Amphetamine Use Revealing Hypertrophic Cardiomyopathy in a Young Patient.

Adrenergic overstimulation in long term can lead to a hyperdynamic myocardium and give rise to hypertrophy and ultimately to heart failure. Amphetamine use is a common cause of neurohormonal activation, which gives rise to such adverse cardiovascular events. However, hypertrophy of myocardium in young patients, even due to apparently obvious causes, should always be considered as a red flag and a further diagnostic downstream should take place, in order to exclude genetic causes. We present a case of a young man with chronic use of amphetamine and an incidental finding of hypertrophic cardiomyopathy

The social impact of the ‘Social solidarity income’ in Greece: A qualitative interpretation

Το παρόν άρθρο ρίχνει φως στην υλοποίηση του «Κοινωνικού Εισοδήµατος Αλληλεγγύης» (εφεξής ΚΕΑ) στη χώρα µας, ένα παράδειγµα ελάχιστου εγγυηµένου εισοδήµατος. Βασισµένο στα ευρήµατα 40 ηµιδοµηµένων συνεντεύξεων µε διοικητικό προσωπικό των λεγόµενων «Κέντρων Κοινότητας» και ωφελούµενους, το άρθρο εστιάζει στον «κοινωνικό αντίκτυπο» του ΚΕΑ και, πιο συγκεκριµένα, σε δύο συνδεόµενα µεταξύ τους θέµατα: α) την «κοινωνική κατάσταση» των ωφελούµενων και τις στρατηγικές επιβίωσής τους, πριν και µετά την πρόσβαση στο ΚΕΑ και β) τις όψεις του ΚΕΑ που πρέπει να βελτιωθούν κατά το στάδιο της υλοποίησης. ∆ιατυπώνεται το επιχείρηµα ότι, παρά την ύπαρξη θετικών στοιχείων, όπως π.χ. ότι η επιδοµατική ενίσχυση, που αποτελεί κεντρικό πυλώνα του ΚΕΑ, βοηθά τους ωφελούµενους να αντεπεξέλθουν στις βασικές τους ανάγκες, ο αντίκτυπος του ΚΕΑ στη ζωή των ωφελούµενων είναι σχετικά µικρός και µεγάλο ποσοστό εξακολουθεί να βασίζεται στις στρατηγικές επιβίωσης που χρησιµοποιούσε και προ του ΚΕΑThe present article sheds light on the implementation of the ‘Social Solidarity Income’ (SSI) in Greece, an example of a guaranteed minimum income (GMI). Drawing on the findings of 40 semi-structured interviews carried out in five selected municipalities, the article focuses on the ‘social impact’ of the SSI. More specifically, it investigates two inter-related themes: a) the social situation of beneficiaries and the survival strategies they used before and after receiving the SSI; and b) aspects of the SSI that should be improved at the implementation stage. It is argued that, despite certain positive elements of the SSI, such as the monetary allowance, which is a key component of the SSI and assists beneficiaries in covering their basic needs, the impact of the SSI on the lives of individuals is relatively small and a large proportion of the beneficiaries continue to rely on the survival strategies they used before implementation of the SS

Cardiopulmonary Exercise Test in heart failure: A Sine qua non

A robust literature, over the last years, supports the indication of cardiopulmonary exercise testing (CPET) in patients with cardiovascular diseases. Understanding exercise physiology is a crucial component of the critical evaluation of exercise intolerance. Shortness of breath and exercise limitation is often treated with an improper focus, partly because the pathophysiology is not well understood in the frame of the diagnostic spectrum of each subspecialty. A vital field and research area have been cardiopulmonary exercise test in heart failure with preserved/reduced ejection fraction, evaluation of heart failure patients as candidates for LVAD-Implantation, as well as for LVAD-Explantation and ultimately for heart transplantation. All the CPET variables provide synergistic prognostic discrimination. However, Peak VO2 serves as the most critical parameter for risk stratification and prediction of survival rate
