    Designing a fruit identification algorithm in orchard conditions to develop robots using video processing and majority voting based on hybrid artificial neural network

    The first step in identifying fruits on trees is to develop garden robots for different purposes such as fruit harvesting and spatial specific spraying. Due to the natural conditions of the fruit orchards and the unevenness of the various objects throughout it, usage of the controlled conditions is very difficult. As a result, these operations should be performed in natural conditions, both in light and in the background. Due to the dependency of other garden robot operations on the fruit identification stage, this step must be performed precisely. Therefore, the purpose of this paper was to design an identification algorithm in orchard conditions using a combination of video processing and majority voting based on different hybrid artificial neural networks. The different steps of designing this algorithm were: (1) Recording video of different plum orchards at different light intensities; (2) converting the videos produced into its frames; (3) extracting different color properties from pixels; (4) selecting effective properties from color extraction properties using hybrid artificial neural network-harmony search (ANN-HS); and (5) classification using majority voting based on three classifiers of artificial neural network-bees algorithm (ANN-BA), artificial neural network-biogeography-based optimization (ANN-BBO), and artificial neural network-firefly algorithm (ANN-FA). Most effective features selected by the hybrid ANN-HS consisted of the third channel in hue saturation lightness (HSL) color space, the second channel in lightness chroma hue (LCH) color space, the first channel in L*a*b* color space, and the first channel in hue saturation intensity (HSI). The results showed that the accuracy of the majority voting method in the best execution and in 500 executions was 98.01% and 97.20%, respectively.     Combination of a Giant Dissected Ascending Aortic Aneurysm with Multiple Fistulae into the Cardiac Chambers Caused by Prosthetic Aortic Valve Endocarditis

    The combination of a dissected ascending aortic aneurysm (AA) with multiple fistulae to the periaortic root structures is a life-threatening complication that occurs rarely after infective endocarditis of the prosthetic aortic valve. Many risk factors are potentially associated with this complication, including aortic diameter, connective tissue disease of the aortic wall, hypertension and infection. We report a rare case of dissected ascending AA with fistulae to the left atrium and pulmonary artery and a paravalvular leak in a 47-year-old woman with a history of an aortic valve replacement. The patient had presented to the Imam Ali Hospital, Kermanshah, Iran, in January 2015 with clinical features of heart failure. After initially being treated for congestive heart failure, she underwent open-heart surgery via a classic Bentall procedure and double fistula closure. She was discharged 23 days after the operation in good condition. A six-month follow-up showed normal functioning of the composite conduit prosthetic valve and no fistulae recurrence

    Powder bed fusion additive layer manufacturing of titanium alloys

    Powder bed fusion (PBF) techniques for additive layer manufacturing (ALM) are reviewed with a focus on titanium alloys production. Selective laser melting and electron beam melting are discussed in terms of feedstock production and processing-microstructure relationships. To control the PBF processes, an outline is presented on the computational modelling approaches for simulating process parameters and defects such as residual stresses and porosity at different length scales. It is concluded that by improving powder production techniques, designing new alloys and further developing ALM hardware, PBF techniques can reach commercial maturity.     Effect of Body Mass Index on Postoperative Complications in Beating Coronary Artery Surgery

    BACKGROUND: Body Mass Index (BMI) is considered as an important risk factor in cardiovascular surgery. We designed a historical cohort study for the evaluation of perioperative complications related to BMI in patients who underwent off-pump coronary artery bypass grafting (OPCAB).METHODS: We studied 1120 consecutive patients who underwent OPCAB between January 2008 and December 2011 in Imam Ali Hospital, Kermanshah, Iran. Patients were divided into four groups according to BMI: underweight/low BMI (< 18.5 kg/m2), healthy weight (between 18.5 kg/m2 and 24.9 kg/m2), overweight/high BMI (between 25 kg/m2 and 30 kg/m2), and obese/very high BMI (> 30 kg/m2). RESULTS: In multivariable regression analysis, an important correlation between the underweight/low BMI group and mortality was observed (p=0.037). Postoperative stroke, postoperative atrial fibrillation (AF) and intra-aortic balloon pump (IABP) use were not associated with BMI. In linear regression analysis, significant correlations between low BMI, reintubation, ICU stay time and intubation time were found. Re-exploration for bleeding was significantly correlated with having a low or high BMI. CONCLUSION: Having a low BMI (which is association with malnutrition and respiratory muscles weakness) was significantly associated with reintubation, prolonged intubation time and ICU stay time.     Right Ventricular Thrombus and Cerebral Artery Aneurysm in a Patient with Behçet’s Disease

    We report a 35-year-old woman referred to the Imam Ali Hospital, Kermanshah, Iran, in July 2014 for evaluation of postoperative dyspnoea after neurosurgery performed seven days previously for a ruptured cerebral artery aneurysm. She was known to have Behçet’s disease with a history of recurrent oral and genital aphthous ulcers and uveitis. At referral, her symptoms included vertigo, dysarthria, palpitations and chest pain. Transthoracic echocardiography (TTE) revealed a large thrombus in her right ventricle outflow tract and open-heart surgery was performed eight days after the previous surgery to remove the clot. The postoperative period was complicated by transient acute renal failure, which resolved spontaneously. The patient was discharged 13 days after the cardiac surgery on warfarin, prednisolone, azathioprine and cyclophosphamide. Cyclophosphamide and azathioprine were discontinued after three months as the symptoms had completely resolved; however, prednisolone was continued due to recurrent uveitis. A 10-month follow-up TTE scan revealed no thrombus recurrence and treatment with warfarin and prednisolone was continued

    A Tricuspid Valve Mass Attached to Papillary Muscle

    BACKGROUND: Cardiac myxoma is the most common benign heart tumor which can arise in any of the cardiac chambers, valves or related great veins. Diagnosis of a myxoma arising from the tricuspid valve apparatus is exceptional. We present a rare case of myxoma in a tricuspid valve attached to papillary muscle.CASE DETAILS: A 45-year-old man was referred to our center for the evaluation dyspnea and chest pain. During work-up by transthoracic echocardiography (TTE), a mass was found on the corda tendinea of the anterior papillary muscle of the tricuspid valve. Coronary angiography revealed normal coronary artery. During open heart surgery, an oval and non-pedunculated mass was detected on tricuspid corda tendinea and resected. Pathological examination revealed the presence of a myxoma.CONCLUSION: This experience illustrates a rare case of myxoma which originated from tricuspid corda tendinea, diagnosed by echocardiography, suggesting fibroelastoma. However, the mass was not clear enough but dyspnea and sign and symptom of probably embolization to lung urged us to treat it surgically. No complications attributable to the mass developed in the postoperative course. In the first year of follow-up, non-recurrence of the mass was detected on TEE, and the patient was asymptomatic.KEYWORDS: Myxoma, Papillary muscle, Tricuspid valv

    Hemolytic anemia caused by kinking of dacron grafts implanted in repair of after aortic dissection

    Background: Hemolytic anemia caused by a kinked Dacron graft is a rare complication after repair of acute aortic dissection. We present a case of hemolytic anemia due to kinking of previously implanted Dacron graft for ascending aorta dissection treated by surgery and replaced with new Dacron.Case Details: We report a case of postoperative hemolytic anemia with kinking of the graft at the outer graft layer and intra luminal bulging of the inner graft. Postoperative computed tomography showed Dacron graft kinking at the mid part of the graft. Transthoracic echocardiography (TTE) revealed good function of repair aortic valve. Upon the redo midsternotomy, we found moderate kinking of the graft at the outer graft layer and intra luminal bulging of the inner graft. We performed reconstruction of the aortic root with a new Dacron.Conclusion: The careful literature review showed that there are some few cases of hemolytic anemia after dissection, but no cases in which hemolytic anemia occurred in a patient with kinked Dacron graft after surgery delete treated by replacement of kinked Dacron graft by new one.Keywords: Hemolytic anemia, Surgical repaired aortic dissection, Aortic graft kinkin
