    Effect of arm movement and task difficulty level on balance performance in healthy children:are there sex differences?

    OBJECTIVE: In children, studies have shown that balance performance is worse in boys compared to girls and further studies revealed inferior performance when arm movement was restricted during balance assessment. However, it remains unclear whether restriction of arm movement during balance testing differentially affects children’s balance performance (i.e., boys more than girls). Thus, we compared the influence of arm movement on balance performance in healthy boys versus girls (mean age: ~ 11.5 years) while performing balance tasks with various difficulty level. RESULTS: In nearly all tests, balance performance (i.e., timed one-legged stance, 3-m beam walking backward step number, Lower Quarter Y-Balance test reach distance) was significantly worse during restricted compared to free arm movement but without any differences between sexes or varying levels of task difficulty. These findings indicated that balance performance is negatively affected by restriction of arm movement, but this does not seem to be additionally influenced by children’s sex and the level of task difficulty

    Time-course of balance training-related changes on static and dynamic balance performance in healthy children

    Objective: In healthy children, there is evidence of improvements in static and dynamic balance performance following balance training. However, the time-course of balance training-related changes is unknown. Thus, we determined the effects of balance training after one, three, and six weeks of exercise on measures of static and dynamic balance in healthy children (N = 44, 20 females, mean age: 9.6 ± 0.5 years, age range: 9–11 years). Results: Participants in the intervention group (2 × 25 min balance exercises per week) compared to those in the control group (2 × 25 min track and field exercises and soccer practice per week) significantly improved their static (i.e., by measuring stance time in the One-Legged Stance test) and dynamic (i.e., by counting step number in the 3-m Beam Walking Backward test) balance performance. Late effects (after 6 weeks) occurred most frequently followed by mid-term effects (after 3 weeks) and then early effects (after 1 week). These findings imply that balance training is effective to improve static and dynamic measures of balance in healthy children, whereby the effectiveness increases with increasing training period.     Effect of Arm Movement and Task Difficulty on Balance Performance in Children, Adolescents, and Young Adults

    BACKGROUND: Studies have shown that restricted compared to free arm movement negatively affects balance performance during balance assessment and this is reinforced when the level of task difficulty (e.g., varying stance/walk conditions, sensory manipulations) is increased. However, it remains unclear whether these findings apply to individuals with differences in the development of the postural control system. Thus, we examined the influence of arm movement and task difficulty on balance performance in children, adolescents, and young adults. METHODS: Static, dynamic, and proactive balance performance were assessed in 40 children (11.5 ± 0.6 years), 30 adolescents (14.0 ± 1.1 years), and 41 young adults (24.7 ± 3.0 years) using the same standardized balance tests [i.e., one-legged stance (OLS) time with eyes opened/closed and/or on firm/foam ground, 3-m beam (width: 6, 4.5, or 3 cm) walking backward step number, Lower Quarter Y-Balance test (YBT-LQ) reach distance] with various difficulty levels under free vs. restricted arm movement conditions. RESULTS: In all but one test, balance performance was significantly better during free compared to restricted arm movement. Arm by age interactions were only observed for the YBT-LQ and post hoc analyses revealed significantly greater performance differences between free and restricted arm movement, especially, in young adults. Arm by age by task difficulty interactions were found for the OLS and the 3-m beam walking backward test. Post hoc analyses showed significantly greater performance differences between free and restricted arm movement during high vs. low levels of task difficulty and this was more pronounced in children and adolescents. CONCLUSIONS: Regardless of age, static, dynamic, and proactive balance performance benefited from arm movements and this was especially noted for youth performing difficult balance tasks

    Party system closure and openness: conceptualization, operationalization and validation

    The degree of closure of the governmental arena is a central aspect of the stabilization of party systems, and yet little systematic effort has been devoted to its operationalization. The article proposes a new index, examines its reliability and validity, and reports the ranking of 60 party systems. By redefining the units of measurement we suggest new indicators that are uniform and transparent in their logic of construction, can be applied both to specific government-changes and to time periods, and are sensitive to the degree of change. The article finds a hierarchy among the components of party system closure, dominated by coalition formula. While new and established democracies can both produced closed patterns, the analysis of inter-war European party systems shows that closed systems are less prone to authoritarian takeover. The article demonstrates the power of inertia: the completely closed configurations stand out as the most durable ones

    Multiple shades of grey: Opening the black box of public sector executives' hybrid role identities

    Public sector reforms of recent decades in Europe have promoted managerialism and aimed at introducing private sector thinking and practices. However, with regard to public sector executives' self-understanding, managerial role identities have not replaced bureaucratic ones; rather, components from both paradigms have combined. In this article, we introduce a bi-dimensional approach (attitudes and practices) that allows for different combinations and forms of hybridity. Empirically, we explore the role identities of public sector executives across Europe, building on survey data from over 7,000 top public officials in 19 countries (COCOPS survey). We identify country-level profiles, as well as patterns across countries, and find that administrative traditions can account for these profiles and patterns only to a limited extent. Rather, they have to be complemented by factors such as stability of the institutional environment (indicating lower shares of hybrid combinations) or extent of reform pressures (indicating higher shares of hybrid combinations)

    Circulating adrenomedullin estimates survival and reversibility of organ failure in sepsis: the prospective observational multinational Adrenomedullin and Outcome in Sepsis and Septic Shock-1 (AdrenOSS-1) study

    Background: Adrenomedullin (ADM) regulates vascular tone and endothelial permeability during sepsis. Levels of circulating biologically active ADM (bio-ADM) show an inverse relationship with blood pressure and a direct relationship with vasopressor requirement. In the present prospective observational multinational Adrenomedullin and Outcome in Sepsis and Septic Shock 1 (, AdrenOSS-1) study, we assessed relationships between circulating bio-ADM during the initial intensive care unit (ICU) stay and short-term outcome in order to eventually design a biomarker-guided randomized controlled trial. Methods: AdrenOSS-1 was a prospective observational multinational study. The primary outcome was 28-day mortality. Secondary outcomes included organ failure as defined by Sequential Organ Failure Assessment (SOFA) score, organ support with focus on vasopressor/inotropic use, and need for renal replacement therapy. AdrenOSS-1 included 583 patients admitted to the ICU with sepsis or septic shock. Results: Circulating bio-ADM levels were measured upon admission and at day 2. Median bio-ADM concentration upon admission was 80.5 pg/ml [IQR 41.5-148.1 pg/ml]. Initial SOFA score was 7 [IQR 5-10], and 28-day mortality was 22%. We found marked associations between bio-ADM upon admission and 28-day mortality (unadjusted standardized HR 2.3 [CI 1.9-2.9]; adjusted HR 1.6 [CI 1.1-2.5]) and between bio-ADM levels and SOFA score (p < 0.0001). Need of vasopressor/inotrope, renal replacement therapy, and positive fluid balance were more prevalent in patients with a bio-ADM > 70 pg/ml upon admission than in those with bio-ADM ≤ 70 pg/ml. In patients with bio-ADM > 70 pg/ml upon admission, decrease in bio-ADM below 70 pg/ml at day 2 was associated with recovery of organ function at day 7 and better 28-day outcome (9.5% mortality). By contrast, persistently elevated bio-ADM at day 2 was associated with prolonged organ dysfunction and high 28-day mortality (38.1% mortality, HR 4.9, 95% CI 2.5-9.8). Conclusions: AdrenOSS-1 shows that early levels and rapid changes in bio-ADM estimate short-term outcome in sepsis and septic shock. These data are the backbone of the design of the biomarker-guided AdrenOSS-2 trial. Trial registration: ClinicalTrials.gov, NCT02393781. Registered on March 19, 2015

    The Critical Juncture Concept’s Evolving Capacity to Explain Policy Change

    This article examines the evolution of our understanding of the critical junctures concept. The concept finds its origins in historical intuitionalism, being employed in the context of path dependence to account for sudden and jarring institutional or policy changes. We argue that the concept and the literature surrounding it—now incorporating ideas, discourse, and agency—have gradually become more comprehensive and nuanced as historical institutionalism was followed by ideational historical institutionalism and constructivist and discursive institutionalism. The prime position of contingency has been supplanted by the role of ideas and agency in explaining critical junctures and other instances of less than transformative change. Consequently, the concept is now capable of providing more comprehensive explanations for policy change

    Can we rate public support for democracy in a comparable way? Cross-national equivalence of democratic attitudes in the World Value Survey

    In this study we examine the cross-cultural equivalence of two scales that measure attitudes toward democracy across 36 countries in the World Value Survey (WVS) 2000. We examine the equivalence of these scales in order to explore if we can meaningfully compare democratic attitudes across countries. Multiple group confirmatory factor analyses (MGCFA) is applied to answer this question. The analyses indicate that the scales may be compared but only to a certain extent and not across all the countries. We close this article by discussing the implications of the findings
