of highly oxidized precursors, and, in some cases, fragmentation reactions that involve the loss of small, reduced fragments.National Science Foundation (U.S.) (Grant CHE-1012809)National Science Foundation (U.S.) (Grant AGS-1056225

    The aging of organic aerosol in the atmosphere : chemical transformations by heterogeneous oxidation

In this work, we endeavor to improve the general understanding of the narrow class of oxidation reactions that occur at the interface between the particle surface and the gas-phase. The heterogeneous oxidation of pure erythritol (C4H1 00 4 ) and levoglucosan (C6H1 00 5) particles by hydroxyl radical (OH) was studied first in order to evaluate the effects of atmospheric aging on the mass and chemical composition of atmospheric organic aerosol, particularly that resembling fresh secondary organic aerosol (SOA) and biomass-burning organic aerosol (BBOA). In contrast to what is generally observed for the heterogeneous oxidation of reduced organics, substantial volatilization is observed in both systems. As a continuation of the heterogeneous oxidation experiments, we also measure the kinetics and products of the aging of highly oxidized organic aerosol, in which submicron particles composed of model oxidized organics -- 1,2,3,4-butanetetracarboxylic acid (C8H100 8), citric acid (C6 H8 0 7), tartaric acid (C4H6 0 6 ), and Suwannee River fulvic acid -- were oxidized by gas-phase OH in the same flow reactor, and the masses and elemental composition of the particles were monitored as a function of OH exposure. In contrast to studies of the less-oxidized model systems, particle mass did not decrease significantly with heterogeneous oxidation, although substantial chemical transformations were observed and characterized. Lastly, the immense complexity inherent in the formation of SOA -- due primarily to the large number of oxidation steps and reaction pathways involved -- has limited the detailed understanding of its underlying chemistry. In order to simplify this inherent complexity, we give over the last portion of this thesis to a novel technique for the formation of SOA through the photolysis of gas-phase alkyl iodides, which generates organic peroxy radicals of known structure. In contrast to standard OH-initiated oxidation experiments, photolytically initiated oxidation forms a limited number of products via a single reactive step. The system in which the photolytic SOA is formed is also repurposed as a generator of organic aerosol for input into a secondary reaction chamber, where the organic particles undergo additional aging by the heterogeneous oxidation mechanism already discussed. Particles exiting this reactor are observed to have become more dramatically oxidized than comparable systems containing SOA formed by gas-phase alkanes undergoing "normal" photo-oxidation by OH, suggesting simultaneously the utility of gas-phase precursor photolysis as an effective experimental platform for studying directly the chemistry involved in atmospheric aerosol formation and also the possibility that heterogeneous processes may play a more significant role in the atmosphere than what is predicted from chamber experiments.     Gambling problems among patients in primary care : a cross-sectional study of general practices

    BACKGROUND: Primary care is an important context for addressing health-related behaviours, and may provide a setting for identification of gambling problems. AIM: To indicate the extent of gambling problems among patients attending general practices, and explore settings or patient groups that experience heightened vulnerability. DESIGN AND SETTING: Cross-sectional study of patients attending 11 general practices in Bristol, South West England. METHOD: Adult patients (n = 1058) were recruited from waiting rooms of practices that were sampled on the basis of population characteristics. Patients completed anonymous questionnaires comprising measures of mental health problems (for example, depression) and addictive behaviours (for example, risky alcohol use). The Problem Gambling Severity Index (PGSI) measured gambling problems, along with a single-item measure of gambling problems among family members. Estimates of extent and variability according to practice and patient characteristics were produced. RESULTS: There were 0.9% of all patients exhibiting problem gambling (PGSI ≥5), and 4.3% reporting problems that were low to moderate in severity (PGSI 1-4). Around 7% of patients reported gambling problems among family members. Further analyses indicated that rates of any gambling problems (PGSI ≥1) were higher among males and young adults, and more tentatively, within a student healthcare setting. They were also elevated among patients exhibiting drug use, risky alcohol use, and depression. CONCLUSION: There is need for improved understanding of the burden of, and responses to, patients with gambling problems in general practices, and new strategies to increase identification to facilitate improved care and early intervention

    Millimeter Dust Emission in the GQ Lup System

    We present Submillimeter Array observations of the GQ Lup system at 1.3 millimeters wavelength with 0\farcs4 (\sim60 AU) resolution. Emission is detected from the position of the primary star, GQ Lup A, and is marginally resolved. No emission is detected from the substellar companion, GQ Lup B, 0\farcs7 away. These data, together with models of the spectral energy distribution, suggest a compact disk around GQ Lup A with mass 3\sim 3 MJup_{Jup}, perhaps truncated by tidal forces. There is no evidence for a gap or hole in the disk that might be the signature of an additional inner companion body capable of scattering GQ Lup B out to 100\sim100 AU separation from GQ Lup A. For GQ Lup B to have formed {\it in situ}, the disk would have to have been much more massive and extended.Comment: 16 pages, 2 figures, accepted to A

    Disk Imaging Survey of Chemistry with SMA (DISCS): I. Taurus Protoplanetary Disk Data

    Chemistry plays an important role in the structure and evolution of protoplanetary disks, with implications for the composition of comets and planets. This is the first of a series of papers based on data from DISCS, a Submillimeter Array survey of the chemical composition of protoplanetary disks. The six Taurus sources in the program (DM Tau, AA Tau, LkCa 15, GM Aur, CQ Tau and MWC 480) range in stellar spectral type from M1 to A4 and offer an opportunity to test the effects of stellar luminosity on the disk chemistry. The disks were observed in 10 different lines at ~3" resolution and an rms of ~100 mJy beam-1 at ~0.5 km s-1. The four brightest lines are CO 2-1, HCO+ 3-2, CN 2_3-1_2 and HCN 3-2 and these are detected toward all sources (except for HCN toward CQ Tau). The weaker lines of CN 2_2-1_1, DCO+ 3-2, N2H+ 3-2, H2CO 3_03-2_02 and 4_14-3_13 are detected toward two to three disks each, and DCN 3-2 only toward LkCa 15. CH3OH 4_21-3_12 and c-C3H2 are not detected. There is no obvious difference between the T Tauri and Herbig Ae sources with regard to CN and HCN intensities. In contrast, DCO+, DCN, N2H+ and H2CO are detected only toward the T Tauri stars, suggesting that the disks around Herbig Ae stars lack cold regions for long enough timescales to allow for efficient deuterium chemistry, CO freeze-out, and grain chemistry.Comment: 29 pages, 4 figures, accepted for publication in Ap

    Economic burden and comorbidities of attention-deficit/hyperactivity disorder among pediatric patients hospitalized in the United States

    <p>Abstract</p> <p>Background</p> <p>This retrospective database analysis used data from the Healthcare Cost and Utilization Project's Nationwide Inpatient Sample (NIS) to examine common primary diagnoses among children and adolescents hospitalized with a secondary diagnosis of attention- deficit/hyperactivity disorder (ADHD) and assessed the burden of ADHD.</p> <p>Methods</p> <p>Hospitalized children (aged 6-11 years) and adolescents (aged 12-17 years) with a secondary diagnosis of ADHD were identified. The 10 most common primary diagnoses (using the first 3 digits of the ICD-9-CM code) were reported for each age group. Patients with 1 of these conditions were selected to analyze demographics, length of stay (LOS), and costs. Control patients were selected if they had 1 of the 10 primary diagnoses and no secondary ADHD diagnosis. Patient and hospital characteristics were reported by cohort (i.e., patients with ADHD vs. controls), and LOS and costs were reported by primary diagnosis. Multivariable linear regression analyses were undertaken to adjust LOS and costs based on patient and hospital characteristics.</p> <p>Results</p> <p>A total of 126,056 children and 204,176 adolescents were identified as having a secondary diagnosis of ADHD. Among children and adolescents with ADHD, the most common diagnoses tended to be mental health related (i.e., affective psychoses, emotional disturbances, conduct disturbances, depressive disorder, or adjustment reaction). Other common diagnoses included general symptoms, asthma (in children only), and acute appendicitis. Among patients with ADHD, a higher percentage were male, white, and covered by Medicaid. LOS and costs were higher among children with ADHD and a primary diagnosis of affective psychoses (by 0.61 days and 51),adjustmentreaction(by1.71daysand51), adjustment reaction (by 1.71 days and 940), or depressive disorder (by 0.41 days and 124)versuscontrols.LOSandcostswerehigheramongadolescentswithADHDandaprimarydiagnosisofaffectivepsychoses(by1.04daysand124) versus controls. LOS and costs were higher among adolescents with ADHD and a primary diagnosis of affective psychoses (by 1.04 days and 352), depressive disorder (by 0.94 days and 517),conductdisturbances(by0.86daysand517), conduct disturbances (by 0.86 days and 1,330), emotional disturbances (by 1.45 days and 1,626),adjustmentreaction(by1.25daysand1,626), adjustment reaction (by 1.25 days and 702), and neurotic disorders (by 1.60 days and $541) versus controls.</p> <p>Conclusion</p> <p>Clinicians and health care decision makers should be aware of the potential impact of ADHD on hospitalized children and adolescents.</p

    Association of early imaging for back pain with clinical outcomes in older adults

    IMPORTANCE: In contrast to the recommendations for younger adults, many guidelines allow for older adults with back pain to undergo imaging without waiting 4 to 6 weeks. However, early imaging may precipitate interventions that do not improve outcomes. OBJECTIVE: To compare function and pain at the 12-month follow-up visit among older adults who received early imaging with those who did not receive early imaging after a new primary care visit for back pain without radiculopathy. DESIGN, SETTING, AND PARTICIPANTS: Prospective cohort of 5239 patients 65 years or older with a new primary care visit for back pain (2011-2013) in 3 US health care systems. We matched controls 1:1 using propensity score matching of demographic and clinical characteristics, including diagnosis, pain severity, pain duration, functional status, and prior resource use. EXPOSURES: Diagnostic imaging (plain films, computed tomography [CT], magnetic resonance imaging [MRI]) of the lumbar or thoracic spine within 6 weeks of the index visit. PRIMARY OUTCOME: back or leg pain-related disability measured by the modified Roland-Morris Disability Questionnaire (score range, 0-24; higher scores indicate greater disability) 12 months after enrollment. RESULTS: Among the 5239 patients, 1174 had early radiographs and 349 had early MRI/CT. At 12 months, neither the early radiograph group nor the early MRI/CT group differed significantly from controls on the disability questionnaire. The mean score for patients who underwent early radiography was 8.54 vs 8.74 among the control group (difference, -0.10 [95% CI, -0.71 to 0.50]; mixed model, P = .36). The mean score for the early MRI/CT group was 9.81 vs 10.50 for the control group (difference,-0.51 [-1.62 to 0.60]; mixed model, P = .18). CONCLUSIONS AND RELEVANCE: Among older adults with a new primary care visit for back pain, early imaging was not associated with better 1-year outcomes. The value of early diagnostic imaging in older adults for back pain without radiculopathy is uncertain
