Nationally representative results on SARS-CoV-2 seroprevalence and testing in Germany at the end of 2020
Pre-vaccine SARS-CoV-2 seroprevalence data from Germany are scarce outside hotspots, and socioeconomic disparities remained largely unexplored. The nationwide representative RKI-SOEP study (15,122 participants, 18â99 years, 54% women) investigated seroprevalence and testing in a supplementary wave of the Socio-Economic-Panel conducted predominantly in OctoberâNovember 2020. Self-collected oral-nasal swabs were PCR-positive in 0.4% and Euroimmun anti-SARS-CoV-2-S1-IgG ELISA from dry-capillary-blood antibody-positive in 1.3% (95% CI 0.9â1.7%, population-weighted, corrected for sensitivityâ=â0.811, specificityâ=â0.997). Seroprevalence was 1.7% (95% CI 1.2â2.3%) when additionally correcting for antibody decay. Overall infection prevalence including self-reports was 2.1%. We estimate 45% (95% CI 21â60%) undetected cases and lower detection in socioeconomically deprived districts. Prior SARS-CoV-2 testing was reported by 18% from the lower educational group vs. 25% and 26% from the medium and high educational group (pâ<â0.001, global test over three categories). Symptom-triggered test frequency was similar across educational groups. Routine testing was more common in low-educated adults, whereas travel-related testing and testing after contact with infected persons was more common in highly educated groups. This countrywide very low pre-vaccine seroprevalence in Germany at the end of 2020 can serve to evaluate the containment strategy. Our findings on social disparities indicate improvement potential in pandemic planning for people in socially disadvantaged circumstances.Peer Reviewe
Corona Monitoring Nationwide (RKI-SOEP-2): Seroepidemiological Study on the Spread of SARS-CoV-2 Across Germany
SARS-CoV-2, the coronavirus, spread across Germany within just a short period of time. Seroepidemiological studies are able to estimate the proportion of the population with antibodies against SARS-CoV-2 infection (seroprevalence) as well as the level of undetected infections, which are not captured in official figures. In the seroepidemiological study Corona Monitoring Nationwide (RKI-SOEP-2), biospecimens and interview data were collected in a nationwide population-based subsample of the Socio-Economic Panel (SOEP). By using laboratory-analyzed blood samples to detect antibodies to the SARS-CoV-2 virus, we were able to identify a history of vaccination or infection in study participants. By combining these results with survey data, we were able to identify groups within the population that are at increased risk of infection. By linking the RKI-SOEP-2 survey data with data from other waves of the SOEP survey, we will be able to examine the medium- to long-term impacts of the COVID-19 pandemic, including effects of long COVID, in diverse areas of life. Furthermore, the data provide insight into the populationâs willingness to be vaccinated as well as related attitudes and conditions. In sum, the RKI-SOEP-2 survey data offer a better understanding of the scope of the epidemic in Germany and can help in identifying target groups for infection control in the present and future pandemics.Peer Reviewe
Seroepidemiological study on the spread of SARS-CoV-2 in Germany:
The SARS-CoV-2 coronavirus has spread rapidly across Germany. Infections are likely to be under-recorded in the notification data from local health authorities on laboratory-confirmed cases since SARS-CoV-2 infections can proceed with few symptoms and then often remain undetected. Seroepidemiological studies allow the estimation of the proportion in the population that has been infected with SARS-CoV-2 (seroprevalence) as well as the extent of undetected infections.
The âCORONA-MONITORING bundesweitâ study (RKI-SOEP study) collects biospecimens and interview data in a nationwide population sample drawn from the German Socio-Economic Panel (SOEP).
Participants are sent materials to self-collect a dry blood sample of capillary blood from their finger and a swab sample from their mouth and nose, as well as a questionnaire. The samples returned are tested for SARS-CoV-2 IgG antibodies and SARS-CoV-2 RNA to identify past or present infections.
The methods applied enable the identification of SARS-CoV-2 infections, including those that previously went undetected. In addition, by linking the data collected with available SOEP data, the study has the potential to investigate social and health-related differences in infection status. Thus, the study contributes to an improved understanding of the extent of the epidemic in Germany, as well as identification of target groups for infection protection
SARS-CoV-2 Transmissibility Within Day Care CentersâStudy Protocol of a Prospective Analysis of Outbreaks in Germany
Introduction: Until today, the role of children in the transmission dynamics of SARS-CoV-2 and the development of the COVID-19 pandemic seems to be dynamic and is not finally resolved. The primary aim of this study is to investigate the transmission dynamics of SARS-CoV-2 in child day care centers and connected households as well as transmission-related indicators and clinical symptoms among children and adults.
Methods and Analysis: COALA (âCorona outbreak-related examinations in day care centersâ) is a day care center- and household-based study with a case-ascertained study design. Based on day care centers with at least one reported case of SARS-CoV-2, we include one- to six-year-old children and staff of the affected group in the day care center as well as their respective households. We visit each child's and adult's household. During the home visit we take from each household member a combined mouth and nose swab as well as a saliva sample for analysis of SARS-CoV-2-RNA by real-time reverse transcription polymerase chain reaction (real-time RT-PCR) and a capillary blood sample for a retrospective assessment of an earlier SARS-CoV-2 infection. Furthermore, information on health status, socio-demographics and COVID-19 protective measures are collected via a short telephone interview in the subsequent days. In the following 12 days, household members (or parents for their children) self-collect the same respiratory samples as described above every 3 days and a stool sample for children once. COVID-19 symptoms are documented daily in a symptom diary. Approximately 35 days after testing the index case, every participant who tested positive for SARS-CoV-2 during the study is re-visited at home for another capillary blood sample and a standardized interview. The analysis includes secondary attack rates, by age of primary case, both in the day care center and in households, as well as viral shedding dynamics, including the beginning of shedding relative to symptom onset and viral clearance.
Discussion: The results contribute to a better understanding of the epidemiological and virological transmission-related indicators of SARS-CoV-2 among young children, as compared to adults and the interplay between day care and households.Peer Reviewe
The appropriateness of prescribing antibiotics in the community in Europe: study design

97417.pdf (publisher's version ) (Open Access)ABSTRACT: BACKGROUND: Over 90% of all antibiotics in Europe are prescribed in primary care. It is important that antibiotics are prescribed that are likely to be effective; however, information about antibiotic resistance in the community is incomplete. The aim of our study is to investigate the appropriateness of antibiotic prescribing in primary care in Europe by collecting and combining patterns of antibiotic resistance patterns and antibiotic prescription patterns in primary care. We will also evaluate the appropriateness of national antibiotic prescription guidelines in relation to resistance patterns. METHODS/DESIGN: Antibiotic resistance will be studied in an opportunistic sample from the community in nine European countries. Resistance data will be collected by taking a nose swab of persons (N = 4,000 per country) visiting a primary care practice for a non-infectious disease. Staphylococcus aureus and Streptococcus pneumoniae will be isolated and tested for resistance to a range of antibiotics in one central laboratory. Data on antibiotic prescriptions over the past 5 years will be extracted from the electronic medical records of General Practitioners (GPs). The results of the study will include the prevalence and resistance data of the two species and 5 years of antibiotic prescription data in nine European countries.The odds of receiving an effective antibiotic in each country will be calculated as a measure for the appropriateness of prescribing. Multilevel analysis will be used to assess the appropriateness of prescribing. Relevant treatment guidelines of the nine participating countries will be evaluated using a standardized instrument and related to the resistance patterns in that country. DISCUSSION: This study will provide valuable and unique data concerning resistance patterns and prescription behaviour in primary care in nine European countries. It will provide evidence-based recommendations for antibiotic treatment guidelines that take resistance patterns into account which will be useful for both clinicians and policy makers. By improving antibiotic use we can move towards controlling the resistance problem globally
PrÀvalenz und Charakteristika von Kindern und Jugendlichen mit speziellem Versorgungsbedarf im Kinder- und Jugendgesundheitssurvey (KiGGS) in Deutschland
Um zu bevölkerungsreprĂ€sentativen EinschĂ€tzungen der PrĂ€valenz und der Charakteristika von Kindern und Jugendlichen mit gesundheitsbedingtem Versorgungsbedarf zu gelangen, sind Screening-Instrumente entwickelt worden. Diese zielen auf eine Erfassung von Konsequenzen körperlicher, seelischer und verhaltensbedingter Störungen ab, unabhĂ€ngig von den zugrunde liegenden medizinischen Diagnosen. Eines der bestuntersuchten und unter Machbarkeitsaspekten bewĂ€hrtesten Instrumente, der CSHCN-(Children with Special Health Care Needs)Screener, wurde in den Elternfragebogen des Kinder- und Jugendgesundheitssurveys (KiGGS) in Deutschland integriert. Die gewichtete GesamtprĂ€valenz von Kindern und Jugendlichen mit speziellem Versorgungsbedarf betrug 16,0% fĂŒr Jungen und 11,4% fĂŒr MĂ€dchen. Bei Kindern im Vorschul- und Schulalter lag nach den Befragungsergebnissen ein spezieller Versorgungsbedarf 2- bis 3-mal hĂ€ufiger vor als bei Kleinkindern. Bis zu einem Alter von 14 Jahren war ein deutlich höherer Versorgungsbedarf bei Jungen als bei MĂ€dchen fĂŒr alle Altersgruppen ersichtlich. Am deutlichsten ausgeprĂ€gt war der Geschlechtsunterschied bei den 3- bis 10-JĂ€hrigen. Kinder und Jugendliche mit Migrationshintergrund wiesen einen signifikant niedrigeren Versorgungsbedarf auf als Kinder ohne Migrationshintergrund. Dies traf insbesondere auf die Jungen (8,0% vs. 17,1%) zu. Signifikante Unterschiede im Versorgungsbedarf nach Sozialstatus, GröĂe des Wohnortes oder Zugehörigkeit des Wohnortes zu den alten oder neuen LĂ€ndern wurden nicht beobachtet. Mit Ausnahme eines fehlenden Zusammenhangs zwischen Versorgungsbedarf und sozioökonomischem Status zeigen die hier berichteten Ergebnisse gute Ăbereinstimmung mit Beobachtungen im US-amerikanischen National Survey of CSHCN.In order to arrive at population-based estimates on the prevalence and characteristics of children and adolescents with specific health care needs (CSHCN), screening instruments focussing on the consequences of physical, mental and behavioral problems rather than on medical diagnoses have been developed. One of the most feasible and widely tested instruments, the CSHCN screener was added to the self-administered questionnaire for parents of children participating in the German Health Interview and Examination Survey for Children and Adolescents (KiGGS). The overall weighted prevalence of CSHCN was 16.0% among boys and 11.4% among girls. Children at kindergarten or school age were more than 2-3 times more likely to screen positive compared to toddlers. Up to 14 years, the sex difference persisted through all age groups and was most pronounced between the ages of 3 and 10 years. Children with a migrant background had significantly lower rates of CSHCN compared to non-migrants. This was particularly true for boys (8.0% vs. 17.1%). CSHCN status was not related to social status, urbanization or residence in former West vs. former East Germany. Except for the lack of association with social status, these results are in good accordance with observations from the US National Survey of CSHCN
A bead-based multiplex assay covering all coronaviruses pathogenic for humans for sensitive and specific surveillance of SARS-CoV-2 humoral immunity
Serological assays measuring antibodies against SARS-CoV-2 are key to describe the epidemiology, pathobiology or induction of immunity after infection or vaccination. Of those, multiplex assays targeting multiple antigens are especially helpful as closely related coronaviruses or other antigens can be analysed simultaneously from small sample volumes, hereby shedding light on patterns in the immune response that would otherwise remain undetected. We established a bead-based 17-plex assay detecting antibodies targeting antigens from all coronaviruses pathogenic for humans: SARS-CoV-2, SARS-CoV, MERS-CoV, HCoV strains 229E, OC43, HKU1, and NL63. The assay was validated against five commercial serological immunoassays, a commercial surrogate virus neutralisation test, and a virus neutralisation assay, all targeting SARS-CoV-2. It was found to be highly versatile as shown by antibody detection from both serum and dried blot spots and as shown in three case studies. First, we followed seroconversion for all four endemic HCoV strains and SARS-CoV-2 in an outbreak study in day-care centres for children. Second, we were able to link a more severe clinical course to a stronger IgG response with this 17-plex-assay, which was IgG1 and IgG3 dominated. Finally, our assay was able to discriminate recent from previous SARS-CoV-2 infections by calculating the IgG/IgM ratio on the N antigen targeting antibodies. In conclusion, due to the comprehensive method comparison, thorough validation, and the proven versatility, our multiplex assay is a valuable tool for studies on coronavirus serology
