    Selecciones continuas para portadores clásicos y simultáneos

    En Teoría de Selecciones continuas, uno de los problemas abiertos que mas interesa es la existencia de selecciones continuas para portadores con blanco no metrizable. En el presente trabajo se ha tenido como una de las metas principales la solución del problema citado. En el primer capítulo se exponen definiciones y propiedades elementales de la teoría, y se dá una nueva demostración del teorema fundamental de existencia de selecciones para portadores con blanco en un de Frechet. En el segundo capítulo se introduce por primera vez en la Teoría de Selecciones, el concepto de portador simultáneo, el que permite obtener teoremas de existencia de selecciones para portadores con blanco no metrizable, por ejemplo los teoremas 7 y 8 valen para cierto tipo de portadores con blanco en un espacio nuclear. También en éste capítulo se obtiene como corolario del teorema 2 del parágrafo 3 el teorema 2.1 de Lindenstrauss-Corson. En los capítulos tercero y cuarto se orienta la investigación en una nueva dirección, generalizando el concepto de portador equi-LCn, n introduciendo el de fuertemente equi-Cn, y limitándose a portadores con dominio en un complejo geométrico finito. Los resultados mas importantes son el teorema 1, que se aplica en 1a demostración del teorema 2, teorema fundamental, que vincula un portador fuertemente equi-Cn con un encaje finito de familias de entornos abiertos del blanco, y que interviene principalmente en la demostración de los resultados que le siguen, teorema de existencia de selecciones continuas para portadores de cierto tipo con blanco en un espacio no-EVT, y para portadores con blanco en un espacio no metrizable.Tesis digitalizada en SEDICI gracias a la Biblioteca del Departamento de Matemática de la Facultad de Ciencias Exactas (UNLP).Facultad de Ciencias Exacta

    An innovative technique to improve safety of volatile anesthetics suction from the cardiopulmonary bypass circuit

    Context: Myocardial injury during cardiac surgery on cardiopulmonary bypass (CPB) is a major determinant of morbidity and mortality. Preclinical and clinical evidence of dose- and time-related cardioprotective effects of volatile anesthetic drugs exist and their use during the whole surgery duration could improve perioperative cardiac protection. Even if administering volatile agents during CPB are relatively easy, technical problems, such as waste gas scavenging, may prevent safe and manageable administration of halogenated vapors during CPB. Aims: The aim of this study is to improve the safe administration of volatile anesthesia during CPB. Settings and Design: Tertiary teaching hospital. Subjects and Methods: We describe an original device that collects and disposes of any volatile anesthetic vapors present in the exit stream of the oxygenator, hence preventing its dispersal into the operating theatre environment and adaptively regulates pressure of oxygenator chamber in the CPB circuit. Results: We have so far applied a prototype of this device in more than 1300 adult cardiac surgery patients who received volatile anesthetics during the CPB phase. Conclusions: Widespread implementation of scavenging system like the one we designed may facilitate the perfusionist and the anesthesiologist in delivering these cardioprotective drugs with beneficial impact on patients' outcome without compromising on safety

    Historical geography II: traces remain

    The second report in this series turns to focus on the trace in relation to life-writing and biography in historical geography and beyond. Through attention to tracing journeys, located moments and listening to the presence of ghosts (Ogborn, 2005), this report seeks to highlight the range of different ways in which historical geographers have explored lives, deaths, and their transient traces through varied biographical terrains. Continuing to draw attention in historical geography to the darkest of histories, this piece will pivot on moments of discovering the dead to showcase the nuanced ways in which historical geography is opening doors into uncharted lives and unspoken histories

    Does side of onset influence the pattern of cerebral atrophy in Parkinson's disease?

    Imaging studies have revealed widespread neurodegeneration in Parkinson's disease (PD), but only a few considered the issue of asymmetrical clinical presentations. To investigate if the side of onset influences the pattern of gray matter (GM) atrophy in PD. Sixty patients (57.87 +/- 10.27 years) diagnosed with idiopathic PD according to the U.K. Brain Bank criteria, 26 with right-sided disease onset (RDO) and 34 with left-sided disease onset (LDO), were compared to 80 healthy controls (HC) (57.1 +/- 9.47 years). We acquired T1-weighted images on a 3 T scanner. Images were processed and analyzed with VBM8 (SPM8/Dartel) on Matlab R2012b platform. Statistic assessments included a two-sample test (family-wise error p < 0.05) with extent threshold of 20 voxels. Compared to HC, LDO patients had GM atrophy in the insula, putamen, anterior cingulate, frontotemporal cortex, and right caudate, while the RDO group showed atrophy at the anterior cingulate, insula, frontotemporal, and occipital cortex. This study revealed widespread GM atrophy in PD, predominantly in the left hemisphere, regardless of the side of onset.     Comparison of Tuberculin Skin test and Quantiferon immunological assay for latent Tuberculosis infection

    Background. Correct identification of individuals with latent tuberculosis infection (LTBI) is a crucial element of the elimination strategy, allowing their adequate treatment. In addition to tuberculin skin test (TST), the Quantiferon test (QFT, based on whole blood γ-interferon release) had been recently proposed. Aim of the study is to compare this test to TST for identification of LTBI in a non-selected population, in order to verify their value in identifying truly infected individuals (entitled to receive preventive chemotherapy), and to exclude from treatment those having a positive TST for other reasons (e.g. after BCG vaccination). Methods. 136 consecutive persons (78 males, mean age 34±9 years) referred to the clinic for TST were recruited (78 born in low - or middle - income countries). Based on their history, the cases were divided into 4 groups: 1) recently traced contacts of whom 18 TST negative and 28 TST positive; 2) 22 screening subjects, all TST negative; 3) BCG vaccinated subjects (14); and 4) 54 subjects already undergoing treatment of LTBI for exposure to TB. Results. The overall agreement between TST and QFT was 72% (64% in TST positive and 88.4% in TST negative subjects). The proportion of TST positive/QFT negative BCG vaccinated individuals was 23.1%. The K coefficient was 0.474 in recently traced contacts, 0.366 in BCG vaccinated individuals and 0.451 overall. Conclusions. The study results suggest that agreement between TST and QFT is lower in TST positive than in negative subjects, being lower in individuals treated for LTBI. Quantiferon does not seem to have brought significant improvement in the diagnosis of LTBI
