    Eficiencia de los métodos diagnósticos en el estudio del sangrado uterino anormal en la peri y postmenopausia

    Indexación: ScieloEl sangrado uterino anormal es un importante síntoma que puede indicar la presencia de cáncer o hiperpla-sia endometrial. El objetivo de este estudio es evaluar la eficiencia de los diversos métodos diagnósticos en pacientes peri y postmenopáusicas con sangrado uterino anormal y proponer una vía de manejo en estas pacientes. Diferentes métodos diagnósticos serán evaluados, incluyendo ecografíatransvaginal, sonohiste-rografía, Doppler, resonancia magnética nuclear, biopsia por Pipelle, dilatación y curetage e histeroscopia. Está demostrado que la ecografía transvaginal tiene una alta eficiencia para el reconocimiento de lesiones difusas, mientras que sonohisterografía tiene una alta eficiencia para identificar lesiones focales. Por otro lado, la biopsia por Pipelle ha demostrado descartar patología difusa con una alta eficiencia. Se concluye que la mejor vía para el estudio de pacientes con sangrado uterino anormal es realizar una ecografíatransvaginal y una histerosonografía en el mismo momento, seguido por una biopsia mediante Pipelle. Finalmente, puede ser sugerido que por esta vía es altamente probable que todos los casos de cáncer e hiperplasia endometrial puedan ser identificados.Abnormal uterine bleeding is an important symptom that can mean presence of endometrial cáncer or endometrial hyperplasia. The aim of this study is to assess the effectiveness of selected diagnostic methods in peri and potmenopausal women with abnormal uterine bleeding, and to propose a way to manage in these patients. Different diagnostic methods are assessed, which include transvaginal ultrasonography, sonohysterography, Doppler technique, magnetic resonance images, biopsy by Pipelle, dilatation and curettage, and hysteroscopy. It is founded that ultrasonography has a high efficiency when it comes for identifying diffuse lesions, while sonohysterography has a high efficiency for identify focal lesions. On the other hand biopsy by Pipelle has demonstrated to rule out diffuse pathology with a high efficiency. It is concluded that the best way to manage patients with abnormal uterine bleeding by performing ultrasonography and sonohysterography at the same time followed by a Pipelle biopsy.     Methane production from mixed tropical savanna and forest vegetation in Venezuela

    International audienceMeasurements of methane concentrations in the boundary layer in the northern part of the Guayana shield, Venezuela, during the wet season (October 1988), showed the presence of substantial methane surface emissions. The measuring site is within the savanna climate region, but is affected by emissions from savanna and forest vegetation. From day versus night concentration measurements, with higher concentrations during night, a methane source strength near the site of 3?7×1011 molecules/cm2/s can be estimated, which includes emissions from small tracts of flooded soils, termites and especially tropical vegetation. Extrapolated to the entire savanna, this may imply a methane source of ~30?60 Tg yr?1 similar to the one calculated for tropical vegetation on the basis of recently published in vitro plant emission experiments by Keppler et al. (2006), which indicate emissions of ~30 Tg yr?1 for tropical savannas and grasslands and ~78 Tg yr?1 for tropical forests

    New insights in the global cycle of acetonitrile: release from the ocean and acetonitrile: release from the ocean and Venezuela

    International audienceCUsing the proton transfer reaction mass spectrometry (PTR-MS) technique, acetonitrile was measured during the wet season in a Venezuelan woodland savanna. The site was located downwind of the Caribbean Sea and no biomass burning events were observed in the region. High boundary layer concentrations of 211 ±36 pmol/mol (median, ± standard deviation) were observed during daytime in the well mixed boundary layer, which is about 60 pmol/mol above background concentrations recently measured over the Mediterranean Sea and the Pacific Ocean. Most likely acetonitrile is released from the warm waters of the Caribbean Sea thereby enhancing mixing ratios over Venezuela. Acetonitrile concentrations will probably still be much higher in biomass burning plumes, however, the general suitability of acetonitrile as a biomass burning marker should be treated with care. During nights, acetonitrile dropped to levels typically around 120 pmol/mol, which is consistent with a dry deposition velocity of ~0.14 cm/s when a nocturnal boundary layer height of 100 m is assumed

    Indexacion: ScieloRESUMEN: El apéndice fibroso hepático es una estructura anatómica del espacio subfrénico, poco descrita en la literatura universal y de importancia clínica. Se diseñó un protocolo para ser realizado en cadáveres donde se determina su presencia, características y correlación estadística con factores antropométricos. En 24 cadáveres se determina que el apéndice fibroso hepático siempre está presente, y que no existe correlación estadísticamente significativa, pero sí se observa variabilidad entre las dimensiones biométricas analizadas. SUMMARY: The hepatic fibrous appendix is an anatomical structure of the subfrenic space scarcely described in the universal literature and of vital clinical importance. For their study a protocol was designed to be carried out in cadavers where its presence is determined, characteristic and statistical correlation with anthropometric factors. In 24 cadavers it is determined that the hepatic fibrous appendix is always present, and that correlation doesn't exist statistically significant among the analyzed anthropological factors, but it's possible to observe variability in it's dimensions

    The core population and kinematics of a massive clump at early stages: an ALMA view

    High-mass star formation theories make distinct predictions on the properties of the prestellar seeds of high-mass stars. Observations of the early stages of high-mass star formation can provide crucial constraints, but they are challenging and scarce. We investigate the properties of the prestellar core population embedded in the high-mass clump AGAL014.492-00.139, and we study the kinematics at the clump and the clump-to-core scales. We have analysed an extensive dataset acquired with the ALMA interferometer. Applying a dendrogram analysis to the Band o-H2D+\rm H_2D^+ data, we identified 22 cores. We have fitted their average spectra in local-thermodinamic-equilibrium conditions, and we analysed their continuum emission at 0.8mm0.8 \, \rm mm. The cores have transonic to mildly supersonic turbulence levels and appear mostly low-mass, with Mcore<30MM_\mathrm{core}< 30 \, \rm M_\odot. Furthermore, we have analysed Band 3 observations of the N2H+\rm N_2H^+ (1-0) transition, which traces the large scale gas kinematics. Using a friend-of-friend algorithm, we identify four main velocity coherent structures, all of which are associated with prestellar and protostellar cores. One of them presents a filament-like structure, and our observations could be consistent with mass accretion towards one of the protostars. In this case, we estimate a mass accretion rate of M˙acc2×104Myr1 \dot{M}_\mathrm{acc}\approx 2 \times 10^{-4} \rm \, M_\odot \, yr^{-1}. Our results support a clump-fed accretion scenario in the targeted source. The cores in prestellar stage are essentially low-mass, and they appear subvirial and gravitationally bound, unless further support is available for instance due to magnetic fields.Comment: Accepted for publication in Ap

    Biometrics of the Posterior Communicating Artery and the Posterior Cerebral Artery in its Precommunicating Segment (P1) of the Arterial Circle of Brain (Willis)

    Indexación: ScieloEl conocimiento anatómico y clínico preciso del círculo arterial del cerebro, se hace cada vez más necesario, por la compleja relación neural que presentan las diversas arterias que entran en su formación y además por su gran variabilidad. Utilizamos 36 encéfalos humanos frescos, provenientes de especímenes autopsiados adultos, cuyos datos bioantropológicos fueron previamente registrados. El calibre de las aa. comunicantes posteriores fue, en promedio, de 1,08 mm (DE 0,45 ) en ambos lados y su longitud de 17,51 mm( DE 7,9) en el lado derecho y de 16,9 mm (DE 8,0 ) en el lado izquierdo. La ACP en el segmento P1 presentó un calibre de 2,56 mm (DE 077) en el lado derecho y de 2,32 mm (DE 0,64) en el lado izquierdo. La longitud de estas arterias correspondió a 9,43 mm (DE 8,92) en el lado derecho y de 8,82 mm (DE 7,33 ) en el lado izquierdo.     Laparoscopic radical nephrectomy of angiomyolipoma with renal vein involvement

    Indexación: Web of Science; Scielo.Objetivo: Se presenta el caso clínico de un Angiomiolipoma renal con extensión a vena renal, patología de muy baja frecuencia. Caso clínico: Mujer de 36 años, quien consulta por dolor en flanco izquierdo. En una tomografía computada se encuentra una lesión tumoral extensa del riñón izquierdo compatible con un Angiomiolipoma renal infiltrante e invasión tumoral de la vena renal. Se realiza nefrectomía laparoscópica, con extirpación de trombo tumoral. El tiempo operatorio fue de 127 min y el sangrado estimado de 20 ml. No hubo complicaciones intra ni postoperatorias. La paciente se encuentra actualmente asintomática luego de 12 meses de seguimiento. La histología mostró un Angiomiolipoma renal con invasión del riñón y un tumor sólido en el lumen de la vena renal. Conclusión: La invasión de vena renal por un Angiomiolipoma es extremadamente raro. Su resolución laparoscópica es posible, con sólo una comunicación previa en la literatura.Background: Angiomyolipomas associated with tuberous sclerosis may invade the renal vein and generate intramural thrombi. Case report: We report a 36-years-old woman, consulting for left flank pain. CT scan showed a large tumor in the left kidney consistent with the diagnosis of infiltrating renal angiomyolipoma with tumor invasion of the vein. Laparoscopic nephrectomy was performed, with removal of tumor thrombus. The operative time was 127 minutes and estimated bleeding 20 ml. There were no intraoperative or postoperative complications. The patient is currently asymptomatic after 12 months of follow up. The pathological study of the surgical piece showed a renal angiomyolipoma with invasion of the kidney and a solid tumor in the lumen of the renal vein.http://ref.scielo.org/7pqpx

    The Proteome of Biologically Active Membrane Vesicles from Piscirickettsia salmonis LF-89 Type Strain Identifies Plasmid-Encoded Putative Toxins

    Indexación: Scopus.Piscirickettsia salmonis is the predominant bacterial pathogen affecting the Chilean salmonid industry. This bacterium is the etiological agent of piscirickettsiosis, a significant fish disease. Membrane vesicles (MVs) released by P. salmonis deliver several virulence factors to host cells. To improve on existing knowledge for the pathogenicity-associated functions of P. salmonis MVs, we studied the proteome of purified MVs from the P. salmonis LF-89 type strain using multidimensional protein identification technology. Initially, the cytotoxicity of different MV concentration purified from P. salmonis LF-89 was confirmed in an in vivo adult zebrafish infection model. The cumulative mortality of zebrafish injected with MVs showed a dose-dependent pattern. Analyses identified 452 proteins of different subcellular origins; most of them were associated with the cytoplasmic compartment and were mainly related to key functions for pathogen survival. Interestingly, previously unidentified putative virulence-related proteins were identified in P. salmonis MVs, such as outer membrane porin F and hemolysin. Additionally, five amino acid sequences corresponding to the Bordetella pertussis toxin subunit 1 and two amino acid sequences corresponding to the heat-labile enterotoxin alpha chain of Escherichia coli were located in the P. salmonis MV proteome. Curiously, these putative toxins were located in a plasmid region of P. salmonis LF-89.     Biometrical Analysis of the Anterior Communicating Artery and the Anterior Cerebral Artery in the Precommunicating Segment of the Cerebral Arterial Circle

    Indexación: ScieloLa conformación del círculo arterial cerebral tiene relevancia en la clínica neuroquirúrgica por la relación compleja que presentan las arterias que lo originan y su gran variabilidad. Debido a la alta frecuencia con que se observan aneurismas en las arteria comunicante anterior y en el segmento precomunicante (A1) de la arteria cerebral anterior se efectuó un análisis biométrico de ellas. El trabajo se realizó en 36 cerebros frescos procedentes de especímenes cuyos datos bioantropológicos estaban registrados. El calibre de la arteria comunicante anterior fue de 1.68 mm y la longitud, en promedio, de 2.50 mm. En el lado derecho el segmento A1 de la arteria cerebral anterior tenía una longitud de 12.91mm y un calibre de 2.39 mm. En el lado izquierdo, la longitud de este segmento fue de 12.77 mm y presentó un calibre de 2.46mm. En un 29% de las muestras analizadas, se presentaron arterias comunicantes dobles, con una longitud de 2.92 mm y un calibre de 0.95 mm. Se observó variabilidad en los componentes que constituyen el círculo arterial cerebral, cuando se consideran variables como lado, sexo e índice cefálico. Due to the high frequency with that aneurysms are observed in the anterior communicant artery and the anterior cerebral artery in the precommunicating segment (A1), we have decided to make an analysis biometrical in them. The work had done in 36 available fresh brains in the laboratories and they come from specimens whose data bioanthropological were registered. The caliber of the anterior communicating artery corresponds to 1.68 mm and the longitude to 2.50 mm. The segment A1 of the anterior cerebral artery it corresponds to a longitude of 12.91mm and it presents a caliber of 2.39 mm in the right side. In the left side the longitude of this segment is of 12.77 mm and it presents a caliber of 2.46. In 29% of the analyzed samples, they register double communicating arteries, with a longitude of 2.92 mm and a caliber of 0.95 mm.
