    Hepatoprotective potential of glyceryl trinitrate against chemically induced oxidative stress and hepatic injury in rats

    Glyceryl trinitrate (GTN) has been used widely as a potent vasodilator to treat heart conditions, such as angina pectoris and chronic heart failure. This study aims to elucidate the effect of exogenous nitric oxide (NO) administration, using GTN, on carbon tetrachloride (CCl4)-induced oxidative stress and liver injury in rats. The results obtained demonstrated that NO generated by the administration of GTN affords protection against CCl4-induced oxidative stress and liver injury. Administration of CCl4 resulted in a significant (p < 0.001) increase in lipid peroxidation and tissue damage markers (aspartate and alanine transaminase and lactate dehydrogenase) release in serum. Parallel to these changes, CCl4 also caused downregulation of antioxidant enzymes including glutathione peroxidase (GPx), glutathione reductase (GR), and glutathione-S-transferase (GST), and several fold induction in γ-glutamyl transpeptidase (GGT) activity. Subsequent administration of GTN resulted in significant (p < 0.001) recovery of GSH-metabolizing enzymes in a dose-dependent manner. Further, administration of NO inhibitor, NG-nitro-l-arginine methyl ester (l-NAME), exacerbated CCl4-induced oxidative tissue injury. Overall, the study suggests that GTN might suppress oxidant-induced tissue injury and hepatotoxicity in rats

    Ethnobotanical, phytochemical and pharmacological aspects of daphne mucronata (thymeleaceae)

    Daphne mucronata is a shrub well known as a medicinal plant in different regions of Asia. Ethnobotanical, phytochemical and pharmacological studies have revealed strong anti-cancer potential of the plant. Literature reports the evaluation of the initial bioactivity profile and extraction of the plant followed by different chromatographic techniques to obtain fractions. As an outcome,  isolation and identification of coumarins, flavonoids, triterpenoids, lignin  cumarinolignans, glucosides, daphnecin, aquillochin, daphnine and umbelliferone from the plant have been reported. Of these compounds, a diterpene, named gnidilatimonoein, has shown promising anticancer potency in in vitro tests on various cancer cell lines.     Prevalence of Vitamin B12 deficiency in patients of type 2 diabetes mellitus on metformin: A case control study from Pakistan

    Introduction: Diabetes Mellitus is the most common endocrine disorder and metformin is the most commonly prescribed oral hypoglycemicagent. Metformin is well known to cause viamin B12 deficiency due to effect on calcium-dependent membrane action in the terminal ileum leading to malabsorption of vitamin B12. The purpose of this study is to determine prevalence and associations of Vitamin B12 deficiency in patients of type 2 diabetes mellitus treated with metformin. Methods: This case control study was carried out in department of medicine, Combined Military Hospital, Kharian from 1st Jan 2012 to 30 december 2012.We enrolled 114 outdoor patients of type 2 diabetes mellitus currently on metformin for atleast 12 months, by consecutive sampling, and 105 age and sex matched patients taken as control. Patients with vitamin B12 levels of less than 150 pg/ml were said to be B12 deficient. The results were analyzed on SPSS version 16.Results: Serum B12 levels were low in 35 patients (31%) on metformin as compared to only 9 patients (8.6%) among controls,(p value 0.002). Mean B12 levels were significantly low in metformin group 311 pg/ml (±194.4), p value 0.03. Dose of metformin had inverse correlation with B12 levels and the difference was statistically significant with pvalue &lt; 0.001.Conclusion: Our study demonstrated significantly high prevalence of  vitamin B12 deficiency in patients treated with metformin with significant effect of dose and duration of metformin use on B12 levels. Physicians must recognize this important fact and screen diabetics on metformin therapy for underlying B12 deficiency

    Potential Drug-Drug Interactions in Psychiatric Ward of a Tertiary Care Hospital: Prevalence, Levels and Association with Risk Factors

    Purpose: To identify the prevalence of potential drug-drug interactions (pDDIs) in a psychiatric ward, their levels and association with risk factors.Methods: This study was conducted in the psychiatric ward of Ayub Teaching Hospital, Abbottabad, Pakistan. Medical records of 415 patients were retrospectively reviewed for pDDIs using Micromedex Drug-Reax software. Logistic regression was applied to determine association of pDDIs with age, gender, hospital stay and number of drugs.Results: In our study, we identified total number of 825 pDDIs of 126 types, with median number of 1 pDDIs per patient. Overall 64.8 % of the patients had at least one pDDI; 27.2 % at least one major pDDI; and 58.5 % patients at least one moderate pDDI. Among 825 identified pDDIs, most were of moderate (75.6 %) or major (20.8 %) severity, good (66.4 %) or fair (29 %) type of scientific evidence; and delayed onset (71 %). The most frequent major and moderate pDDIs included haloperidol + procyclidine (127 cases), haloperidol + olanzapine (49), haloperidol + promethazine (47), haloperidol + fluphenazine (41), diazepam + divalproex sodium (40), haloperidol + trihexyphenidyl (37), lorazepam + divalproex sodium (34), fluphenazine + procyclidine (33) and olanzapine + divalproex sodium (32). There was significant association of occurrence of pDDIs with hospital stay of 7 days or longer (p = 0.005) and taking 7 or more drugs (p &lt; 0.001).&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Conclusion: A high prevalence of pDDIs in the psychiatric ward was recorded, a majority of which were of moderate severity.     Correlation and path analysis for yield and yield components in sunflower (Helianthus annus. L)

    Ten (10) sunflower genotypes were evaluated in a triplicated randomized complete blocked design in field for plant height, stem diameter at base, internodal length, head diameter, number of whorls head-1, achene weight head-1, 100-achene weight, achene oil and protein content. Association among various plant traits and direct and indirect effects of various traits on achene weight head-1 were estimated. Correlations of head diameter, 100-achene weight, stem diameter at base, internodal length and oil contents were positive and significant with achene weight head-1. Path analysis showed that direct effects of plant height, head diameter, 100-achene weight and oil contents were positive while direct effects of stem diameter at base, internodal length, number of fertile whorls head-1 and protein contents were negative on achene weight head-1.     Heavy Metal Levels in Vegetables and Soil Cultivated with Industrial Wastewater from Different Sites of Chunian and Jamber, District, Kasur

    In human diet, vegetables play important role to maintain the physiological conditions. Due to anthropogenic activities and pollution, the food items become contaminated. The present study was performed to evaluate the level of heavy metals in the vegetables irrigated with wastewater across Chunian and Jamber, district, Kasur. Level of heavy metals from the study area like Zinc, Lead and chromium in the soil, water and vegetables was compared. The four sites of each city and 10 vegetables e.g. potato, radish, carrot, fenugreek, spinach, tomato, Onion, Turnip, Cauliflower, Pangalo were selected to conduct the experiment. The vegetables were irrigated with industrial wastewater and the concentration of heavy metals was measured by the atomic absorption spectrophotometer (AAS). We concluded that the level of heavy metals was beyond the FAO limits in irrigated water due to industrial waste. In Jamber and Chunian, the level of Zn and Pb was high and beyond the FAO safe limits in the all water sample, the level of Cr was much higher only in the water sample of one site from Jamber. The concentration of zinc was higher in soil samples as compared to lead and chromium. Zn and Pb in vegetables of study area were labeled as priority pollutants but this concentration was within the safe limits set by FAO. However, constant inspection of heavy metals is recommended to avoid accumulation in the food chain and thus avoid human health risks.     Manganese toxicity with ephedrone abuse manifesting as parkinsonism: a case report

    Introduction: Neurologic consequences of manganese toxicity have been recognized since 1837. A new form of presumed manganese poisoning has been reported in drug addicted persons from Eastern Europe and the Baltic states who have intravenously injected self-prepared methcathinone hydrochloride (ephedrone), which is synthesized from pseudoephedrine hydrochloride using potassium permanganate as a potent oxidant. This clinical syndrome is under-recognized in Western Europe and there are no reported cases in the literature from Ireland. Case presentation: We report a 30-year-old Eastern European man who presented with a two-year history of gait disturbance. A neurological assessment revealed features of parkinsonism which included hypophonia, hypomimia, mild bradykinesia and rigidity with no resting tremor. He held his arms slightly abducted from his sides when walking, with a reduction in arm swing. Magnetic resonance imaging of his brain showed a high signal on T1 in the globus pallidus and serum manganese levels were raised. He had no response to levodopa. Conclusion: Manganism secondary to ephedrone abuse causing parkinsonism has emerged in Western Europe in recent years due to mass immigration and often remains unrecognized. This paper highlights the various features of this rare cause of parkinsonism and aids in its recognition and subsequent diagnosis. Neurologists in Western Europe will increasingly encounter such patients

    Space vector pulse-width modulation technique for an eleven-phase voltage source inverter with sinusoidal output voltage generation

    This paper discusses the space vector pulse width modulation (SVPWM) scheme for an eleven-phase two-level voltage source inverter (VSI), aimed at producing a sinusoidal output voltage waveform. Generalised space vector theory is used to realise the SVPWM. As per the general inverter switching theory, there are 211 = 2048 switching states that yield 2046 active voltage space vectors and one zero voltage vector, which results with two switching states. Out of the total of 2046 active voltage vectors, the most suitable set comprising 110 active voltage vectors is identified and is utilised in the implementation of the SVPWM. The sinusoidal voltage is obtained by controlling the duty cycles of the applied voltage space vectors in such a way that the non-zero reference voltage in the first (d-q) plane is achieved, while simultaneously zeroing the average voltage in the other four (x-y) planes in accordance with the zero references. The theoretical results are verified by experimentation using a passive resistive-inductive load. Finally, experimentally obtained THD values of the phase voltage and current for the eleven-phase SVPWM are compared with the corresponding values obtained using SVPWM for other odd phase numbers.     Compliance of smokeless tobacco supply chain actors and products with tobacco control laws in Bangladesh, India and Pakistan: protocol for a multicentre sequential mixed-methods study

    Introduction: South Asia is home to more than 300 million smokeless tobacco (ST) users. Bangladesh, India and Pakistan as signatories to the Framework Convention for Tobacco Control (FCTC) have developed policies aimed at curbing the use of tobacco. The objective of this study is to assess the compliance of ST point-of-sale (POS) vendors and the supply chain with the articles of the FCTC and specifically with national tobacco control laws. We also aim to assess disparities in compliance with tobacco control laws between ST and smoked tobacco products. Methods and analysis: The study will be carried out at two sites each in Bangladesh, India and Pakistan. We will conduct a sequential mixed-methods study with five components: (1) mapping of ST POS, (2) analyses of ST samples packaging, (3) observation, (4) survey interviews of POS and (5) in-depth interviews with wholesale dealers/suppliers/manufacturers of ST. We aim to conduct at least 300 POS survey interviews and observations, and 6-10 in-depth interviews in each of the three countries. Data collection will be done by trained data collectors. The main statistical analysis will report the frequencies and proportions of shops that comply with the FCTC and local tobacco control policies, and provide a 95% CI of these estimates. The qualitative in-depth interview data will be analysed using the framework approach. The findings will be connected, each component informing the focus and/or design of the next component. Ethics and dissemination: Ethical approvals for the study have been received from the Health Sciences Research Governance Committee at the University of York, UK. In-country approvals were taken from the National Bioethics Committee in Pakistan, the Bangladesh Medical Research Council and the Indian Medical Research Council. Our results will be disseminated via scientific conferences, peer-reviewed research publications and press releases
