Caso clínico de la unidad de medicina interna hipertensión arterial de origen renovascular
Hombre de 28 años, natural y procedente de Bogotá, consulto por cefalea progresiva fronta-occipital de treinta horas de evolución, acompañada de vomito en dos oportunidades y de trastorno de la conciencia, con relajación del esfínter vesical. La esposa informo episodio similar un mes ames, cuando fue hospitalizado, encontrándose HTA y hemorragia subaracnoidea. Salió contratamiento triconjugado (captopril, nifedipina, propanolol), el cual suspendió ocho días antes del ingreso

Este volumen acoge la memoria académica de la Cuarta edición del Congreso Internacional de Ciencia, Tecnología e Innovación para la Sociedad, CITIS 2017, desarrollado entre el 29 de noviembre y el 1 de diciembre de 2017 y organizado por la Universidad Politécnica Salesiana (UPS) en su sede de Guayaquil.
El Congreso ofreció un espacio para la presentación, difusión e intercambio de importantes investigaciones nacionales e internacionales ante la comunidad universitaria que se dio cita en el encuentro. El uso de herramientas tecnológicas para la gestión de los trabajos de investigación como la plataforma Open Conference Systems y la web de presentación del Congreso http://citis.blog.ups.edu.ec/, hicieron de CITIS 2017 un verdadero referente entre los congresos que se desarrollaron en el país.
La preocupación de nuestra Universidad, de presentar espacios que ayuden a generar nuevos y mejores cambios en la dimensión humana y social de nuestro entorno, hace que se persiga en cada edición del evento la presentación de trabajos con calidad creciente en cuanto a su producción científica.
Quienes estuvimos al frente de la organización, dejamos plasmado en estas memorias académicas el intenso y prolífico trabajo de los días de realización del Congreso Internacional de Ciencia, Tecnología e Innovación para la Sociedad al alcance de todos y todas

mencionen específicamente otras fuentes o autores.Los rumiantes que padecen infecciones severas de nematodos gastrointestinales (NGI) manifiestan signos clínicos como diarrea, pérdida de peso, cuello de botella (edema mandibular), anemia, debilidad y muerte. En la actualidad, el control de los problemas por NGI se basa en el uso de antihelmínticos (AH) comerciales, repetidamente a todos los animales del rebaño.
El uso frecuente y descontrolado de los AH ha provocado la aparición de NGI resistentes a los tres
grupos de AH. El problema de parásitos resistentes a las drogas es un problema mundial que también se presenta en los rebaños caprinos de México (véase la tecnología Diagnóstico de la Resistencia a los Antihelmínticos en Nematodos Parásitos de Caprinos). Para evitar la aparición de NGI resistentes a los AH es necesario reducir el uso de AH en los animales de los rebaños caprinos. Para esto se están desarrollando sistemas de desparasitación que se basan en tratar farmacológicamente únicamente a aquellas cabras que realmente estén infectadas. Es decir, se busca evitar desparasitar a aquellas cabras que tienen infecciones moderadas o bajas de NGI.Ruminants suffering from severe gastrointestinal nematode infections (GIN) manifest clinical signs such as diarrhea, weight loss, bottleneck (jaw edema), anemia, weakness and death. Currently, control of NGI problems is based on the use of commercial anthelmintics (HA), repeatedly to all animals in the herd.
Frequent and uncontrolled use of HAs has led to the emergence of NGI resistant to all three groups of HAs. HA groups. The problem of drug-resistant parasites is a worldwide problem that also occurs in Mexican goat herds (see technology Diagnosis of Anthelmintic Resistance in Parasitic Nematodes of Goats). To avoid the emergence of HA-resistant NGI, it is necessary to reduce the use of HA in goat herd animals. For this purpose, deworming systems are being developed that are based on pharmacologically treating only those goats that are actually infected. El libro es la continuación de una iniciativa de hace ya unos años (2012), donde varios países del área de distribución de la familia, decidieron unirse al ver como este recurso estaba disminuyendo o bien, era prácticamente desconocido. Este grupo tiene un gran impacto principalmente por su importancia como recurso pesquero ornamental, aunque también se usa como alimento, en la pesca deportiva y tiene incidencia en la salud pública. Para poder mitigar la posible sobreexplotación, surgió la necesidad de regular el comercio e intentar convertir la pesca ornamental de rayas en una actividad sostenible. Por ello, desde 2012 el IAvH junto a varios países propusieron la inclusión de varias especies al Apéndice III de Cites. Una de las limitantes de esta propuesta y por la cual no prosperó en su inicio, fue la falta de información sobre las especies.Bogotá, D. C

Colombian surgical outcomes study insights on perioperative mortality rate, a main indicator of the lancet commission on global surgery – a prospective cohort studyResearch in context
Summary: Background: Surgical care holds significant importance in healthcare, especially in low and middle-income countries, as at least 50% of the 4.2 million deaths within the initial 30 days following surgery take place in these countries. The Lancet Commission on Global Surgery proposed six indicators to enhance surgical care. In Colombia, studies have been made using secondary data. However, strategies to reduce perioperative mortality have not been implemented. This study aims to describe the fourth indicator, perioperative mortality rate (POMR), with primary data in Colombia. Methods: A multicentre prospective cohort study was conducted across 54 centres (hospitals) in Colombia. Each centre selected a 7-day recruitment period between 05/2022 and 01/2023. Inclusion criteria involved patients over 18 years of age undergoing surgical procedures in operating rooms. Data quality was ensured through a verification guideline and statistical analysis using mixed-effects multilevel modelling with a case mix analysis of mortality by procedure-related, patient-related, and hospital-related conditions. Findings: 3807 patients were included with a median age of 48 (IQR 32–64), 80.3% were classified as ASA I or II, and 27% of the procedures had a low-surgical complexity. Leading procedures were Orthopedics (19.2%) and Gynaecology/Obstetrics (17.7%). According to the Clavien–Dindo scale, postoperative complications were distributed in major complications (11.7%, 10.68–12.76) and any complication (31.6%, 30.09–33.07). POMR stood at 1.9% (1.48–2.37), with elective and emergency surgery mortalities at 0.7% (0.40–1.23) and 3% (2.3–3.89) respectively. Interpretation: The POMR was higher than the ratio reported in previous national studies, even when patients had a low–risk profile and low-complexity procedures. The present research represents significant public health progress with valuable insights for national decision-makers to improve the quality of surgical care.
