    The air quality perceptions of the residents of Bayview, Mossel Bay

    Background: In developing countries, it often occurs that little attention is given to air pollution emissions due to a lack of proper town planning, household combustion processes, energy production and the continuous growth in the transport sector (Norman et al., 2007:783). There is an increase in urban air pollution in most of the major cities of developing countries which is amplified by population growth and industrialization (World Resource Institute, 1998, 1999:1). Air pollution studies are not complete, and may fail if the quality of life and the perceptions of the studied community are not taken into consideration. This paper investigates the air quality perceptions of a high income residency surrounded by industrial activities and Mossel Bay was rated as to have potentially poor air quality by the South African Department of Environmental Affairs and Tourism. Methods: A cross-sectional survey was carried out in Bayview, Mossel Bay. The perceptions of the respondents were collected by a structured questionnaire. Components of perceptions that were tested included general opinion regarding air quality, visual perceptions of air quality, type of pollutants such as smoke and dust, perceptions regarding the source of air pollution, perceptions regarding the municipal health institution controlling air quality in Bayview, etc. These perceptions were investigated by age, gender, socio-economic status etc

    Prevalence of Salmonella in juvenile dogs affected with parvoviral enteritis

    Salmonellosis is a disease of major zoonotic importance and canine parvovirus is a potentially fatal cause of canine enteritis with a world-wide distribution. Persistent isolation of Salmonella during routine environmental sampling surveys of a hospital ward, reserved for the treatment of dogs with canine parvovirus infection, prompted investigation into a possible source. We hypothesised that dogs affected by canine parvovirus would have a higher prevalence of faecal salmonellae compared to an apparently healthy cohort. Seventy-four client-owned dogs naturally infected with canine parvovirus and 42 apparently healthy client-owned dogs were included in the study. This prospective, longitudinal, observational study was conducted over an 18-month period. Fresh faecal samples were collected from dogs aged 6 weeks to 9 months diagnosed with canine parvovirus infection and admitted for treatment, and from apparently healthy dogs presented for vaccination or routine hospital procedures. Faeces were submitted for the isolation, antimicrobial susceptibility testing and serotyping of salmonellae. The prevalence of faecal Salmonella shedding was 22% and 31% for the affected and apparently healthy dogs, respectively, which was not statistically different. No significant associations between Salmonella status and possible risk factors or continuous variables such as age, body weight and duration of hospitalisation were identified. All the Salmonella isolates (n = 32) were resistant to penicillin G, lincomycin and tylosin. Salmonellae from nine different serotypes were identified. The prevalence of Salmonella shedding in both groups was higher than that commonly reported, yet similar to those in previous reports on young dogs, shelter dogs or dogs fed a raw meat diet

    A case-based, small-group cooperative learning course in preclinical veterinary science aimed at bridging basic science and clinical literacy

    In 1999 a dedicated problem-based learning course was introduced into the lecture-based preclinical veterinary curriculum of the University of Pretoria. The Introduction to Clinical Studies Course combines traditional lectures, practical sessions, student self-learning and guided tutorials. The self-directed component of the course utilises case-based, small group cooperative learning as an educational vehicle to link basic science with clinical medicine. The aim of this article is to describe the objectives and structure of the course and to report the results of the assessment of the students' perceptions on some aspects of the course. Students reacted very positively to the ability of the course to equip them with problem-solving skills. Students indicated positive perceptions about the workload of the course. There were, however, significantly lower scores for the clarity of the course objectives. Although the study guide for the course is very comprehensive, the practice regarding the objectives is still uncertain. It is imperative to set clear objectives in non-traditional, student-centred courses. The objectives have to be explained at the outset and reiterated throughout the course. Tutors should also communicate the rationale behind problem based learning as a pedagogical method to the students. Further research is needed to verify the effectiveness of this course in bridging the gap between basic science and clinical literacy in veterinary science. Ongoing feedback and assessment of the management and content are important to refine this model for integrating basic science with clinical literacy

    Evaluation of acute kidney injury in dogs with complicated or uncomplicated Babesia rossi infection

    Dogs with babesiosis can present with multiple complications, including acute kidney injury (AKI). The objective of this study was to characterize AKI in dogs with babesiosis caused by Babesia rossi at presentation and after treatment. Thirty-five client-owned dogs with B. rossi infection and 10 control dogs were included in this prospective observational study. Blood and urine were collected in Babesia-infected dogs at presentation (T-0, n = 35), after 24 h (T-24h, n = 11), and after 1 month (T-1m, n = 9). The following urinary kidney injury biomarkers were assessed: urinary protein to creatinine ratio (UPC), urinary glomerular injury biomarkers (immunoglobulin G (uIgG) and C-reactive protein (uCRP)), and urinary tubular injury biomarkers (retinol-binding protein (uRBP) and neutrophil gelatinase-associated lipocalin (uNGAL)). Serum functional renal biomarkers were creatinine (sCr) and symmetric dimethylarginine (sSDMA). Post-mortem kidney biopsies were analyzed by light and transmission electron microscopy. At T-0, all kidney injury biomarkers were significantly higher in Babesia-infected dogs compared to healthy controls (P 0.05). At T-24h, all urinary tubular injury biomarkers and UPC decreased significantly (P 0.5). Significant changes in functional renal biomarkers were not seen after treatment (P > 0.05). Dogs with complicated babesiosis had significantly higher glomerular injury biomarkers, UPC, and sSDMA compared to uncomplicated cases (P 0.1). Dogs with babesiosis caused by B. rossi showed transient kidney injury, which was detected by all kidney injury biomarkers, but remained undetected by functional biomarkers. All infected dogs, irrespective of disease severity, suffered comparable kidney injury based on tubular injury biomarker concentrations, while loss of function was seen more often in dogs with complicated babesiosis based on sSDMA results

    Epidemiological reference ranges for low-density lipoprotein cholesterol and apolipoprotein B for identification of increased risk of ischaemic heart disease

    Although there is widespread acceptance that total cholesterol (TC) value reference ranges should be based on epidemiological rather than statistical considerations, the epidemiological action limits for Iow-density lipoprotein cholesterol (LDL-C) are still incomplete and only statistical reference ranges for apolipoprotein B (Apo-B) levels are available. The combined use of epidemiological reference ranges for TC and incomplete or statistical reference ranges for LDL-C and Apo-B is illogical, since these parameters may fall into discordant risk categories that will hamper and complicate the management of hypercholesterolaemia. Based on a study of Iipograms obtained from ± 3 000 inhabitants of two industrialised Transvaal towns, the agerelated epidemiological reference ranges for LDL-C and ApoB were established. A comparison with published observational studies of other populations, in which comparable lipid, lipoprotein and apolipoprotein methodologies were used, reflected the severity of these lipid-related abnormalities in white South Africans, especially after the age of 30 years. In addition, the serum TC values found in this survey were not significantly different from those obtained 10 years ago

    Research strategy for missional transformation. In this study, an innovative research process was developed to support a missional ecclesiology. The research strategy was designed as a practice-oriented research process in service of faith communities as ‘problem owners’ of the research. The goal is to inform and serve the process of missional transformation. The approach taken was defined as a process of discernment to participate in the missio Dei, appreciating the work of the Holy Spirit and the reciprocal relation between confession and praxis. Scripture and tradition are constitutive elements of the language house that forms the congregational life and imagination. The innovate process comprises three cycles moving through four quadrants in the deployment of a missional strategy, the four quadrants being: guidance, research, design and training. This was developed along 12 movements: (1) articulate the pain, (2) clarify the question, (3) develop the prototype, (4) testing, (5) practice capacities, (6) observe patterns, (7) build a model, (8) implementation, (9) accepting into the culture, (10) describe breakthroughs, (11) support the learning community, and (12) institutional alignment. Intradisciplinary and/or interdisciplinary implications: The research includes the following disciplines: Missiology, Missionary Ecclesiology and Practical Theology. It has wide-ranging implications, as it presents an innovative and comprehensive research process that can significantly influence research on missional transformation

    A comparison of the pharmacokinetics of Aspen Ceftriaxone and Rocephin in community-acquired meningitis

    BACKGROUND. Community-acquired bacterial meningitis (CABM) is a life-threatening condition that is common among immunocompromised individuals. Intravenous ceftriaxone, of which Rocephin (ROC) is the originator brand, is recommended as first-line therapy in South Africa. Despite concerns regarding therapeutic equivalence with generic agents, this is the first study that has been conducted comparing clinical pharmacokinetics (PK) of a generic ceftriaxone formulation with the originator. OBJECTIVE. To compare the PK and safety of Aspen Ceftriaxone (AC) and ROC in the treatment of adult CABM. METHODS. A total of 63 eligible patients were randomised 1:1 to receive 2 g of either medication twice daily for a duration based on the identity of the causative organism and their physician’s clinical judgment. The primary endpoint of this study was the comparison of clinical PK, specifically the concentrations of each drug in the cerebrospinal fluid with corresponding paired plasma samples. While this study was underpowered to assess efficacy, safety could be evaluated on the basis of reported adverse events. RESULTS. The two patient groups were epidemiologically similar. There were no statistically significant differences in PK between either agent, nor any difference with regard to safety. CONCLUSION. AC can be considered as equivalent to ROC with regard to PK and safety in patients with CABM.Aspen Pharmacare Ltd, SA.http://www.samj.org.zaam2014ay201
