Surgery is required only in about 1 in 10 cases of slipped disc. It may be considered if there is evidence of severe nerve compression, symptoms not improved using other treatments, having difficulty in standing or walking, have severe symptoms such as progressive muscle, weakness or altered bladder function. A microdiscectomy is typically performed for a herniated lumbar disc and is actually more effective for treating leg pain (also known as radiculopathy) than lower back pain. Impingement on the nerve root (compression) can cause substantial leg pain. While it may take weeks or months for the nerve root to fully heal and any numbness or weakness to get better, patients normally feel relief from leg pain almost immediately after a microdiscectomy spine surgery. Backache during pregnancy must not be overlooked as only minor ailment due to mechanical stress. Patient should be investigated on lines of spinal pathology

Background: The aim of study is to study role of antioxidants to reduce free radical induced oxidative stress in preeclampsia. Methods: A prospective study conducted with 326 patients in UISEMH, Kanpur at Department of Obstetrics and Gynaecology from January 2011 - January, 2012 with 326 patients. There were divided into 2 groups - control and study group on the basis of measurement of activity of superoxide dismutase at 20 weeks, control group constituted patients with normal SOD level (Normal values >0.702 U/mg of protein). Study group constituted 180 patients with decreased SOD level at 20 weeks, they were further divided into 2 groups - study group A and study group B, according to intervention given in form of Vit. C 1000 mg OD from 20 weeks till delivery. SOD was re-measured in control and study group at 32 weeks. Result was studied in terms of value of SOD at 32 weeks and incidence of PIH and IUGR, mode of delivery, complications in forms of mild and severe PET, Abruptio placentae, antepartum eclampsia and IUGR.Results: The study showed that mean level of SOD at 20 weeks in study group was much less as compared to control group, also SOD value in group A at 32 weeks is much less as compared to group B in which intervention was given t value ~ 46.877, p value <0.0001; 95% CI - 0.285223 - 0.310391, which is statistically highly significant. Hence, in our study there was statistically significant improvement in SOD enzyme levels after giving intervention in study group. Conclusions: Since, preeclampsia is multifactorial disorder in which oxidative stress plays very important role in pathophysiology amongst various other factors. The antioxidant supplementation to such patients who already have reduced anti-oxidant status definitely has role in decreasing incidence of PIH and its severity

Polymorphisms in renin-angiotensin-aldosterone system and vascular endothelial growth factor may cross talk in preeclampsia: a pilot study of maternal and fetal dyads in Indian population

    Get PDF
Background: Preeclampsia (PE) is a multi-system disorder complicating 5-7% pregnancies and one of the leading causes of fetal/maternal morbidity and mortality worldwide. Methods: We have analyzed the association of common genetic polymorphisms of renin-angiotensin-aldosterone system (RAAS) and angiogenesis pathway in preeclamptic/eclamptic mother-fetal dyad samples, as we have hypothesized that there is a cross-talk between the maternal and fetal genotypes. Maternal venous and fetal umbilical vein blood of 50 primigravidae with preeclampsia /eclampsia and 100 matched normotensive controls of north Indian origin were collected. Results: A significant association was observed in the prevalence of VEGFA (vascular endothelial growth factor) polymorphism (rs25648 T>C) with PE. Moreover, difference in the prevalence of the AGT (rs7079 A>C) polymorphism was observed between mild vs. severe PE. Fetal genotypes showed a strong association with respect to RENIN (rs11240688 A>G) and AGT (rs11122576 G>A) to preeclampsia. Conclusions: Preeclamptic mothers and their fetuses have shown association with genes that are interacting partners in the regulation of angiogenesis and it would be interesting to expand the study to include more genes and connected pathways for the better understanding of the interplay of the biological process that goes dysregulated in this multi-systemic disorder.

Implementing services for Early Infant Diagnosis (EID) of HIV: a comparative descriptive analysis of national programs in four countries

<p>Abstract</p> <p>Background</p> <p>There is a significant increase in survival for HIV-infected children who have early access to diagnosis and treatment. The goal of this multi-country review was to examine when and where HIV-exposed infants and children are being diagnosed, and whether the EID service is being maximally utilized to improve health outcomes for HIV-exposed children.</p> <p>Methods</p> <p>In four countries across Africa and Asia existing documents and data were reviewed and key informant interviews were conducted. EID testing data was gathered from the central testing laboratories and was then complemented by health facility level data extraction which took place using a standardized and validated questionnaire</p> <p>Results</p> <p>In the four countries reviewed from 2006 to 2009 EID sample volumes rose dramatically to an average of >100 samples per quarter in Cambodia and Senegal, >7,000 samples per quarter in Uganda, and >2,000 samples per quarter in Namibia. Geographic coverage of sites also rapidly expanded to 525 sites in Uganda, 205 in Namibia, 48 in Senegal, and 26 in Cambodia in 2009. However, only a small proportion of testing was done at lower-level health facilities: in Uganda Health Center IIs and IIIs comprised 47% of the EID collection sites, but only 11% of the total tests, and in Namibia 15% of EID sites collected >93% of all samples. In all countries except for Namibia, more than 50% of the EID testing was done after 2 months of age. Few sites had robust referral mechanisms between EID and ART. In a sub-sample of children, we noted significant attrition of infants along the continuum of care post testing. Only 22% (Senegal), 37% (Uganda), and 38% (Cambodia) of infants testing positive by PCR were subsequently initiated onto treatment. In Namibia, which had almost universal EID coverage, more than 70% of PCR-positive infants initiated ART in 2008.</p> <p>Conclusions</p> <p>While EID testing has expanded dramatically, a large proportion of PCR- positive infants are initiated on treatment. As EID services continue to scale-up, more programmatic attention and support is needed to retain HIV-exposed infants in care and ensure that those testing positive initiate treatment in a timely manner. Namibia's experience demonstrates that it is feasible for a rural, low-income country to achieve high national coverage of infant testing and treatment.</p

Biodegradation of Endocrine-Disrupting Chemicals and Residual Organic Pollutants of Pulp and Paper Mill Effluent by Biostimulation

    No full text
Effluent discharged from the pulp and paper industry contains various refractory and androgenic compounds, even after secondary treatment by activated processes. Detailed knowledge is not yet available regarding the properties of organic pollutants and methods for their bioremediation. This study focused on detecting residual organic pollutants of pulp and paper mill effluent after biological treatment and assessing their degradability by biostimulation. The major compounds identified in the effluent were 2,3,6-trimethylphenol, 2-methoxyphenol (guaiacol), 2,6-dimethoxyphenol (syringol), methoxycinnamic acid, pentadecane, octadecanoic acid, trimethylsilyl ester, cyclotetracosane, 5,8-dimethoxy-6-methyl-2,4-bis(phenylmethyl)napthalen-1-ol, and 1,2-benzendicarboxylic acid diisononyl ester. Most of these compounds are classified as endocrine-disrupting chemicals and environmental toxicants. Some compounds are lignin monomers that are metabolic products from secondary treatment of the discharged effluent. This indicated that the existing industrial process could not further degrade the effluent. Supplementation by carbon (glucose 1.0%) and nitrogen (peptone 0.5%) bio-stimulated the degradation process. The degraded sample after biostimulation showed either disappearance or generation of metabolic products under optimized conditions, i.e., a stirring rate of 150 rpm and temperature of 37 ± 1°C after 3 and 6 days of bacterial incubation. Isolated potential autochthonous bacteria were identified as Klebsiella pneumoniae IITRCP04 (KU715839), Enterobacter cloacae strain IITRCP11 (KU715840), Enterobacter cloacae IITRCP14 (KU715841), and Acinetobacter pittii strain IITRCP19 (KU715842). Lactic acid, benzoic acid, and vanillin, resulting from residual chlorolignin compounds, were generated as potential value-added products during the detoxification of effluent in the biostimulation process, supporting the commercial importance of this process

We studied the fresh root extract of Vetiver zizanioides (Linn.) Nash (VET) with a CDRI-CIMAP antimalarial α/β arteether (ART) together for their antimalarial potential. Our results showed additive to synergistic antimalarial activity of VET and ART with sum fractional inhibitory concentrations Σ FICs 1.02±0.24 and 1.12±0.32 for chloroquine sensitive (CQS) and chloroquine resistant (CQR) strain of Plasmodium falciparum (William H. Welch), respectively. Further, these combinations were explored against multidrug resistant rodent malaria parasite i.e. <i style="mso-bidi-font-style: normal">P. yoelii nigeriensis. Analysis of in vivo interaction of ART and VET showed that 10 mg/kg×5days of ART with 1000 mg/kg of VET ×5 days cured 100% mice infected with MDR parasite, while the same dose of ART could produce only up to 30% cure and VET fraction was not curative at all. Synergism/additiveness, found between VET and ART is reported for the first time. The curative dose of ART in the combination was reduced to its one fourth, and thus limits the side effects, if any. Although antimalarial potential of ART was enhanced by VET, action mechanism of later needs to be elucidated in detail.</span
