    Surgical alternative for treatment of vertical root fracture: A case report

    Vertical root fracture (VRF) has been a great challenge in dentistry; most fractures often result in tooth extraction. Inflammation of tissues around the fractured root is the main reason for tooth extraction. Based on the strategic importance of some fractured teeth, treatment may be necessary and often complicated. However, performing a proper repair or even splinting the fractured segments may result in tooth preservation. Accordingly, in this case we report a new method for fractured tooth preservation. The surgical exposition of the fracture tooth was carried out through the radicular portion of the element via ultrasonic preparation, filling with composed resin and a synthetic hydroxyapatite graft. All these were performed around the tooth which received five sections of low-power laser. The patient was followed for two years with no signs or symptoms of inflammation and gingival recession. In conclusion, the used treatment protocol could be considered as a promising approach for VRF treatment, especially in cases where there is advanced or moderate bone loss in the surrounding sites of the fractured tooth

    Non-Invasive Methods and the Use of Infiltrating Resins for the Control of Caries Progression in Deciduous Teeth: A Systematic Review and Meta-Analysis

    Objective: To evaluate the influence of non-invasive treatment associated with the use of infiltrating resin for managing caries lesions in primary teeth. Material and Methods: A systematic review was performed by selecting articles from 6 online databases, using a search algorithm and eligibility criteria for data extraction and data synthesis for the papers included. Clinical trials involving primary teeth with incipient caries lesions (1/2 of the enamel or 1/3 of the outer dentin) were included, presenting full text and answering the study\u27s guiding question. This study used the RoB 2 tool for the risk of bias assessment and GRADE for certainty of evidence. Random effects meta-analyses were implemented, and lesion progression treatment effects were estimated through relative risk (RR) and associated 95% confidence intervals. Results: A total of 440 studies were found. After analyzing the inclusion criteria and removal of duplicates, eight studies were analyzed for quality evidence. Five of the eight studies included in this review contributed to the meta-analysis, all with some reflections regarding the risk of bias. Overall, the results of the meta-analysis showed that non-invasive treatment, when associated with the use of infiltrating resins, significantly reduced the risk of caries progression in relation to the treatment without this addition for follow-up periods ranging from 12 months to 2 years (RR 0.51 [0.40-0.65]). Conclusion: There is moderate certainty of evidence that the use of infiltrating resins associated with non-invasive treatments decreases the risk of caries progression in primary teeth with incipient caries lesions (1/2 of the enamel or 1/3 of the dentin outer) when combined with non-invasive control methods alone

    QUALITY OF SWEET CORN SEEDS TREATED BEFORE AND AFTER STORAGE

    This work evaluated the effect of industrial treatment with different pesticides and seed sizes in the physiological performance of sweetcorn and the content of active ingredients of insecticides before and three months after storage. Four seed lots of a sweetcorn single-cross hybrid (L1, L2, L3 and L4) and different sieves (S1, S2 and S3) were used for two experiments. Experiment 1: industrial seed treatment, T1=untreated seed (control), T2= Maxim Advanced®, T3= Cruiser® 350 FS and T4= Poncho®. Experiment 2: quantification of the content of active ingredients, commercial products Cruiser® 350 FS and Poncho and the efficiency of distribution were measured using HPLC-UV. The results showed that industrial seed treatment with Maxim Advanced®, Cruiser® 350 FS and Poncho® can be performed three months before sowing without damage to sweetcorn seeds HPLC is efficient in quantifying the content of active ingredients on sweetcorn seeds treated with Cruiser® 350 FS and Poncho®. The industrial seed treatment shall be calibrated for each lot, which is justified by the different amounts of active ingredient between lots and seed sizes.RESUMO: Este trabalho avaliou o efeito do tratamento industrial com diferentes pesticidas e tamanhos de sementes sobre o comportamento fisiológico do milho doce e o teor de ingredientes ativos dos inseticidas antes e três meses após o armazenamento. Quatro lotes de um único híbrido cruzado de milho doce (L1, L2, L3 e L4) e diferentes peneiras (S1, S2 e S3) foram utilizados para dois experimentos. Experimento 1: tratamento industrial de sementes, T1 = sementes não tratadas (controle), T2 = Maxim Advanced®, T3 = Cruiser® 350 FS e T4 = Poncho®. Experimento 2: quantificação do teor de princípios ativos, produtos comerciais Cruiser® 350 FS e Poncho e a eficiência de distribuição foi medida por HPLC-UV. Os resultados mostraram que o tratamento de sementes industriais com Maxim Advanced®, Cruiser® 350 FS e Poncho® pode ser realizado três meses antes da semeadura sem danificar as sementes de milho doce. A HPLC é eficiente para quantificar o conteúdo de ingredientes ativos em sementes de milho doce tratadas com Cruiser® 350 FS e Poncho®. O tratamento industrial de sementes será calibrado para cada lote, o que se justifica pelas diferentes quantidades de ingrediente ativo entre os lotes e os tamanhos das sementes

    Abstract INTRODUCTION: Chikungunya infection presents with distinct clinical features depending on the patient age group. METHODS: Medical records of children with positive IgM for the chikungunya virus who were hospitalized in a pediatric ward in Fortaleza, Ceará, Brazil were analyzed. RESULTS: Fourteen children with a median age of 4 months (36 days to 15 years) were included. All patients presented with fever persisting for an average of 5 days. The joints were involved in 6 (42.8%) children, and 8 (57.1%) children presented with bullous rash. CONCLUSIONS: Systemic involvement and atypical clinical manifestations characterize severe forms of chikungunya infection in children

    Geoeconomic variations in epidemiology, ventilation management, and outcomes in invasively ventilated intensive care unit patients without acute respiratory distress syndrome: a pooled analysis of four observational studies

    Background: Geoeconomic variations in epidemiology, the practice of ventilation, and outcome in invasively ventilated intensive care unit (ICU) patients without acute respiratory distress syndrome (ARDS) remain unexplored. In this analysis we aim to address these gaps using individual patient data of four large observational studies. Methods: In this pooled analysis we harmonised individual patient data from the ERICC, LUNG SAFE, PRoVENT, and PRoVENT-iMiC prospective observational studies, which were conducted from June, 2011, to December, 2018, in 534 ICUs in 54 countries. We used the 2016 World Bank classification to define two geoeconomic regions: middle-income countries (MICs) and high-income countries (HICs). ARDS was defined according to the Berlin criteria. Descriptive statistics were used to compare patients in MICs versus HICs. The primary outcome was the use of low tidal volume ventilation (LTVV) for the first 3 days of mechanical ventilation. Secondary outcomes were key ventilation parameters (tidal volume size, positive end-expiratory pressure, fraction of inspired oxygen, peak pressure, plateau pressure, driving pressure, and respiratory rate), patient characteristics, the risk for and actual development of acute respiratory distress syndrome after the first day of ventilation, duration of ventilation, ICU length of stay, and ICU mortality. Findings: Of the 7608 patients included in the original studies, this analysis included 3852 patients without ARDS, of whom 2345 were from MICs and 1507 were from HICs. Patients in MICs were younger, shorter and with a slightly lower body-mass index, more often had diabetes and active cancer, but less often chronic obstructive pulmonary disease and heart failure than patients from HICs. Sequential organ failure assessment scores were similar in MICs and HICs. Use of LTVV in MICs and HICs was comparable (42·4% vs 44·2%; absolute difference -1·69 [-9·58 to 6·11] p=0·67; data available in 3174 [82%] of 3852 patients). The median applied positive end expiratory pressure was lower in MICs than in HICs (5 [IQR 5-8] vs 6 [5-8] cm H2O; p=0·0011). ICU mortality was higher in MICs than in HICs (30·5% vs 19·9%; p=0·0004; adjusted effect 16·41% [95% CI 9·52-23·52]; p<0·0001) and was inversely associated with gross domestic product (adjusted odds ratio for a US$10 000 increase per capita 0·80 [95% CI 0·75-0·86]; p<0·0001). Interpretation: Despite similar disease severity and ventilation management, ICU mortality in patients without ARDS is higher in MICs than in HICs, with a strong association with country-level economic status
