    Applications of three-dimensional printing in ophthalmology

    Three-dimensional (3D) printing is increasingly used to produce customised objects and is a promising alternative to traditional manufacturing methods in diverse fields, such as dentistry and orthopaedics. Already in use in other medical specialities, adoption in ophthalmology has been limited to date. This review aims to provide an overview of 3D printing technology with respect to current and potential applications in ophthalmic practice. Medline, Embase and internet search were performed with "3D printing", "ophthalmology", "dentistry", "orthopaedics" and their synonyms used as main search terms. In addition, search terms related to clinical applications such as "surgery" and "implant" were employed. 3D printing has multiple applications in ophthalmology, including in diagnosis, surgery, prosthetics, medications and medical education. Within the past decade, researchers have produced 3D printed models of objects such as implants, prostheses, anatomical models and surgical simulators. Further development is necessary to generate optimal biomaterials for various applications, and the quality and long-term performance of 3D models needs to be validated

    Systemic adjuvant chemotherapy for advanced malignant ocular medulloepithelioma

    BACKGROUND: Ocular medulloepithelioma (diktyoma) is a rare and potentially malignant paediatric tumour of the non-pigmented ciliary epithelium. Adjuvant chemotherapy can be given in advanced cases, but the indications and regimens remain to be defined. The aim was to identify whether adjuvant chemotherapy offers treatment benefit in advanced ocular medulloepithelioma. METHODS: This was a retrospective case series of subjects referred to a single specialist ocular oncology centre for advanced ocular medulloepithelioma subsequently treated with enucleation, including those needing adjuvant systemic vincristine, etoposide and carboplatin. A case-note review was performed for included subjects meeting referral criteria. The outcomes were histopathology characteristics, recurrence, metastases and survival. RESULTS: Between March 2010 and June 2017, four male patients (mean age 31 months) underwent enucleation for ocular medulloepithelioma. Adjuvant chemotherapy was commenced in 3 patients (75%) due to malignant histopathological features. With a mean follow-up time of 81.5 months (median 71 months, range 49-135 months) none of the patients have had recurrence, metastases or death from the tumour. CONCLUSIONS: This series is unique in reporting the management of advanced malignant ocular medulloepithelioma with adjuvant systemic vincristine, etoposide and carboplatin for advanced tumours with malignant features. This regimen appears to be safe and may be effective in preventing metastatic spread

    Effect of processing route on ballistic performance of Ti-6Al-4V armour plate

    Titanium alloys are a key part of an armour designer’s toolset due to their high ballistic mass efficiency. However, titanium’s high cost has restricted its use in military land-based vehicles. In this study, the effect of different processing routes on the microstructure, texture, ballistic performance V50 and deformation modes of Ti-6Al-4V armour plate is investigated. Plates were fabricated via conventional hot rolling and the powder metallurgy techniques of HIP and FAST. The plate failure modes varied significantly with the microstructure while the formation of adiabatic shear bands seems to be influenced by crystallographic texture. The rolled plate exhibited the best performance due to a strong transverse-type texture perpendicular to the projectile direction

    Yield sensing technologies for perennial and annual horticultural crops: a review

    Yield maps provide a detailed account of crop production and potential revenue of a farm. This level of details enables a range of possibilities from improving input management, conducting on-farm experimentation, or generating profitability map, thus creating value for farmers. While this technology is widely available for field crops such as maize, soybean and grain, few yield sensing systems exist for horticultural crops such as berries, field vegetable or orchards. Nevertheless, a wide range of techniques and technologies have been investigated as potential means of sensing crop yield for horticultural crops. This paper reviews yield monitoring approaches that can be divided into proximal, either direct or indirect, and remote measurement principles. It reviews remote sensing as a way to estimate and forecast yield prior to harvest. For each approach, basic principles are explained as well as examples of application in horticultural crops and success rate. The different approaches provide whether a deterministic (direct measurement of weight for instance) or an empirical (capacitance measurements correlated to weight for instance) result, which may impact transferability. The discussion also covers the level of precision required for different tasks and the trend and future perspectives. This review demonstrated the need for more commercial solutions to map yield of horticultural crops. It also showed that several approaches have demonstrated high success rate and that combining technologies may be the best way to provide enough accuracy and robustness for future commercial systems

    PRELP secreted from mural cells protects the function of blood brain barrier through regulation of endothelial cell-cell integrity

    INTRODUCTION: Proline/arginine-rich end leucine-rich repeat protein (PRELP), is a small secreted proteoglycan expressed by pericytes and vascular smooth muscle cells surrounding the brain vasculature of adult mouse. METHODS: We utilised a Prelp knockout (Prelp−/−) mouse model to interrogate vasculature integrity in the brain alongside performing in vitro assays to characterise PRELP application to endothelial cells lines. Our findings were supplemented with RNA expression profiling to elucidate the mechanism of how PRELP maintains neurovasculature function. RESULTS: Prelp−/− mice presented with neuroinflammation and reducedneurovasculature integrity, resulting in IgG and dextran leakage in the cerebellum and cortex. Histological analysis of Prelp−/− mice revealed reducedcell-cell integrity of the blood brain barrier, capillary attachment of pericytes andastrocyte end-feet. RNA-sequencing analysis found that cell-cell adhesion andinflammation are affected in Prelp−/− mice and gene ontology analysis as well as gene set enrichment analysis demonstrated that inflammation related processes and adhesion related processes such as epithelial-mesenchymal transition and apical junctions were significantly affected, suggesting PRELP is a regulator of cell-cell adhesion. Immunofluorescence analysis showed that adhesion junction protein expression levels of cadherin, claudin-5, and ZO-1, was suppressed in Prelp−/− mice neurovasculature. Additionally, in vitro studies revealed that PRELP application to endothelial cells enhances cell-cell integrity, induces mesenchymal-endothelial transition and inhibits TGF-β mediated damage to cell-cell adhesion. DISCUSSION: Our study indicates that PRELP is a novel endogenous secreted regulator of neurovasculature integrity and that PRELP application may be a potential treatment for diseases associated with neurovascular damage

    Two Secreted Proteoglycans, Activators of Urothelial Cell–Cell Adhesion, Negatively Contribute to Bladder Cancer Initiation and Progression

    Osteomodulin (OMD) and proline/arginine-rich end leucine repeat protein (PRELP) are secreted extracellular matrix proteins belonging to the small leucine-rich proteoglycans family. We found that OMD and PRELP were specifically expressed in umbrella cells in bladder epithelia, and their expression levels were dramatically downregulated in all bladder cancers from very early stages and various epithelial cancers. Our in vitro studies including gene expression profiling using bladder cancer cell lines revealed that OMD or PRELP application suppressed the cancer progression by inhibiting TGF-β and EGF pathways, which reversed epithelial–mesenchymal transition (EMT), activated cell–cell adhesion, and inhibited various oncogenic pathways. Furthermore, the overexpression of OMD in bladder cancer cells strongly inhibited the anchorage-independent growth and tumorigenicity in mouse xenograft studies. On the other hand, we found that in the bladder epithelia, the knockout mice of OMD and/or PRELP gene caused partial EMT and a loss of tight junctions of the umbrella cells and resulted in formation of a bladder carcinoma in situ-like structure by spontaneous breakdowns of the umbrella cell layer. Furthermore, the ontological analysis of the expression profiling of an OMD knockout mouse bladder demonstrated very high similarity with those obtained from human bladder cancers. Our data indicate that OMD and PRELP are endogenous inhibitors of cancer initiation and progression by controlling EMT. OMD and/or PRELP may have potential for the treatment of bladder cancer

    Late Solitary Extraocular Recurrence from Previously Resected Iris Melanoma.

    Purpose To report on cases of late extraocular relapse of previously resected iris melanoma, without concurrent intraocular recurrence. Design Retrospective case series. Methods A retrospective chart review of 4 patients diagnosed with late subconjunctival relapse of previously resected iris melanoma. Results Three females and one male underwent iris tumour resection and presented to our service with suspicious conjunctival lesions at a median of 22 years later (mean: 21). None showed intraocular relapse. Treatment of the conjunctival tumours included excisional biopsy (n=4) followed by cryotherapy (n=3) and/or brachytherapy (n=3). In all cases, histopathology confirmed malignant melanoma, with no intraepithelial component or associated melanosis. Genetic sequencing (n=3) showed wildtype BRAF and NRAS in all. GNA11 mutation was found in 1 case. On array CGH (n=3), gain of 6p was found in 2 cases and gain of 8 in 2. Overall, findings were strongly suggestive of a diagnosis of late extraocular relapse from previously resected iris melanoma. In a median of 2.5 years (mean: 7.7) from the subconjunctival relapse, no further episodes of intra/extraocular recurrence were recorded, and all patients were free from distant metastasis. Conclusions Patients undergoing iris melanoma resection are at risk of developing late solitary extraocular relapse even over 30 years after surgery. In the absence of an intraocular component, diagnosis may be challenging, as tumours mimic a primary conjunctival lesion. Management by excisional biopsy followed by adjuvant therapy was successful, and histopathology and genetic analysis supported a diagnosis of extraocular uveal tumour spread rather than a primary conjunctival tumour
