    Rural small firms' website quality in transition and market economies

    Purpose – The purpose of this paper is to investigate website quality in rural firms in four countries, by using Gonzalez and Palacios's Web Assessment Index (WAI). There is an assertion in the literature that quality is lower amongst rural firms than urban firms, and lower amongst small firms than large firms. The disadvantages of lack of access to skills and economic peripherality in rural areas are attributed to this. Concurrently, there is reason to surmise that the websites of firms in transition economies may be higher quality than those in market economies. The paper aims to explore websites in distinct rural regions to investigate if variation occurs. Design/methodology/approach – To evaluate website quality the WAI was applied to a sample of 60 rural firms representing 15 each in Scotland, New Zealand, Southern Russia and Hunan Province in China. Analysis of the categorical data was performed using a variety of established methods. Findings – The WAI is of use in terms of website quality management. Additionally, comparisons between the quality of websites in the sample of small rural firms with those of large firms in previous studies support the contention that large firms generally have better quality websites. Results also illustrate that there are some differences in website quality between rural small businesses in the different locations. In particular, small rural firms in Hunan Province in China had websites of observable better quality than those elsewhere. The authors conclude that skills, knowledge and infrastructure have a bearing on the sophistication of small firms' websites. Research limitations/implications – Implications include that variation in the rural economy by region prevails as the rural economy is not, as often implied, a homogeneous concept. Practical implications – There are implications in terms of exploring the effects of regulation, culture and infrastructure on rural small to medium-sized enterprises (SMEs). The internet may indeed contribute to rural economies, but only insofar as it is facilitated by infrastructure and access to skills, and by culture and perceived usefulness by business owners. Originality/value – The paper contributes to the understanding of rural entrepreneurship as a heterogeneous concept by comparing practice in four distinct rural regions. It also adds weight to the emerging identification of exogenous factors as being at least as much a factor in determining the use of ICT in rural SMEs as endogenous motivations, skills and resources. </jats:sec

    “I Put a Mask on” the human side of deportation effects on Latino youth

    Recent research on immigration has looked at forced deportation issues and specifically on the mental health issues of immigrant parents separated from their children rather than from the child’s experience. Hispanic adolescents residing in the United States who live with the fear of being separated from their parents either through forced parental deportation or as a result of being detained themselves may face serious health and mental health problems during the crucial developmental stage of adolescence and pre-adolescence. This study looks at twenty children ages 11-18 (males and females). Qualitative methods were used including focus groups and individual in-depth interviews to examine issues among youth who were at risk of being deported and/or whose parents had been deported or were at risk of deportation. Evidence from the study demonstrated that the youth have complex understandings of the stress of living in undocumented families that can be categorized in individual, social, and structural levels.http://jswhr.com/journals/jswhr/Vol_2_No_2_December_2014/3.pdfPublished versio

    Sternal non-union in a professional hockey player: considerations for return to play

    We describe a healthy 40-year old professional hockey player with an asymptomatic sternal non-union following aortic root surgery. The purpose of this case report is to make orthopedic surgeons aware of the possibility of this complication following sternotomy, and to discuss the considerations involved in return to play in contact sports. We will discuss our work-up, evaluation, and management of a sternal non-union in a professional athlete. Patient's consent has been obtained

    Radiofrequency catheter ablation of supraventricular tachycardia substrates after mustard and senning operations for d-transposition of the great arteries

    OBJECTIVES The purpose of this study was to determine the efficacy and risks of radiofrequency ablation of various forms of supraventricular tachycardia after Mustard and Senning operations for d-transposition of the great arteries. BACKGROUND In this patient group, the reported success rate of catheter ablation of intraatrial reentry tachycardia is about 70% with a negligible complication rate. There are no reports of the use of radiofrequency ablation to treat other types of supraventricular tachycardia. METHODS Standard diagnostic criteria were used to determine supraventricular tachycardia type. Appropriate sites for attempted ablation included 1) intraatrial reentry tachycardia: presence of concealed entrainment with a postpacing interval similar to tachycardia cycle length; 2) focal atrial tachycardia: a P-A interval ≤-20 ms; and 3) typical variety of atrioventricular (AV) node reentry tachycardia: combined electrographic and radiographic features. RESULTS Nine Mustard and two Senning patients underwent 13 studies to successfully ablate all supraventricular tachycardia substrates in eight (73%) patients. Eight of eleven (73%) patients having intraatrial reentry tachycardia, 3/3 having typical AV node reentry tachycardia, and 2/2 having focal atrial reentry tachycardia were successfully ablated. Among five patients having intraatrial reentry tachycardia (IART) and not having ventriculoatrial (V-A) conduction, two suffered high-grade AV block when ablation of the systemic venous portion of the medial tricuspid valve/inferior vena cava isthmus was attempted. CONCLUSIONS Radiofrequency catheter ablation can be effectively and safely performed for certain supraventricular tachycardia types in addition to intraatrial reentry. A novel catheter course is required for slow pathway modification. High-grade AV block is a potential risk of lesions placed in the systemic venous medial isthmus

    Health promotion in Australian multi-disciplinary primary health care services: case studies from South Australia and the Northern Territory

    This paper reports on the health promotion and disease prevention conducted at Australian multi-disciplinary primary health care (PHC) services and considers the ways in which the organizational environment affects the extent and type of health promotion and disease prevention activity. The study involves five PHC services in Adelaide and one in Alice Springs. Four are managed by a state health department and two by boards of governance. The study is based on an audit of activities and on 68 interviews conducted with staff. All the sites undertake health promotion and recognize its importance but all report that this activity is under constant pressure resulting from the need to provide services to people who have health problems. We also found an increased focus on chronic disease management and prevention which prioritized individuals and behavioural change strategies rather than addressing social determinants affecting whole communities. There was little health promotion work that reflected a salutogenic approach to the creation of health. Most activity falls under three types: parenting and child development, chronic disease prevention and mental health. Only the non-government organizations reported advocacy on broader policy issues. Health reform and consequent reorganizations were seen to reduce the ability of some services to undertake health promotion. The paper concludes that PHC in Australia plays an important role in disease prevention, but that there is considerable scope to increase the amount of community-based health promotion which focuses on a salutogenic view of health and which engages in community partnerships.This work was supported by the National Health and Medical Research Council (grant number 535041), an Australian Research Council Federation Fellowship (to F.B.) and the Canada Research Chair program (to R.L.)

    ISRU at a Lunar Outpost: Implementation and Opportunities for Partnerships and Commercial Development

    The NASA Lunar Architecture Team (LAT), which was commissioned to help answer the question "how" will humans return to the Moon, and the Synthesis Team and the recently released Global Exploration Strategy, which was commissioned to help answer the question "why" will humans return to the Moon and go on to Mars have identified the ability to extract and use in-situ resources as important to extending human frontiers, reduce dependence on Earth, and further economic and commercial expansion into space. The extraction and processing of space resources into useful products, known as In-Situ Resource Utilization (ISRU), can have a substantial impact on mission and architecture concepts. In particular, the ability to make propellants, life support consumables, and fuel cell reagents can significantly reduce the cost, mass, and risk of sustained human activities beyond Earth. Potential lunar resources include solar wind implanted volatiles, vast quantities of metal and mineral oxides, possible water/ice at the poles, abundant solar energy, regions of permanent light and darkness, the vacuum of space itself, and even scavenging leftover descent propellants and/or trash and waste from human crew activities. Suitable processing can transform these raw resources into useful materials and products. The establishment of a human lunar Outpost, as proposed by NASA at the 2nd Space Exploration Conference in Houston in December 2006, opens up the possibility for the first time of breaking our reliance on Earth supplied consumables and learn to "live off the land". The ISRU phasing and capability incorporation strategy developed during LAT Phase I & II is based on the premise that while ISRU is a critical capability and key to successful implementation of the US Vision for Space Exploration, it is also an unproven capability for human lunar exploration and can not be put in the critical path of architecture success until it has been proven. Therefore, ISRU needs to take incremental steps toward the desired end state. However, at the same time, the lunar architecture needs to be open enough to take advantage of ISRU when proven available

    Collecting Symptoms and Sensor Data With Consumer Smartwatches (the Knee OsteoArthritis, Linking Activity and Pain Study):Protocol for a Longitudinal, Observational Feasibility Study

    BACKGROUND: The Knee OsteoArthritis, Linking Activity and Pain (KOALAP) study is the first to test the feasibility of using consumer-grade cellular smartwatches for health care research. OBJECTIVE: The overall aim was to investigate the feasibility of using consumer-grade cellular smartwatches as a novel tool to capture data on pain (multiple times a day) and physical activity (continuously) in patients with knee osteoarthritis. Additionally, KOALAP aimed to investigate smartwatch sensor data quality and assess whether engagement, acceptability, and user experience are sufficient for future large-scale observational and interventional studies. METHODS: A total of 26 participants with self-diagnosed knee osteoarthritis were recruited in September 2017. All participants were aged 50 years or over and either lived in or were willing to travel to the Greater Manchester area. Participants received a smartwatch (Huawei Watch 2) with a bespoke app that collected patient-reported outcomes via questionnaires and continuous watch sensor data. All data were collected daily for 90 days. Additional data were collected through interviews (at baseline and follow-up) and baseline and end-of-study questionnaires. This study underwent full review by the University of Manchester Research Ethics Committee (#0165) and University Information Governance (#IGRR000060). For qualitative data analysis, a system-level security policy was developed in collaboration with the University Information Governance Office. Additionally, the project underwent an internal review process at Google, including separate reviews of accessibility, product engineering, privacy, security, legal, and protection regulation compliance. RESULTS: Participants were recruited in September 2017. Data collection via the watches was completed in January 2018. Collection of qualitative data through patient interviews is still ongoing. Data analysis will commence when all data are collected; results are expected in 2019. CONCLUSIONS: KOALAP is the first health study to use consumer cellular smartwatches to collect self-reported symptoms alongside sensor data for musculoskeletal disorders. The results of this study will be used to inform the design of future mobile health studies. Results for feasibility and participant motivations will inform future researchers whether or under which conditions cellular smartwatches are a useful tool to collect patient-reported outcomes alongside passively measured patient behavior. The exploration of associations between self-reported symptoms at different moments will contribute to our understanding of whether it may be valuable to collect symptom data more frequently. Sensor data-quality measurements will indicate whether cellular smartwatch usage is feasible for obtaining sensor data. Methods for data-quality assessment and data-processing methods may be reusable, although generalizability to other clinical areas should be further investigated.
