Differential neuropsychological profiles in Parkinsonian patients with or without vascular lesions.
The purpose of this study is to compare the neuropsychological profile of patients affected by parkinsonism and vascular lesions to that in patients with PD alone (PD) and to evaluate whether the brain vascular lesion load is associated with neuropsychological variables. Thirty-six nondemented patients
with parkinsonism were divided into 3 groups of 12 patients each, according to both clinical history and the presence of brain vascular lesions and/or dopaminergic denervation as revealed by magnetic resonance and dopamine transporter imaging, respectively. The first group had vascular lesions without
dopaminergic denervation (VP group); the second group had vascular lesions and dopaminergic denervation (DD) (VP+DD group); and the third group consisted of patients with dopaminergic denervation (PD group) without vascular lesions. All
patients underwent neurological and neuropsychological assessments. The groups differed in disease duration, age at onset, and cerebrovascular risk factors. The VP and VP+DD groups performed worse than the PD group on frontal/executive tasks. Regardless of the presence of dopaminergic denervation, cerebrovascular lesions in hemispheric white matter, basal ganglia, and cerebellum have an important effect in determining early onset and severity of cognitive impairment in patients with parkinsonism
Pathological gambling in Parkinson's disease. A comprehensive review
Pathological gambling (PG) and other Impulse Control Disorders (ICDs), such as hypersexuality, compulsive eating and buying, are often reported in Parkinson's disease (PD). The prevalence of PG is 2.2%-7% in treated PD patients, which is higher than the background population rate. As other non motor symptoms in PD, PG is frequently under-reported by patients and caregivers and may be under-recognized by the treating physicians. Factors associated with PG include male sex, younger age or younger age at PD onset, personal or family history of substance abuse or ICD, a personality profile characterized by impulsiveness, and treatment with dopamine agonists (DA) more than with levodopa (l-dopa). The DA effect seems to be a class effect and not specific for any DA. Neurofunctional studies suggest that medication-induced downregulation of frontostriatal connections and upregulation of striatum might combine to induce impulsive behavior. A dysfunction of fronto-subcortical circuits in PD patients with PG is also supported by neuropsychological findings of impaired executive control and monitoring abilities. Management of ICDs in PD is complex, and until now only discontinuation and/or tapering of DA treatment seem to be an effective management strategy for ICDs in PD. There is no empirical evidence supporting the use of psychiatric drugs for PG such as antipsychotics and antidepressants. Data regarding the effect of deep brain stimulation (DBS), particularly of subthalamic nucleus, on PG and ICDs in PD are still limited and sometimes conflicting since improvement of PG or new onset of PG after surgery have been reported
Experimental assessment of the acoustic performance of nozzles designed for clean agent fire suppression
Discharge through nozzles used in gas-based fire protection of data centers may generate noise that causes the performance of hard drives to decay considerably; silent nozzles are employed to limit this harmful effect. This work focuses on proposing an experimental methodology to assess the impact of sound emitted by gaseous jets by comparing various nozzles under several operating conditions, together with relating that impact to design parameters. A setup was developed and repeatability of the experiments was evaluated; standard and silent nozzles were tested regarding the discharge of inert gases and halocarbon compounds. The ability of silent nozzles to contain the emitted noise—generally below the 110 dB reference threshold—was proven effective; a relationship between Reynolds number and peak noise level is suggested to support the reported increase in noise maxima as released flow rate increases. Hard drives with lower speed were the most affected. Spectral analysis was conducted, with sound at the higher frequency range causing performance decay even if lower than the acknowledged threshold. Independence of emitted noise from the selected clean agent was also observed in terms of released volumetric flow rate, yet the denser the fluid, the lower the generated noise under the same released mass flow rate
Ferritin heavy chain Is the host factor responsible for HCV-Induced inhibition of apoB-100 production and is required for efficient viral infection
Hepatic fat export occurs by apolipoprotein B-100-containing lipoprotein production, whereas impaired production leads to liver steatosis. Hepatitis C virus (HCV) infection is associated to dysregulation of apoB-100 secretion and steatosis; however, the molecular mechanism by which HCV affects the apoB-100 secretion is not understood. Here, combining quantitative proteomics and computational biology, we propose ferritin heavy chain (Fth) as being the cellular determinant of apoB-100 production inhibition. By means of molecular analyses, we found that HCV nonstructural proteins and NS5A appear to be sufficient for inducing Fth up-regulation. Fth in turn was found to inhibit apoB-100 secretion leading to increased intracellular degradation via proteasome. Notably, intracellular Fth down-regulation by siRNA restores apoB-100 secretion. The inverse correlation between ferritin and plasma apoB-100 concentrations was also found in JFH-1 HCV cell culture systems (HCVcc) and HCV-infected patients. Finally, Fth expression was found to be required for robust HCV infection. These observations provide a further molecular explanation for the onset of liver steatosis and allow for hypothesizing on new therapeutic and antiviral strategies

Road traffic crashes are a main cause of physical harm and mortality. That means a heavy burden in terms of short, medium and long-term disability and public costs
for society. Among the most relevant cases of severe accidents is driving under the influence of alcohol and psychoactive drugs. That\u2019s the reason of the international
strong effort to combating the phenomenon. In Italy the draft law concerning the criminal offence of vehicular homicide and road traffic personal injuries is currently being discussed. Previously the law n. 120/2010 has already changed the regulation in matter of drink-driving and driving under the influence of controlled substances with stiffer penalties for transgressors and (at least some) guidance as to assess the infringement. The draft law suggests that clinical and toxicological evidences of actual driving impairment has to be proved to reach significance in court, nevertheless no standard and effective procedure has being established for detection, at least for what concerns controlled substances.
The purpose of this article is to put in evidence, in accordance with a medico-legal perspective, some of the issues that emerge from this complex matter (i.e. the need of a list of the impairing substance under detection, of well-defined legal cut-off values, of a better definition of the problems following the therapeutic use of drugs of abuse and other concerns), issues, those, that are now more troubling whereas the introduction of new crimes and punishments is becoming more concrete
