    Effect of Chlorhexidine on durability of two self-etch adhesive systems

    Despite of the rapid development in the field of dental adhesives, the issue of reduction in dentin bond durability has still not been resolved. The activity of dentinal endogenous enzymes such as MMPs is one of the most important causes of failure in resin composite restorations. The aim of this study was to investigate the influence of Chlorhexidine on micro-tensile bond strength of two types of commercially available self-etch adhesives. Twenty four sound and freshly extracted molars were selected. Four standardized flat mid-coronal dentinal disks were prepared from each tooth. The specimens were randomly assigned to 6 groups (n=16). Groups A(control group) and B were treated with Clearfill SE Bond based on the manufacturer?s instructions. Groups C and D were treated with 2% Chlorhexidine 60 seconds before applying Clearfill SE Bond. Groups E and F were treated with Peak Universal Bond according to the manufacturer?s instructions. All groups were stored in distilled water in room temperature. Microtensile bond strength in groups A, C, and E were tested 24 hours after preparation, while microtensile bond strength in groups B, D, and F were tested after 3 months storage and 3000 thermal cycles(5-55 °C). Statistical analysis was performed with SPSS 20 and µTBS test results were analyzed using the Two-way ANOVA test. µTBS was not significantly different between groups A, C, and E after 24 hours (P>0.5). There was no significant difference between groups B (Clearfill SE Bond + Aging) and D (Clearfill SE Bond + 2% CHX + Aging). The Peak Universal µTBS significantly decreased after the aging procedure (P<0.001). Based on the findings of this study, pretreatment with 2% CHX had no negative effect on the Clearfill SE Bond µTBS. However the µTBS of 0.2% CHX contained Peak Universal adhesive decreased significantly after aging

People around the globe publish and share information related to real-world events they experience every day. This provides a valuable opportunity to analyze the content of this information to detect real-world happenings, however, it is quite challenging task. In this work, we propose a novel graph-based approach named the Dynamic Heartbeat Graph (DHG) that not only detects the events at an early stage, but also suppresses them in the upcoming adjacent data stream in order to highlight new emerging events. This characteristic makes the proposed method interesting and efficient in finding emerging events and related topics. The experiment results on real-world datasets (i.e. FA Cup Final and Super Tuesday 2012) show a considerable improvement in most cases, while time complexity remains very attractive

    SURVIVAL OUTCOMES IN EARLY GLOTTIC CARCINOMA; A SINGLE INSTITUTION EXPERIENCE

    Purpose: Laryngeal cancers are amongst the most common cancers affecting head and neck region. In this study, we analyse the overall survival (OS) following hypofractionated radiotherapy (RT) in early stage glottic carcinoma treated at Shaukat Khanum Memorial Cancer Hospital and Research Centre, Lahore. Methods: Between October 2003 and June 2009, 87 patients with early stage glottic carcinoma were treated with hypofractionated RT. All patients were included in the study. The ratio of male: female is 94%:6%. Mean age was 62 years (range 31–83 years). 66% of the patients were smokers. AJCC stage was T1a in 76%, T1b 20% and T2 in 4% of the patients. Histological distribution was; squamous cell carcinoma 97%, verrucous carcinoma 2% and squamous cell spindle variant 1%. Median follow-up time was 59 months (range 4–122 months). RT dose was 55 Gy in 20 fractions over a period of 4 weeks. Median RT treatment time was 28 days (range 23–35 days). Patients that lost to follow-up were contacted through telephone. Results: The 10-year OS was 83%. Patterns of failure was 7 local and 1 distant while 1 patient had persistent disease. 15 patients were dead at the time of study. Cause of death; 13 patients died due to Ischemic heart disease and 2 due to primary disease. Conclusion: Hypofractionated RT 55 Gy in 20 fractions seems to achieve good OS while offering potential for optimizing resources usage. Key words: Glottic carcinoma, hypofractionated, overall survival, radiotherapy

    Enhanced Heartbeat Graph for emerging event detection on Twitter using time series networks

Detecting events from Twitter stream requires complex techniques. Event-related trending topics consist of a group of words which successfully detect and identify events. Event detection techniques must be scalable and robust, so that they can deal with the huge volume and noise associated with social media. Existing event detection methods mostly rely on burstiness, mainly the frequency of words and their co-occurrences. However, burstiness sometimes dominates other relevant details in the data which could be equally significant. Besides, the topological and temporal relationships in the data are often ignored. In this work, we propose a novel graph-based approach, called the Enhanced Heartbeat Graph (EHG), which detects events efficiently. EHG suppresses dominating topics in the subsequent data stream, after their first detection. Experimental results on three real-world datasets (i.e., Football Association Challenge Cup Final, Super Tuesday, and the US Election 2012) show superior performance of the proposed approach in comparison to the state-of-the-art techniques

    Ultrasound-guided Intralesional Bleomycin Injection (IBI) for Treatment of Cutaneous Hemangiomas and Vascular Malformations

    Purpose: To report the therapeutic outcome of ultrasound-guided intralesional injection of bleomycin in the treatment of cutaneous hemangiomas and vascular malformations. Material & Methods: The medical records of patients with cutaneous hemangiomas and vascular malformations treated with the intralesional injection of bleomycin under ultrasound guidance between August 2009 and June 2013 at the Indus Hospital, Karachi were reviewed retrospectively using a computerized medical record information management system. Data were extracted using a pre-coded performa that included patient demographics, type and location of lesion, number of treatments, presenting/pre- and post-treatment clinical symptoms (pain, swelling, heaviness, size, discoloration), ultrasound appearance and vascularity, and post-treatment side effects. The dose range of bleomycin was 0.5-1.0 mg/kg, but not exceeding 15 mg in a single session. A maximum of four treatments were given in any given patient except for one, who presented with recurrence after a year of complete resolution. Therapeutic outcome was determined using review of ultrasound images and recorded clinical assessment. Treatment response was categorized as: (i) complete resolution [more than 90% reduction]; (ii) substantial reduction [more than 50% reduction]; (iii) mild reduction [25% reduction]; or, (iv) no improvement [ \u3c 10% reduction]. Results: A total of 30 patients (16 female, 14 male), ranging in age from 8 months to 48 years (mean age 10.2 years), were treated from 2009 to 2013. There were 23 hemangiomas. Seven were vascular malformations, of which five were lymphatic malformations and two were venous malformations. Twenty-eight lesions were located in the head and neck region, and two were peripheral.. In 24 of the 30 patients (76%), treatment had been completed. In six patients (21%) treatment was ongoing at the time of this report. Seventeen of the 23 hemangiomas (74%) were completely resolved clinically and on ultrasound, five (22%) showed substantial improvement and one (4%) showed mild improvement. In five of the seven vascular malformations (71%) lymphatic malformations resolved completely, and two (29%) venous malformations showed substantial improvement. Of the 13 patients presenting with discoloration, there was complete resolution in one (7.7%), marked reduction in 11 (84.6%) and mild reduction in one (7.7%). Of seven patients presenting with pain, there was complete resolution in two (28.6%), marked reduction in two (28.6%), mild reduction in two (28.6%), and no improvement in one (14.3%). There were no pulmonary complications. Conclusion: Ultrasound-guided intralesional injection of bleomycin is an option to consider for the treatment of certain types of cutaneous hemangiomas and vascular malformations. Prospective studies should be undertaken to understand the various factors contributing to therapeutic success

    Early Versus Interval Cholecystectomy in Mild Acute Biliary Pancreatitis. A Retrospective Study in Tertiary Care Hospital

    Objective: The objective is to determine the adequacy of early cholecystectomy (EC) versus interval cholecystectomy (IC) in terms of recurrence, duration of hospital admission, and perioperative complications after mild acute biliary pancreatitis (MABP). Methodology: After endorsement from the ethical committee, clinical data and files of all the admitted patients having MABP in the general surgery department of Holy Family Hospital, Rawalpindi, was collected retrospectively from August 2017 to July 2020. The patients’ demographic profile, clinical findings, diagnostic investigations, timing of cholecystectomy, operating surgeons, operative time, biliary complications, intraoperative bleeding, conversion rate, duration of admission, and recurrence were reviewed. Patients presented with abdominal pain, vomiting, jaundice, or fever. The diagnosis was confirmed on the basis of a CT scan of the abdomen showing an inflamed pancreas and stones in the gallbladder. Outcomes were compared and reviewed between the two groups. Results: In this research proposal, 263 patients admitted were analyzed. EC was performed were discharged after conservative management and followed up after 12 weeks for interval laparoscopic cholecystectomy (IC). It was observed in patients of IC (EC 2 [1.2%] vs IC 7[7%]; p value &lt;0.01) while duration of hospitalization was prolonged as compared to patients of EC (EC 5.53+0.58days vs IC 5.82+0.78days; p value &lt;0.001). lar in both EC and IC. Conclusion: EC performed after MABP is associated with shorter duration of hospitalization and reduced recurrence with similar rate of perioperative complications
