    Pacientes com transtornos relacionados ao uso de substâncias psicoativas atendidos em uma unidade de emergência psiquiátrica

    Study type: Prevalence study. Objective: To describe the socio demographic and clinical profile of patients with mental and behavioral disorders due to psychoactive substance (PAS) use, evaluated by the Psychiatric team of the Referenced Emergency Unit (REU) at the State University Clinical Hospital of Campinas (UNICAMP-HC). Methods and casuistic: This is a descriptive and quantitative study, which analyzed patients with mental and behavioral disorders due to PAS use, evaluated by the Psychiatric team of the REU-HC-UNICAMP during a period of 12 months, starting on May 2010. Results: There were evaluated 350 patients with PAS related disorders, representing 23.8% of those who sought psychiatric care. Most were males and younger than other patients of the emergency unit. The main reason for seeking help was psychiatric complaint other than PAS related disorders (23.1%), followed by desire to be treated oneself/being admitted to inpatient psychiatric treatment (20.8%) and intoxication (12.5%); 38.5% had a diagnosis of multiple PAS dependence and 34.8% were diagnosed with isolated alcohol dependence; 36% had psychiatric comorbidities and, when compared to other patients, they received more medications during attendance and were less directed to primary care units. Conclusion: There is a high rate of consultations of patients with PAS related problems in the psychiatric emergency unit. Objetivo: Descrever o perfil sócio demográfico e clínico de pacientes com transtornos relacionados ao uso de substâncias psicoativas (SPA) atendidos pela Psiquiatria da Unidade de Emergência Referenciada (UER) do Hospital de Clínicas (HC) da UNICAMP. Métodos e casuística: Estudo descritivo e quantitativo, que avaliou os pacientes com transtornos relacionados ao uso de SPA atendidos pela Psiquiatria da UER do HC-UNICAMP no período de 12 meses a partir de maio de 2010. Resultados: Foram avaliados 350 pacientes com transtornos por uso de SPA, representando 23.8% do total dos que procuraram atendimento psiquiátrico. Maioria do sexo masculino e mais jovens do que os demais pacientes. O principal motivo de atendimento foi queixa psiquiátrica outra que não o uso de SPA (23,1%), seguido do desejo de se tratar/internar (20,8%) e intoxicação (12,5%). 38,5% tinham diagnóstico de dependência de múltiplas SPA e 34,8% dependência exclusiva de álcool; 36% apresentavam comorbidade psiquiátrica. Quando comparados com os demais pacientes avaliados pela Psiquiatria, receberam mais medicações no atendimento e foram menos encaminhados à atenção primária. Conclusão: Há elevada taxa de atendimento de pessoas com problemas relacionados ao uso de SPA na emergência psiquiátrica.     A qualitative analysis on what Brazilian psychiatrists expect from current diagnostic classifications

    INTRODUCTION: The objective of the present study was to analyze the qualitative answers of an anonymous standardized survey including qualitative and quantitative questions about the Brazilian psychiatrists' perceptions on their use of the multiaxial diagnostic systems ICD-10 and DSM-IV and about their expectations regarding future revisions of these classifications (ICD-11 and DSM-V). METHODS: The questionnaire, elaborated by Graham Mellsop (New Zealand), was translated into Portuguese and sent through mail to 1,050 psychiatrists affiliated to the Brazilian Psychiatry Association. The quantitative analysis is presented elsewhere. RESULTS: One hundred and sixty questionnaires returned (15.2%). From these, 71.1% of the open questions where answered. The most needed and/or desirable qualities in a psychiatric classification were found to be: simplicity, clarity of criteria, objectivity, comprehensibility, reliability, and ease to use. Axis I of the ICD-10 was reported to be the most used due to its instrumental character in addition to being the official classification also for legal and bureaucratic purposes. The DSM-IV was also used in the everyday practice, mostly for education and research purposes, by psychiatrists with academic affiliations. The less frequent use of the multiaxial systems was explained by the lack of training and familiarity, the overload of information and by the fact they are not mandatory. Based on the respondents' answers, we concluded that some diagnostic categories must be revised, such as: mental retardation, eating disorders, personality disorders, sleeping disorders, child and adolescent disorders, affective, and schizoaffective disorders. CONCLUSION: This material offers a systematic overview of the psychiatrists' opinions and expectations concerning the diagnostic instruments used in their daily practice.INTRODUÇÃO: Pretende-se analisar o perfil de respostas qualitativas a um questionário padronizado anônimo, com itens abertos e fechados, sobre a percepção dos psiquiatras brasileiros acerca do uso dos sistemas diagnósticos multiaxiais CID-10 e DSM-IV e de suas expectativas quanto às próximas revisões (CID-11 e DSM-V). MÉTODO: O questionário, elaborado por Graham Mellsop (Nova Zelândia), foi traduzido e enviado para 1.050 psiquiatras afiliados à Associação Brasileira de Psiquiatria. RESULTADOS: Retornaram 160 questionários (15,2%). Nestes, 71,1% das questões abertas foram respondidas. As principais qualidades julgadas necessárias ou desejadas em uma classificação psiquiátrica foram: simplicidade, clareza de critérios, objetividade, compreensibilidade, confiabilidade e facilidade de uso. O eixo I da CID-10 foi citado como o mais utilizado pelo caráter instrumental e pelo fato de ser a classificação oficial, inclusive para fins burocráticos e legais. O DSM-IV também é bastante utilizado no cotidiano, sobretudo para ensino e pesquisa, por psiquiatras com afiliações acadêmicas. O uso menos frequente de sistemas multiaxiais foi justificado pela falta de treino e de familiaridade, pela sobrecarga de informações e pelo fato de não serem obrigatórios. Avaliou-se que algumas categorias diagnósticas, entre outras, devem ser revistas, como: retardo mental, transtornos alimentares, de personalidade, do sono, da infância e adolescência, quadros ansiosos, afetivos, esquizoafetivos. CONCLUSÃO: O material fornece um panorama sobre as opiniões e expectativas dos psiquiatras a respeito dos instrumentos diagnósticos dos quais se servem ativamente em sua prática diária e cujas revisões estão em pleno processo de elaboração.11211

    Green onion production under strategies of replacement and frequencies of circulation of brackish nutritive solutions

    The cultivation of vegetables in semi-arid regions, especially in the context of the use of brackish water, has been made possible by the use of the hydroponics technique. Thus, two experiments were carried out between December 2016 and January 2017 in a protected environment at the Federal Rural University of Pernambuco (UFRPE), Recife – PE, Brazil (8° 1”7” South latitude and 34° 56” 53” West longitude, and average altitude of 6.5 m), aiming at evaluating the production of green onion (cv. “Todo dia” Evergreen - Nebuka) in plants exposed to brackish nutrient solution (1.5, 3.0, 4.5, 6.0, 7.5 and 9.0 dS m-1), applied at two frequencies of circulation (twice a day - at 8 and 16 hours, and three times per day - at 8, 12 and 16 hours) in low-cost hydroponics system. In Experiment I, the nutrient solution evapotranspirated by the plants was replaced with the respective brackish water used in its preparation, and in Experiment II with UFRPE supply water (0.12 dS m-1). In both cases, a completely randomized experimental design was used, in a 6 x 2 factorial scheme, with five replications. It was concluded that under replacement with brackish water, the increase in the frequency of circulation attenuated the losses imposed by the salinity to the biometric variables and of the production of fresh and dry phytomass of the plants; the water supply replenishment had a greater mitigating role in relation to the damage caused by the salinity with the increase of the electrical conductivity of the nutrient solution

    Pharmacogenetics of Risperidone and Cardiovascular Risk in Children and Adolescents

    Objective. To identify the frequency of obesity and metabolic complications in child and adolescent users of risperidone. Potential associations with clinical parameters and SNPs of the HTR2C, DRD2, LEP, LEPR, MC4R, and CYP2D6 genes were analyzed. Methods. Samples from 120 risperidone users (8–20 years old) were collected and SNPs were analyzed, alongside assessment of chronological and bone ages, prescribed and weight-adjusted doses, use of other psychotropic drugs, waist circumference, BMI z-scores, blood pressure, HOMA-IR index, fasting levels of serum glucose, insulin, cholesterol, triglycerides, transaminases, and leptin. Results. Thirty-two (26.7%) patients were overweight and 5 (4.2%) obese. Hypertension was recorded in 8 patients (6.7%), metabolic syndrome in 6 (5%), and increased waist circumference in 20 (16.7%). The HOMA-IR was high for 22 patients (18.3%), while total cholesterol and triglycerides were high in 20 (16.7%) and 41 (34.2%) patients, respectively. SNP associations were found for LEP, HTR2C, and CYP2D6 with BMI; CYP2D6 with blood pressure, ALT, and HOMA-IR; HTR2C and LEPR with leptin levels; MC4R and DRD2 with HOMA-IR; HTR2C with WC; and LEP with ALT. Conclusions. Although not higher than in the general pediatric population, a high frequency of patients was overweight/obese, with abnormalities in metabolic parameters and some pharmacogenetic associations

    Effect of racial crossing on the seminal parameters of rams submitted to heat stress

    The aim of this study was to evaluate the effect of racial crossing on seminal parameters of eight Santa Inês and crossbred (Santa Inês x Dorper) rams submitted to heat stress, and to monitor the return of these parameters to previously reported. Before to place the insulation bags, two collects of semen through electroejaculation were performed. The insulation pouches were made with double-layer plastic, internally lined with cotton, and fixed around the spermatic funiculus and scrotum with adhesive tape and bandage remaining on the testes of the animals for seven days. The first collect was performed on the day that the pouches were taken (day 0) and thereafter, every seven days, totalizing 15 measurements. Data were submitted to analysis of variance (ANOVA). The analyzed variables were subjected to Dunnett test at 5% probability to compare the values obtained before treatment with those obtained in the following days. In this study it was found that the animals restored normal seminal parameter after the insulation effects, however, the return rate differed slightly among the studied breeds. The crossbred animals restored the seminal patterns, on average, a week before Santa Inês. It is concluded that the racial crossing influences the semen parameters of rams submitted to heat stress.     Instituído pelo Ministério da Saúde (MS), o Programa Nacional de Imunização (PNI) foi elencado para cumprir os objetivos da Organização Mundial da Saúde (OMS), e controlar principalmente o sarampo, tuberculose, difteria, tétano, coqueluche, poliomielite e manter até então, a já erradicada varíola. Este estudo objetivou-se em discutir sob a perspectiva da Atenção Primária à Saúde (APS) os desafios da cobertura vacinal nas capitais da região norte do Brasil. Trata-se de um estudo epidemiológico, descritivo, com abordagens quantiqualitativa. A pesquisa incluiu os 6 milhões de habitantes residentes das sete capitais dos estados que compõem a região norte do país, analisados sob a perspectiva da cobertura vacinal/taxa de abandono. Quanto a cobertura vacinal em relação ao índice populacional, nota-se maior abrangência em Palmas (TO), onde os índices na média comparativa entre os dois anos pesquisados encontraram-se em 74,30% e a maior taxa de abandono vacinal em Macapá (AP), 42,26%. Ressalta-se a importância da educação popular em saúde, principalmente na Atenção Primária à Saúde (APS), principal porta dos usuários aos serviços disponibilizados pelo Sistema Único de Saúde (SUS). 

    Soil macrofauna communities in Brazilian land-use systems

    Soil animal communities include more than 40 higher-order taxa, representing over 23% of all described species. These animals have a wide range of feeding sources and contribute to several important soil functions and ecosystem services. Although many studies have assessed macroinvertebrate communities in Brazil, few of them have been published in journals and even fewer have made the data openly available for consultation and further use. As part of ongoing efforts to synthesise the global soil macrofauna communities and to increase the amount of openly-accessible data in GBIF and other repositories related to soil biodiversity, the present paper provides links to 29 soil macroinvertebrate datasets covering 42 soil fauna taxa, collected in various land-use systems in Brazil. A total of 83,085 georeferenced occurrences of these taxa are presented, based on quantitative estimates performed using a standardised sampling method commonly adopted worldwide to collect soil macrofauna populations, i.e. the TSBF (Tropical Soil Biology and Fertility Programme) protocol. This consists of digging soil monoliths of 25 x 25 cm area, with handsorting of the macroinvertebrates visible to the naked eye from the surface litter and from within the soil, typically in the upper 0-20 cm layer (but sometimes shallower, i.e. top 0-10 cm or deeper to 0-40 cm, depending on the site). The land-use systems included anthropogenic sites managed with agricultural systems (e.g. pastures, annual and perennial crops, agroforestry), as well as planted forests and native vegetation located mostly in the southern Brazilian State of Paraná (96 sites), with a few additional sites in the neighbouring states of São Paulo (21 sites) and Santa Catarina (five sites). Important metadata on soil properties, particularly soil chemical parameters (mainly pH, C, P, Ca, K, Mg, Al contents, exchangeable acidity, Cation Exchange Capacity, Base Saturation and, infrequently, total N), particle size distribution (mainly % sand, silt and clay) and, infrequently, soil moisture and bulk density, as well as on human management practices (land use and vegetation cover) are provided. These data will be particularly useful for those interested in estimating land-use change impacts on soil biodiversity and its implications for below-ground foodwebs, ecosystem functioning and ecosystem service delivery.Quantitative estimates are provided for 42 soil animal taxa, for two biodiversity hotspots: the Brazilian Atlantic Forest and Cerrado biomes. Data are provided at the individual monolith level, representing sampling events ranging from February 2001 up to September 2016 in 122 sampling sites and over 1800 samples, for a total of 83,085 ocurrences
