    Knowledge, Atitude and Perception regarding National Health Programmes among villagers of Chauras, Tehri-Garhwal, Uttarakhand

    Background and Objective: Since India became independent, several measures have been undertaken by the national government to improve the health of the people. Prominent among these measures are the national health programmes. The main objective of these National Health programmes are protection and promotion of national and individual health. The main objective of this study was to assess the knowledge, attitude and perception regarding various national health programmes among the villagers. Methods: It is a descriptive and observational study. The study subjects comprised 273 respondents belonging to 15 to 64 years age group. The collection tool used was a pre designed questionnaire, which was pre-tested. Results: 60% of respondents were adults, about 16 percent were educated up to primary level and more than 40% belonged to scheduled castes. Nearly 20% were aware about National AIDS Control Programme and 6.59% had clear knowledge about HIV/AIDS. Only 4.02% knew about the national vector borne disease control programme and 24% women clearly knew about exclusive breast feeding. Peripheral health workers were the most common source of information regarding these programmes. 64% of respondents opined that these national health programmes are good. Conclusion: Low level of knowledge was observed among the respondents regarding National Health Programmes

    Work capacity and surgical output for cataract in the national capital region of Delhi and neighbouring districts of north India.

    OBJECTIVES: To ascertain time taken for cataract surgery by ophthalmologists in the National Capital Region of Delhi and neighbouring districts, to determine what work output is feasible with the available ophthalmologists. METHODS: The time-motion study was conducted during January to June 2006 in the National Capital Region of Delhi and neighbouring districts in North India. Data was collected by observing all activities from entry of a patient into the operating theatre to exit. A total of 156 cataract surgeries performed by 45 ophthalmologists in 38 hospitals were observed. A stop watch was used to record activity time, rounded off to the nearest 10 seconds. Case duration, surgical and clinical times were calculated. RESULTS: Ninety percent ophthalmologists completed surgery in 41.3 minutes. The 10th and 90th percentile for case duration time was 15.5 and 78.4 minutes respectively. Median surgical time was lowest for ophthalmologists working in the NGO sector (10 minutes), compared to the government (23.5 minutes), and private sector (17.3 minutes).     The energy consumption in telecommunication networks is expected to grow considerably, especially in core networks. In this chapter, optimization of energy consumption is approached from two directions. In a first study, multilayer traffic engineering (MLTE) is used to assign energy-efficient paths and logical topology to IP traffic. The relation with traditional capacity optimization is explained, and the MLTE strategy is applied for daily traffic variations. A second study considers the core network below the IP layer, giving a detailed power consumption model.     A guide to chemokines and their receptors

    The chemokines (or chemotactic cytokines) are a large family of small, secreted proteins that signal through cell surface G‐protein coupled heptahelical chemokine receptors. They are best known for their ability to stimulate the migration of cells, most notably white blood cells (leukocytes). Consequently, chemokines play a central role in the development and homeostasis of the immune system, and are involved in all protective or destructive immune and inflammatory responses. Classically viewed as inducers of directed chemotactic migration, it is now clear that chemokines can stimulate a variety of other types of directed and undirected migratory behaviour, such as haptotaxis, chemokinesis, and haptokinesis, in addition to inducing cell arrest or adhesion. However, chemokine receptors on leukocytes can do more than just direct migration, and these molecules can also be expressed on, and regulate the biology of, many non‐leukocytic cell types. Chemokines are profoundly affected by post‐translational modification, by interaction with the extracellular matrix (ECM), and by binding to heptahelical ‘atypical’ chemokine receptors that regulate chemokine localisation and abundance. This guide gives a broad overview of the chemokine and chemokine receptor families; summarises the complex physical interactions that occur in the chemokine network; and, using specific examples, discusses general principles of chemokine function, focussing particularly on their ability to direct leukocyte migration

    Study of electrically active defects in epitaxial layers on silicon

    Electrically active defects in silicon-based epitaxial layers on silicon substrates have been studied by Deep-Level Transient Spectroscopy (DLTS). Several aspects have been investigated, like, the impact of the pre-epi cleaning conditions and the effect of a post-deposition anneal on the deep-level properties. It is shown that the pre-cleaning thermal budget has a strong influence on the defects at the substrate/epi layer interface. At the same time, a post-deposition Forming Gas Anneal can passivate to a large extent the active defect states. Finally, it is shown that application of a post-deposition anneal increases the out-diffusion of carbon from a Si:C stressor layer into the p-type CZ substrate

    Skills of different mesoscale models over Indian region during monsoon season: Forecast errors

    Performance of four mesoscale models namely, the MM5, ETA, RSM and WRF, run at NCMRWF for short range weather forecasting has been examined during monsoon-2006. Evaluation is carried out based upon comparisons between observations and day-1 and day-3 forecasts of wind, temperature, speci.c humidity, geopotential height, rainfall, systematic errors, root mean square errors and specific events like the monsoon depressions. It is very difficult to address the question of which model performs best over the Indian region? An honest answer is 'none'. Perhaps an ensemble approach would be the best. However, if we must make a final verdict, it can be stated that in general, (i) the WRF is able to produce best All India rainfall prediction compared to observations in the day-1 forecast and, the MM5 is able to produce best All India rainfall forecasts in day-3, but ETA and RSM are able to depict the best distribution of rainfall maxima along the west coast of India, (ii) the MM5 is able to produce least RMSE of wind and geopotential fields at most of the time, and (iii) the RSM is able to produce least errors in the day-1 forecasts of the tracks, while the ETA model produces least errors in the day-3 forecasts

    Spinal Cord Injury Repair by Intrathecal Infusion of Stromal Cell-Derived Factor-1/CXC Chemokine Receptor 4 in a Rat Model

    Background: Stromal cell-derived factor-1 (SDF-1)/CXC Chemokine receptor 4 (CXCR4) is an important cytokine, with multiple functions, which plays a crucial role in the recruitment of multiple stem cell types in the defect sites of central nervous system (CNS). Various strategies have been managed to improve functional recovery after spinal cord injury (SCI). One of these strategies is the use of factors to limit damage and increase recovery. Objectives: In this study we investigated the effect of SDF-1 in spinal cord injury repair in a rat model. Materials andMethods: Adult male Wistar rats were randomly divided to four groups (n = 5) as follows: Sham, SCI, SDF-1 and Vehicle. Spinal cord injury model was created by contusion of T8-T9 by clips and SDF-1 infusion pump implanted in the neck region. One week after injury, 5-Bromo-20-Deoxyuridine (BrdU) was injected to trace the proliferative cells. Basso-Beattie-Bresnahan (BBB) test was performed to evaluate locomotor activity following SCI. Immunohistochemistry test was performed to determine proliferating cells, and real time polymerase chain reaction (PCR) was performed to detect the CXCR4 cells in tissue. Results: Significant improvements in locomotor function were detected in the SDF-1 group compared with the SCI and vehicle groups (P < 0.05). The results showed that SDF-1 treatment increased proliferative cells at the spinal cord injury site. Real time PCR revealed that these proliferative cells are CXCR4 positive that intake Bromodeoxyuridine (Brdu). Conclusions: These results showed that the administration of SDF-1a increases the number of proliferating cells in the injured area in the spinal cord and improves functional recovery

    Automatic eduction and statistical analysis of coherent structures in the wall region of a confine plane

    This paper describes a vortex detection algorithm used to expose and statistically characterize the coherent flow patterns observable in the velocity vector fields measured by Particle Image Velocimetry (PIV) in the impingement region of air curtains. The philosophy and the architecture of this algorithm are presented. Its strengths and weaknesses are discussed. The results of a parametrical analysis performed to assess the variability of the response of our algorithm to the 3 user-specified parameters in our eduction scheme are reviewed. The technique is illustrated in the case of a plane turbulent impinging twin-jet with an opening ratio of 10. The corresponding jet Reynolds number, based on the initial mean flow velocity U0 and the jet width e, is 14000. The results of a statistical analysis of the size, shape, spatial distribution and energetic content of the coherent eddy structures detected in the impingement region of this test flow are provided. Although many questions remain open, new insights into the way these structures might form, organize and evolve are given. Relevant results provide an original picture of the plane turbulent impinging jet

    Renal impairment in a rural African antiretroviral programme

    Background: There is little knowledge regarding the prevalence and nature of renal impairment in African populations initiating antiretroviral treatment, nor evidence to inform the most cost effective methods of screening for renal impairment. With the increasing availability of the potentially nephrotixic drug, tenofovir, such information is important for the planning of antiretroviral programmes Methods: (i) Retrospective review of the prevalence and risk factors for impaired renal function in 2189 individuals initiating antiretroviral treatment in a rural African setting between 2004 and 2007 (ii) A prospective study of 149 consecutive patients initiating antiretrovirals to assess the utility of urine analysis for the detection of impaired renal function. Severe renal and moderately impaired renal function were defined as an estimated GFR of ≤ 30 mls/min/1.73 m2 and 30–60 mls/min/1.73 m2 respectively. Logistic regression was used to determine odds ratio (OR) of significantly impaired renal function (combining severe and moderate impairment). Co-variates for analysis were age, sex and CD4 count at initiation. Results: (i) There was a low prevalence of severe renal impairment (29/2189, 1.3% 95% C.I. 0.8–1.8) whereas moderate renal impairment was more frequent (287/2189, 13.1% 95% C.I. 11.6–14.5) with many patients having advanced immunosuppression at treatment initiation (median CD4 120 cells/μl). In multivariable logistic regression age over 40 (aOR 4.65, 95% C.I. 3.54–6.1), male gender (aOR 1.89, 95% C.I. 1.39–2.56) and CD4<100 cells/ul (aOR 1.4, 95% C.I. 1.07–1.82) were associated with risk of significant renal impairment (ii) In 149 consecutive patients, urine analysis had poor sensitivity and specificity for detecting impaired renal function. Conclusion: In this rural African setting, significant renal impairment is uncommon in patients initiating antiretrovirals. Urine analysis alone may be inadequate for identification of those with impaired renal function where resources for biochemistry are limited

    Zona pellucida-induced acrosome reaction in human spermatozoa is potentiated by glycodelin-A via down-regulation of extracellular signal-regulated kinases and up-regulation of zona pellucida-induced calcium influx

    Background Glycodelin-A interacts with spermatozoa before fertilization, but its role in modulating sperm functions is not known. Zona pellucida-induced acrosome reaction is crucial to fertilization and its dysfunction is a cause of male infertility. We hypothesized that glycodelin-A, a glycoprotein found in the female reproductive tract, potentiates human spermatozoa for zona pellucida-induced acrosome reaction. Methods Glycodelin isoforms were immunoaffinity purified. The sperm intracellular cAMP concentration, protein kinase-A (PKA) and extracellular signal-regulated kinase (ERK) activities, and intracellular calcium were measured by ELISA, kinase activity assay kits and Fluo-4AM technique, respectively. The phosphorylation of inositol 1,4,5-trisphosphate type-1 receptor (IP3R1) mediated by ERK was determined by western blotting. Zona pellucida-induced acrosome reaction was detected by Pisum sativum staining. Results Pretreatment of spermatozoa with glycodelin-A significantly up-regulated adenylyl cyclase/PKA activity and down-regulated the activity of ERK and its phosphorylation of IP3R1, thereby enhancing zona pellucida-induced calcium influx and zona pellucida-induced acrosome reaction. Glycodelin-F or deglycosylated glycodelin-A did not have these actions. Treatment of spermatozoa with a protein kinase inhibitor abolished the priming activity of glycodelin-A, whilst ERK pathway inhibitors mimic the stimulatory effect of glycodelin-A on zona pellucida-induced acrosome reaction. Conclusions Glycodelin-A in the female reproductive tract sensitizes spermatozoa for zona pellucida-induced acrosome reaction in a glycosylation-specific manner through activation of the adenylyl cyclase/PKA pathway, suppression of extracellular signal-regulated kinase activation and up-regulation of zona pellucida-induced calcium influx. © 2010 The Author.postprin
