    Postoperative cognitive change after cardiac surgery predicts long-term cognitive outcome

    Objectives Postoperative cognitive dysfunction (POCD) is a common consequence of coronary artery bypass grafting. However, domain-specific associations between postoperative changes and long-term performance are poorly known. The aim of this study was to investigate whether domain-specific cognitive changes after cardiac surgery predict long-term cognitive outcome. Materials and Methods We assessed 100 patients (86 men, mean age 60) before coronary artery bypass grafting, with re-examinations after one week, three months, and a mean of 6.7 years. The extensive neuropsychological test battery was organized into seven functional cognitive domains. Cognitive decline and improvement were defined with the reliable change index derived from 17 matching healthy controls. Analyses were adjusted for baseline cognitive performance, age, gender, education and cardiovascular risks factors. Results On group level, one week after surgery 71% patients showed cognitive decline and 9% improvement in any functional domain, as compared to preoperative results. Three months postsurgery, decline was observed in 47% and improvement in 25% of patients. Executive functioning was the most sensitive domain to both decline and improvement. Postoperative dysfunction predicted long-term cognitive deterioration six years after operation, particularly in the domain of executive functioning. Conclusions POCD after coronary artery bypass grafting is an essential risk factor for long-term deterioration and an indication for neuropsychological follow-up. Assessment of change in executive functioning after coronary artery bypass grafting may help to identify patients at risk for unfavorable long-term outcome.Peer reviewe

    Evolutionary and ecological processes influencing chemical defense variation in an aposematic and mimetic Heliconius butterfly

    Chemical defences against predators underlie the evolution of aposematic coloration and mimicry, which are classic examples of adaptive evolution. Surprisingly little is known about the roles of ecological and evolutionary processes maintaining defence variation, and how they may feedback to shape the evolutionary dynamics of species. Cyanogenic Heliconius butterflies exhibit diverse warning color patterns and mimicry, thus providing a useful framework for investigating these questions. We studied intraspecific variation in de novo biosynthesized cyanogenic toxicity and its potential ecological and evolutionary sources in wild populations of Heliconius erato along environmental gradients, in common-garden broods and with feeding treatments. Our results demonstrate substantial intraspecific variation, including detectable variation among broods reared in a common garden. The latter estimate suggests considerable evolutionary potential in this trait, although predicting the response to selection is likely complicated due to the observed skewed distribution of toxicity values and the signatures of maternal contributions to the inheritance of toxicity. Larval diet contributed little to toxicity variation. Furthermore, toxicity profiles were similar along steep rainfall and altitudinal gradients, providing little evidence for these factors explaining variation in biosynthesized toxicity in natural populations. In contrast, there were striking differences in the chemical profiles of H. erato from geographically distant populations, implying potential local adaptation in the acquisition mechanisms and levels of defensive compounds. The results highlight the extensive variation and potential for adaptive evolution in defense traits for aposematic and mimetic species, which may contribute to the high diversity often found in these systems.Peer reviewe

    Use of benzodiazepines and benzodiazepine-related drugs in the Nordic countries between 2000 and 2020

We aimed to describe and compare their use across the Nordic countries. Data on the use of clonazepam, BZ-sedatives, BZ-hypnotics, and benzodiazepine-related drugs (BZRD) in adults (≄20 years) were obtained from nationwide registers in Denmark, Finland, Iceland, Norway, and Sweden, 2000–2020. Main measures were therapeutic intensity (TI:DDD/1000 inhabitants [inhab.]/day) and annual prevalence (users/1000 inhab./year). Overall, TI of BZ and related drugs decreased in all Nordic countries from 2004 to 2020. However, there were considerable differences between countries in TI. In 2020, the TI of BZ and related drugs ranged from 17 DDD/1000 inhab./day in Denmark to 93 DDD/1000 inhab./day in Iceland. BZRD accounted for 55–78% of BZ use in 2020, followed by BZ sedatives at 20–44%, BZ-hypnotics at <1–5%, and clonazepam at <1–2%. Annual prevalence of BZ use increased with age in all countries, and the highest annual prevalence was observed among people ≄80 years. Overall, the use of BZ and related drugs has decreased in all Nordic countries from 2004 to 2020, however, with considerable differences in their use between countries. The highest prevalence was observed among the oldest age groups—despite warnings against their use in this population.Peer reviewe

    Five-frequency Galileo long-baseline ambiguity resolution with multipath mitigation

For long-baseline over several hundreds of kilometers, the ionospheric delays that cannot be fully removed by differencing observations between receivers hampers rapid ambiguity resolution. Compared with forming ionospheric-free linear combination using dual- or triple-frequency observations, estimating ionospheric delays using uncombined observations keeps all the information of the observations and allows extension of the strategy to any number of frequencies. As the number of frequencies has increased for the various GNSSs, it is possible to study long-baseline ambiguity resolution performance using up to five frequencies with uncombined observations. We make use of real Galileo observations on five frequencies with a sampling interval of 1 s. Two long baselines continuously receiving signals from six Galileo satellites during corresponding test time intervals were processed to study the formal and empirical ambiguity success rates in case of full ambiguity resolution (FAR). The multipath effects are mitigated using the measuremen ts of another day when the constellation repeats. Compared to the results using multipath-uncorrected Galileo observations, it is found that the multipath mitigation plays an important role in improving the empirical ambiguity success rates. A high number of frequencies are also found to be helpful to achieve high ambiguity success rate within a short time. Using multipath-uncorrected observations on two, three, four and five frequencies, the mean empirical success rates are found to be about 73, 88, 91, and 95% at 10 s, respectively, while the values are increased to higher than 86, 95, 98, and 99% after mitigating the multipath effects
