    Predisposition To Metabolic Acidosis Induced By Topiramate.

    Metabolic acidosis induced by topiramate is a well documented but infrequent adverse event. The objective was to demonstrate the lowering of carbon dioxide serum levels, which is usually asymptomatic but may facilitate the occurrence of metabolic acidosis in patients using topiramate. We evaluated, prospectively, the carbon dioxide serum levels of 18 patients seen at the epilepsy clinic of our university hospital, before and 3 months after introducing topiramate. Five patients were female and 13 were male, age ranging from 2 to 16 years old (mean=9. 3). Carbon dioxide mean serum levels were 25 and 21.2 mmol/L (normal = 22 to 30), before and 3 months after introducing topiramate, respectively. Dose ranged from 2.08 to 11.76 mg/kg/day (mean=6. 7mg/kg/day). Adverse events were anorexia, nausea and somnolence. We conclude that the lowering of carbon dioxide serum levels induced by topiramate is mostly asymptomatic, but may facilitate the occurrence of metabolic acidosis. Since patients in use of topiramate have refractory epilepsy, they may need epilepsy surgery, and must be carefully monitored for the risk of metabolic acidosis during surgery.581021-

Epilepsia foi a manifestação clínica presente em três casos, cuja investigação laboratorial revelou haver distúrbio do metabolismo do cálcio, pseudo-hipoparatireoidismo em um dos casos com certeza e, provavelmente, também nos outros dois pacientes. A primeira finalidade deste estudo é divulgar entidade potencialmente tratável, como causa de síndrome epiléptica na infância.     The objective of this work was to study the immunological behavior of newborn Mangalarga and Angle-Arabian foals, the acquiring process of maternal immunoglobulins and its correlation to the levels of colostral immunoglobulin. The Mangalarga and Angle-Arabian foals serum immunoglobulins were analysed by the ZST (Zinc Sulfate Turbidity) method. The results were tested by mathematical models which study the biological process of passibe immunity transfer in that period of observation: to the Mangalarga foals the tested model was: y = 23.9274 - 0.39766X + 6.4675 10-3 x², with r = 0.91 and P < 0.01 to Angle-Arabian foals was: y = 34.161 - 0.756062X + 0.015604X² - 1.013 10-3 x³, with r = 0.96 and P < 0.05. Estimation of the colostral IgG concentration was based on total protein concentration measured by Micro-Kjeldhal method. The results of this study allowed the following conclusions: (1) higher passive IgG in foals serum delayed the stablishment of normal Ig level; (2) animals which acquired lower quantities of passive IgG showed more intense response of endogenous Ig production; (3) colostrum estimated IgG concentration showed positive correlation Serum IgG concentration of the foals at the point of maximal absorption showed positive correlation with colostrum estimated IgG.O objetivo deste trabalho foi observar o comportamento imunológico de potros recém-nascidos das raças Mangalarga e Anglo-Árabe no que se refere ao processo de aquisição de anticorpos maternos e sua correlação com os níveis de imunoglobulina do colostro. Foram utilizados 7 potros Anglo-Árabe e 6 potros Mangalarga para amostragem de sangue imediatamente após o nascimento (antes de qualquer ingestão de colostro), 24 e 48 horas, 5, 10, 15, 20, 25, 30, 40, 50, 60 e 70 dias após o nascimento, e suas respectivas mães cujo colostro foi amostrado imediatamente após a parição, antes da primeira mamada. A quantificação das imunoglobulinas séricas foi efetuada pelo método do ZST (Zinc Sulfate Turbidity) e para a análise dos resultados foram testados modelos matemáticos que estudam o processo em questão. Para a raça Mangalarga o modelo matemático foi: y = 23,9274 - 0,39766x + 6,4675 10-3 x², com r = 0,91 e P < 0,01 e para a raça Anglo Árabe foi y = 34,161 - 0,756062x + 0,015604x² - 1,013 10-3 x³, com r = 0,96 e P < 0,05. A concentração de IgG do colostro foi estimada com base na concentração de proteína total avaliada pelo método Micro-Kjeldhal. Este estudo permitiu concluir: (1) a presença de maior quantidade de IgG (imunoglobulina G) passiva no sangue dos potros, retardou o estabelecimento dos níveis normais de Ig (imunoglobulina G + M+A+E); (2) animais que adquiriram menor quantidade de IgG passiva apresentaram resposta mais intensa de produção endógena de Ig; (3) a concentração estimada de IgG no colostro apresentou correlação positiva com a concentração sérica dos potros no pico da absorção

    Phenytoin is an effective antiepileptic drug, although, it can be associated with many side effects, including dyskinesia. OBJECTIVE: To describe the clinical characteristics of phenytoin induced dyskinesia. METHODS: We investigated the occurrence of involuntary movements in patients followed at our adult and pediatric epilepsy clinics during the period of one year. RESULTS: Three patients presented with phenytoin-induced dyskinesia: one adult with axial and orofacial dyskinesia, and two children with choreoathetosis. They did not have other signs of phenytoin intoxication and had complete recovery after phenytoin withdrawal. CONCLUSION: Phenytoin induced dyskinesia may occur during either chronic or initial treatment and with normal serum phenytoin levels. However, it occurs most often in patients on polytherapy, usually after increasing dosage and with toxic serum levels. Other signs of phenytoin intoxication may be present in these patients, but often the dyskinesia is the only side effect, which may delay the diagnosis and treatment. The clinical characteristics of the involuntary movements vary and may be focal or generalized, most often characterized by choreoathetosis and dyskinesias. These may last for hours, days or even years, but frequently disappear completely after phenytoin withdrawal.Fenitoína é droga anti-epiléptica eficaz, mas pode estar associada a vários efeitos colaterais, inclusive discinesia. OBJETIVO: Descrever as características clínicas da discinesia induzida por fenitoína. MÉTODO: Avaliamos a ocorrência de movimentos involuntários em pacientes seguidos nos ambulatórios de epilepsia durante o período de um ano. RESULTADOS: Três pacientes apresentaram discinesia induzida por fenitoína: um adulto com discinesia orofacial e duas crianças com coreoatetose. Eles não tinham outros sinais de intoxicação por fenitoína e apresentaram recuperação completa após a retirada da fenitoína. CONCLUSÃO: Discinesia induzida pela fenitoína pode ocorrer durante tratamento crônico ou no início do tratamento e com níveis séricos normais de fenitoína. Entretanto, na maioria das vezes ocorre em politerapia e geralmente após aumento da dose e com níveis tóxicos de fenitoína. Outros sinais de intoxicação podem estar presentes, mas muitas vezes a discinesia é o único efeito adverso, o que pode atrasar o diagnóstico e tratamento. Os movimentos involuntários podem ser focais ou generalizados. Podem durar horas, dias ou até anos, mas freqüentemente desaparecem completamente após a retirada da fenitoína.35636

    Stereodynamical Effects by Anisotropic Intermolecular Forces

    Electric and magnetic field gradients, arising from sufficiently strong anisotropic intermolecular forces, tend to induce molecular polarization which can often modify substantially the results of molecular collisions, especially at low rotational temperatures and low collision energies. The knowledge of these phenomena, today still not fully understood, is of general relevance for the control of the stereo-dynamics of elementary chemical-physical processes, involving neutral and ionic species under a variety of conditions. This paper reports on results obtained by combining information from scattering, spectroscopic and reactivity experiments, within a collaboration between the research groups in Perugia and Trento. We addressed particular attention to the reactions of small atomic ions with polar neutrals for their relevance in several environments, including interstellar medium, planetary atmospheres, and laboratory plasmas. In the case of ion-molecule reactions, alignment/orientation is a general phenomenon due to the electric field generated by the charged particle. Such phenomenon originates critical stereo-dynamic effects that can either suppress (when the orientation drives the collision complex into non-reactive or less reactive configurations), or enhance the reactivity (when orientation confines reagents in the most appropriate configuration for reaction). The associated rate coefficients show the propensity to follow an Arrhenius and a non-Arrhenius behavior, respectively
