Leitlinienbasierte Anforderungsprofile von bildgebenden Verfahren im Uropoetischen und Reproduktionssystem - Eine vergleichende Untersuchung von evidenzbasierten Leitlinien
Die evidenzbasierte Medizin schlĂ€gt eine BrĂŒcke zwischen der sogenannten
universitÀren Spitzenmedizin und der tÀglichen Routine von Medizinern.
In der Radiologie haben die Entwicklungen der evidenzbasierten Medizin
bislang wenig Beachtung gefunden. Das hat zur Folge, dass in der Radiologie
bisher formal wenig Àrztliches Handeln evidenzbasiert ist.
Fehlende oder schwer zugÀngliche Informationen zur Angemessenheit einer
Untersuchungsmethode sind ein Hauptgrund fĂŒr eine Vielzahl von
unangemessenen radiologischen Leistungen.
Diese unangemessenen Leistungen verursachen zusÀtzliche hohe Kosten in
einem Gesundheitssystem, in dem radiologische Verfahren bis zu 15% der
Kosten ausmachen.
Evidenzbasierte Leitlinien stellen eine wichtige SĂ€ule zur Entscheidungsfindung
bezĂŒglich der Anforderung von angemessenen Untersuchungsmethoden dar.
In dieser Arbeit werden durch eine systematische vergleichende Untersuchung
von nationalen und internationalen Leitlinien, klinikinterne evidenzbasierte
Anforderungsprofile von bildgebenden Verfahren im Uropoetischen und
Reproduktionssystem formuliert.
Diese Anforderungsprofile enthalten Ăberweisungskriterien und sollen als
grundlegende Empfehlung zur Anforderung von radiologischen Verfahren bei
Erkrankungen im Uropoetischen und Reproduktionssystem dienen können.
Fragen zur bildgebenden Diagnostik werden in der EbM benachteiligt.
Evidenzbasiertes Wissen zum sinnvollen und angemessenen Einsatz von
bildgebenden Verfahren ist dabei in zahlreichen Leitlinien verstreut. Rein
radiologische Leitlinien existieren nur vereinzelt.
Uneinheitliche Empfehlungsinhalte und unĂŒbersichtliche Einteilungssysteme zur
Evidenzlage erschweren Nutzung und reduzieren Möglichkeiten zur
Implementierung.

Klinikumsinterne Empfehlungen sind das Ergebnis der vergleichenden
Leitlinienuntersuchung mit einer gleichzeitigen Anpassung an die lokalen
VerhÀltnisse.
Diese internen Empfehlungen werden farbkodiert und in tabellarischer Form im
Intranet des Klinikums Marburg implementiert.
Grundlagentabellen zur Evidenz- und Empfehlungslage enthalten die
zusammengefĂŒgten Ergebnisse aus der Leitlinienuntersuchung. Sie bilden die
Basis fĂŒr die internen Empfehlungen und enthalten die aus den Leitlinien
gewonnenen Informationen mit Verweisen auf die ursprĂŒngliche Evidenz âund
Empfehlungseinstufung.
Um eine vergleichende Untersuchung von Leitlinien mit uneinheitlichen
Klassifizierungssystemen zu ermöglichen, wird ein angepasstes
Einteilungssystem vorgeschlagen. Dieses System beinhaltet dabei zwei
Empfehlungsgrade und drei Evidenzstufen.
Zur graphischen Darstellung der Ergebnisse in den Tabellen wurde eine
zweidimensionale Lösung gefunden: Die EmpfehlungsstÀrke wird farblich nach
dem Ampelprinzip kodiert, die Evidenzlage durch eine Schattierung unterlegt.
In den internen Empfehlungen wurde zur besseren Ăbersichtlichkeit auf die
Musterkodierung zur Evidenzlage verzichtet.
Die Empfehlungen sind von jedem Computerarbeitsplatz des Klinikums
abrufbar und enthalten Empfehlungen zu möglichen bildgebenden Verfahren in
einer konkreten klinischen Situation. ErgÀnzend finden sich darin Anmerkungen
fĂŒr den Einsatz der Untersuchungsmethode. Jede Tabelle enthĂ€lt auĂerdem
einen Link zu der jeweils zugehörigen Grundlagentabelle in der die
ursprĂŒnglichen Leitlinienempfehlungen mit der Evidenz- und EmpfehlungsstĂ€rke
zusammengefasst sind.
Annahme, Nutzung und die Wirksamkeit der Intranetempfehlungen werden in
Zukunft zu evaluieren sein
Targeting the host response in sepsis: current approaches and future evidence
Sepsis, a dysregulated host response to infection characterized by organ failure, is one of the leading causes of death worldwide. Disbalances of the immune response play an important role in its pathophysiology. Patients may develop simultaneously or concomitantly states of systemic or local hyperinflammation and immunosuppression. Although a variety of effective immunomodulatory treatments are generally available, attempts to inhibit or stimulate the immune system in sepsis have failed so far to improve patientsâ outcome. The underlying reason is likely multifaceted including failure to identify responders to a specific immune intervention and the complex pathophysiology of organ dysfunction that is not exclusively caused by immunopathology but also includes dysfunction of the coagulation system, parenchymal organs, and the endothelium. Increasing evidence suggests that stratification of the heterogeneous population of septic patients with consideration of their host response might led to treatments that are more effective. The purpose of this review is to provide an overview of current studies aimed at optimizing the many facets of host response and to discuss future perspectives for precision medicine approaches in sepsis
Deriving Digital Energy Platform Archetypes for Manufacturing â A Data-Driven Clustering Approach
External factors such as climate change and the current energy crisis due to global conflicts are leading to the increasing relevance of energy consumption and energy procurement in the manufacturing industry. In addition to the growing call for sustainability, companies are increasingly struggling with rising energy costs and the reliability of the power grid, which endangers the competitiveness of companies and regions affected by high energy prices. Appropriate measures for energy-efficient and, not least, energy-flexible production are necessary. In addition to innovations and optimizations of plants and processes, digital energy platforms for the visualization, analysis, optimization, and control of energy flows are becoming essential. Over time, several digital energy platforms emerged on the market. The number and the different functionalities of the platforms make it challenging for classic manufacturing companies to keep track and select the right digital energy platform. In literature, the characteristics and functionalities of digital energy platforms have already been identified and structured. However, a classification of existing platforms into archetypes makes it easier for companies to select the platforms providing the missing functionality. To tackle this issue, we conducted an explorative and data-driven cluster analysis based on 49 existing digital energy platforms to identify digital energy platform archetypes and derive implications for research and practice. The results show five different archetypes that differ primarily in terms of functionalities on energy market integration. The identified archetypes provide a well-founded overview of the similarities and differences of digital energy platforms. Decision makers in manufacturing companies will benefit from the archetypes in future analyses as decision support in procurement processes and modifications of digital energy platforms
Structuring the Digital Energy Platform Jungle: Development of a Multi-Layer Taxonomy and Implications for Practice
Rising and volatile energy prices are forcing production companies to optimize their consumption patterns and reduce carbon emissions to remain competitive. Demand-side management (DSM) or energy flexibility (EF) is a promising option for the active management of electricity demand. With DSM, energy procurement costs can be effectively reduced, for example, by reducing peak loads and taking advantage of volatile energy prices. In addition, renewable energies can be better integrated to reduce carbon emissions while stabilizing the power grid. Although the benefits of DSM for production companies are well known, implementation is not yet widespread. A key barrier is the high requirements of IT systems and the associated effort and complexity involved in setting them up. Companies often lack appropriate IT systems or have historically grown systems that do not allow continuous communication from the machine to the energy market. A variety of different platforms promise solutions to address these challenges. However, when selecting platforms, it is often unclear which aspects and functionalities of a platform are relevant for a companyâs specific application. To address this gap, we developed a multi-layer taxonomy of digital platforms for energy-related applications in the industry that includes a general, as well as a more specific data-centric and transaction-centric perspective. We develop, revise, and evaluate our taxonomy using insights from literature and analysis of 46 commercially available platforms or platforms developed through research projects. Based on our taxonomy, we derive implications for research and practice. Our results contribute to the descriptive knowledge of digital platforms in energy-related applications. Our taxonomy enables researchers and practitioners to classify such platforms and make informed decisions about their deployment

We explored the impact of time from diagnosis to treatment on outcomes in newly diagnosed acute myeloid leukemia undergoing venetoclax-based therapy in two distinct cohorts.
By querying the Study Alliance Leukemia database and the global health network TriNetX, we identified 138 and 717 patients respectively with an average age of 76 and 72 years who received venetoclax-based firstline therapy. When comparing patients who started treatment earlier or later than 10 days after initial diagnosis, no significant difference in median overall survival was observed - neither in the SAL cohort (7.7 vs. 9.6 months, p=.42) nor in the TriNetX cohort (7.5 vs. 7.2 months, p=.41). Similarly, severe infections, bleeding, and thromboembolic events were equally observed between early and later treatments, both in the overall patient groups and specific subgroups (age â„75 years or leukocytes â„20x109/L).
This retrospective analysis indicates that delaying the start of venetoclax-based therapy in newly diagnosed acute myeloid leukemia might be a safe option for selected patients, provided that close clinical monitoring is performed

= 13 TeV pp collision data recorded by the ATLAS detector at the Large Hadron Collider at CERN. This search examines 22 final states categorized by the objects that are produced in association with the Higgs boson. These objects include isolated electrons or muons, hadronically decaying Ï-leptons, additional photons, missing transverse momentum, and hadronic jets, as well as jets that are tagged as containing a b-hadron. No significant excesses above Standard Model expectations are observed and limits on the production cross section at 95% confidence level are set. Detector efficiencies are reported for all 22 signal regions, which can be used to convert detector-level cross-section limits reported in this paper to particle-level cross-section constraints
Measurement of the ttÂŻttÂŻ production cross section in pp collisions at âs=13 TeV with the ATLAS detector
A measurement of four-top-quark production using proton-proton collision data at a centre-of-mass energy of 13 TeV collected by the ATLAS detector at the Large Hadron Collider corresponding to an integrated luminosity of 139 fbâ1 is presented. Events are selected if they contain a single lepton (electron or muon) or an opposite-sign lepton pair, in association with multiple jets. The events are categorised according to the number of jets and how likely these are to contain b-hadrons. A multivariate technique is then used to discriminate between signal and background events. The measured four-top-quark production cross section is found to be 26+17â15 fb, with a corresponding observed (expected) significance of 1.9 (1.0) standard deviations over the background-only hypothesis. The result is combined with the previous measurement performed by the ATLAS Collaboration in the multilepton final state. The combined four-top-quark production cross section is measured to be 24+7â6 fb, with a corresponding observed (expected) signal significance of 4.7 (2.6) standard deviations over the background-only predictions. It is consistent within 2.0 standard deviations with the Standard Model expectation of 12.0 ± 2.4 fb
Measurement and interpretation of same-sign W boson pair production in association with two jets in pp collisions at s = 13 TeV with the ATLAS detector
This paper presents the measurement of fducial and diferential cross sections for both the inclusive and electroweak production of a same-sign W-boson pair in association with two jets (W±W±jj) using 139 fbâ1 of proton-proton collision data recorded at a centre-of-mass energy of âs = 13 TeV by the ATLAS detector at the Large Hadron Collider. The analysis is performed by selecting two same-charge leptons, electron or muon, and at least two jets with large invariant mass and a large rapidity diference. The measured fducial cross sections for electroweak and inclusive W±W±jj production are 2.92 ± 0.22 (stat.) ± 0.19 (syst.)fb and 3.38±0.22 (stat.)±0.19 (syst.)fb, respectively, in agreement with Standard Model predictions. The measurements are used to constrain anomalous quartic gauge couplings by extracting 95% confdence level intervals on dimension-8 operators. A search for doubly charged Higgs bosons H±± that are produced in vector-boson fusion processes and decay into a same-sign W boson pair is performed. The largest deviation from the Standard Model occurs for an H±± mass near 450 GeV, with a global signifcance of 2.5 standard deviations
